Case report: Epiglottitis in the setting of COVID-19.

Epiglottitis is an uncommon, potentially fatal infection of the epiglottis that can lead to complete upper airway obstruction. Isolated pathogens are usually nasopharyngeal bacteria, most commonly Haemophilus influenzae type b (Hib). While the incidence of epiglottitis has diminished significantly over the years due to widespread vaccination against Hib, the proportion of epiglottitis caused by other pathogens has increased. In this report, we introduce an unseen presentation of acute epiglottitis associated with severe acute respiratory distress syndrome--coronavirus2 (SARS-CoV-2), who presen

A Retrospective Cohort Study of Acute Epiglottitis in Adults.

INTRODUCTION: Adult epiglottitis is a disease process distinct from pediatric epiglottitis in microbiology, presentation, and clinical course. While traditionally considered more indolent and benign than in children, adult epiglottitis remains a cause of acute airway compromise with a mortality rate from 1-20%. Our objective was to characterize the disease course and evaluate the rate and type of airway management in this population at a tertiary, academic referral center. METHODS: We conducted a retrospective chart review of all adult patients (age ≥ 18) who were definitively diagnosed with

The clinical characteristics of patients with an isolate epiglottic collapse.

OBJECTIVE: We analyzed the clinical characteristics of patients with isolated epiglottic collapse (IEC) who had an epiglottic anteroposterior (AP) collapse as the only cause of snoring during drug-induced sleep endoscopy (DISE). METHODS: A retrospective analysis of 334 consecutive DISE examinations was performed. Patients who had only epiglottic AP collapse were designated as the IEC group. There were four age- and sex-matched controls for each case, and these patients were designated as the control group. Demographic factors, polysomnography findings, cephalometry, and awake nasopharyngoscop

An unexpected asymptomatic epiglottal site of Kimura disease.

INTRODUCTION: Kimura disease (KD) is a chronic lymphoproliferative disorder of unknown etiology that affects the skin and lymph nodes, mostly observed in males of Asian descent. The natural history of asymptomatic epiglottal KD remains unknown. This rare site of KD is often only diagnosed when tumor growth starts to obstruct the upper airways. OBSERVATION: A 34-year-old North African male presented with fatigue and multiple, slowly progressive, fluctuating skin nodules in the right mandibular and retroauricular regions. Computed tomography of the head and neck revealed a large soft tissue tum

Subtotal epiglottectomy for the management of epiglottic retroversion in a dog.

A six-year-old male neutered Yorkshire terrier was evaluated for severe, acute-onset, inspiratory dyspnoea. Laryngoscopy revealed retroversion of the epiglottis with intermittent occlusion of the rima glottidis during inspiration. The dog underwent both temporary and permanent epiglottopexy procedures that were unsuccessful. Subtotal epiglottectomy was performed and resulted in permanent resolution of dyspnoea without evidence of dysphagia or aspiration. This case highlights potential complications associated with management of this condition as well as describing the successful use of subtota

[Epiglottitis and necrotizing mediastinitis in a middle-aged man].

Acute epiglottitis is nowadays a rare bacterial infection. Airway management and promptly started antimicrobial medication are essential in the treatment. Descending necrotizing mediastinitis is a life-threatening bacterial infection that typically spreads from an odontogenic, pharyngeal or neck infection. Despite the vagueness of symptoms, diagnosis must be made quickly. Owing to the high mortality rate, aggressive surgical treatment is indicated.

An uncommon shape of epiglottis misleading as acute epiglottitis.

We report a case of a 31-year-old Japanese woman with an uncommon shape of epiglottis misleading as acute epiglottitis. She presented with high fever, arthralgia, general fatigue and strong throat pain. Laryngeal fiberscopy showed a markedly swollen epiglottis suggestive of acute epiglottitis, though she had no signs of asphyxia. A careful observation and detailed medical history ruled out acute epiglottitis. However, without clinical discretion, it might have been misinterpreted as acute epiglottitis. She was diagnosed later with Behçet's disease, which is reported to present pharyngeal steno

Epiglottic repositioning procedure for supraglottic stenosis/collapse.

OBJECTIVES: Supraglottic stenosis/collapse is an uncommon condition. It can be difficult to detect and may be a cause of failed decannulation following tracheostomy. We present a novel technique to correct supraglottic stenosis/collapse. METHODS: We performed a retrospective analysis of the records of patients in whom attempts at decannulation had failed at our center between 2003 and 2007. A subgroup with supraglottic stenosis/collapse with posterior displacement of the base of the epiglottis was identified. Our epiglottic repositioning procedure was performed in these patients. Through an e

Four cases of acute epiglottitis with a peritonsillar abscess.

We report four cases of acute epiglottitis with a peritonsillar abscess originating from the inferior pole of the palatine tonsil. All cases were male, and presented with acute onset of sore throat and dysphagia. Flexible laryngoscopy revealed swollen epiglottis and swelling at the base of tongue along the edge of the epiglottis in all cases. Computed tomography (CT) revealed the position and extent of a peritonsillar abscess. Surgical drainage was not performed. Abscesses decreased in size following intravenous antibiotics and corticosteroids. We surmise that inflammatory exudates extending w

Dyspnea caused by epiglottic retroversion in two dogs.

CASE DESCRIPTION: An 8-year-old castrated male Boxer and a 10-year-old spayed female Yorkshire Terrier were evaluated because of dyspnea. In both dogs, the dyspnea persisted after elongated soft palate resection. CLINICAL FINDINGS: Laryngoscopic examination revealed caudal displacement of the epiglottis into the rima glottidis in both dogs. Excessive mobility of the epiglottis during respiration with episodic obstruction of the rima glottidis by the epiglottis was observed during fluoroscopic examination. TREATMENT AND OUTCOME: The epiglottis of both dogs was fixed in a horizontal plane by r

Treatment of wide-based epiglottic cyst by microdebrider.

Epiglottic cyst is not an unusual disease in the larynx. Although it is usually a benign lesion, airway problems may occur due to a large cyst. Surgical excision is usually the treatment of choice, with low recurrence. Because of the limited view and mobility of laryngoscopes, there is sometimes difficulty in the procedure of excision or marsupialization, especially in wide-based epiglottic cysts. The microdebrider has been widely used in treatment of otolaryngological diseases, and specialized blades were designed for laryngeal lesions. We report the use of a microdebrider for treatment of wi

Treatment in the field of 27 horses with epiglottic entrapment.

Twenty-seven horses were treated for epiglottic entrapment by using an oral, hand-assisted bistoury knife technique, under general anaesthesia; 26 of them returned successfully to racing, but one developed a permanently displaced soft palate. After the surgery 13 of the horses had an increased handicap rating and 13 had a decreased rating.

Airway obstruction caused by a congenital epiglottic cyst.

Congenital epiglottic cyst is a rare affliction with potential for airway obstruction. A newborn boy was referred to our department for evaluation of respiratory distress and inspiratory stridor 7 h after birth. Through a transnasal fiberoptic laryngoscopy examination, a diagnosis of an obstructive upper laryngeal cyst was made. Immediate endoscopic surgery was performed 20 h after birth to completely remove the lesion. Two days after surgery, the patient resumed normal breathing pattern and showed no further episodes of stridor or airway obstruction.

[Acute epiglottitis in adults. A series of 41 cases].

Acute epiglottis is not strictly a childhood disease. We report our experience with acute epiglottis in adults with a retrospective series of 41 patients seen from 1986 to 1994. The main complaint was painful dysphagia. The diagnosis was confirmed a examination of the larynx. Therapeutic management in a hospital setting is required due to the risk of ventilatory obstruction (therapy to be adapted to the patient's ventilatory status).

Acute epiglottitis in the adult caused by Neisseria meningitidis.

Acute epiglottitis is being increasingly encountered in the adult population. While it has many similarities to its pediatric counterpart, the presentation and associated pathogens are distinct. In addition, although the recent literature has suggested that acute epiglottitis in the adult follows a benign course this is not always the case. Here we describe a case of acute, fulminant epiglottitis in an immunocompetent host, caused by Neisseria meningitidis.
