Coracoacromial arch decompression in rotator cuff surgery.

In rotator cuff surgery it is important to obtain adequate decompression of the coracoacromial arch. However, it is difficult to localize the impingement site preoperatively. Based on histological and morphological studies and the clinical findings in 45 patients and 15 cadavers, we have tried to determine the impingement site. In addition, as a part of these investigations, we assessed the clinical outcome for 100 patients. Soft tissue decompression was indicated in 12 shoulders, anterior acromioplasty in 67 and anterior acromioplasty with coracoplasty in 21. According to Neer's criteria ther

Portal decompression using the inferior mesenteric vein.

We report five patients with variceal hemorrhage, in three cases secondary to diffuse thrombosis of the portal, superior mesenteric and splenic veins. Mesenteric angiography demonstrated patency of the inferior mesenteric vein (IMV) in each, and successful portal decompression by anastomosis of the IMV to the left renal vein (n = 4) or the inferior vena cava (n = 1) was accomplished. Bleeding was permanently controlled: four patients have survived from one to eight years post-operatively. Because shunt procedures utilizing the IMV are technically straightforward, subtotally decompress the port

Endoscopic suction decompression of idiopathic epidural lipomatosis.

BACKGROUND: Epidural lipomatosis can be idiopathic or associated with obesity and steroid excess. This disorder can be treated by weight control, reduction of steroid load, or operative decompression. We describe a non-obese patient with neurogenic claudication who had localized epidural fat from L4 to S1. Minimally invasive treatment consisted of a small laminotomy and endoscopically guided fat aspiration. METHODS: A small bilateral L5-S1 laminotomy was performed. Using a special malleable endoscopic aspirator, loose fat was removed both dorsal and ventral to the dural sac. RESULTS: At surg

Endoscopic orbital decompression for thyroid eye disease.

BACKGROUND: Thyroid eye disease is a disorder of immune function resulting in extraocular myopathy and an increase in retrobulbar fat. These changes lead to the clinical features found in thyroid eye disease including proptosis, eyelid retraction, restrictive myopathy and sometimes compressive optic neuropathy. Orbital decompression is undertaken for optic nerve compression, exposure keratopathy or cosmesis. Previously this involved an external approach with the removal of the medial orbital wall along with the floor. An intranasal endoscopic approach with the removal of the medial orbital wal

The sphenoidal sinus in optic nerve decompression.

Surgical decompression of the orbit may be necessary in dysthyroid patients with compressive optic neuropathy. Two-wall decompression with a trans-antral or a trans-conjunctival approach is commonly used. However, in some patients the initial improvement following this surgical procedure is not maintained. Although this may be due to disease progression, a variant of orbital anatomy can contribute to sub-optimal decompression. We report three cases in which recurrence of compressive optic neuropathy occurred following two-wall decompression. The sphenoidal sinus was placed anteriorly in these

Endoscopic decompression for acute colonic pseudo-obstruction.

BACKGROUND: Acute colonic pseudo-obstruction is often treated by colonoscopic decompression. Efficacy, safety, and outcome of endoscopic decompression was assessed. METHODS: Colonoscopic decompressions from 1988 to 1994 were reviewed. Resolution without further endoscopic intervention was defined as clinical success. RESULTS: Acute colonic pseudo-obstruction was diagnosed in 50 patients. Thirty-three cases followed surgery or trauma and 17 developed during severe medical illness. Orthopedic joint surgery was most common. Nineteen of 50 patients (38%) had severe underlying medical disease. Fo

Extracranial optic canal decompression: indications and technique.

Optic canal decompression may be beneficial in cases of indirect optic nerve trauma, with or without canal fracture. Although no definitive data exist to clarify its role, several clinical series have reported on optic canal decompression for the treatment of intracanalicular optic nerve trauma, providing only limited information of the details of the procedure. We describe extradural optic canal decompression using a transethmoidal/transorbital approach. Removal of > 180 degrees of the bony canal is possible with this method. The technique can be accomplished by orbital surgeons familiar with

Transforaminal arthroscopic decompression of lateral recess stenosis.

The purpose of this study was to evaluate the feasibility and efficacy of arthroscopic decompression of lateral recess stenosis, determine potential associated complications, and present an alternative method to access the lateral recess of the lumbar spine. Forty patients were selected in whom the authors found clinical and computerized tomography evidence of lateral recess stenosis and sequestered foraminal herniations. All 40 were treated with a posterolateral arthroscopic technique, and 38 were available for this follow-up evaluation. A satisfactory result was obtained in 31 patients (82%)

Multilevel cervical spondylosis. Laminoplasty versus anterior decompression.

Poor overall outcome and a high incidence of postoperative kyphosis and progressive myelopathy have driven surgeons away from decompressive laminectomy as a treatment for multilevel cervical spondylosis. Dr. Henry Bohlman advocates anterior decompression and fusion as the best approach to the pathophysiology of this disorder, while Dr. Kiyoshi Hirabayashi believes that laminoplasty represents an excellent strategy for patients with degenerative disease, as well as those with ossification of the posterior longitudinal ligament.

Microvascular decompression for trigeminal neuralgia: prognostic factors.

A series of 66 patients with trigeminal neuralgia (TN) treated by microvascular decompression (MVD) is analyzed in order to define the importance of various prognostic factors. A typical pattern of pain, presence of facial hypaesthesia and previous ablative trigeminal procedures showed an unfavorable influence on the results of MVD. Duration of symptoms was longer in patients who failed to benefit from MVD. Pain distribution and the type of vascular compression did not influence the outcome.

Microneurovascular decompression: early experiences in Srinagarind Hospital.

These results support other studies that indicate that M.V.D. is the best procedure to treat these patients. There are some points to emphasize M.V.D. in order to achieve the best results and lowest complication rate. These include good preoperative patient preparation; more lateral suboccipital craniectomy to expose the cranial nerves with minimal retraction; adequate drainage of CSF from the cistern before attempting to identify the nerve; thorough inspection of the affected cranial nerve, especially in the root entry (exit) zone; adequate mobilization of blood vessels and stable interpositi

[Arthroscopic subacromial decompression. 1-3 year results].

In 148 patients with impingement lesion type I or type II, we performed an arthroscopic subacromial decompression (ASD). 122 patients ran a follow up one to three years post-operatively. All patients were pre- and postoperatively documented by a 100 point shoulder score. The mean score was 57.9 (+/- 11.5) preoperatively. Postoperatively there was a significant increase to 80.7 (+/- 17.9) (p < 0.05). 15% of the patients with a postoperative score less than 70 points were determined as failures. Patients with a preoperative pain history of more than one year had a significantly worse result (79.

Hypoglobus following orbital decompression for dysthyroid ophthalmopathy.

Hypoglobus following orbital decompression is not a rare complication. Hypoglobus requiring surgery to elevate the globe following orbital decompression is considered clinically significant hypoglobus. A retrospective analysis of 157 consecutive patients who had their orbits expanded by removing the medial wall and floor of the orbit found seven patients (4.5%) who required further surgery to elevate the globe. The seven patients who developed clinically significant hypoglobus following orbital decompression all developed a set of distinct clinical signs. These included a "setting sun" appeara

Nasolacrimal drainage system obstruction after orbital decompression.

We reviewed 123 cases of orbital decompression in 63 patients with dysthyroid ophthalmopathy. Of 90 cases of transantral ethmoidal orbital decompression, 14 (16%) resulted in epiphora. The tearing began between 11 and 18 months after surgery. All patients had obstruction distal to the common internal punctum. The delayed onset suggested progressive cicatricial obstruction of the nasolacrimal drainage system. Damage to adjacent tissues probably caused scarring to extend into the system.
