Results of the ANCHOR prospective, multicenter registry of EndoAnchors for type Ia endoleaks and endograft migration in patients with challenging anatomy.

OBJECTIVE: Proximal attachment site complications continue to occur after endovascular repair of abdominal aortic aneurysms (EVAR), specifically type Ia endoleak and endograft migration. EndoAnchors (Aptus Endosystems, Sunnyvale, Calif) were designed to enhance endograft proximal fixation and sealing, and the current study was undertaken to evaluate the potential benefit of this treatment. METHODS: During the 23-month period ending in December 2013, 319 subjects were enrolled at 43 sites in the United States and Europe. EndoAnchors were implanted in 242 patients (75.9%) at the time of an init

Treatment of type II endoleaks associated with left subclavian artery coverage during thoracic aortic stent grafting.

OBJECTIVE: Increasing experience with thoracic aortic stent grafts has led to a more aggressive approach to thoracic aortic pathologies in the distal aortic arch and proximal descending thoracic aorta. To increase the length of the proximal landing zone, it is sometimes necessary to cover the left subclavian artery with the thoracic stent-graft, introducing the risk of retrograde filling of the excluded aorta from the left subclavian artery. It is currently unclear how best to manage these patients to prevent persistent risk of aneurysm expansion or rupture. We report our experience with a min

Treatment of a Type II Endoleak from an Atypical Mediastinal Collateral Branch Following Total Arch Replacement with Frozen Elephant Trunk.

We present a rare case of type II endoleak (T2EL) from an atypical mediastinal artery following total arch replacement with a frozen elephant trunk (FET) for chronic aortic dissection. A 63-year-old male with a history of multiple thoracic aortic surgeries, including FET, developed enlargement of an aortic arch aneurysm on follow-up. Computed tomography and diagnostic angiography revealed a T2EL due to a newly developed mediastinal collateral artery arising from the left subclavian artery feeding the sac. Selective transcatheter embolization with N-butyl-2-cyanoacrylate successfully eliminated

Thoracic Endovascular Aortic Repair for Blunt Thoracic Aortic Injury: Impact of Arch Type on the Rate of Type 1 Endoleak.

BACKGROUND: In recent years, thoracic endovascular aortic repair (TEVAR) has emerged as the standard of care for management of blunt thoracic aortic injury (BTAI), resulting in reduced rates of in-hospital mortality. We identified factors, including arch type, that may impact endoleaks among these patients. METHODS: The Aortic Trauma Foundation registry was used to identify patients with BTAI managed with TEVAR from 2012 to 2023 for whom arch type was documented. Type 1 endoleak rates, graft specifications, lesion characteristics, demographics, and mechanism of injury were compared. RESULTS:

Relining of a Bridging Covered Stent to Treat a Rare Cause of Type IIIc Endoleak Following Fenestrated Endovascular Aneurysm Repair.

PURPOSE: To present a rare cause of type III endoleak via the left renal artery (LRA) fenestration following fenestrated endovascular aneurysm repair (FEVAR) and to describe a successful reintervention for treating this endoleak. TECHNIQUE: The patient presented with a type IIIc endoleak following FEVAR, due to inadvertent placement of the LRA bridging balloon expandable covered stent (BECS) via the superior mesenteric artery (SMA) fenestration, but deployed outside the SMA fenestration. The proximal part of the BECS was positioned outside of the main body. This caused a type IIIc endoleak vi

Proximal iliac limb extension and embolization: a new technique of complete endovascular management of an unfavorably sited type III endoleak.

Type III endoleak is an uncommon but life-threatening complication of endovascular aortic repair, and such leaks at certain sites can be challenging to treat through an endovascular route. A 77-year-old man presented with severe abdominal pain and was found to have an abdominal aortic aneurysm with contained rupture due to an unfavorably cited type IIIb endoleak. He was successfully treated with an endovascular approach using bilateral iliac limb proximal extension combined with embolization of endoleak sac, endoleak site and the feeding recess, preserving flow through both the iliac limbs obv

Prospective Multicentre Cohort Study of Fenestrated and Branched Endografts After Failed Endovascular Infrarenal Aortic Aneurysm Repair with Type Ia Endoleak.

OBJECTIVE: Failed endovascular infrarenal aortic aneurysm repair (EVAR) due to development of late type Ia endoleak exposes patients to the risk of rupture and should be treated. The purpose of this study was to evaluate the results of fenestrated/branched EVAR (F/BEVAR) for treatment of failed EVAR with type Ia endoleak. METHODS: From January 2010 to December 2019, a prospective multicentre study was conducted (ClinicalTrials.gov identifier: NCT04532450) that included 85 consecutive patients who had undergone F/BEVAR to treat a type Ia endoleak following EVAR. The primary outcome was overall

Intraoperative retrograde TEVAR to control endoleak after emergent total arch replacement and frozen elephant trunk repair for ruptured Kommerell's diverticulum.

We report an emergent complex hybrid repair of a type A intramural hematoma with a tear of the aortic arch at the site of Kommerell's diverticulum and an aberrant right subclavian artery. We identified a type IA endoleak intraoperatively, which was managed immediately with proximal extension. Performing this operation in the hybrid operating room facilitated optimal surgical management.

Treatment of Type I Endoleak after Thoracic Endovascular Aortic Repair and Stent Extension via False Lumen Catheterization and Coil Embolization.

Thoracic endovascular aortic repair (TEVAR) is a minimally invasive alternative to conventional open surgical reconstruction for the treatment of thoracic aortic dissection. However, the possibility of a type I endoleak may contribute to false lumen enlargement and rupture. Here, we present a case of successful treatment of type I endoleak after TEVAR and endograft extension for a complicated descending thoracic aortic dissection that was embolized by coils via a false lumen channel through the distal entry tear. This endovascular technology might offer a new option to treat thoracic type I en

Successful image-guided percutaneous embolization of a ruptured abdominal aortic aneurysm sac due to type II endoleak after endovascular repair.

The rupture of an aneurysm sac secondary to endoleak type II after endovascular abdominal aortic aneurysm repair is a rare occurrence, routinely corrected by open surgery. Computed tomography (CT)-guided percutaneous embolization of the aneurysm sac is a safe, efficient, and minimally invasive treatment option. We present a case of an endoleak type II associated to aneurysm sac rupture successfully treated by CT-guided percutaneous embolization of the aneurysm sac.

Prophylactic Perigraft Arterial Sac Embolization During EVAR: Minimizing Type II Endoleaks and Improving Sac Regression.

BACKGROUND: Type II endoleaks (ELII) are the most common complication following endovascular aneurysm repair (EVAR). Persistent ELII require continual surveillance and have been shown to increase the risk of Type I and III endoleaks, sac growth, need for intervention, conversion to open or even rupture, directly or indirectly. These are often difficult to treat following EVAR, and there are limited data regarding the effectiveness of prophylactic treatment of ELII. The aim of this study is to report the midterm outcomes of prophylactic perigraft arterial sac embolization (pPASE) performed in p

The association between perioperative embolization of hypogastric arteries and type II endoleaks after endovascular aortic aneurysm repair.

OBJECTIVE: Type II endoleaks (T2ELs) are the most common type of endoleak after endovascular aneurysm repair (EVAR). The iliolumbar artery arising from the hypogastric artery is often a major source of T2ELs, and transarterial embolization of the iliolumbar artery through the hypogastric artery is sometimes performed to interrupt sac expansion during follow-up. Considering the equivocal results of an association between hypogastric embolization and T2ELs in previous studies, this topic has re-emerged after the advent of iliac branch devices. This study reviewed our series to clarify whether hy

Effect of antiplatelet therapy on aneurysmal sac expansion associated with type II endoleaks after endovascular aneurysm repair.

OBJECTIVE: Endovascular aneurysm repair (EVAR) of abdominal aortic aneurysms (AAAs) has gained widespread use through a solid reputation of safety and effectiveness. However, some issues, such as endoleaks and sac growth over time, still arise as important concerns. Antiplatelet therapy, mandatory as secondary prevention of cardiovascular disease, may play a role in both phenomena by interfering with blood clotting properties and the inflammatory process associated with AAA. We analyzed whether different antiplatelet therapies were independent risk factors for type II endoleak (T2E) persistenc

Laparo- and thoracoscopic aortic aneurysm neck optimization and treatment of potential endoleaks type IA and II in a porcine model.

BACKGROUND: Endovascular repair of aortic aneurysms has a higher incidence of late complications, and open conversion (OC) associated with high mortality may be required. As alternatives to OCs, we propose minimal invasive laparo-/thoracoscopic approaches, either to control endoleaks after endovascular repair, or to convert non-endovascular treatable cases due to a hostile neck anatomy by inserting a peri-aortic PTFE collar before endovascular repair. Such interventions may reduce complications and the necessity for OCs in the future. METHODS: In twelve pigs, were 10 had infra-/juxtrarenal AA

Late Open Abdominal Aneurysm Reconstruction and Graft Salvage in a Patient With Persistent Endoleak Type II Following Endovascular Aneurysm Repair.

This study highlights a case of late open conversion repair (OCR) for persistent Type II endoleak after endovascular aneurysm repair (EVAR), presenting a 78-year-old male with a history of EVAR for an infrarenal abdominal aortic aneurysm. Despite conservative management of the initial endoleak, the aneurysm sac's progressive growth necessitated open reconstruction to salvage the graft. Successful postoperative outcomes emphasize the critical need for meticulous intervention strategies and surveillance in managing persistent Type II endoleaks. This case underlines the importance of a tailored a

Retrograde filling of an internal iliac artery aneurysm via the profunda femoris artery: A case report of type IIa endoleak.

INTRODUCTION: Internal iliac artery aneurysms (IIAAs) are an uncommon but clinically significant vascular condition that can lead to life-threatening complications, such as rupture and endoleaks, following endovascular repair. Endoleaks particularly type IIa, occur when there is retrograde flow into the aneurysm sac from collateral vessels, and their presence can jeopardize the success of repair procedures. This case report illustrates a rare occurrence of a type IIa endoleak attributed to retrograde filling from the profunda femoris artery, providing insights into the diagnostic complexities
