Donor finger morbidity in cross-finger flaps.

We reviewed 16 patients who had cross-finger flap procedures carried out between 1991 and 1996 at the West Midlands Regional centre for Plastic and Reconstructive Surgery. We looked specifically at the donor finger morbidity of each patient with a median follow up interval of 43 months. At follow up there were 10 patients with cold intolerance, eight patients with subjective joint stiffness and a documented reduced range of finger joint movement. Skin graft reconstruction of the secondary defect was associated with poor colour match in eight cases (seven hyperpigmented, one hypopigmented) and v

Little finger abduction and adduction testing in ulnar nerve lesions.

We aimed to evaluate the abduction and adduction of the little finger based on a new clinical test in the context of ulnar nerve lesions. We tested little finger abduction and adduction in 34 patients with an isolated ulnar nerve injury and 20 patients with cubital tunnel syndrome. With their forearms supinated, patients were asked to fully abduct their little finger. Then, the examiner held the patients' index, middle, and ring fingers in extension and maximal radial deviation, and then asked the patients to touch their little finger to their radially deviated ring finger. In patients with ul

Diagnosis of vascular injuries caused by hand-transmitted vibration.

OBJECTIVE: For a reliable objective diagnosis of vascular injuries in hand-arm vibration syndrome (HAVS), the standardized cold provocation tests--finger skin temperature measurement during hand(s) immersion in cold water (FST test) and finger systolic blood pressure measurement during local cold exposure (FSBP test)--are widely used. In recent years there is a growing controversy regarding the diagnostic value of these tests. The aim of this study was to describe particularly the diagnostic performance of FST and FSBP tests, and also to focus on the problems and uncertainties regarding the te

[Injuries and overuse syndromes due to rock climbing on artificial walls].

The purpose of this study was to obtain an overview of acute injuries and chronic overuse syndromes due to rock climbing on artificial climbing walls. Strategies for prevention should then be developed based on these experiences. From October 1995 to December 1996 314 climbers of both sexes and all degrees of climbing abilities were individually interviewed at five different indoor climbing arenas with the help of a special questionnaire. Type, quantity and cause of typical injuries and overuse syndromes on artificial climbing walls were determined. Injuries (n = 204) as well as overuse syndro

Percutaneous A1 pulley release: a cadaveric study.

Percutaneous A1 pulley release was performed on 17 fresh-frozen cadaveric hands with a 14-gauge angiocath needle. Each hand was then explored to assess the adequacy of release and the degree of injury to adjacent structures. Complete release of the A1 pulley was obtained in 45 of the 66 fingers and in 10 of the 17 thumbs. Significant injury to the flexor tendons was observed in two digits. All tendon injuries occurred along the line of the fibers. There were no digital nerve injuries. The release was within 2 mm of a thumb digital nerve in seven hands and the little finger ulnar digital nerve

Robot-related injuries in the workplace: An analysis of OSHA Severe Injury Reports.

Industrial robots are increasingly commonplace, but research on prototypical accidents and injuries has been sparse, hindering evidence-based safety strategies. Using Severe Injury Reports (SIRs) from the U.S. Occupational Safety and Health Administration (OSHA), we identified 77 robot-related accidents from 2015-2022. Of these, 54 involved stationary robots, resulting in 66 injuries, mainly finger amputations and fractures to the head and torso. Mobile robots caused 23 accidents, leading to 27 injuries, mainly fractures to the legs and feet. A two-stage deductive-inductive thematic analysis w

[Injury of the extensor mechanism in the zone I - mallet deformity].

Mallet finger is deformity of the extensor mechanism in the zone I, which is frequently encountered at surgical outpatientęs - departments. Active extension of distal phalanx is impossible. These injuries ranging from disruption of the extensor mechanism alone without skin disruption, with skin disruption to those that have articular involvement and subluxation of distal interphalangeal joint. The management of mallet finger injuries depends on injury pattern and includes splinting, closed reduction, percutaneous pinning with Kirschner wire, open reduction and internal fixation of extensor mec

Traumatized index finger pollicization for thumb reconstruction.

PURPOSE: When the index finger is injured or severed in conjunction with a traumatic amputation of the thumb, transfer of the injured index finger can restore the important function of the thumb. The purpose of this study is to evaluate the results of the transfer of an injured index finger for traumatic loss of the thumb. METHODS: Seven patients treated by pedicled transfer of a traumatized index finger after amputation to the ipsilateral thumb were reviewed retrospectively. Postoperative evaluations included thumb range of motion, opposition and pinch function, grasp and pinch strength, sen

Epidemiology of Flexor Tendon Injuries of the Hand in a Northern Finnish Population.

BACKGROUND: Flexor tendon injuries cause significant morbidity in working-age population. The epidemiology of these injuries in adult population is not well known. The aim of this study was to describe the epidemiology of flexor tendon injuries in a Northern Finnish population. MATERIAL AND METHODS: Data on flexor tendon injuries, from 2004 to 2010, were retrieved from patient records from four hospitals, which offer surgical repair of the flexor tendon injuries in a well-defined area in Northern Finland. The incidence of flexor tendon injury as well as the gender-specific incidence rates was

Differences in the Outcomes of Early Active Mobilization Following Flexor Tendon Repair Between Zone 1 and Distal Zone 2 Injuries.

PURPOSE: Complete release of the A4 pulley is recommended for flexor tendon repair to allow smooth tendon excursion, provided the proximal sheath is mostly intact. However, it is unclear whether the active range of motion of finger joints differs when the flexor digitorum profundus tendon is repaired at different levels with A4 pulley release. We evaluated differences in the outcomes of flexor tendon repair of zone 1 and zone 2A injuries using a six-strand suture (the Yoshizu #1 technique) and complete release of the A4 pulley, followed by an early active mobilization protocol. METHODS: This

Transfer of innervated latissimus dorsi free musculocutaneous flap for the restoration of finger flexion.

Transfer of functioning free muscle for the restoration of finger flexion is an uncommon procedure. We present our experience with five patients with severe forearm injuries in whom a latissimus dorsi musculocutaneous free tissue transfer was performed in an attempt to provide soft tissue coverage and active digital flexion. Four patients had active finger flexion with volitional control of the transferred muscle between the tenth and the fifteenth weeks. On average, active flexion lags were 2.0 cm in the index finger, 2.1 cm in the long finger, 2.3 cm in the ring finger, and 1.4 cm in the sma

Absence of flexor digitorum profundus muscle and variation of flexor digitorum superficialis muscle in a little finger: two case reports.

PURPOSE: Knowledge of rare variants of the FDP is of high clinical importance for physicians examining patients for tendon lacerations and especially for hand surgeons operating tendon injuries. METHODS: During routine dissection at our Department of Anatomy both cases were observed. RESULTS: Variations of flexor digitorum superficialis and flexor digitorum profundus muscles of the little finger were observed in two cadavers. In both cases, the flexor digitorum profundus muscle for the little finger was absent. Moreover, in the first case, the flexor digitorum superficialis muscle for the li

Endoscopic Retrograde Approach for Trigger Finger Release: A Cadaver Study.

Trigger finger is one of the most common causes of disability and pain in the hand. Current surgical techniques for trigger finger release fall short in that they are performed blindly with trauma to, or require incision of, the palmar fascia, which can be a source of significant and long-lasting morbidity. Retrograde endoscopic release of the A1 pulley was performed through a single incision at the proximal digital crease in cadaveric specimens. The fingers were then dissected to assess for completeness of release and inspected for injury to nearby structures. Complete release of the A1 pulle

Long-Term Outcomes After Toe-To-Thumb Transfers for Burn Reconstruction in Children.

Children are one of the most vulnerable populations to burns, and hands are frequently burned anatomical structures. Restoring function in a severely burned pediatric hand is challenging. We present our experience with pediatric toe-to-thumb transfers for burn reconstruction. A retrospective review was conducted of all pediatric toe-to-thumb transfer patients between 2009 and 2014. Children younger than the age of 18 who underwent secondary reconstruction after electrical or thermal burn injuries with at least a 5-year follow-up were included. Functional outcomes were measured with the modifie

Jersey finger type IV: a case report.

Avulsion of the flexor digitorum profundus, usually known as jersey or rugby finger, is a rare condition that can only be treated surgically. It is mostly reported in sports injuries, in which the diagnosis is easily made. It is less frequent in household accidents, but should not be ignored because good results are time-dependent. Type IV jersey finger is the combination a fracture of the volar base of the distal phalanx and avulsion of the flexor digitorum profundus. This is a rare variety, and the surgical approach has not been discussed in detail. We introduce here a case report describing

Factors influencing final range of motion in the fingers after fractures of the hand.

We evaluated 123 patients in which we treated 150 fractured fingers primarily. Sixty-seven percent of these fractured fingers required open reduction with internal fixation; the remainder had closed reduction with internal or external fixation, or immediate arthrodeses. Active and passive range-of-motion (ROM) exercises were usually begun within 3 days after injury. The active and passive ROM of all joints of each finger in the involved hand were measured at regular intervals until no further change in ROM occurred. An associated crush injury, a flexor or extensor tendon injury, or a skin loss
