[Demodicidosis in humans as a current problem in dermatology].

Demodex mites are common parasites in the hair follicles and in the pilosebaceous glands. Demodex folliculorum and D. brevis have been identified in humans and play a role in three facial conditions: Pityriasis folliculorum, rosacea-like demodicidosis and Demodicidosis gravis. The highly infected sites by this mites are: forehead, area around the orbit, area around the nose, chin and area around the mouth. Infection of the skin by D. folliculorum is more frequent among females and in the elderly people. The presence of mites can be determined by microscopic inspection of secretion from sebum g

The rapid effect of pulsed dye laser on demodex density of facial skin.

BACKGROUND: Recently, treatment with acaricides, which is aimed at reducing excessive proliferation of demodex mites, has gained popularity due to its providing a significant improvement in the symptoms of diseases, such as rosacea, seborrhoeic dermatitis, and perioral dermatitis. The effect of IPL on demodex mites was reported in skin biopsy specimens in three patients; however, to the best of our knowledge, no study exists to date, which evaluates the effect of pulsed dye laser (PDL) on demodex density (Dd) in larger patient group. We aim here in to observe the Dd before and after PDL therap

The Role of Skin Microbiota in Facial Dermatoses and Related Factors: A Narrative Review.

Inflammatory facial dermatoses (atopic dermatitis [AD], acne vulgaris, contact dermatitis, seborrheic dermatitis, rosacea, perioral dermatitis, and demodicosis, etc.) often profoundly impact patients' appearance and psychological well-being. In this narrative review, we wanted to present the current knowledge on the role of skin microbiota in common facial dermatoses. Skin keratinocytes are the primary producers of antimicrobial peptides (AMPs) and express Toll-like receptors (TLRs), which stimulate the T helper (Th1) immune response, with the production of interferon (IFN). They can also prod

Off-label Uses of Topical Pimecrolimus.

Pimecrolimus is a topical calcineurin inhibitor currently approved for second-line use in the management of mild-to-moderate atopic dermatitis in patients age 2 years and older. Given the safety profile and nonsteroidal mechanism of pimecrolimus, there has been significant interest in its use in the treatment of a variety of dermatological conditions. This article reviews research that has been published on the off-label uses of topical pimecrolimus, with a focus on published RCTs. Convincing evidence exists supporting pimecrolimus' efficacy in oral lichen planus and seborrheic dermatitis. For

Demodex: The worst enemies are the ones that used to be friends.

Demodex mites are common ectoparasites of the human pilosebaceous units. Most adults are infested with Demodex mites without clinical symptoms. Demodex mite will only become a pathogenic organism when there is an abnormal increase in the number of Demodex mite density. This situation happens when the equilibrium between Demodex mites, skin microenvironment and human immunity system changes. Demodex infestation can cause multiple skin disorders, which are grouped under the term demodicosis or demodicidosis. Clinical manifestations of demodicosis can mimic other known skin diseases such as folli

Granulomatous periorificial dermatitis: a variant of granulomatous rosacea in children?

BACKGROUND: We report a case of granulomatous periorificial dermatitis (GPD). A 13-year-old Jamaican boy presented with pink to flesh-colored perioral and periocular papules that erupted during a summer visit to Jamaica. The child was initially diagnosed with sarcoidosis and treated with oral corticosteroids, but the eruption recurred 2 years later. He was referred to Dermatology. Biopsy of one of the facial papules revealed a dense granulomatous infiltrate with surrounding prominent lymphocytes. A diagnosis of GPD was made. OBJECTIVE AND CONCLUSION: Also known as facial Afro-Caribbean childh

[Cefixime treatment in different bacterial infections in the ENT region].

The clinical efficacy and tolerance of cefixime were evaluated in an open uncontrolled clinical trial including 37 patients suffering from ENT-infections. The MIC90 values of this new oral cephalosporin against gram-negative pathogens are less than 1 mg/l. The antibacterial activity of cefixime against gram-negative pathogens is stronger than that of other oral cephalosporins. Cefixime was administered for five to twelve days twice daily in a dose of 200 mg. In one patient the treatment with cefixime was discontinued after five days due to perioral dermatitis. In 33 of 36 patients cure or dist

Biotinidase deficiency--clinching the diagnosis rapidly can make all the difference!

A 2-month-old male infant, born of second degree consanguineous parentage, presented with seizures not responding to phenytoin and phenobarbitone. His perinatal period had been uneventful and there was no family history of seizures. On examination, he had failure to thrive, perioral and perianal rash, alopecia with hypopigmented hair, seborrhoeic dermatitis, bilateral blepharitis, respiratory distress and stridor. Neurological examination revealed hypertonia of all the four limbs, exaggerated deep tendon reflexes and papilloedema. Biotinidase deficiency was suspected within 24 h of admission a

Periocular allergic contact dermatitis following topical Mitomycin C eye drop application.

A 63-year-old women had an extensive limbal papilliform tumoural lesion of her left eye. Mitomycin C (MMC) was applied to the area at a dose of 0.2 mg/ml after total surgical excision of the lesion. The lesion was diagnosed as invasive squamous cell carcinoma on histopathology and topical 0.02% MMC was prescribed four times daily to the left eye. A severe per ocular contact dermatitis of the left eye developed two days after starting MMC. The patch test result was positive. The per ocular dermatitis resolved after discontinuation of the topical MMC and treatment with a topical corticosteroid.

Heterozygous Variants of the SLC39A4 Gene and Possible Increased Risk for Developing Acrodermatitis Enteropathica with Kaposi's Varicelliform Eruption.

BACKGROUND Acrodermatitis enteropathica (AE) is a rare autosomal recessive disorder caused by solute carrier family 39 member 4 (SLC39A4) gene variants that impair zinc absorption. Although typically associated with bacterial or fungal superinfection, its concurrence with Kaposi's varicelliform eruption (KVE) is exceptionally rare - only 1 case was previously reported. This report describes a case of KVE complicating AE in a patient with novel compound heterozygous SLC39A4 variants, highlighting the immunovirological implications of zinc deficiency. CASE REPORT An 8-year-old girl who had chron

[Folliculitis barbae in herpes simplex infection].

A 60-year-old male athlete developed a folliculitis in the beard region after several competitions. After identification of herpes simplex antigen within the lesions, systemic therapy with acyclovir led to rapid improvement. In folliculitis resistant to antibiotic and anti-inflammatory therapy, viral and mycotic infections as well as eosinophilic folliculitis should be considered as differential diagnostic possibilities.

STK4 (MST1) deficiency in two siblings with autoimmune cytopenias: A novel mutation.

Combined immunodeficiencies (CIDs) are heterogeneous group of disorders characterized by abrogated/impaired T cell development and/or functions that resulted from diverse genetic defects. In addition to the susceptibility to infections with various microorganisms, the patients may have lymphoproliferation, autoimmunity, inflammation, allergy and malignancy. Recently, three groups have independently reported patients having mutations in STK4 gene that cause a novel autosomal recessive (AR) CID. We describe here two siblings with a novel STK4 mutation identified during the evaluation of a group

New hydroxyisoflavans as contact sensitizers in cocus wood Brya ebenus DC (Fabaceae).

A 15-year-old girl developed perioral dermatitis and swelling of the lips, having played a flute made of cocus wood for 2 years. Patch tests with wood shavings, as well as with subsequently isolated constituents named Cocus I and Cocus II, were strongly positive. After complete purification, both contact allergens were identified as isoflavan derivatives. While Cocus I was shown to be 7,8-dihydroxy-2',4',5'-trimethoxyisoflavan, the structure of Cocus II could not be elucidated exactly. It could have been either 7,8,3'-trihydroxy-2',4'-dimethoxyisoflavan or 7,8,2'-trihydroxy-3',4'-dimethoxyflav

Subacute toxicity of dietary T-2 toxin in mice: morphological and hematological effects.

Changes in hematopoietic and lymphoid tissues of young Swiss mice fed a balanced semipurified diet containing T-2 toxin (20 ppm) were examined after one, two, three, four or six weeks. During the first three weeks of exposure of T-2 toxin, lymphoid tissues, bone marrow and splenic red pulp became hypoplastic, resulting in anemia, lymphopenia and eosinopenia. Subsequently, during continued exposure to T-2 toxin, hematopoietic cells regenerated in bone marrow and splenic red pulp and became hyperplastic by six weeks. Granulopoiesis and thrombopoiesis resumed in advance of erythropoiesis. All lym

Contact allergy in the mouth: diversity of clinical presentations and diagnosis of common allergens relevant to dental practice.

Delayed-type hypersensitivity reaction or type IV allergic reaction can cause different oral manifestations. They can be localized or diffusely visible on oral mucosa and usually appear 24-72 hours after antigen input. The antigens that cause this type of reaction are mostly external, such as contact allergens (particularly metals) and drugs. It has been shown that the most common oral manifestations are cheilitis, gingivitis, stomatitis, perioral dermatitis, burning mouth syndrome, lichenoid reaction and orofacial granulomatosis. The most important part of diagnosis is the use of patch testin
