The use of a joystick technique facilitates closed reduction and percutaneous fixation of multidirectionally unstable supracondylar humeral fractures in children.

BACKGROUND: Multidirectionally unstable supracondylar fracture of the distal humerus presents with severe instability in both flexion and extension. Closed reduction and percutaneous fixation is challenging and may not be obtained by the traditional reduction maneuver. METHODS: We retrospectively evaluated 8 children (4 boys and 4 girls) with a mean age at presentation of 7.6 years (range, 5.3 to 10.9 y) who underwent closed reduction and percutaneous fixation using a joystick technique for the treatment of multidirectionally unstable supracondylar fractures. Clinical and functional results w

[Closed reduction osteosynthesis of non-articular proximal fractures of the 1st metacarpal bone. Crossed ascending double pinning].

In non-articular fractures of the base of the base of the first metacarpal, this technique permits firm fixation, and immediate mobilization, with an excellent result. It consists of the insertion of two Kirschner wires, on either side, from the neck to the base of the first metacarpal, while reducing the fracture without opening. Two front and side view X-Rays control the correct reduction and position of the wires, which are cut under the skin closure. When making the two lateral incisions, it is very important to avoid injuring the two dorsal branches of the radial nerve. The Kirschner wire

A novel closed reduction technique for treating femoral shaft fractures with intramedullary nails: Kirschner wire and the fulcrum technique.

Femoral shaft fractures (FSF) are usually treated with closed reduction and intramedullary nailing. In this study, we report a novel technique with Kirschner wire to achieve closed reduction. From January 2020 to January 2023, this technique was used on 32 patients with FSF (AO 32 A and 32 B). The fracture was reduced with a radiolucent traction table, 3.0 mm Kirschner wire and the fulcrum technique was conducted. The operative effect was evaluated on the basis of the operative time, reduction time, fluoroscopy time, and intraoperative blood loss. All 32 patients were treated by closed reducti

A Novel Method of Closed Reduction and Percutaneous Pinning With Six K-wires for Intra-articular Fractures of the Distal Radius.

The existing method of closed reduction cannot reduce the complete intra-articular fracture of the distal radius; none of the available methods of percutaneous pinning alone can fix the intra-articular fracture of the distal radius. A prospective study including 108 complete intra-articular fractures of AO type B and C in 108 patients was conducted from 2013 to 2018. The subjects were aged between 18 and 84 years and included 61 men and 47 women. The cohort was divided into three age groups: ages 18 to 44, ages 45 to 59, and ages 60 to 84. The surgical technique includes an innovative method o

The design of an "H" joystick for closed reduction and its application in segmental and comminuted femoral shaft fractures: an innovative technique.

BACKGROUND: Closed reduction and locked intramedullary nailing has become a common surgical method in the treatment of femoral shaft fractures. Overlap and rotation displacements can usually be corrected through the use of an orthopedic traction table. However, lateral displacement and angulation persist. METHODS: In this paper, we describe a joystick that can be used in the closed reduction of a fracture. It can correct lateral displacement and angulation, and has the advantage of multi-direction reduction. The device described in this paper includes two parallel horizontal joysticks, one ve

A Comparison of Complications and Union Rates in Intramedullary Nailing of Femoral Shaft Fractures Treated With Open Versus Closed Reduction.

Intramedullary rod fixation is a common technique for treatment of femoral shaft fractures, with both open and closed reduction techniques described. The purpose of this study was to assess union and complication rates among patients treated with open vs closed reduction and intramedullary nailing of closed femoral shaft fractures. A total of 107 patients undergoing intramedullary fixation of nonpathologic femoral shaft fractures (AO/OTA type 32) between January 2012 and June 2017 were retrospectively studied. Those undergoing open reduction prior to intramedullary nailing were compared with t

Improved Results in Closed Reduction of Zygomatic Arch Fractures by the Use of Intraoperative Cone-Beam Computed Tomography Imaging.

PURPOSE: The use of intraoperative imaging could help overcome the insufficiency of closed reduction that can result from the absence of visible feedback to evaluate the accuracy of the reduction in isolated zygomatic arch fractures. The aim of the present study was to evaluate the use of intraoperative 3-dimensional cone-beam computed tomography (CBCT) to improve the accuracy of reduction, by measuring the remaining cortical step and dislocation angle. We also evaluated the effect of CBCT on the need for repeat surgery. PATIENTS AND METHODS: We performed a retrospective cohort study of patie

Does the quality of preoperative closed reduction of displaced ankle fractures affect wound complications after surgical fixation?

BACKGROUND: Displaced ankle fractures are initially closed reduced and splinted with the goal of restoring gross ankle alignment. The benefits of an exact closed reduction are unclear and possibly detrimental and unnecessary if multiple attempts are made. The purpose of this study was to determine whether the quality of preoperative closed reduction in patients with operative ankle fractures affects post-operative wound complications. METHODS: A retrospective analysis was performed of patients with isolated, closed, operative ankle fractures treated at two level 1 trauma centers who had an in

Arthroscopy-assisted closed reduction and percutaneous nail fixation of unstable ankle fractures: description of a minimally invasive procedure.

When one is surgically managing an unstable ankle fracture, anatomic reduction of the syndesmosis is typically accomplished using an open surgical approach. We propose an arthroscopically assisted technique that restores normal anatomy while using a percutaneously placed intramedullary nail to fix the fibula. The patient is positioned supine, and the ankle is placed under traction by use of a tensor bandage. Standard anteromedial and anterolateral arthroscopy portals are used. The joint is examined for bony, ligamentous, and chondral injury. Lateral malleolus fracture reduction is accomplished

A novel closed reduction with extension block and flexion block using Kirschner wires and microscrew fixation for mallet fractures.

BACKGROUND: Some patients with mallet fractures who undergo extension block pinning complain of exposed wires, which delay their return to sports and causes inconvenience while performing tasks that require the use of hands during the early postoperative period. The purpose of this retrospective study was to present and evaluate a novel surgical procedure for mallet fractures. METHODS: We treated 20 patients (14 males and six females; mean age, 38.4 years; range 17-68 years) with displaced mallet fractures involving >30 % of the articular surface using the closed reduction and microscrew fixa
