Risk of Developing Lower Limb Ischemia in Patients With Extracorporeal Cardiopulmonary Resuscitation: A Secondary Analysis of the Study of Advanced Cardiac Life Support for Ventricular Fibrillation With Extracorporeal Circulation in Japan II (SAVE-J II) Study.

OBJECTIVES: While potential risks for limb ischemia have been explored in studies of venoarterial extracorporeal membrane oxygenation (ECMO), they remain inadequately defined for patients undergoing extracorporeal cardiopulmonary resuscitation (ECPR). We identified risk factors for the development of lower limb ischemia during ECPR using a large nationwide ECPR cohort. DESIGN: A post hoc analysis was conducted using a nationwide, multicenter, retrospective study (Study of Advanced Cardiac Life Support for Ventricular Fibrillation with Extracorporeal Circulation in Japan II [SAVE-J II]). Depth of anesthesia by Narcotrend® and postoperative characteristics in children undergoing cardiac surgery under extracorporeal circulation: a retrospective comparison of two anesthetic regimens.

BACKGROUND: Depth of anesthesia may be insufficient in pediatric cardiac anesthesia if a total intravenous anesthetic regimen with opioids and midazolam is used during cardiopulmonary bypass. The advantages of sevoflurane-based balanced anesthesia may be (1) a more graduated regulation of the depth of anesthesia during cardiopulmonary bypass and (2) a reduction in postoperative ventilation time for children in comparison with total intravenous anesthesia. AIM: To evaluate a possibly positive effect of sevoflurane-based balanced anesthesia in children undergoing cardiac surgery we analyzed whe

EXPRESS: Cerebral Microbleed Burden After Aortic and Cardiac Surgery Is Associated With Extracorporeal Circulation Exposure: Evidence From Young Patients With Heritable Connective Tissue Disorders.

BACKGROUND AND PURPOSE: Cerebral microbleeds (CMBs) occur after surgery involving extracorporeal circulation (ECC), though confounding risk factors may obscure this association. Patients with heritable connective tissue diseases (HCTDs) requiring early surgery provide an ideal model to investigate ECC-related CMBs. We aimed to assess CMB prevalence, distribution, and surgical correlates in a genetically confirmed HCTD population. METHODS: We retrospectively analyzed 111 HCTD patients with brain magnetic resonance. CMBs number and location were correlated with clinical and surgical data, inclu

Soluble Urokinase Plasminogen Activator Receptor (suPAR) Plasma Concentration Is Reduced Using Minimized Extracorporeal Circulation: Results of a Secondary Analysis of a Prospective Observational Study.

Background: Minimized extracorporeal circulation (miECC) was developed to mitigate the adverse effects of cardiopulmonary bypass (CPB), yet its impact on soluble urokinase plasminogen activator receptor (suPAR) is unclear. SuPAR has been linked to adverse outcomes, including acute kidney injury (AKI). This study investigated perioperative suPAR kinetics in patients undergoing cardiac surgery with miECC or conventional CPB (cCPB) and explored its association with AKI, postoperative delirium (POD), and infections. Methods: This study is a secondary analysis of an observational cohort of 79 cardi

Effect of extracorporeal circulation on structure of main components of animal plasma - ATR-IR and 2D-COS studies. Part II: Presence of antithrombotic drugs.

An application of extracorporeal circulation (ECC) requires using antithrombotic drugs to maintain blood fluidity and prevent thrombotic complications. The molecular mechanism of this process is not fully elucidated; therefore, we undertook an examination of the effect of antithrombotic drugs (aspirin and clopidogrel) on changes in the structure of the main plasma components during ECC. The spectral variations were monitored using ATR-IR (Attenuated Total Reflection Infrared) spectroscopy and analyzed by moving-window approach and two-dimensional correlation spectroscopy (2D-COS). For samples

Extracorporeal therapy in the treatment of sepsis: In vitro assessment of the effect of an absorbent cartridge on the circulating bacterial concentration and its interaction with the antibiotic therapy.

Sepsis is one of the major causes of death worldwide. In its physiopathological process, a broad spectrum of pro and antiinflammatory mediators plays a strategic role, leading to a sepsis induced state of immunoparalysis. The rationale behind the employment of extracorporeal purification techniques as a complement to therapy for sepsis is based on their ability to remove the mediators involved. Until now, attention was focused on the immunomodulation allowed by purification therapies. However, the focus of studies on the application possibilities that these techniques offer as a supplement to

Hemocompatible nucleosome-inspired heparin-mimicking hydrogel microspheres for safe and efficient extracorporeal removal of circulating histones in critically ill patients.

Circulating histones have been identified as essential mediators that lead to hyperinflammation, platelet aggregation, coagulation cascade activation, endothelial cell injury, multiple organ dysfunction, and death in severe patients with sepsis, multiple trauma, COVID-19, acute liver failure, and pancreatitis. Clinical evidence suggests that plasma levels of circulating histones are positively associated with disease severity and survival in patients with such critical diseases. However, safe and efficient therapeutic strategies targeting circulating histones are lacking in current clinical pr

[Recommendations of the S3 Guideline "Use of Extracorporeal Circulation (ECLS/ ECMO) for Cardiac and Circulatory Failure" of the Association of Scientific Medical Societies in Germany].

In recent years, the use of mechanical support for patients with cardiac or circulatory failure has continuously increased, leading to 3,000 ECLS/ECMO (extracorporeal life support/extracorporeal membrane oxygenation) implantations annually in Germany. Due to the lack of guidelines, there is an urgent need for evidence-based recommendations addressing the central aspects of ECLS/ECMO therapy. In July 2015, the generation of a guideline level S3 according to the standards of the Association of the Scientific Medical Societies in Germany (AWMF) was announced by the German Society for Thoracic and

Effects of stored and fresh blood transfusions on postoperative pulmonary function. A clinical study on patients following extracorporeal circulation in association with aortic valve surgery.

The effects on postoperative pulmonary function of fresh versus stored blood when priming the heart-lung machine were studied in 49 patients subjected to aortic valvular replacement. The onset of extracorporeal circulation was considered to mimic the conditions of massive blood transfusion. The patients receiving heparinized fresh blood required more oxygen in the respirator than those receiving stored blood, but still had a significantly lower arterial oxygen tension during the first and second postoperative days. The fresh blood patients also showed a tendency to require a larger amount of d

Contributions to the biology of the hypoxic liver. Note II. Histologic, electron microscopic and biochemical aspects in the course of open heart surgery under extracorporeal circulation.

A study was carried out on the evolution of histological and ultrastructural lesions of liver fragments harvested at different time intervals in the course of extracorporeal circulation in 62 patients operated for acquired and congenital heart disease, as well as that of serologic tests, pre-, intra- and postoperatively up to seven days. Morphologically, it is only the ultrastructural examination that detects the accentuation of preexisting hypoxic lesions within the framework of a state of "controlled shock", noting especially accentuated dilation of the endoplasmic reticulum, lysosomal activ

[Transfusion management of a patient with multiple anti-erythrocyte antibodies (anti-c + anti-K + anti-Fyb + anti-Jkb) subjected to a valvular reoperation using extracorporeal circulation].

A woman with multiple anti-red cell antibodies (anti-c, anti-K, anti-Jkb, anti-Fyb) due to previous transfusions was admitted to the hospital for valvular replacement. To ensure enough transfusional supply, a high number of packed red cell units was typed and screened prior to previous operation. Haemorrhagic complications developing during surgery led to transfusion of 32 blood units, 20 of them compatible. Besides this, measures directed to prevent complications derived from haemolytic disease were instituted. These included intravenous fluids and diuretics to maintain circulatory integrity

Disruption of the Circulating Proteome in Neonates Receiving Extracorporeal Membrane Oxygenation Following Congenital Heart Disease Surgery: A Nested Case-Control Study.

This nested case-control study identified broad dysregulation of the circulating proteome in neonates receiving postoperative extracorporeal membrane oxygenation support after congenital heart disease surgery, including differential responses in those not surviving to hospital discharge. Tissue hypoxia and mitochondrial-associated proteins may represent novel candidate biomarkers for poor extracorporeal membrane oxygenation outcomes.

Circulating CD4+CD7- lymphocyte burden and rapidity of response: predictors of outcome in the treatment of Sézary syndrome and erythrodermic mycosis fungoides with extracorporeal photopheresis.

BACKGROUND: Extracorporeal photopheresis (ECP) is an effective treatment for cutaneous T-cell lymphoma. Controversy has arisen regarding its ability to improve survival rates in Sézary syndrome (SS). We describe our experience with ECP in the treatment of SS and erythrodermic mycosis fungoides, with particular emphasis on early predictors of long-term outcome. OBSERVATIONS: We included 17 patients (15 with SS and 2 with erythrodermic mycosis fungoides) who received ECP as initial treatment. Four of these patients were moribund on presentation (Eastern Cooperative Oncology Group Performance St

[The influence of arterial partial pressure of CO2 and of the arterial value of pH on the oxygen consumption of the whole body during extracorporal circulation (author's transl)].

134 patients with coronary heart disease, with defects of the cardiac values or with inborn heart diseases were studied referring to the influences of the arterial partial pressure of CO2 and the arterial value of pH on oxygen consumption of the whole body during total hypothermic extracorporal circulation. Oxygen consumption of the whole body increases during respiratory or metabolic alkalosis whereas respiratory or metabolic acidosis decreases oxygen consumption significantly. These typical changes of oxygen consumption of the whole body during extraorporal circulation due to alkalosis or ac
