How should glucocorticoid and mineralocorticoid replacement be optimised in a young patient with classic 21-hydroxylase congenital adrenal hyperplasia?

This article aims to provide a simple clinical approach to understanding and managing steroid supplementation in young adults and adult patients with classical congenital adrenal hyperplasia secondary to 21-hydroxylase deficiency (21OHD). It aims to overcome the clinical inertia that leads to glucocorticoid excess in these patients, particularly addressing the adult endocrinologist at the transition when they meet the patient with 21OHD with 'fresh eyes'.

Klinefelter syndrome.

Klinefelter syndrome, affecting males, is a collection of characteristics that occurs as a result of two or more X chromosomes. The syndrome was named after Harry Klinefelter, an American endocrinologist, and is common - occurring in all races. It is thought that one male in every 500 live births is affected and the incidence is rising. However, this may be due to increasing awareness, reflective of the sophistication of the methods to diagnose.

Perspectives on Newer Antiobesity Drugs (Semaglutide and Tirzepatide) among Healthcare Professionals Treating Obesity in South Asia: A Cross-sectional Survey.

INTRODUCTION: Obesity has reached epidemic proportions in South Asia, contributing significantly to metabolic diseases and economic burdens. Newer antiobesity medications (AOMs), such as semaglutide and tirzepatide, show promising efficacy but face barriers such as high costs and limited familiarity among healthcare professionals (HCPs) in low- and middle-income countries (LMICs). This study explored perceptions, prescribing practices, efficacy, safety concerns, and barriers to AOM use among HCPs managing obesity in South Asia. METHODS: A cross-sectional online survey was conducted from May t

EnVision CF: Building the Next Generation of Leaders in Cystic Fibrosis Endocrinology. A Model for Physician-Scientist Workforce Development in Advanced Subspecialty Care.

INTRODUCTION: As people with cystic fibrosis (CF) live longer, the burden of endocrine and metabolic complications has become increasingly apparent. Despite a complex and evolving clinical landscape, until very recently, access to endocrinologists and reproductive health specialists with specific expertise in CF was limited to only a few large CF Centers. DESCRIPTION: Recognizing this gap in skilled care-provider availability, in 2016 the Cystic Fibrosis Foundation launched EnVision CF: Emerging Leaders in CF Endocrinology. This national career development program was designed to train and su

IGF-1 as screening tool for acromegaly and adult-onset growth hormone deficiency in the Netherlands.

OBJECTIVE: Insulin-like growth factor 1 (IGF-1) measurements play a central role in the diagnosis and follow-up of acromegaly and growth hormone deficiency. However, improving health care outcomes for these patients involves an intricate process of laboratory diagnostics and skilled health care professionals. The integrated effects of IGF-1 reports on diagnosis and treatment decisions are yet unknown. DESIGN, PATIENTS AND MEASUREMENTS: Extended quality assessment, distributing the description of five (real) patient cases with accompanying blood samples. Patients suspected or during follow up

Assessment and referral of patients with short stature by primary care physicians in the Arabian gulf region: Current perspectives from a regional survey.

Children with short stature are frequently referred late to pediatric endocrinologists in the Arabian Gulf region. This is likely a contributing factor to late initiation of treatment despite current evidence suggesting that children with short stature have better outcomes with earlier treatment. This delay in referral could be due to a lack of identification or proper assessment of short stature by front-line physicians. To analyze the assessment and perception of short stature in this group of physicians, an expert group of pediatric endocrinologists developed and disseminated an anonymous o

Ectopic ACTH-dependent Cushing syndrome in 3 hospitalized patients: lessons learned from management with osilodrostat.

Severe Cushing syndrome (CS) is a rare diagnosis with high mortality, which limits the ability of endocrinologists to gain experience delivering care. Here we describe our experience treating 3 cases of ectopic ACTH-dependent CS presenting in a single year. The initial diagnosis of each case was made during hospitalization resulting from complications of CS, implicating severe disease and delayed diagnosis. Our experience suggests that rapid initiation and titration of osilodrostat effectively treats hypercortisolism and improves outcomes.

Congenital hypothyroidism in Young-Simpson syndrome.

We describe a patient with the clinical spectrum of Young-Simpson syndrome. This rare genetic disorder is characterized by congenital hypothyroidism, mental retardation and blepharophimosis. Young-Simpson syndrome is, at present, poorly known to endocrinologists and pediatricians, and should be included in the differential diagnosis of congenital hypothyroidism. It is important to underline that the association of congenital hypothyroidism, blepharophimosis and ptosis allows an exact clinical diagnosis, since the majority of other clinical aspects are common to other disorders.

Acromegaly secondary to growth hormone releasing hormone secretion.

BACKGROUND: Acromegaly secondary to growth hormone releasing hormone (GHRH) secretion is exceptionally rare. AIM: To report a case of acromegaly diagnosed in 1984 and assumed to be pituitary in origin. Sixteen years later, the cause was found to be a GHRH secreting neuroendocrine pancreatic tumour. METHOD: A case report. CONCLUSION: Although ectopic GHRH production is very rare, endocrinologists should be aware of this possibility in acromegaly patients if a pituitary tumour was not detected using pituitary imaging.

Graves' orbitopathy: a multidisciplinary approach.

Orbitopathy is the main extra thyroidal manifestation of Graves' disease. It is a very challenging condition, which requires a cooperation between many specialists (endocrinologists, ophthalmologists, radiologists, radiotherapeutic, orbital surgeons) for an optimal clinical management. An accurate diagnostic assessment is required, in order to plan an adequate treatment of Graves' orbitopathy. Medical therapy, radiotherapy or surgery may be necessary to control the disease. In this review, the authors analyze the various therapeutic strategies, as well the more recent therapies based on pharma

[Every sixth patient in amiodarone treatment develops thyrotoxicosis or hypothyreosis].

Amiodarone is associated with multiple side effects among which a substantial number of patients are suffering from amiodarone-induced thyroid disease. In this review we present difficulties in diagnosing, sub typing and treating amiodarone-induced thyroid disease and give views on the future of this drug and one of the alternatives, dronedarone. Amiodarone holds a place in modern medicine due to its unique antiarrhythmic effects and associated thyroid diseases requires collaboration between cardiologists and endocrinologists due to the complexity hereof.

[Bisphosphonates in the therapy of fibrous dysplasia. Relevant data and practical aspects].

Fibrous dysplasia covers a variety of rather different clinical pictures. Its therapy therefore has to be individually adapted. In addition to the orthopaedic surgeon, endocrinologists and craniofacial surgeons are also involved in the process. In the present contribution the role of modern bisphosphonates as a conservative treatment strategy is discussed with regard to the prevalent therapeutic possibilities. The literature as well as personal experience show them to be an especially valid option for the treatment of polyostotic fibrous dysplasia and the various syndromes.

Parathormone (PTH), produced by parathyroid glands, is the main regulator of calcium homeostasis. Hypoparathyroidism (hypoPT), due to decrease of PTH production, is a rare disease. Symptoms are multiple, altering function of several organs and leading to a decrease of quality of life. Acquired etiologies, including thyroïdectomy, the main cause of hypoPT, can be distinguished from congenital etiologies, including genetic defects. HypoPT, which is classically treated by supplementation by calcium and active vitamin D, can now be treated by recombinant injection in certain indications as a poor

Do Not Forget Nephrotic Syndrome as a Cause of Increased Requirement of Levothyroxine Replacement Therapy.

Nephrotic syndrome increases L-thyroxine requirements because of urinary loss of free and protein-bound thyroid hormones. We report 2 hypothyroid patients referred to us because of high serum TSH, even though the L-thyroxine daily dose was maintained at appropriate levels or was increased. The cause of nephrotic syndrome was multiple myeloma in one patient and diabetic glomerulosclerosis in the other patient. As part of the periodic controls for diabetes, urinalysis was requested only in the second patient so that proteinuria could be detected. However, as in the first patient, facial puffines
