A Modified Arthroscopic Technique of Gracilis Tendon Graft with Double Fixation System for Treatment of Type V Acromioclavicular Injuries: A Case Report.

CASE: A 48-year-old male patient with a type V acromioclavicular injury with a 3-tendon acute cuff tear, anterior glenohumeral dislocation, and an axillary posttraumatic neuropathy is presented. The rotator cuff tear was sutured and an all-arthroscopic-modified coracoclavicular ligaments (CCLs) reconstruction technique was performed with a gracilis tendon graft and a double knotless suture fixation system. CONCLUSIONS: An arthroscopic approach allows the surgeon to identify and treat associated glenohumeral lesions in type V acromioclavicular dislocations. In addition, the modified CCL recons

Evaluating Thigh Muscle Volume and Compensatory Muscle Changes in Bone-Patellar Tendon-Bone, Hamstring Autograft, and Allograft Anterior Cruciate Ligament Reconstructions: A Minimum 5-Year Follow-up.

BACKGROUND: Graft selection in anterior cruciate ligament reconstruction (ACLR) may lead to long-term, graft-specific muscle adaptations, yet their effect on functional outcomes remains poorly understood. Magnetic resonance imaging (MRI)-based 3D modeling enables precise quantification of muscle volume changes, offering insight into the relationship between postoperative muscle preservation and patient-reported outcomes. PURPOSE: To radiologically evaluate thigh muscle volume changes and patient-reported outcomes at minimum 5-year follow-up among patients after primary anterior cruciate ligam

Stimulated gracilis neosphincter: a new procedure for anal incontinence.

BACKGROUND: The gracilis muscle has been used previously to construct an anal neosphincter, but this was not successful since a short-lived muscle contraction was insufficient to restore continence. Recently, a procedure was described in which conversion to a fatigue-resistant muscle was achieved by chronic low frequency electrical stimulation, and the resultant ability to sustain a constant contraction was associated with improved continence. Our initial results with this procedure, using a standardized operation and treatment protocol in 12 consecutive patients, is reported. METHODS: Seven

Reconstruction of isolated hypoplasia of the patella by a modified Galeazzi procedure: a case report.

BACKGROUND: Isolated Patellar Aplasia Hypoplasia is a very rare autosomal dominant disorder. Its treatment depends on the clinical manifestations that can vary widely. The lack of active extension, which can be responsible for frequent falls due to a knee instability, is the most frequent and disabling manifestation. We report an original technique that is a modification of the Galeazzi technique for recurrent dislocation of the patella to gain active extension in case of PTLAH. CASE REPORT: A 7-year-old Caucasian boy with isolated Patellar Aplasia Hypoplasia and an extension lag of the right

Sternohyoid flap for facial reanimation: a comprehensive preclinical evaluation of a novel technique.

IMPORTANCE: Neuromuscular reanimation of the face provides the correct specific neural functional input and thereby prevents synkinesis. Unfortunately, this ideal situation is rarely encountered in the clinical setting. OBJECTIVES: To assess the technical feasibility of and define the surgical procedure for harvesting the sternohyoid muscle as a novel free flap for use in facial reanimation indications. DESIGN, SETTING, AND PARTICIPANTS: Fresh, postmortem, nonfixed cadavers were used to define the anatomy and perform the flap harvest procedures. Twenty-four flap harvests were performed. Angi

[The diagnostic value of magnetic resonance imaging on neuromuscular diseases].

The article concentrated on the study of skeletal muscles of four extremities in 12 cases of different kinds of neuromuscular diseases and 4 volunteers with MRI. The results revealed: MRI could clearly display individual muscle, muscle groups or abnormal muscles morphologically. It could also demonstrate the difference between the neurogenic and myogenic changes. In patients with progressive muscular dystrophy (PMD), the gracilis m. and sartorius m. were relatively intact. In patients with primary myopathies, the severity of MRI changes was consistent with the severity of the illness and the E

[Participation of the beta-receptor system in the genesis of the carotid aortic baroreceptor reflex in the dog (author's transl)].

The effects of propranolol, given by close arterial injection, on the reflex vasodilatation induced in the atropinized isolated gracilis muscle by rapid i.v. administration of norepinephrine were studied in dogs. The integrated areas of vasodilatation were significantly reduced after treatment with propranolol. Thus the conclusion was that adrenergic beta-receptors participate in the reflex vasodilatation, although how they act needs further elucidation.

Gracilis Flap Repair for Reoperative Rectovaginal Fistula.

BACKGROUND: Surgical treatment of recurrent rectovaginal fistulas is notoriously difficult. Placement of the gracilis muscle between the vagina and anus is an advanced technique used to close persistent fistulas. We have utilized this procedure for recalcitrant fistulas and hypothesized that a gracilis interposition would offer a good treatment option for patients with refractory rectovaginal fistulas, regardless of underlying etiology. OBJECTIVE: The purpose of this study was to investigate healing rates of gracilis interposition in patients with refractory rectovaginal fistulas. DESIGN: Fo

Free functioning gracilis transfer for traumatic brachial plexus injuries in children.

PURPOSE: To report our technique and experience with use of free functioning muscle transfer (FFMT) in reconstruction of traumatic brachial plexus injuries (BPIs) in children as well as its complications and outcomes. METHODS: Twelve patients with complete BPI underwent FFMT for reconstruction between 2000 and 2012. Eight had single-stage gracilis transfer for restoration of elbow flexion, and 4 children had double free gracilis muscle transfer for restoration of elbow flexion and prehension. Mean duration of follow-up was 27 months (range, 14-55 mo). RESULTS: Eleven out of 12 patients achie

Management of 18 difficult vesicovaginal and urethrovaginal fistulas with modified Ingelman-Sundberg and Martius operations.

Our experience in the management of difficult vesicovaginal and urethrovaginal fistulas is presented. The fistulas were secondary to radiation damage and extensive local fibrosis owing to previous attempts to repair surgically. Satisfactory surgical repair of the fistulas was obtained by interposition of viable gracilis muscle and labial fibrofatty tissue at the repair site. The patients have been followed for 1 to 2 years postoperatively.

MRI patterns of thigh muscle involvement in immune-mediated necrotizing myopathy and dermatomyositis.

BACKGROUND: Immune-mediated necrotizing myopathy (IMNM) and dermatomyositis (DM) are characterized by weakness, hyperCKemia, associated autoantibodies, and varying extramuscular manifestations. Muscle MRI, currently subordinate to histopathology and serology in idiopathic inflammatory myopathy (IIM) classification, has an evolving role. Our study aims to define thigh muscle MRI involvement in IMNM and DM by direct comparison. METHODS: This single-center, retrospective, cross-sectional study included 25 participants, who met IIM classification criteria (14 IMNM, 11 DM) and had available thigh

[Surgical treatment of sternoclavicular joint instability with tenodesis].

BACKGROUND: Instability of the sternoclavicular joint is a very uncommon disorder of the shoulder girdle. Acute traumatic dislocations are commonly treated nonoperatively. But severe displacement or chronic instability with recurrent symptomatic subluxation may require surgical intervention. We present our results with open reduction and internal fixation through an autologous gracilis tendon transplant or fiber tape in 8 patients treated surgically. The operative stabilisation of the sternoclavicular joint reduces pain level and improves function of the shoulder. This technique provides an ef

Single-stage dynamic reanimation of the smile in irreversible facial paralysis by free functional muscle transfer.

Unilateral facial paralysis is a common disease that is associated with significant functional, aesthetic and psychological issues. Though idiopathic facial paralysis (Bell's palsy) is the most common diagnosis, patients can also present with a history of physical trauma, infectious disease, tumor, or iatrogenic facial paralysis. Early repair within one year of injury can be achieved by direct nerve repair, cross-face nerve grafting or regional nerve transfer. It is due to muscle atrophy that in long lasting facial paralysis complex reconstructive methods have to be applied. Instead of one sin

Technical considerations in two-stage functioning free muscle transplantation reconstruction of both flexor and extensor functions of the forearm.

We report on two patients with severe injuries of the forearm who were reconstructed using functioning free muscle transplantation (FFMT) for individual replacement of flexor and extensor function. In both cases a two-stage procedure was performed: The extensor reconstruction preceded the flexor reconstruction by 4-6 months. The extensor digitorum communis and flexor digitorum profundus were successfully reconstructed in both cases using bilateral gracilis FFMT. In one case the flexor pollicis longus and extensor pollicis longus were also reconstructed using the adductor longus in addition to

Bilateral gracilis neosphincter construction for treatment of faecal incontinence.

Neosphincter formation with gracilis muscle is used for faecal incontinence refractory to conservative measures and after failed sphincter repair. In this study both gracilis muscles were used to create a neosphincter to determine whether this provides superior physiological and clinical results. Ten patients of median age 39 (range 18-73) years were treated. The mean resting and squeeze pressures before operation were 16 (range 0-40) and 44 (range 0-68) cmH2O respectively. The operation was covered by a defunctioning loop left iliac fossa colostomy. Nine of the ten patients have had the stoma

[In vitro caffeine test on skeletal muscle samples from swine].

Excitable porcine gracilis muscle samples were subjected to an in vitro caffeine test. Muscle samples were taken from pig carcasses or as biopsy from anaesthetized and halothane tested pigs. The caffeine threshold of the muscle was determined, which was the caffeine concentration of the bath that produced a contracture of the muscle sample. The in vitro caffeine test is used in human medicine to diagnose malignant hyperthermia susceptibility. In this study, the caffeine test was adapted to the particular demands of skeletal muscle samples taken from pig carcasses. The used method is described

Indications and outcomes of free tissue transfer to the lower extremity in children: review.

Thirty-four free tissue transfers for reconstruction of various tissue defects to the lower extremities were performed in 32 children over a 20-year period (1980 to 1999). Patient ages ranged from 3 to 19 years (mean: 10.8 years). Four donor sites were used: gracilis muscle, latissimus dorsi muscle, iliac crest osteocutaneous, and vascularized fibula. Fourteen patients had tissue defects related to trauma: nine received a gracilis flap; five received a latissimus dorsi flap. Five patients had tissue defects related to malignant tumor resection: three patients with Ewing's sarcoma received free

Relationship of muscle morphology to hip displacement in cerebral palsy: a pilot study investigating changes intrinsic to the sarcomere.

BACKGROUND: Cerebral palsy (CP) is the most common cause of childhood disability, typified by a static encephalopathy with peripheral musculoskeletal manifestations-most commonly related to spasticity-that are progressive with age. Hip displacement is one of the most common manifestations, observed to lead to painful degenerative arthritis over time. Despite the key role that spasticity-related adductor muscle contractures are thought to play in the development of hip displacement in CP, basic science research in this area to date has been limited. This study was initiated to correlate hip add
