Compressed Air Working in Chennai During Metro Tunnel Construction: Occupational Health Problems.

Chennai metropolis has been growing rapidly. Need was felt of a metro rail system. Two corridors were planned. Corridor 1, of 23 km starting from Washermanpet to Airport. 14.3 km of this would be underground. Corridor 2, of 22 km starting from Chennai Central Railway station to St. Thomas Mount. 9.7 km of this would be underground. Occupational health centre's role involved selection of miners and assessing their fitness to work under compressed air. Planning and execution of compression and decompression, health monitoring and treatment of compression related illnesses. More than thirty five

Diving injuries to the inner ear.

Most of the previous literature concerning otologic problems in compressed gas environments has emphasized middle ear barotrauma. With recent increases in commercial, military, and sport diving to deeper depths, inner ear disturbances during these exposures have been noted more frequently. Studies of inner ear physiology and pathology during diving indicate that the causes and treatment of these problems differ depending upon the phase and type of diving. Humans exposed to simulated depths of up to 305 meters without barotrauma or decompression sickness develop transient, conductive hearing lo

Ear problems and injuries in athletes.

The ear is an unique organ--the principal structure involved in both hearing and balance. Although not common, problems with the ear may be encountered in specific sporting populations. Common conditions affecting the ear in the athlete include otitis externa, an infection of the external ear; external auditory canal exostoses, or abnormal bony growths in the canal; and otitis media, an infection of the middle ear. Given its position on the head, the ear is subject to trauma, often resulting in an auricular hematoma. Divers, due to pressure changes on descent and ascent, are subject to both ea

[Disorders caused by heat, cold, and abnormal pressure].

Exposure to heat disturbs the homeostasis of body water, serum osmosis, and core temperature, resulting in the development of heat cramp, heat syncope, heat exhaustion, and heat stroke. Commonly coexisting risks are humidity, windlessness, infrared radiation, physical exertion, continuous work, chemical protective clothing, and lack of acclimatization. Exposure to cold constricts peripheral arteries and reduces metabolism, resulting in the development of chilblains, frostbite, immersion foot, and hypothermia. Wind, water immersion, and alcohol drinking will aggravate the symptoms. Exposure to

CT finding of VGE in the portal veins and IVC in a diver with abdominal pain: a case report.

The finding of abdominal venous gas emboli (VGE) on computerized tomography (CT) is reported for the first time in a recreational diver. The patient presented 2-3 hours after surfacing from two deep air dives and subsequently complained of visual blurring and abdominal pain. Gas bubbles in the inferior vena cava (IVC) and portal veins were found incidentally by computerized tomography (CT) during his work-up for abdominal pain. The patient was treated for decompression sickness (DCS) with a US Navy Treatment Table 6 and achieved complete resolution of symptoms. The routine use of CT for venous

Cutis marmorata and cerebral arterial gas embolism.

Dr Kemper and colleagues reported that, when air was injected into the cerebral circulation of pigs, they developed a rash that looked very similar to cutis marmorata of cutaneous decompression illness (DCI) and to livido reticularis. They postulated that cutaneous DCI in divers may be centrally mediated as a result of cerebral gas embolism. It would be helpful if Kemper et al. described the distribution of the rash in their pigs. In divers, cutaneous DCI is generally confined to parts of the body with significant amounts of subcutaneous fat, such as the trunk and thighs, and the rash often cr

Sickle cell trait and diving: review and recommendations.

INTRODUCTION: Sickle cell trait, once thought to be a benign condition, has been associated with complications which may be of concern to the diver, such as splenic infarction and exercise-related sudden death. Medical clearance to dive for those with this common hemoglobinopathy has been a source of debate, with differing recommendations for both civilian and military candidates. To assess this disparity, a review was conducted of published findings and opinions regarding sickle cell trait and fitness to dive. METHODS: Literature search was conducted using PubMed/Medline. Keywords included s

Ocular tear film bubble formation after breath-hold diving.

Bubbles in the ocular tear film have been observed following both dry-chamber, simulated compressed air dives and in-water, recreational compressed air dives. The current paper reports on the formation of tear film bubbles in a breath-hold diver following repeated, extended breath-hold excursions to a maximum depth of -28.5 m. It is believed that this is the first time that ocular tear film bubbles have been reported in breath-hold divers.

Venous and arterial bubbles at rest after no-decompression air dives.

PURPOSE: During SCUBA diving, breathing at increased pressure leads to a greater tissue gas uptake. During ascent, tissues may become supersaturated, and the gas is released in the form of bubbles that typically occur on the venous side of circulation. These venous gas emboli (VGE) are usually eliminated as they pass through the lungs, although their occasional presence in systemic circulation (arterialization) has been reported and it was assumed to be the main cause of the decompression sickness. The aims of the present study were to assess the appearance of VGE after air dives where no stop

In-water recompression for severe decompression illness in competitive freediving: a case report.

INTRODUCTION: Decompression illness (DCI) is a recognised risk of freediving. When timely hyperbaric oxygen therapy (HBOT) is unavailable, oxygen-based in-water recompression (IWR) may be considered. METHODS: A 49-year-old competitive freediver developed severe neurological DCI within minutes of surfacing from a dive to 117 metres of seawater at a sanctioned competition, with altered mental status, right facial droop, right hemiparesis and aphasia. With an anticipated 24-48 h delay to HBOT, emergency IWR was initiated with a full-face mask and 100% oxygen. Incremental ascent from 18 msw over

Retroperitoneal extensive free air bubbles due to decompression illness.

Decompression illness (DCI) is a rare condition caused by air bubbles that arise because of a rapid decrease in ambient pressure. These air bubbles exert both physical and chemical effects associated with a range of findings from asymptomatic clinical presentation to death. In the literature, changes in consciousness, severe musculoskeletal and abdominal pain, respiratory distress, and skin changes have been described. The diagnosis of DCI is difficult, but anamnesis and physical examination are helpful. Radiologic evaluation is useful for determining possible complications in patients with se

Inner ear barotrauma in divers: an evidence-based tool for evaluation and treatment.

OBJECTIVE: To systematically search the literature for studies evaluating the typical presentation and testing that is performed for divers with inner ear symptoms and then to create a tool for clinicians when evaluating a diver with inner ear symptoms. METHODS: Nine databases, including PubMed/MEDLINE were systematically searched through 31 January 2018. The PRISMA statement was followed. RESULTS: Three-hundred and two manuscripts were screened, 69 were downloaded and 21 met criteria to be included in this review. The articles were evaluated for symptomatic trends and initial evaluation wor

The persistent foramen ovale and migraine.

There is association between migraine, particularly migraine with aura, and large persistent foramen ovale (PFO) and other right-to-left shunts. Migraine is often improved by closure of a PFO in patients who have a large shunt. Inheritance of migraine is linked to the inheritance of large atrial shunts in some families. The data indicate that in some patients PFOs play a causal role in migraine with aura. These observations may also improve understanding of the etiology of migraine in patients who do not have a shunt.

Physiological incidents during 39 years of hypobaric chamber training in Japan.

BACKGROUND: Hypobaric chamber training for military aircrew is very important for flight safety. Since we began hypobaric training in our laboratory in 1960, some trainees have suffered physiological incidents. This study will characterize the physiological incidents during hypobaric chamber training at the Japan Air Self-Defense Force (JASDF). METHODS: All available training records from 1960-1998 were reviewed and the frequency of physiological incidents counted and analyzed. RESULTS: There were 29,677 trainees and 58,454 exposures. Overall frequency of physiological incidents was 6.3%. Ph

Cardiac problems associated with dysbarism.

Patient salvage rates depend on the extent of injury, speed and appropriateness of treatment, and the patient's age and state of health in addition to other variables. Undertreatment of DII may result in significant morbidity and associated costs. If there is any doubt regarding whether a patient might benefit from treatment, a trial at pressure is indicated. Among those with AGE who reach recompression within 5 minutes, the death rate may be only 5%, and those who survive have little morbidity. Among those whose recompression delay increases to 5 hours, however, mortality approaches 10%, and
