Appendicovesicostomy and newer alternatives for the Mitrofanoff procedure: results in the last 100 patients at Riley Children's Hospital.

PURPOSE: We present our experience using the various Mitrofanoff techniques to create a continent catheterizable stoma as an adjunct to continent urinary tract reconstruction in children and young adults. MATERIALS AND METHODS: Between 1990 and 1998 a Mitrofanoff procedure was performed at our institution in 55 male and 45 female patients with a mean age of 10.5 years. The etiology of incontinence was diverse but more than 90% of the patients had neurogenic bladder, the epispadias-exstrophy complex or a cloacal anomaly. Surgery included appendicovesicostomy in 57 cases, a Yang-Monti ileovesic

Obstructed Hemivagina: Anatomical Variation, Management, and Challenges of Classification- A Single Centre 10-Year Retrospective Cohort Study.

OBJECTIVE: Obstructed hemivagina is a rare congenital anomaly typically presenting in adolescence and is commonly associated with ipsilateral renal anomalies. Diagnosis is frequently delayed, and existing classification systems provide limited support for surgical planning. This study aimed to describe clinical and anatomical variations in obstructed hemivaginas, evaluate surgical outcomes following specialist management, and identify anatomical features relevant to systematic description. METHODS: This retrospective cohort study included all patients who underwent surgery for obstructed hemi

Bulking agent treatment of incontinent catheterizable channels in pediatric patients and young adults.

BACKGROUND: Catheterizable continent channels (CCC) provide means for urinary continence when urethral catheterization is not feasible. However, some patients present with stomal incontinence warranting further interventions. The purpose of this study is to evaluate the effectiveness of endoscopic injection (EI) of bulking agent (Deflux©) as a minimally invasive treatment for CCC incontinence and to explore patient-specific variables influencing outcomes in a pediatric cohort. METHODS: Hospital's pediatric urology procedure registry was retrospectively reviewed to identify all patients with a

Use of clomiphene citrate and birth defects, National Birth Defects Prevention Study, 1997-2005.

BACKGROUND: Clomiphene citrate (CC) is the first line drug for subfertility treatment. Studies assessing the association between CC and birth defects have been inconclusive. METHODS: We used data from the National Birth Defects Prevention Study, a population-based, multi-site case-control study of major birth defects. Women from 10 US regions with deliveries affected by at least one of >30 birth defects (cases) and mothers of live born infants without a major birth defect (controls) who delivered October 1997-December 2005 were interviewed. The exposure of interest was reported CC use in the

Construction of a female urethra using the vaginal wall and a buttock flap: experience with 40 cases.

METHODS: From 1975 to 1996 a urethra was constructed using tubularized anterior vaginal wall covered with a buttock flap in 40 female patients in whom urinary incontinence was associated with a short or absent urethra. The underlying pathology included bilateral single ectopic ureters, cloacal malformation, urogenital sinus malformation, previous failed surgery of the urethra, severe trauma, myelodysplasia, female hypospadias, ectopic ureterocele with destruction of the urethra, congenital epispadias, bladder exstrophy, and previous hysterectomy and vaginectomy for clear cell carcinoma caused

The Shanfield anastomosis revisited: Its applications and early outcome.

BACKGROUND: Shanfield first described a simple ureteric implantation technique involving a U-stitch anchoring the spatulated end of the transplant ureter to the interior of the intact bladder through a small stab wound. We present an extrapolation of this principle to Mitrofanoff channels and native ureteric reimplantations and further extend it to a laparoscopic approach in some. METHODS: A retrospective case-note reviewing the Shanfield ureteric reimplantation in fifteen children between October 2014 and May 2017 was performed. RESULTS: Fifteen children (females n = 9), median age 6 years

Teratogenic effects of suramin on the chick embryo.

Suramin, a polysulfonated naphthylamine, has been used for the chemotherapy of trypanosomiasis and onchocerciasis since about the 1920s. Currently, it is also being tested as an anticancer agent. It is hoped that suramin might stop the progression of some kinds of cancer since it has been found to inhibit the proliferation and migration of cells and the formation of new blood vessels. These processes are not only essential for the development and progression of cancer, but also for normal embryonic development. Suramin might, therefore, be a potent teratogen. In the literature, however, we hav

Vaginal reconstruction in anorectal malformations: A 17-year single-center experience.

BACKGROUND/PURPOSE: Vaginal anomalies frequently accompany anorectal malformations (ARMs) in females, yet management strategies remain poorly standardized. This study presents our 17-year single-center experience with vaginal reconstruction in ARM patients with documented Müllerian and/or vaginal anomalies, focusing on surgical decision-making, technique selection, and outcomes. METHODS: This study was conducted retrospectively on all children with ARMs who had documented Müllerian and/or vaginal anomalies who underwent vaginal reconstruction at our tertiary referral center between January 20

Outcomes in the giant omphalocele population: A single center comprehensive experience.

BACKGROUND/PURPOSE: Morbidity and mortality in the giant omphalocele population is complicated by large abdominal wall defects, physiologic aberrancies, and congenital anomalies. We hypothesized different anomalies and treatment types would affect outcomes. METHODS: A 2009-2018 retrospective chart review of giant omphaloceles was performed. Exclusions included cloacal exstrophy, transfer after 3 weeks, surgery prior to transfer, conjoined twins, or not yet achieving fascial closure. Thirty-five patients met criteria and mortality and operative morbidity categorized them into favorable (n = 20

Bladder neck closure in conjunction with enterocystoplasty and Mitrofanoff diversion for complex incontinence: closing the door for good.

PURPOSE: Bladder neck closure is an irreversible procedure requiring compliance with catheterization of a cutaneous stoma and historically has been reserved for the definitive treatment of intractable incontinence after prior failed procedures. We assessed long-term outcomes of our patients undergoing bladder neck closure including continence status, additional surgical interventions, postoperative complications, conception and sexual function, and satisfaction with bladder neck closure. MATERIALS AND METHODS: We performed a retrospective review of all patients who underwent bladder neck clos

Impact of intestinal lengthening on the nutritional outcome for children with short bowel syndrome.

Survival among children with short bowel syndrome has increased with the use of supportive nutritional techniques including parenteral and enteral nutrition. Further improvement in outcome has been sought by using intestinal lengthening procedures to lengthen the bowel, improve intestinal motility, initiate a progressive increase in intestinal mucosal mass, and thereby improve tolerance to enteral nutrition. The authors examine the growth parameters and the tolerance to enteral nutrition in children with refractory short bowel syndrome before and after intestinal lengthening procedures. For se

Ketamine-Based Sedation Among Young Infants During MRI at Mzuzu Central Hospital: Initial Experience in a Resource-Limited Setting in Malawi.

BACKGROUND: Magnetic resonance imaging (MRI) in young infants requires effective immobilization to ensure diagnostic image quality. Although non-pharmacological approaches such as the feed-and-swaddle technique are widely recommended, they are not always successful. Sedation in neonates remains controversial, particularly in resource-limited settings. This study aimed to evaluate the feasibility and observed safety profile of intramuscular ketamine for MRI sedation in young infants at Mzuzu Central Hospital, Malawi. METHODS: This retrospective case series included five young infants (aged 3-9

Omphalocele-exstrophy-imperforate anus-spinal defect syndrome in a monozygotic twin pregnancy: a case report and literature review.

BACKGROUND: The Omphalocele, Exstrophy, Imperforate Anus, and Spinal Defects Complex (OEIS complex) is a rare birth defect with a prevalence of 1 in 200,000-400000 pregnancies and 1 in 82,000-200000 live births. While cases in female monozygotic twins have been reported, the perinatal outcomes of the unaffected co-twin remain insufficiently addressed. CASE PRESENTATION: We report a spontaneously conceived monochorionic monoamniotic twin pregnancy where Twin B was diagnosed with OEIS. Prenatal ultrasound at 12 + 4 weeks identified an abdominal cystic mass in Twin B, which enlarged progressivel

A review of adopted colorectal patients: a parent's perspective.

PURPOSE: After treating many adopted patients with congenital colorectal conditions, our goal was to understand if parents were properly counseled about their child's medical needs before adoption. METHODS: A comprehensive questionnaire was developed. Recruitment occurred by social media and colorectal database. RESULTS: 48 parents participated. Adopted children were primarily male (60%), internationally adopted (75%), and a median age of 2.5 years (range newborn-13yo). While 96% of parents received medical records, 41% had incorrect/missing information. Most patients had an anorectal malfor

Surgery in disorders of sex development (DSD) with a gender issue: If (why), when, and how?

Ten years after the consensus meeting on disorders of sex development (DSD), genital surgery continues to raise questions and criticisms concerning its indications, its technical aspects, timing and evaluation. This standpoint details each distinct situation and its possible management in 5 main groups of DSD patients with atypical genitalia: the 46,XX DSD group (congenital adrenal hyperplasia); the heterogeneous 46,XY DSD group (gonadal dysgenesis, disorders of steroidogenesis, target tissues impairments …); gonosomic mosaicisms (45,X/46,XY patients); ovo-testicular DSD; and "non-hormonal/non

Long-term followup and outcome of continent catheterizable vesicocstomy using the Rink modification.

PURPOSE: Multiple techniques have been described to create a Mitrofanoff channel in the pediatric population. A small subset of patients only requires creation of a catheterizable channel without bladder augmentation. These patients are ideal candidates for a procedure that avoids the use of intestine, especially in the absence of a suitable appendix. We used a modification of the Casale vesicostomy, as described by Rink, to create a continent vesicostomy in these children. We report our long-term experience with this technique. MATERIALS AND METHODS: We retrospectively reviewed the medical r
