Early management of proximal humeral fractures with hemiarthroplasty: a systematic review.

We performed a comprehensive systematic review of the literature to examine the role of hemiarthroplasty in the early management of fractures of the proximal humerus. In all, 16 studies dealing with 810 hemiarthroplasties in 808 patients with a mean age of 67.7 years (22 to 91) and a mean follow-up of 3.7 years (0.66 to 14) met the inclusion criteria. Most of the fractures were four-part fractures or fracture-dislocations. Several types of prosthesis were used. Early passive movement on the day after surgery and active movement after union of the tuberosities at about six weeks was described i

[Use of angle-stable implants for proximal humeral fractures: prospective study].

PURPOSE OF THE STUDY: To evaluate and compare proximal humeral fractures treated either by plate osteosynthesis with angular-stable screws or by intramedullary nailing, and to define the indications optimal for use of either technique. MATERIAL: The study comprised 97 patients. The proximal humeral internal locking system (PHILOS) plate was used in 49 patients (31 women and 18 men); with age average 57.4 years (women, 64.5 and men, 45.3 years). By the AO classification, 12 patients with type A, 15 with type B, and 22 with type C fractures. The Targon PH nail was used in 48 patients (32 women

Effectiveness of locked nailing for displaced three-part proximal humeral fractures.

BACKGROUND: Published reports about locked nailing for displaced comminuted proximal humeral fractures are few and the results contradictory. METHODS: Locked nailing was used to treat 27 selective patients with displaced three-part proximal humeral fractures. The average age of patients was 54.3 years. The operative indications were persistent severe displacement, intact or minimally displaced lesser tuberosity, tolerance of anesthesia, and adherence to rehabilitation programs. Three patients had associated shoulder dislocation and two had fractures with diaphyseal extension. By transdeltoid

[Treatment outcome after primary hemi-alloarthroplasty in dislocated humeral head fractures].

Humeral head fractures often lead to a high complication rate because of vulnerable vascularization of the humeral head, pretraumatical soft tissue lesions and osteoporotic bone. Compared with open reduction and internal fixation primary hemiarthroplasty seems to be an encouraging alternative treatment in case of dislocated and comminuted fractures. To evaluate our results following primary hemiarthroplasty we examined the outcome of 27 patients treated between November 1993 and May 1997 by primary hemiarthroplasty (15 Neer II, 12 Aequalis) in our institution within two weeks after trauma. The

Surgical results of open reduction and plating of humeral shaft fractures.

Thirty-five patients who sustained humeral shaft fractures were treated by open reduction internal fixation using AO techniques between 1992 and 1997. Open fractures occurred in 8 patients. Primary radial nerve palsy was present in 5 cases. In 16 patients an open fracture or multiple trauma, or both were indications for surgery. Eight osteosynthesis were performed after failed conservative treatment. The complications encountered were non-union (2 cases), osteomyelitis (2 cases), secondary radial nerve palsy (3 cases) and repeat surgery (4 cases). Bony union averaged 5.3 months radiographicall

[The value of combining multiple screwed plates in humeral shaft fractures].

The authors have reviewed 246 fresh fractures of the shaft of the humerus treated by primary plating over a period of 22 years. It has been customary to treat the majority of fractures of the shaft of the humerus by conservative means, with use of open reduction and fixation by a plate for complicated fractures with associated injuries, or in cases of nonunion. Over the period of review the authors have gradually increasing their use of primary plating as opposed to the traditional methods. They fell that the reliability of union, together with the early mobilisation and return of the arm to n

Complications of Seidel intramedullary nailing of narrow diameter humeral diaphyseal fractures.

Forty acute blunt fractures of the humeral diaphysis were treated with Seidel nails (9-mm diameter) between April 1988 and August 1992. Follow up was available for 36 patients. Average Injury Severity Score (ISS) was 22. Forty-eight pelvic and lower extremity fractures necessitated humeral weight bearing in 23 patients. Patients were grouped by canal diameter measured at the point of distal humeral fixation: < or = 9 mm or > or = 10 mm. Data were analyzed with regard to complications, fracture pattern, and time to union. Due to difficulties reaming the humeral canal, five different reaming sys

Proximal humeral fractures. Late results in relation to classification and treatment.

One hundred and eighty-eight proximal humeral fractures were reviewed retrospectively in relation to the Neer classification and the treatment given. The displacement criteria were found to have prognostic significance. Minimally displaced fractures had excellent outcome, whereas the results following any treatment of the displaced fractures were unsatisfactory in about one half of the cases. The indications for closed or open treatment can only be defined by prospective studies, and the methods of treatment should be improved.

Similar Complication Rates for Humeral Shaft Fractures Treated With Humeral Nails Versus Open Reduction and Internal Fixation With Plating.

Humeral shaft fractures account for an estimated 3% of all adult fractures. When operative treatment is indicated, intramedullary (IM) nailing and open reduction and internal fixation with plating are the 2 most common techniques. Early IM nails were associated with poor outcomes, but with advancements in nail design, patient outcomes have been improving with this technique. This study used a nationwide database to quantify and compare the rates of postoperative complications between IM nails and plating for humeral shaft fractures in a national population. An administrative database was queri

Modified use of a proximal humeral internal locking system (PHILOS) plate in extra-articular distal-third diaphyseal humeral fractures.

BACKGROUND: Surgical treatment of extra-articular distal-third diaphyseal humeral fractures is controversial in terms of surgical approach and position of implant. The aim of this study is to evaluate the clinical and radiological outcomes of a modified application of the proximal humeral internal locking system (PHILOS) plate in extra-articular distal-third diaphyseal humeral fractures. MATERIALS AND METHODS: A total of 23 patients with extra-articular distal humerus fractures were treated using either open plating or the minimally invasive plate osteosynthesis (MIPO) technique with upside d

Proximal humeral fractures: non-operative treatment versus intramedullary nailing in 2-, 3- and 4-part fractures.

BACKGROUND: Little is known about non-operative treatment of proximal humerus 4-part and severe displaced fractures as those are normally treated operatively. In this study, we present a historical collective of two level I trauma centers, where all humeral head fractures, despite displaced fractures, received non-operative treatment within a 10 years period. Functional and clinical results of 2-, 3- and 4-part fractures were compared to those after fixed angle intramedullary nailing by matched pair analysis. METHODS: Between 2000 and 2010, 167 patients with 2-, 3- and 4-part humeral head fra

Humeral capitellum fracture combined with humeral lateral column injury: A novel classification system and treatment algorithm.

BACKGROUND: Shear humeral capitellum fracture (CF) treatment can be complicated by comminution of the distal lateral humeral column (LHC). Although treatment with a 3.5 mm posterolateral distal humerus plate with support (PDHPWS) has been proposed, its indications have not yet been outlined. The purpose of this study was to describe a classification system for this fracture pattern and provide a therapeutic algorithm to avoid complications associated with PDHPWS fixation. METHODS: Thirty-four patients who underwent surgical treatment for CF with LHC comminution were enrolled. The humeral capi

Humeral fractures after reverse total shoulder arthroplasty: functional outcomes and complications.

INTRODUCTION: Periprosthetic humeral fracture is one of the major complications of shoulder arthroplasty. Optimal therapeutic management of this complication remains debated across treatment modalities. The aim of this study was to determine the most appropriate strategy by comparing functional outcomes and complications among the different treatments for humeral fractures occurring after reverse total shoulder arthroplasty (rTSA). HYPOTHESES: There were no differences in complication rates or functional outcomes between nonoperative treatment and surgical management (osteosynthesis or prosth

[Research progress on medial support augmentation techniques for proximal humeral fractures].

OBJECTIVE: To conduct a comprehensive analysis of the historical evolution, current progress, and future prospects of medial support augmentation techniques for proximal humeral fractures (PHFs). METHODS: The recent domestic and international research literature on medial support augmentation techniques for PHF was extensively reviewed. Based on a comprehensive of the literature, the review focused on the biomechanical principles of the medial column of the proximal humerus, providing an in-depth critique of the development trajectory, biomechanical efficacy, and clinical outcomes of various

Augmented versus non-augmented locking-plate fixation in proximal humeral fractures.

AIMS: Proximal humeral fractures are the third most common fracture among the elderly. Complications associated with fixation include screw perforation, varus collapse, and avascular necrosis of the humeral head. To address these challenges, various augmentation techniques to increase medial column support have been developed. There are currently no recent studies that definitively establish the superiority of augmented fixation over non-augmented implants in the surgical treatment of proximal humeral fractures. The aim of this systematic review and meta-analysis was to compare the outcomes of

Humeral trochlear morphology does not influence coronoid fractures in elbow dislocation.

PURPOSE: Traumatic elbow dislocation is the second most frequent joint dislocation, even though the elbow is a congruent and stable joint. Individual variability in anatomical congruence of the elbow and how it relates to simple or complex instability has rarely been studied in the literature; we hypothesized that a greater articular coverage by the humeral trochlea would be more likely to result in complex dislocation. The aim of this study is to analyze trochlear morphology in simple (SED) and complex elbow dislocation (CED), to assess whether the degree of humeroulnar joint congruence influ

Reverse Shoulder Arthroplasty for Failed Operative Treatment of Proximal Humeral Fractures.

INTRODUCTION: Failure after operative treatment of complex proximal humeral fractures (PHF) can prove challenging even for experienced surgeons. Reverse shoulder arthroplasty (RSA) seems to offer a satisfactory revision procedure with good clinical outcomes. MATERIALS AND METHODS: We present a case series of 14 patients, who were treated during a 3.5 years period (from 01/2016 until 06/2019). They all underwent revision surgery with RSA for failed operative treatment of PHF. Their mean age was 68 years (range, 51-84 years). 2 patients (14.3%) had been primarily treated with open reduction and
