Klebsiella pneumoniae resistant to third-generation cephalosporins in five African and two Vietnamese major towns: multiclonal population structure with two major international clonal groups, CG15 and CG258.

The molecular epidemiology of third-generation cephalosporin-resistant (3GC-R) Klebsiella pneumoniae in developing countries is poorly documented. From February 2007 to March 2008, we collected 135 3GC-R K. pneumoniae isolates from seven major towns in Maghreb (Morocco), West Africa (Senegal, Côte d'Ivoire), Central Africa (Cameroon), East Africa (Madagascar) and Southeast Asia (Vietnam). Their genetic diversity, assessed by multilocus sequence typing, was high (60 sequence types), reflecting multiclonality. However, two major clonal groups, CG15 (n = 23, 17% of isolates) and CG258 (n = 18, 13

Cysteine-to-serine mutants dramatically reorder the active site of human ABO(H) blood group B glycosyltransferase without affecting activity: structural insights into cooperative substrate binding.

A common feature in the structures of GT-A-fold-type glycosyltransferases is a mobile polypeptide loop that has been observed to participate in substrate recognition and enclose the active site upon substrate binding. This is the case for the human ABO(H) blood group B glycosyltransferase GTB, where amino acid residues 177-195 display significantly higher levels of disorder in the unliganded state than in the fully liganded state. Structural studies of mutant enzymes GTB/C80S/C196S and GTB/C80S/C196S/C209S at resolutions ranging from 1.93 to 1.40 A display the opposite trend, where the unligan

Structural diversity and supramolecular aggregation in calcium, strontium, and barium salicylates incorporating 1,10-phenanthroline and 4,4'-bipyridine: probing the softer side of group 2 metal ions with pyridinic ligands.

Group 2 metal complexes [Ca(SA)2(phen)]n (1), [Sr2(SA)4(phen)4] (2), and [Ba(SA)2(phen)2]n (3) (SA = salicylate) have been obtained by the addition of 1,10-phenanthroline (phen) to the corresponding metal salicylates, while the bipyridine derivatives {[Ca3(SA)6(H2O)4](4bpy)2}n (4), {[Sr(SA)2(H2O)3](4bpy)1.5(H2O)}n (5), and {[Ba(SA)2(H2O)3]-(4bpy)1.5(H2O)}n (6) have been synthesized starting from the respective metal carbonates, salicylic acid (SA-H), and 4,4'-bipyridine (4bpy). The new compounds have been characterized by elemental analysis, pH measurements, thermal analysis, and spectroscopic

Structuring Digital Mental Health Care Navigation: Co-Design Nominal Group Technique Study to Develop the MChart Definition and Typology of the Characteristics of Digital Mental Health Care Navigation Tools.

BACKGROUND: Australia's mental health care system has been characterized by complexity and fragmentation, as highlighted by numerous reports, commissions, and inquiries. In response, digital mental health care navigation tools have emerged as a promising solution to help individuals locate appropriate mental health services. The rapid proliferation of these tools-without a clear understanding of their definitions and characteristics-risks creating confusion rather than clarity for users. Terms such as "navigation" and "navigators" are often used interchangeably, further complicating the landsc

Impact of the volume-based procurement for intraocular lenses combined with the diagnosis-related group's payment reform on hospitalization costs and expenditure structure among cataract patients: an interrupted time series analysis.

BACKGROUND: Cataract surgery is one of the most resource-intensive procedures in China, with intraocular lens (IOL) costs accounting for a substantial proportion of total hospitalization expenditure. To address persistently high prices of high-value medical consumables, our hospital implemented the Beijing-Tianjin-Hebei "3 + N" regional alliance procurement for IOLs in January 2022, followed by DRG payment reform in January 2024. However, the synergistic effects of these two sequential policies on cataract surgery costs-particularly on both cost levels and expenditure composition-have not been

Phosphorus-nitrogen compounds part 27. Syntheses, structural characterizations, antimicrobial and cytotoxic activities, and DNA interactions of new phosphazenes bearing secondary amino and pendant (4-fluorobenzyl)spiro groups.

A number of partly (7-9) and fully (10a-12d, Scheme 1) substituted mono(4-fluorobenzyl)spiro cyclotriphosphazenes was prepared. The structures of the compounds were determined by MS, FTIR, 1D and 2D NMR techniques. The crystal structures of 9, 11b and 12b were verified by X-ray diffraction analysis. In vitro cytotoxic activity of the phosphazenes (10a-12d) against HeLa cervical cancer cell lines was evaluated. Compound 12c was found to be the most effective, as it is a cytotoxic reagent that might activate apoptosis by altering mitochondrial membrane potential. Compounds 10b, 11b and 12b showe

Congenital syphilis as a failure of the maternal-fetal prevention cascade: Clinical implications for pregnant migrant and structurally vulnerable women in high-income settings - an ESCMID study group for infections in Travellers and migrants (ESGITM) perspective.

BACKGROUND: Congenital syphilis remains a preventable but persistent cause of fetal loss, neonatal morbidity, and early-life complications. In high-income settings, its occurrence rarely reflects the absence of diagnostic tools or effective therapy; but more often exposes missed opportunities across antenatal care, sexually transmitted infection prevention, maternal treatment, partner management, and neonatal follow-up. This review investigates congenital syphilis from the viewpoint of the possible failures of the maternal-fetal prevention pathway, with a specific focus on pregnant migrant and

[Basic structural features of a cardiac telemedicine center for patients with heart failure and implanted devices, cardiac arrhythmias, and increased risk of sudden cardiac death : Recommendations of the working group 33 Telemonitoring of the German Cardiac Society].

Heart failure is one of the most common diseases. It is associated with high morbidity and mortality. Since heart failure is age-associated, the number of patients with heart failure is constantly increasing. At the same time, the imbalance between the need for treatment and the provision of care is growing. Telemonitoring/telemedicine offers patients in rural or remote areas access to high-quality health care and enables fast access to specialists. The working group 33 Telemonitoring of the German Cardiac Society describes the characteristics and possible applications of telemonitoring/teleme
