Choroidal Structural and Vascular Changes in Early Primary Angle Closure Glaucoma.

PRÉCIS: Thicker CT and higher CVI in the 1-12 mm circumferential annular area were noted in early PACG patients compared with healthy controls. More importantly, CVI may serve as a more sensitive indicator for disease progression. PURPOSE: To investigate changes of choroidal thickness (CT) and choroidal vascularity index (CVI) in eyes with early primary angle closure glaucoma (PACG) based on swept source optical coherence tomography angiography (SS-OCTA). METHODS: A total of 86 early PACG patients (86 eyes) and 79 healthy individuals (79 eyes) were enrolled. All subjects underwent 12×12 mm m

Dynamic brain states and molecular signatures in primary angle-closure glaucoma.

BACKGROUND: Primary angle-closure glaucoma (PACG) has traditionally been regarded as an ocular disorder, but accumulating evidence suggests broader central nervous system involvement. Although previous neuroimaging studies have identified static functional abnormalities, the dynamic properties of large-scale brain networks and their associated molecular signatures in PACG remain insufficiently understood. METHODS: We applied Leading Eigenvector Dynamics Analysis to resting-state functional MRI data from 44 patients with PACG and 57 healthy controls to characterize recurrent whole-brain dynami

Acute Angle-Closure Glaucoma Secondary to Vitreous Prolapse Following Ocular Trauma.

Secondary acute angle-closure glaucoma (AACG) caused by vitreous prolapse in a phakic patient is exceedingly rare. We report a case of AACG secondary to vitreous prolapse, occurring 2 weeks after orbital trauma in a 34-year-old male healthcare worker. The patient presented with acute left eye pain, nausea, and vision loss after performing cardiopulmonary resuscitation (CPR) at his workplace. Two weeks earlier, he sustained a nonoperative orbital floor fracture and commotio retinae from a motor vehicle accident. Examination revealed decreased vision and an intraocular pressure (IOP) of 59 mmHg.

Efficacy of Micropulse Laser Cyclophotocoagulation Therapy in Primary Angle Closure Glaucoma.

PRÉCIS: In primary angle closure glaucoma (PACG), micropulse cyclophotocoagulation resulted in a significant reduction in intraocular pressure (IOP), and the number of antiglaucoma medications without any sight-threatening complications. However, the IOP lowering efficacy decreased over time. PURPOSE: The purpose of this study was to evaluate the efficacy and safety of micropulse trans-scleral cyclophotocoagulation (MP-TSCPC) in PACG. PATIENTS AND METHODS: Thirty-three phakic PACG eyes with patent laser peripheral iridotomy and IOP uncontrolled on maximum tolerable antiglaucoma medications (

Brain Functional Network Analysis of Patients with Primary Angle-Closure Glaucoma.

OBJECTIVES: Recent resting-state functional magnetic resonance imaging (fMRI) studies have focused on glaucoma-related neuronal degeneration in structural and spontaneous functional brain activity. However, there are limited studies regarding the differences in the topological organization of the functional brain network in patients with glaucoma. In this study, we aimed to assess both potential alterations and the network efficiency in the functional brain networks of patients with primary angle-closure glaucoma (PACG). METHODS: We applied resting-state fMRI data to construct the functional

Multimodal etiology of drug induced angle closure with topical glaucoma therapy.

PURPOSE: We present a case of acute onset of bilateral choroidal effusions leading to angle closure glaucoma attributed to multiple mechanism of actions causing ciliary body and aqueous flow disruption in the setting of topical glaucoma therapy with latanoprost, brimonidine 0.2%, and Brinzolamide 0.1%. OBSERVATION: The patient presented with ocular hypertension in the setting of bilateral choroidal effusions, leading to angle closure without pupillary block. After cessation of the glaucoma drops and starting steroids and cycloplegics, the patient's symptoms resolved. CONCLUSIONS AND IMPORTAN

Topiramate-induced acute bilateral angle closure glaucoma confirmed by ultrasound biomicroscopy.

Topiramate is a sulfamate-substituted monosaccharide used in the treatment of epilepsy and migraines. This case report demonstrates a 29-year-old female who complained of severe bilateral loss of vision 14 days after using topiramate for her migraine. On initial examination, visual acuities were counting fingers in both eyes. Intraocular pressures were 55 mmHg and 58 mmHg in the right and left eyes, respectively. Anterior segment examination showed bilateral conjunctival chemosis, mild corneal edema, and markedly shallow anterior chambers with closed angles on gonioscopy. Ultrasound biomicrosc

Ocular Biometry in Primary Angle-Closure Glaucoma Associated with Retinitis Pigmentosa.

BACKGROUND: Retinitis pigmentosa (RP) comprises a group of inherited disorders in which patients typically lose night vision in adolescence and then lose peripheral vision in young adulthood before eventually losing central vision later in life. A retrospective case-control study was performed to evaluate differences in ocular biometric parameters in primary angle-closure glaucoma (PACG) patients with and without concomitant RP to determine whether a relationship exists between PACG and RP. METHODS: We used ultrasound biomicroscopy (UBM) to measure anterior chamber depth (ACD). A-scan biometr

Acute angle-closure glaucoma in retinopathy of prematurity following pupil dilation.

BACKGROUND: Pupil dilation is a known risk factor for acute angle-closure glaucoma. Regular retinal evaluation is necessary for retinopathy of prematurity (ROP) cases. An acute attack of angle-closure glaucoma following pupil dilation in regressed ROP has never been reported. CASE PRESENTATION: A five-year-old girl presented to the hospital for a routine retina check-up. The patient was born prematurely with a gestation age of 27 weeks and a body weight of 980 grams. She had a history of stage 4A ROP in the right eye and received scleral buckling. After pupil dilation with 1 % tropicamide and

Oseltamivir (Tamiflu)-induced bilateral acute angle closure glaucoma and transient myopia.

A 27-year-old woman developed bilateral acute angle closure glaucoma (AACG) and transient myopia after taking oseltamivir for four days. On the fourth day, she received systemic and topical intraocular pressure (IOP)-lowering agents, and IOP decreased in both eyes. However, her visual acuity was unchanged. A myopic shift of -5.25 D OD and -5.0 D OS was estimated to have occurred in the acute phase. A-scan ultrasonography and Pentacam showed markedly shallow anterior chambers and increased lens thickness. Ultrasound biomicroscopy revealed an annular ciliochoroidal effusion with forward displace

Central corneal thickness in Chinese subjects with primary angle closure glaucoma.

PURPOSE: To characterize central corneal thickness (CCT) in Chinese eyes with primary angle closure glaucoma (PACG). METHODS: CCT was measured by ultrasound pachymetry in PACG eyes and compared with primary open-angle glaucoma (POAG) eyes. POAG eyes were further subdivided into high-tension glaucoma (HTG) and normal-tension glaucoma (NTG) for analysis. PACG eyes had glaucomatous optic neuropathy and visual field loss, an angle in which at least 180 degrees of angle in which the posterior trabecular meshwork was not visible, and raised intraocular pressure (IOP) and/or peripheral anterior syne

Secondary angle-closure glaucoma due to posterior synechiae formation following phacovitrectomy.

Iris posterior synechiae formation after phacovitrectomy is usually regarded as a trivial problem. We report a case of secondary angle-closure glaucoma due to 360°PS 5 years after a phacovitrectomy for recurrent vitreous haemorrhage secondary to diabetic retinopathy. To our knowledge this is the first such reported case of this potentially sight-threatening complication of phacovitrectomy. We discuss our successful management of our patient, as well as potential risk factors and preventative measures.

Painless transient monocular loss of vision resulting from angle-closure glaucoma.

The main symptoms of acute primary angle-closure glaucoma (PACG) include pain in the head and/or the eyes in addition to the characteristic halo and blurred vision. When loss of vision is accompanied by such pain, PACG is usually suspected. However, no information has been available regarding painless acute angle-closure glaucoma. Recently, we treated a patient with painless acute PACG mimicking amaurosis fugax as a type of transient ischemic attack. Generally, amaurosis fugax causes transient monocular loss of vision due to occlusion of the ophthalmic artery. The formation of microemboli deri
