The natural history of the patient with an infected total hip replacement.

Deep infection, the most serious local complication of total hip replacement, prompted a study of the records of 135 patients (137 hips) thus afflicted in a nationwide survey of Canada. Particular attention has been paid to the natural history of the infection, and the problems of diagnosis are described. Twenty-one patients died after the insertion, or removal, of the prosthesis, and of the survivors of the original 135 patients only eighteen have been able to retain the prosthesis without further problems with the wound. The remaining patients had the prosthesis removed, and most dry wounds.

Short-term results of the S-ROM-A femoral prosthesis operative strategies for Asian patients with osteoarthritis.

The S-ROM-A femoral prosthesis (DePuy, Warsaw, IN) is a modular cementless femoral stem system that was developed to adapt to hip joint disorders of Asian patients. Clinical results of total hip arthroplasties using the S-ROM-A femoral prosthesis were evaluated in 68 hips. The average age at the time of surgery was 57.1 years. The mean follow-up period was 27.8 months. The Harris hip score improved from 47.1 points preoperatively to 91.4 points at final follow-up. No dislocations were observed. Implant fixation was good without any evidence of osteolysis or loosening. The S-ROM-A femoral prost

Outcome after reinfection following reimplantation hip arthroplasty.

Between 1976 and 1992, reinfection developed in 34 patients treated for an infected total hip arthroplasty with removal of the prosthesis and implantation of another prosthesis. These patients included 15 men and 19 women with a mean age of 62 years. Infection recurred an average of 2.2 years after reimplantation of the new prosthesis. Followup after the diagnosis of reinfection averaged 5.1 years. Reinfection after an attempt at reimplantation total hip arthroplasty was seldom compatible with a good functional outcome. Resection arthroplasty was reliable in eradicating reinfection but led to

Wear and performance of a tripolar total hip replacement.

BACKGROUND: An unconstrained tripolar hip replacement matches a large capacity two-piece metal/polyethylene acetabular component with a bipolar prosthesis. This combination of accepted technology is different than the relatively new dual mobility prosthesis. The goal is to protect against dislocation and allow close to a normal range of motion (ROM). So far there has not been enough information about wear and performance of tripolar hip replacement to support its wide use. METHODS: Twenty-four tripolar prostheses were retrieved from 23 patients after a mean of 14 years (range, 5-21 years). Al

[Total hip replacement in the treatment of severe deformity of the hip caused by advanced ankylosing spondylitis].

OBJECTIVE: To evaluate the clinical results of the total hip replacement in the treatment of severe deformity of the hip joint caused by advanced ankylosing spondylitis. METHODS: Forty-five hips with severely deformities in 33 patients suffered from advanced ankylosing spondylitis treated by total hip replacement were followed-up from 1992 to 2005 in our department. The pain index, deformity angle, range of motion of the joint, changes of weight baring capacity of a single leg, and the postoperative complications were compared and analysed in this study. RESULTS: The pain was complete releas

Dislocation of hip prostheses.

BACKGROUND: In the literature, the dislocation rate for hip prostheses is stated to be 2-3%. Dislocation may have several causes, such as aspects of the patient, the surgeon, the type of prosthesis or the operation. The objective of this study was to identify the frequency, causes and treatment of dislocation in our department. MATERIAL AND METHODS: The material encompasses all patients operated on with a hip prosthesis at the Department of Orthopaedics, Haugesund Hospital, from 1987 through 2011. Information has been collected retrospectively from databases and patient records after an avera

A rare case of primary psoas abscess causing hip pain in a patient with hip replacement.

The number of primary hip replacement procedures performed each year has exponentially increased, with a subsequent increase in associated revision procedures. Prosthetic infection is a serious concern for all arthroplasty surgeons. Psoas abscess is a rare condition with unknown etiology, and diagnosis is often missed or delayed, with subsequent increased morbidity, mortality and even possible inappropriate management. We report a patient presenting with a total hip replacement complaining of sepsis and right hip pain. No focus for the infection was isolated and an open washout of the right hi

[Changes of biomechanical parameters in dysplasia of the hip by total hip replacement].

AIM: Dysplastic hips or dislocated hips lead to loss of muscle strength claudication, and finally to coxarthrosis. This study analytically compares the hip joint forces in normal, dysplastic hips and hips after implantation of a hip prosthesis for several positions. METHOD: The results of 173 total hip replacements in 153 cases of either severe congenital dysplasia or dislocation were analyzed. A 2 dimensional mathematical model of the hip was developed to evaluate the effects of surgically achievable mechanical alterations such as acetabular placement, femoral shaft-prosthetic neck angle, an

Successful treatment of periprosthetic joint infection caused by Granulicatella para-adiacens with prosthesis retention: a case report.

BACKGROUND: Granulicatella and Abiotrophia spp. are difficult to detect due to their complex nutritional requirements. Infections with these organisms are associated with high treatment failure rates. We report the first implant-associated infection caused by Granulicatella para-adiacens, which was cured with anti-microbial treatment consisting of anti-biofilm-active rifampin and debridement, exchange of mobile parts and retention of the prosthesis. CASE PRESENTATION: Patient with a history of left hip arthroplasty presented with acute onset of fever, pain and limited range of motion of the l

The Influence of Pelvic Tilt on the Anteversion Angle of the Acetabular Prosthesis.

The concept of the "safe area" of the acetabular prosthesis has a long history and has been recognized by many scholars. It is generally believed that postoperative hip dislocation rate is low, when the acetabular anteversion angle is placed in the range of 15° ± 10°. Despite this, hip dislocation is a common complication after total hip arthroplasty. In recent years, more and more scholars have paid attention to the influence of pelvic tilt on the acetabular anteversion angle. The concept of acetabular anteversion changes as the pelvic tilt changes, and is challenging the traditional acetabul

[The treatment concept and results of peri-/sub-prosthetic fractures].

The treatment of choice in ipsilateral femoral shaft fractures in patients with hip prosthesis depends on the location of the fracture lines and the observation of loosening of the prosthesis. Either a change of prosthesis, if necessary in combination with refixation of the greater trochanter or simple open reduction with internal fixation (ASIF) along with autologous and/or homologus spongiosa are the used techniques. The rate of complications in peri- or subprothetic fractures is high. Lack of motion in hip and knee joint occurs frequently.

Factors Associated With Complications in 176 Crowe IV Hips Treated With Total Hip Arthroplasty.

OBJECTIVE: Total hip arthroplasty (THA) in Crowe IV hip was a challenge for surgeons, along with some complications. Currently, most evaluations focused on the surgical techniques and radiographs, which were indirectly measured parameters and easily affected by the operators. Objective factors were not considered. This study aimed to investigate objective factors to predict the complications. METHODS: We retrospectively reviewed a series of Crowe IV patients who received THA between July 2010 and December 2019 in our hospital. Demographics and surgical parameters were collected: gender, age,

Anterior thigh pain after cementless total hip arthroplasty.

We compared the incidence of anterior thigh pain retrospectively following primary uncemented total hip replacement in 70 patients (78 hips) with osteoarthritis. In one group (38 hips), an Endoplus porous-coated, canal-filling prosthesis was inserted through an anterolateral approach. In the other group (40 hips), a Furlong hydroxyapatite-coated prosthesis was inserted through a posterior approach. The latter prosthesis has a relatively longer femoral stem and does not achieve diaphyseal press-fit contact. Three patients in the Furlong group had anterior thigh pain, which eventually resolved i

Is Collum Femoris Preserving Stem (CFP) an Epiphyseal-Stabilized Prosthesis? A Long-Term Single-Center Series Follow Up of 705 Cases.

BACKGROUND: The global increase in total hip arthroplasty (THA) has led to widespread use of cementless femoral stems. The Collum Femoris Preserving (CFP) stem, initially designed as an epiphyseal-stabilized prosthesis, aims to preserve proximal bone and reduce stress shielding. However, long-term observations have revealed unexpected proximal bone resorption and distal sclerosis, challenging this classification. This study aims to reassess the fixation pattern and long-term complications of CFP stems to inform clinical decision-making. METHODS: Between 2006 and 2012, 497 patients (705 hips)

Hip joint geometry effects on cartilage contact stresses during a gait cycle.

The cartilage surface geometry of natural human hip joint is commonly regarded as sphere. It has been widely applied in computational simulation and hip joint prosthesis design. Some new geometry models have been developed and the sphere assumption has been questioned recently. The objective of this study was to analyze joint geometry effects on cartilage contact stress distribution and investigate contact patterns during a whole gait cycle. Hip surface was reconstructed from CT data of a healthy volunteer. Three finite element (FE) models of hip joint were developed from different cartilage g

Metal-on-metal total hip resurfacing arthroplasty in the presence of extra-articular deformities or implants.

BACKGROUND: Hip resurfacing has been proposed as an alternative to total hip replacement in patients who have proximal femoral deformities or retained hardware in the proximal aspect of the femur. In these situations, placement of a conventional stemmed hip prosthesis would be difficult or impossible, possibly necessitating a complex osteotomy or a custom prosthesis. The purpose of this study was to evaluate a series of patients who had extra-articular deformities of the proximal aspect of the femur and/or retained hardware and who were managed with a resurfacing hip prosthesis. METHODS: Fift

Massive femoral osteolysis secondary to loosening of a cemented roughened long stem: a case report.

The surface finish of a femoral stem plays an important role in the longevity of cemented total hip arthroplasty. In efforts to decrease the rate of aseptic loosening, some prostheses have been designed to have a roughened surface that enhances bonding between the prosthesis and cement, but clinical outcomes remain controversial. We present a rare case of massive osteolysis with extreme femoral expansion that developed after cemented revision total hip arthroplasty. The destructive changes in the femur were attributable to abnormal motion of the stem and were aggravated by the roughened precoa
