Pneumopericardium associated with high-frequency jet ventilation during laser surgery of the hypopharynx in a child.

A case of pneumopericardium, without concomitant pneumothorax, resulting from high frequency jet ventilation during laser surgery of the hypopharynx is described. The patient, a four-year-old boy, suffered cardiac arrest as a consequence of the incident. The pneumopericardium was likely to have been caused by obstruction to the expiratory flow during surgery. Other possible explanations for the incident are discussed.

[The effect of high frequency jet ventilation on intracranial pressure in the patients with severe head injury].

The effect of high frequency jet ventilation (HFJV) on intracranial pressure (ICP) in the series of seven patients with severe head injury was studied. These patients received mechanical ventilation for respiratory failure accompanied with neural damage or circulatory shock and for ICP control by decreasing PaCO2. ICP was measured continuously by subarachnoid catheter method connected to Gould P-50 strain-gage transducer and controlled by hyperventilation and/or osmotic diuretics during acute phase. Arterial pressure, central venous pressure and intratracheal pressure were measured continuousl

Volume Targeted Ventilation and High Frequency Ventilation as the Primary Modes of Respiratory Support for ELBW Babies: What Does the Evidence Say?

Respiratory management of the extremely low birth weight (ELBW) newborn has evolved over time. Although non-invasive ventilation is being increasingly used for respiratory support in these ELBW infants, invasive ventilation still remains the primary mode in this population. Current ventilators are microprocessor driven and have revolutionized the respiratory support for these neonates synchronizing the baby's breath to ventilator breaths. High frequency ventilators with the delivery of tidal volumes less than the dead space have been introduced to minimize barotrauma and chronic lung disease.

High-frequency jet ventilation using the Arndt bronchial blocker for refractory hypoxemia during one-lung ventilation in a myasthenic patient with asthma.

A novel method in the management of refractory severe hypoxemia during one-lung ventilation (OLV) in a patient who presented with myasthenia gravis, asthma, a symptomatic mediastinal mass, hiatal hernia, and a moderate pericardial effusion is presented. The patient was scheduled for excision of a large anterior mediastinal mass and creation of a pericardial window through a left thoracotomy. One-lung ventilation was achieved using an Arndt bronchial blocker. High-frequency jet ventilation (HFJV) was applied to the surgical nondependent lung through the lumen of the Arndt endobronchial blocker

Efficiency of Alveolar Ventilation During High-Frequency Modes in Piglets: Effects of Airway and Tissue Heterogeneity.

BACKGROUND: Oscillatory high-frequency ventilation modalities, including high-frequency oscillatory, percussive, and jet ventilation, are commonly used during anesthesia and critical care to improve oxygenation or facilitate airway surgery. These techniques rely on rapid, small-amplitude pressure oscillations to sustain gas exchange while minimizing baro- or volutrauma. However, how effectively such oscillations transmit through the conducting airways to the alveoli in mechanically heterogeneous lungs remains poorly understood, particularly under perioperative conditions associated with atelec

High-Frequency Oscillation vs Mechanical Ventilation for Neonatal Acute Respiratory Distress Syndrome: A Randomized Clinical Trial.

IMPORTANCE: Key clinical features of neonatal acute respiratory distress syndrome (NARDS) are broadly comparable to those observed in pediatric and adult ARDS; however, evidence is insufficient to recommend high-frequency oscillatory ventilation (HFOV) or conventional mechanical ventilation (CMV) as the preferred first-line therapy. OBJECTIVE: To evaluate whether HFOV is superior to CMV in reducing bronchopulmonary dysplasia (BPD) and other neonatal adverse outcomes, including death, among preterm infants (≤34 weeks' gestational age) with NARDS. DESIGN, SETTING, AND PARTICIPANTS: This single

Feasibility of an alternative, physiologic, individualized open-lung approach to high-frequency oscillatory ventilation in children.

BACKGROUND: High-frequency oscillatory ventilation (HFOV) is a common but unproven management strategy in paediatric critical care. Oscillator settings have been traditionally guided by patient age and/or weight rather than by lung mechanics, thereby potentially negating any beneficial effects. We have adopted an open-lung HFOV strategy based on a corner frequency approach using an initial incremental-decremental mean airway pressure titration manoeuvre, a high frequency (8-15 Hz), and high power to initially target a proximal pressure amplitude (∆Pproximal) of 70-90 cm H2O, irrespective of ag

Imposed Work of Breathing During High-Frequency Oscillatory Ventilation in Spontaneously Breathing Neonatal and Pediatric Models.

BACKGROUND: High-frequency oscillatory ventilation (HFOV) is used in cases of neonatal and pediatric acute respiratory failure, sometimes even as the primary ventilatory mode. Allowing patients (at least neonates) on HFOV to breathe spontaneously soon after intubation has been shown to be feasible, and this is becoming a more generally used approach for infants and small children. However, such an approach may increase the imposed work of breathing (WOB), raising the question of whether the imposed WOB varies with the use of newer-generation HFOV devices, which operate according to different f

Efficacy of exogenous surfactant during conventional and high-frequency oscillatory ventilation in preterm infants with RDS.

INTRODUCTION: The immediate effects of exogenous surfactant on lung volume and hemodynamics in preterm infants have been poorly studied. MATERIALS AND METHODS: Lung volume, SpO2, perfusion index, regional SO2, transcutaneous partial pressure of CO2 were simultaneously monitored and recorded ten minutes after surfactant tracheal instillation in elective HFOV ventilated preterm infants with RDS. OUTCOMES: Despite a reduction in gas exchange during the first 5 minutes post surfactant instillation, most likely dependent on airway obstruction, lung volume increases rapidly in HFOV preterm infants

High-frequency jet ventilation improves gas exchange in extremely immature infants with evolving chronic lung disease.

Extremely preterm infants often develop chronic lung disease (CLD) characterized by heterogeneous aeration; poorly supported, floppy airways; and air trapping. High-frequency jet ventilation (HFJV) with high end-expiratory pressure (optimal lung volume strategy [OLVS]) may improve airway patency, lead to better gas distribution, improve gas exchange, and facilitate extubation. In a pilot trial, this study sought to explore the effect of HFJV on oxygenation, ventilation, and ease of extubation in preterm infants with evolving CLD and refractory respiratory failure (RRF). From September 2002 to

Mercury inhalation poisoning and acute lung injury in a child. Use of high-frequency oscillatory ventilation.

Acute mercury inhalation poisoning is a rare cause of acute lung injury. It is commonly fatal in the young child because of progressive pulmonary failure. We describe a 3-month-old infant who survived this insult with the use of high-frequency oscillatory ventilation. This form of support may be an option in the patient with severe adult respiratory distress syndrome (ARDS) or air leak syndromes.

High-frequency jet ventilation in children with the adult respiratory distress syndrome complicated by pulmonary barotrauma.

High-frequency jet ventilation (HFJV) was used in 29 children with severe ARDS complicated by pulmonary barotrauma (PBT). Treatment with HFJV was begun when PBT was progressing over a 24-h period while receiving conventional ventilation (CV). The mean (+/- SD) age was 0.95 +/- 1.21 years (range, 0.03-4 years). The most common diagnosis was viral pneumonia (n = 17); other diagnoses included aspiration pneumonitis (n = 4), bacterial pneumonia (n = 3), multiple trauma (n = 2), and near-drowning (n = 3). The Bunnell Life-Pulse ventilator was used at a rate of 240/min or 300/min, with inspiratory t

Modeling Early Oxygenation Trajectory in PARDS From High-Frequency Mechanical Ventilation Signals Using Deep Sequence Architectures.

OBJECTIVE: PARDS causes substantial morbidity in PICU. We investigated whether high-frequency ventilator waveforms from first hour of invasive ventilation contain predictive information for oxygenation trajectory. METHODS: We analyzed mechanically ventilated pediatric patients using engineered statistical features and learned representations from high-frequency ventilator signals, including breath-by-breath flow waveforms. Engineered features included early oxygenation indices and ventilator variables derived from time- and frequency-domain analyses. Learned representations were obtained from

Evaluating tidal volume stability in extremely preterm infants on high-frequency oscillatory ventilation with volume guarantee.

BACKGROUND: High Frequency Oscillatory Ventilation (HFOV) combined with volume guarantee (HFOV-VG) represents an innovative ventilation mode designed for managing respiratory failure in neonates. This study aimed to assess the stability of High Frequency tidal volume (VThf) in extremely preterm infants ventilated on HFOV with VG during the first 48 h of life. Additional objectives included examining the correlations between VThf, Diffusion Coefficient of Carbon Dioxide (DCO2) and key respiratory markers. METHODS: This retrospective, single-center study included 22 extremely preterm infants tr

Effect of high-frequency oscillatory ventilation with intermittent sigh breaths on carbon dioxide levels in neonates.

BACKGROUND: High-frequency oscillatory ventilation (HFOV) minimizes ventilator-induced lung injuries. Spontaneous sigh breathing may augment the functional residual capacity, increase lung compliance, and recruit atelectatic alveoli. PURPOSE: To evaluate the difference in the partial pressure of carbon dioxide (PaCO2) in neonates receiving invasive HFOV as the primary mode of respiratory support before versus after sigh breaths (Sighs). METHODS: This prospective study was conducted between January and December 2023. Intubated preterm and term neonates who underwent HFOV with an available art
