[Usefulness of transrectal ultrasonography in the diagnosis of anomalies of intersexual conditions].

INTRODUCTION: Between the external genital exploration and the internal genital direct vision, through laparoscopia, there is a black point which is the pelvic floor; we have to know whether there is a vagina or not, what it is like and where it arrives at. The reason for this research is to present our experience with the transrectal ecography, which allows us a very good exploration of the pelvic floor. MATERIAL AND METHODS: We present 6 patients, the youngest is 16 months old and the oldest is 19 years old, who have a diagnostic of 3 congenital adrenal hyperplasia (HSC), 2 gonadal dysgenes

Novel management of vaginal chronic graft-versus-host disease causing haematometra and haematocolpos.

Genital chronic graft-versus-host disease (GVHD) in women posthaematopoietic cell transplantation may cause vaginal mucosal sclerosis. Human papillomavirus (HPV) reactivation, also common post-transplantation, limits local immunosuppressive, but not oestrogen treatment. A 36-year-old nulliparous woman developed coincidental genital chronic GVHD and HPV 22 months after transplant for aplastic anaemia. Topical immunosuppression for GVHD led to an eruption of warts successfully treated with laser surgery and cone biopsy. She maintained normal ovarian function and used extended cycle combined horm

Spontaneous uterine rupture secondary to recurrent haematometra from cervical stenosis.

Cervical stenosis is a challenging condition that often recurs despite intervention. Multiple therapeutic options have been described, but a clearly effective and reliable treatment method has yet to be identified. Patients with recurrent stenosis are at risk of developing severe complications such as chronic pelvic pain and infertility. We describe a case of congenital cervical stenosis with secondary haematometra in which repeated cervical dilatation, hysteroscopic canalisation and administration of medications to retard endometrial development were unsuccessful in relieving the obstruction

Fluid detection in the uterus during and after irradiation for carcinoma of the cervix--clinical implications.

The records of 16 patients with an obstructed, fluid-filled uterus due to carcinoma of the uterus or to its treatment by radiation therapy were analysed. In 12 uteri the presence of malignant tumor was simultaneously established, e.g. primary cervical carcinoma (1), recurrence of cervical (4), endometrial Stage II or III carcinoma (2), second primary tumors, MMT (2), and endometrial carcinoma (3). The uterine fluid consisted of blood (8), pus (3) or was serous (3). Twice the fluid could not be analysed. In our series the prognosis of patients with recurrent cervical cancer or a second primary

Characterising acute gynaecological pathology with ultrasound: an overview and case examples.

This article summarises the ultrasound features of a number of gynaecological emergencies: ectopic pregnancy, haemorrhagic corpus luteum, twisted adnexa, pelvic inflammatory disease, acute myoma necrosis, haematocolpos and haematometra. The basis of all diagnosis in women with acute gynaecological conditions is history and clinical examination. An ultrasound examination should only be performed if it is likely to provide information that would change the likelihood of the diagnosis suspected on the basis of clinical data. If ultrasound findings are abnormal, then it is important to thoroughly

Incidental diagnosis of genital tuberculosis in a prepubertal girl presenting with acute urinary retention: a case report.

BACKGROUND: Genital tuberculosis affecting prepubertal girls is extremely uncommon, posing a great diagnostic challenge. It is a form of extrapulmonary tuberculosis usually seen in the reproductive age group. It can lead to significant health problems, including infertility, pelvic pain, menstrual irregularities in women, or amenorrhea. Genital tuberculosis is even rarer when coupled with an imperforate hymen, thus presenting a rare and diagnostically challenging presentation. Imperforate hymen is an uncommon obstructive congenital abnormality of the female reproductive tract with an incidence

Long-term gynecological outcome of patients with persistent cloaca.

PURPOSE: Persistent cloaca is a complex malformation that remains a difficult reconstructive challenge, and data on long-term outcome are scarce. Gynecological abnormalities are common with cloaca but may remain asymptomatic until puberty or adult life. We evaluate long-term gynecological sequelae in these patients with persistent cloaca. MATERIALS AND METHODS: The records and radiographs of postpubertal patients (mean age 16.8 years, range 10 to 32) treated for cloacal malformation at 1 institution from 1970 to 2001 were retrospectively reviewed. Outcome data at puberty were available in 41

Surgical techniques of uterine evacuation in first- and second-trimester abortion.

Induced abortion is an ancient procedure. Vacuum curettage is a recent innovation and is demonstrably superior to other methods for first-trimester abortions. Patient selection, patient preparation and the necessary instruments are described. The only absolute contraindications for local anaesthesia, vacuum curettage abortions are pregnancies too far advanced and allergy to local anaesthestics. The only mandatory laboratory tests are Rh blood group and cervical culture for gonorrhoea. Rh-negative patients must receive anti-D (Rh0) immunoglobulin. Perioperative antibiotics are of proven benefit

Complications of hysteroscopy.

Most of the complications of hysteroscopy are avoidable and, fortunately, rare. With improved training, experience, and technology, most of these complications should become extinct. There will always be some unavoidable complications as well as difficulties resulting from inexperience. A goal for the future is to teach operating room personnel how to recognize and treat these complications to ensure the best patient outcome possible. Once gynecologic surgeons recognize the safety and efficacy of diagnostic and operative hysteroscopy as a minimally invasive option to treat benign uterine patho
