[Diagnosis of Alzheimer's disease for general practitioners: screening instruments and early symptoms].

Despite the increasing prevalence and the potential benefits of early identification, dementias continue to be under-diagnosed. By some estimates, fewer than half of all dementia patients have been diagnosed. AD is particularly under-diagnosed in primary care settings perhaps because patients with early AD may appear entirely appropriate in the context of a brief office visit. This, coupled with increasingly abbreviated office visits and the lack of routine use of mental status exams, make identification of cognitive deficit challenging in the primary care setting. Detection of these patients

[Professional burn-out of general practitioners in urban areas: prevalence and determinants].

A telephone survey was carried out in 2007 to evaluate prevalence of burnout in private GPs in South-eastern France and its determinants. Doctors were selected at random, and this list was stratified according to gender, age and size of the place of practice. Burnout was evaluated with the French validated version of the Maslach Burnout Inventory. 511 GPs participated (96,4%). The prevalence of the complete burnout syndrome was 1% (CI 95% [0,3-2,3]). Dealing with long working hours on a weekly basis and high-levels of mental strain, managing palliative care, unrealistic patients' expectations,

General Practitioners involvement in enteral tube feeding at home: a qualitative study.

BACKGROUND: Complex medical treatment is moving from hospital to primary care and General Practitioners (GPs) are increasingly asked to undertake new roles. There are now an estimated 19,500 patients being fed in the UK in the community on enteral tube feeding using a variety of different feeding tubes (Percutaneous endoscopic gastrostomy (PEG), Jejunostomy, or nasogastric (NG). The majority of patients are over the age of 65 years when they had artificial feeding initiated and mainly because of dysphagia. The aim of this study was to explore GPs knowledge, attitudes and skills relating to ent

Views of general practitioners and benzodiazepine users on benzodiazepines: a qualitative analysis.

Effectively assisting benzodiazepine users to cease use requires a greater understanding of general practitioners' (GPs) and benzodiazepine users' views on using and ceasing benzodiazepines. This paper reports the findings from a qualitative study that examined the views of 28 GPs and 23 benzodiazepine users (BUs) in Cairns, Australia. A semi-structured interview was conducted with all participants and the information gained was analysed using the Consensual Qualitative Research Approach, which allowed comparisons to be made between the views of the two groups of interviewees. There was common

Variation in requests for imaging investigations by general practitioners: a multilevel analysis.

OBJECTIVES: To describe the variation in the numbers of imaging investigations requested by general practitioners (GPs) and to find likely explanations for this variation. METHODS: Cross-sectional survey of the use of eight imaging investigations by 229 GPs collaborating in 40 local GP groups from five regions in the Netherlands during 1997. A multivariable, multilevel regression analysis was used to link these data with survey data on professional characteristics such as knowledge about and attitude towards test ordering, and with data on contextual factors such as practice type or experienc

General practitioners' management of lumbosacral radicular syndrome compared with a clinical guideline.

OBJECTIVE: To investigate the current treatment policy of general practitioners (GPs) in patients with a lumbosacral radicular syndrome (LRS) compared with their clinical guideline. DESIGN: A cross sectional survey. METHODS: Sixty-three GPs completed questionnaires about their treatment policy in individual LRS patients at baseline and at six months follow-up. Simultaneously, 136 LRS patients of these GPs were interviewed at baseline, and at three and six month's follow-up. RESULTS: Of the 12 recommendations in the guideline related to history taking, four were not adhered to by the GPs in

Population need and geographical access to general practitioners in rural New Zealand.

AIMS: To use a geographical information system (GIS) approach to demonstrate the extent to which different areas in New Zealand vary in their geographical access to GPs, and to analyse the extent to which spatial access varies in relation to different population groups. METHODS: Three methods; population/GP ratios, least cost path analysis (LCPA), and an allocation method (which considered the capacity constraint of GPs) were used to demonstrate differences in geographic accessibility to GPs. Travel time, and distance to the closest GP, was calculated for every census enumeration district in

[Summary of The Dutch College of General Practitioners' 'Irritable bowel syndrome' standard].

The diagnosis of 'irritable bowel syndrome' can usually already be established after history-taking and a limited physical examination, particularly if the symptoms have persisted for many years in the same form. Additional investigations are indicated in the case of diarrhoea which lasts longer than 2 weeks, especially if the patient is also severely unwell or has rectal bleeding; further investigations are also required if older patients experience the symptoms for the first time, if there is blood loss upon defecation, if unintentional weight loss occurs or if a mass is felt on the left sid

Undergraduate ophthalmology education survey of New Zealand ophthalmologists, general practitioners and optometrists.

AIM: To determine what New Zealand ophthalmologists, general practitioners and optometrists consider important ophthalmic topic areas requiring emphasis in the medical undergraduate curriculum. METHOD: A total of 793 questionnaires related to the content and teaching of undergraduate ophthalmology were sent to ophthalmologists, general practitioners and optometrists. Results were analysed separately for the three respondent groups and as a whole. RESULTS: Four hundred and fourteen questionnaires were returned (52% return rate). Overall responses of the three participant groups were similar a

Magnetic resonance imaging of the lumbar spine: direct access for general practitioners.

At the Cardiff Royal Infirmary we have offered general practitioners (GPs) direct access for magnetic resonance imaging (MRI) of the lumbar spine for sciatica or suspected spinal claudication since January 1993. We compared referrals for MRI from GPs and hospital outpatient doctors, and assessed GP patient management following the scan report. No difference in the diagnostic rates for disc herniation and spinal stenosis were found. GP direct access shortens investigation time, potentially reduces waiting lists, and allows GPs to make more informed management decisions.

[Cancer prevention by general practitioners in the Rhône area: a declarative survey].

A telephone questionnaire on primary prevention and cancer screening practices of general practitioners in the Rhône was administered to a sample of 191 general practitioners, 129 of which had been randomly sampled. Questions concerned primary prevention (tobacco, alcohol, diet, exposure to sunlight, occupation) and cancer screening (uterine cervix, breast, colo-rectum, prostate, other cancers). In addition, physicians were also interviewed on difficulties encountered in their practice, and about their initial and further medical training in this field. The response rate was high (85%). The views of general practitioners on community carrier screening for cystic fibrosis.

BACKGROUND: Recent developments in molecular genetics have made it possible to identify carriers of the cystic fibrosis (CF) mutation, regardless of family history, before they have an affected child. Using these techniques, population or 'community' carrier screening can offer informed reproductive choice to individuals and couples who would not otherwise know of their risk of having a CF child. AIM: This study set out to assess the views of general practitioners (GPs) on community carrier screening for CF and to consider the factors that influence their willingness to offer it themselves.

The delivery of interim discharge summaries to general practitioners by the elderly.

Two hundred and four patients discharged from an acute geriatric unit were given an interim discharge summary for delivery to their General Practitioner (GP). All but three summaries were received by the practices. Information regarding mode and timing of receipt was available in 191 patients. The summaries were received in a median of 1.0 day in both those living alone and those living with family; 75% of summaries had been received within two days of discharge and 93% within four days. General practitioners collected the summaries from patients' homes in a high proportion of both groups. Int

Encouraging attendance at screening mammography: knowledge, attitudes and intentions of general practitioners.

We surveyed a random sample of 200 general practitioners from the Sydney metropolitan area about their attitudes to and knowledge of screening mammography. General practitioners' knowledge about some aspects of screening mammography was found to be limited; only 25% of the sample knew that the risk of breast cancer increases with age and only 30% of the sample knew that the evidence for a reduction in mortality as a result of mammographic screening is weakest for women of less than 50 years of age. Ninety-five per cent of general practitioners were of the opinion that breast self-examination a

[Prevalence and incidence of migraine in general practice. A survey of general practitioners in North Rhine-Westphalia, Germany].

BACKGROUND: Headaches are one of the most common symptoms worldwide. The majority of patients suffer from migraine. The prevalence of migraine is about 14% in the general population in industrialized countries. Its impact on primary care is high. Migraine is impairing and therefore a frequent and acute reason for consultation in primary care. Previous studies have attempted to derive the prevalence in the general population from a representative sample. Despite the importance of migraine, there is no data on its prevalence in general practitioner's (GP) practice. METHOD: This survey study of

Respiratory tract infections in general practice: considerable differences in prescribing habits between general practitioners in Denmark and Spain.

OBJECTIVE: The prevalence of antibiotic resistance in a country reflects the local consumption of antibiotics. The majority of antibiotics are prescribed in general practice and most prescriptions are attributable to treatment of respiratory tract infections (RTIs). The aim of this study was to compare general practitioners' (GPs') prescribing of antibiotics for respiratory tract infections in a country with a high prevalence of antibiotic resistance (Spain) with a country with a low prevalence of antibiotic resistance (Denmark). METHODS: A group of GPs in Copenhagen and Barcelona registered

[The sleep apnea syndrome: diagnosis and management in general practice. A descriptive survey of 579 French general practitioners].

General practitioners (GP's) should be the first to recognise the sleep apnoea/hypopnoea syndrome (SAHS) and could play a major role in the follow up of patients treated with continuous positive airways pressure (CPAP) at home. We have carried out a prospective study in order to evaluate the knowledge and diagnostic and therapeutic practice of GP's in relation to SAHS. 579 GP's agreed to participate in the study by means of a telephone questionnaire. Although daytime somnolence and the presence of apnoeas and snoring were the most frequently quoted clinical symptoms suggesting the diagnosis of
