A radiographic study of hip-joint alignment after prosthetic hip arthoplasty.

Postoperative hip alignment was studied on radiographs in cases of total hip arthroplasty (THA) and of Bipolar Head Prosthesis(BHP), both with MX-1. Postoperative anteroposterior-view radiographs of hip joints of patients with a normal hip joint on the unoperated side and without pelvic tilt were used. Thirty-nine THA patients (femoral neck fracture), 26 THA patients (osteonecrosis of the femoral head and osteoarthritis of the hip joint), and 34 BHP patients were selected for this study. Lines and points for measurement of 9 parameters were established on radiographs. The position of the great

Total hip replacement in inflammatory hip disease using the Charnley prosthesis.

The Charnley low-friction arthroplasty was performed on 73 hips in 55 patients with inflammatory joint disease, usually for intolerable pain. Sixty-five of these operations were reviewed at a special clinic or by postal questionnaire; 57 hips were painfree or gave rise to only occasional discomfort, in 58 there was an increased range of movement, and in 46 of the 59 hips assessed clinically, the results were excellent or good. The most serious complication was deep infection, which occurred in 7 operations.

Implant survival after primary total hip arthroplasty due to childhood hip disorders: results from the Danish Hip Arthroplasty Registry.

BACKGROUND: Childhood hip disorders including acetabular dysplasia, congenital hip dislocation, epiphysiolysis, and morbus Calve-Legg-Perthes are well-established risk factors for the development of early osteoarthritis of the hip. These patients have an increased risk of undergoing a total hip arthroplasty (THA) operation early in their life. However, there are very few data on implant survival in such patients. METHODS: We used data from the Danish Hip Arthroplasty Registry to identify patients who had been treated with a primary THA in Denmark between 1995 and 2005. Implant survival during

U-shaped hip capsular incision: An easier way to do hip capsulorrhaphy in developmental dysplasia of the hip - Technical note.

Developmental dysplasia of the hip (DDH) is one of the most common hip anomalies encountered in pediatric patients. Stabilization of the femoral head into the acetabulum is crucial for normal hip joint development. When surgical intervention is decided, open reduction is needed to remove any obstacle that hinders hip reduction. Capsulorraphy is an essential step for minimizing instability of the hip after reduction. The classic T-shaped capsular incision is done by two incisions: vertical limb parallel to femoral neck axis and a transverse one parallel to the inguinal ligament 5mm distally to

Hip Hemi-Arthroplasty vs Total Hip Replacement for Displaced Intra-Capsular Hip Fractures: Retrospective Age and Sex Matched Cohort Study.

INTRODUCTION: The Royal Victoria Hospital in Belfast is the largest volume hospital in the UK Hip Fracture Database. Management of displaced intra-capsular hip fractures is evolving in light of NICE2 and BOA guidelines3, with more patients receiving total hip replacement (THR) over hemi-arthroplasty. With current rationing within the NHS, it is vital that principles of the 'Getting It Right First time' (GIRFT) report4 are implemented and the correct treatment choice made. Our aim was to assess Barthel scores5, complication rate, blood transfusion rate and post op functional ability in two age

Cementless total hip arthroplasty for three different degrees of hip involved secondary to ankylosing spondylitis: an analysis of 195 hips.

BACKGROUND: Hip involved secondary to ankylosis spondylitis (AS) had a huge influence on hip function. Cementless total hip arthroplasty (THA) can improve hip function. However, no previous study compared the outcomes of THA for AS patients with three different degrees of hip involvement. METHODS: The 195 hips were retrospectively analyzed and divided into non-ankylosed group (group A, 94 hips), fibrous ankylosed group (group B, 49 hips), and bony ankylosed group (group C, 52 hips). postoperative range of motion (ROM), harris hip scores (HHS), the short-form 12 health survey (SF-12), length o

Radiographic evaluation of osteoarthritis of the hip: an inter-observer study of 61 hips treated for late-detected developmental hip dislocation.

BACKGROUND AND PURPOSE: Different methods have been used to classify osteoarthritis (OA) of the hip. We evaluated the reliability of different classifications in order to find which grading system is most appropriate for use in clinical practice. PATIENTS AND METHODS: 49 patients (61 affected hips) with late-detected developmental dislocation of the hip (DDH) were studied. The mean age at follow-up was 45 (32-49) years. 3 classifications of OA were compared. The gradings by Kellgren and Lawrence (1957) (K&L) and Croft et al. (1990) are global visual assessments based on osteophytes, cysts, su

Total hip replacement in the congenitally dislocated hip using the Paavilainen technique: 19 hips followed for 1.5-10 years.

BACKGROUND AND PURPOSE: Total hip replacement (THR) in congenitally dislocated hips (CDHs) according to Paavilainen includes placement of the cup in the original acetabulum and an extended trochanteric osteotomy with distal advancement of the trochanter. There have only been a few reports describing the outcome of this technique. Thus, we report the results of 19 THRs using the Paavilainen technique. METHODS: 10 women and 5 men with an average age of 38 (16-73) years at the time of surgery (19 hips) were followed for mean 4.8 (1.5-10) years. The patients were evaluated clinically with the Har

Posterolateral Portal Use During Hip Arthroscopy for Resection of Posterior Hip Cysts.

Hip arthroscopy is a widely accepted, minimally invasive technique for treating various hip joint disorders. The traditional approach typically involves the supine position with traction and an anterolateral entry point. However, this approach presents challenges, including insufficient exposure of posterior hip lesions and an increased risk of nerve injury. In this technical note, we describe a posterolateral portal in the lateral decubitus position, eliminating the need for traction and fluoroscopy. This technique provides direct access to posterior hip lesions and facilitates effective rese

Changes in Fall Risk After Total Hip Arthroplasty for Dysplastic Hip Osteoarthritis.

OBJECTIVE: Total hip arthroplasty (THA) improves function in patients with dysplastic hip osteoarthritis (DHOA). However, its effect on reducing fall risk remains unclear. This study aimed to evaluate fall risk following THA in patients with DHOA. METHODS: This retrospective cohort study included 85 patients who had DHOA and underwent THA between September 2019 and September 2022 and were evaluated as having a preoperative fall risk (Fall Risk Index 5 items [FRI-5] ≥ 6). They were categorized into two groups according to the FRI-5 score 1 year postoperatively. Evaluation parameters included F

Intraoperative Hip Arthrography to Guide Decision-Making in Cerebral Palsy Hip Reconstruction.

BACKGROUND: Hip reconstruction surgery is indicated in nonambulatory children with cerebral palsy (CP). The decision to perform a pelvic osteotomy (PO) with a proximal femoral varus derotational osteotomy (VDRO) is subjective. Our hypothesis was that evaluation of dynamic stability and labral pathoanatomy using intraoperative hip arthrography (IHA) would reduce the performance of concomitant PO without adversely affecting outcomes. The capability of IHA to correctly guide intraoperative decision-making was evaluated using midterm outcomes in children with CP (CwCP) treated with hip reconstruct

Impact of Hip Rotation Angle Following Total Hip Arthroplasty with Leg Lengthening.

Background/Objectives: Few studies report on hip rotation after total hip arthroplasty (THA); however, details of the factors affecting the hip rotation angle are unknown. We aimed to investigate the factors related to hip rotation after THA. Methods: This study included 124 consecutive patients who underwent THA. We retrospectively analyzed the correlation between changes in the rotation angle of the femur relative to the pelvis, global femoral offset, and femoral version and leg lengthening. Moreover, we performed a multivariate regression analysis of these parameters to calculate the effica

Early hip arthroplasty in elderly hip-fracture patients: a perspective from HbA1c.

BACKGROUND: To investigate whether stress-induced hyperglycemia influences postoperative outcomes in hip fracture patients with normal hemoglobin A1c (HbA1c) levels, and to determine if delaying surgery for blood glucose normalization is necessary. METHODS: This multicentre retrospective cohort (August 2017 – November 2022) included 212 patients (≥ 60 years) with femoral-neck or intertrochanteric fractures and admission random glucose ≥ 11.1 mmol/L. Patients were classified as early surgery (< 24 h from admission; median 18 h) or delayed surgery (≥ 24 h; median 38 h) after fasting glucose was

Hip-Spine Relationship in Femoroacetabular Impingement: Does Hip Arthroscopy Affect Pelvic Mobility?

BACKGROUND: Femoroacetabular impingement (FAI) is a well-recognized cause of hip pain in adults. The hip-spine relationship between the femur, pelvis, and lumbosacral spine has garnered recent attention in hip arthroplasty. However, the hip-spine relationship has not been well described in patients with FAI. QUESTIONS/PURPOSES: The goal of this study was to determine whether lumbopelvic mobility is altered after hip arthroscopy. Does lumbopelvic motion, defined as the difference between standing and sitting measurements for sacral slope (SS), pelvic tilt (PT), and pelvic-femoral angle (PFA),

Total Hip Arthroplasty in Ankylosing Spondylitis: A Case Report of Ankylosed Hip.

Ankylosing spondylitis (AS) is a chronic inflammatory arthritic disease that primarily affects the axial skeleton, and its association with the secondary development of osteoarthritis (OA) in peripheral joints, particularly the hips, is increasingly recognized. This case report elucidates the diagnostic and therapeutic challenges encountered in a patient with bilateral hip osteoarthritis secondary to AS. The patient's medical history included AS and a failed attempt at core decompression of the left hip joint. The patient was managed with total hip arthroplasty (THA) on the left side due to pe

Hip passive stiffness is associated with hip kinematics during single-leg squat.

INTRODUCTION: Single-leg squat (SLS) is a test commonly used to assess lower limb function in rehabilitation. Increased hip adduction and internal rotation (IR) is associated with dynamic knee valgus, which is related to hip and knee overload. Proximal and distal factors, such as hip passive stiffness, poor hip muscle strength and excessive foot misalignment may influence hip movement. However, previous studies focus on how proximal and distal factors affected knee joint movement and did not reported the influence on hip joint. OBJECTIVE: This study investigated the association of hip externa

Robotic-Assisted Total Hip Arthroplasty in Patients Who Have Developmental Hip Dysplasia.

INTRODUCTION: Total hip arthroplasty (THA) in the setting of developmental dysplasia of the hip (DDH) presents more inherent complexities than routine primary THA for osteoarthritis. These include acetabular bone deficiency, limb length discrepancy (LLD), and abnormal femoral anteversion. Three-dimensional planning and robot-assisted (RA) bone preparation may simplify these complex procedures and make them more reproducible. The purpose of this study was to evaluate radiographic and clinical outcomes in a cohort of patients who had DDH and underwent an RA THA. MATERIALS AND METHODS: We retros
