Megaureter.

Obstructive megaureter is shown by ureteral dilatation most frequently due to an obstacle at the level of ureterovesical junction. The obstacle is caused by a stenotic tract and/or the absence of peristalsis. The diagnosis of primary obstructive megaureter is usually prenatal. The pediatric surgeon together with the pediatric radiologist and nephrologist should pose the indication for surgery. The main criteria on which this approach should be indicated are the grade of megaureter and the renal function. Megaureters of low-mid grade should be followed by a "wait and see" approach because they

Facial nerve function and hearing preservation acoustic tumor surgery: does the approach matter?

OBJECTIVE: The retrosigmoid and middle fossa approaches to acoustic tumor excision allow for hearing preservation but differ in the angle of approach to the facial nerve. The authors comparatively examined facial nerve results of each approach. STUDY DESIGN: Case series with chart review. SETTING: Multiple physician subspecialty practice. SUBJECTS AND METHODS: The authors reviewed facial nerve outcomes of patients undergoing acoustic tumor excision at a single subspecialty practice that had used a hearing preservation approach for the past 15 years. The retrosigmoid and middle fossa approac

Reduction of high-grade isthmic and dysplastic spondylolisthesis in 5 adolescents.

Treatment of high-grade isthmic and dysplastic spondylolisthesis in children and adolescents remains a challenge. Surgical treatment of spondylolisthesis has been recommended in adolescents with pain refractory to nonoperative modalities, slippage progression, or > 50% slippage on presentation. Controversy exists as to the optimal surgical approach for high-grade spondylolisthesis. In this report, we describe 5 cases of high-grade isthmic and dysplastic spondylolisthesis in adolescents and review the literature on surgical treatment for this entity. Operative records, charts, x-rays, and Scoli

Muscle strength recovery and its effects on outcome after open reduction and internal fixation of acetabular fractures.

OBJECTIVE: To determine the relationship between muscle strength recovery and functional outcome in patients who were treated for an acetabular fracture via an anterior approach. DESIGN: Case series. SETTING: University Medical Center. PATIENTS: Fifteen patients with a displaced acetabular fracture treated via an anterior, ilioinguinal approach were studied. Hip muscle strength of these patients was then compared with the muscle strength of a similar group of patients treated via a Kocher-Langenbeck approach (posterior approach). These patient groups were then combined and muscle strength r

Surgical treatment of high-grade spondylolisthesis.

High-grade spondylolisthesis is defined as Meyerding grade 3, 4, or 5 with an increased L5-SI kyphosis/slip angle. Modern-day treatment of high-grade spondylolisthesis usually involves some reduction of the spondylolisthesis along with pedicle screw instrumentation and circumferential fusion at L5-S1, achieved entirely through a posterior approach or through separate anterior and posterior approaches. Complications include implant failure distally, nonunion, and footdrop.

Achieving 360° fusion in high-grade spondylolisthesis using HMA screws.

Circumferential (360°) fusion yields better long-term outcome in patients with high-grade spondylolisthesis. This result may be achieved by combined anterior and posterior techniques or by an all-posterior approach. As the anterior approach is fraught with numerous complications, many surgeons prefer the all-posterior approach to achieve circumferential fusion. We have described in this chapter the technique of using HMA screws to achieve posterior interbody fusion in situ, to be combined with posterolateral fusion and pedicle screw instrumentation to provide the 360° fusion in high-grade spon

Paramedian supracerebellar transtentorial approach for Yaşargil T2 tentorial incisura meningiomas.

OBJECTIVE: Tentorial incisura meningiomas, particularly those arising from the middle incisural region (Yaşargil T2), are surgically challenging because of their deep location and compression of critical neurovascular structures. These lesions typically have supratentorial or infratentorial extension but can also extend across both compartments. Although approach selection is often guided by the dominant compartment of tumor extension, for lesions with supratentorial-dominant extension, the optimal approach remains controversial, and a standardized strategy has not been established. Multimodal Approach for Treatment of Hemorrhaged Mesencephalic Arteriovenous Malformation.

Brainstem arteriovenous malformations (AVMs) are complex and life-threatening lesions. In our video (Video 1), we illustrate resection of a posterior midbrain hemorrhaged AVM (Spetzler-Martin grade III, Lawton-Young grade III, Supplementary Spetzler-Martin grade 6) in a 55-year-old woman presenting with ventricular hemorrhage and coma. Multimodal therapy with endovascular embolization of an associated prenidal aneurysm of the right posteromedial choroidal artery, followed by microsurgical resection via supracerebellar-infratentorial approach, is demonstrated. A total resection of the AVM was a

Thymic carcinoma: state of the art review.

Thymic carcinoma is a rare neoplasm with distinct clinical and pathological characteristics. The prognosis is often poor with an aggressive course that belies its numerical rarity. Potentially prognostic factors for survival include histopathologic grade, clinical stage, and resectability of the tumor. Five-year survival rates for all patients are approximately 30-50%, with a significant survival time differential between low-grade and high-grade neoplasms. Owing to the paucity of cases, optimal management of thymic carcinoma has yet to be defined. At present, a multimodality approach involvin
