Tinnitus, sound intolerance, and mental health: the role of long-term occupational noise exposure.

PURPOSE: Whereas chronic noise exposure (CNE) is a known risk factor for tinnitus, little is known about how a history of CNE impacts tinnitus characteristics and its comorbid symptoms. METHODS: Seventy-five participants with chronic tinnitus (59m/16f, 22-78 years, 48 with sensory-neural hearing loss, and 27 with a normal audiogram) including 43 individuals with (Tin-CNE group) and 32 without (Tin group) a history of long-term occupational noise exposure were studied. Tinnitus characteristics were rated by a visual analog scale, and tinnitus comorbid symptoms were scored using self-assessment

Fibromyalgia Patients Are Not Only Hypersensitive to Painful Stimuli But Also to Acoustic Stimuli.

Fibromyalgia is a chronic widespread pain syndrome associated with hypersensitivity to nociceptive stimuli. This increased sensitivity of FM patients has been associated with central sensitization of dorsal horn neurons. Increasing evidence, however, suggests that the mechanisms of FM hypersensitivity not only affect pain but include light, smell, and sound. We hypothesized that supraspinal augmentation of sensory input including sound represent a hallmark of FM. We tested 23 FM patients and 28 healthy controls (HC) for sensory augmentation of nociceptive and non-nociceptive sensations: For as

Misophonia: A scoping review of research.

OBJECTIVE: To scope the literature describing misophonia populations, management, and research opportunities. METHOD: Literature searches for research studies describing patients diagnosed with misophonia, defining a patient profile, or outlining development or testing of an intervention for misophonia. A data extraction form was developed and piloted before data from each article were independently charted by two researchers. Researchers then agreed on a final data set for each article. RESULTS: Thirty-one records were included. The misophonia population was described in terms of onset age,

Should we screen for misophonia in patients with eating disorders? A report of three cases.

In this case report, the authors describe three cases of misophonia in people with eating disorders. Misophonia is a condition where a specific trigger sound provokes an intense emotional reaction in an individual. Case 1 is a 29-year-old with childhood eating issues, anorexia nervosa and bulimia nervosa whose trigger was a high-pitched female voice. Case 2 is a 15-year-old diagnosed with anorexia nervosa after misophonia onset. Her trigger was people chewing and eating noisily. Case 3 is a 24-year-old woman who presented with anorexia nervosa prior to misophonia onset. Her trigger was the cli

Development and evaluation of a procedure for fitting multi-channel compression hearing aids.

Hearing aids with multi-channel compression are often fitted on the basis of loudness scaling data obtained using narrow bands of noise or tones. Here, we report the development and evaluation of an alternative fitting procedure based on the use of speech signals. The parameters of the hearing aid (the gains in each channel for high and low input levels) are adjusted adaptively under computer control on the basis of the listener's responses. The goal is that speech at 85 dB SPL should be judged as 'loud', speech at 60 dB SPL should be judged as 'quiet', and speech at both levels should be judg

A small vestibular schwannoma arising from the inferior vestibular nerve.

The investigation of a complaint of unilateral right sensorineural hearing loss led to the identification of a right 3 mm vestibular schwannoma arising from the inferior division of the right vestibular nerve. On investigation the patient was found to have normal caloric function, this being mediated by the superior vestibular nerve. Both transient and distortion product otoacoustic emissions were absent in the right ear, and ABR was abnormal on this side. These findings are of interest as they indicate that this small lesion produced a hearing loss that was both cochlear and retrocochlear. Th

Using auditory reaction time to measure loudness growth in rats.

Previous studies have demonstrated that auditory reaction time (RT) is a reliable surrogate of loudness perception in humans. Reaction time-intensity (RT-I) functions faithfully recapitulate equal loudness contours in humans while being easier to obtain than equal loudness judgments, especially in animals. In humans, loudness estimation not only depends on sound intensity, but on a variety of other acoustic factors. Stimulus duration and bandwidth are known to impact loudness perception. In addition, the presence of background noise mimics loudness recruitment; loudness growth is rapid near th

The neurophysiological approach to misophonia: Theory and treatment.

Clinical observations of hundreds of patients who exhibited decreased tolerance to sound showed that many of them could not be diagnosed as having hyperacusis when negative reactions to a sound depend only on its physical characteristics. In the majority of these patients, the physical characteristics of bothersome sounds were secondary, and patients were able to tolerate other sounds with levels higher than sounds bothersome for them. The dominant feature determining the presence and strength of negative reactions are specific to a given patient's patterns and meaning of bothersome sounds. Mo

Tinnitus: pharmacological topodiagnosis.

The difficulty of accurately localizing the source of subjective tinnitus is well-known. Anamnesis and traditional audiological tests can often suggest a source if its origin as peripheral or merely central (or both). Therefore, several authors, such as Risey, Denk, and Shulman, recently proposed identifying the source of subjective tinnitus through the evaluation of the responses reported by patients to adequate pharmacological treatments. Our study presents a useful plan to perform tinnitus topodiagnosis, which consists of specific audiological tests evaluating the characteristics of symptom

Migraine and auditory dysfunction: beyond comorbidity.

Migraine operates as a systemic disorder of multisensory sensitization. Within this spectrum, auditory manifestations, including sensorineural hearing loss, tinnitus, and hyperacusis, represent prevalent yet frequently overlooked otologic phenotypes. Epidemiological data show that 15%-49% of migraine patients present with comorbid auditory symptoms, and nearly two-thirds exhibit auditory electrophysiological abnormalities. However, optimal clinical management remains constrained by an incomplete understanding of pathogenic mechanisms, variable audiological assessment protocols, and dispersed t

Stapedial reflexes in normal versus recruiting ears.

The latency and magnitude of the stapedial reflex was examined in a group of normal-hearing Ss (N: 15) and in 15 Ss with unilateral sensorineural impairment exhibiting abnormal loudness growth in the impaired ear. Reflexes were determined at successive 2-db steps from threshold to 16 db above using a 2-kc/s stimulus tone. Latency and magnitude measurements were not statistically different between groups nor between ears within the pathological group. These results substantiate dependence of the stapedial reflex on loudness. Furthermore, there appears to be a dependence of the reflex magnitude

Long-Term Outcomes of an Internet-Based Cognitive Behavior Therapy Intervention for Tinnitus: Follow-Up Analysis of a Nonrandomized Clinical Trial.

IMPORTANCE: Determining the durability of treatment effects extending more that 1 year postintervention is crucial for sustainability of health care services for tinnitus. Tinnitus is often disabling and can result in a considerable economic burden. Identifying tinnitus interventions with stable long-term positive effects can improve health care outcomes. Due to the lack of research focused on outcomes longer than 1 year postintervention, the current study was undertaken. OBJECTIVES: To investigate the durability of treatment effects over a 6-year period following internet-based cognitive beh

The immediate effects of acoustic trauma on excitation and inhibition in the inferior colliculus: A Wiener-kernel analysis.

Noise-induced tinnitus and hyperacusis are thought to correspond to a disrupted balance between excitation and inhibition in the central auditory system. Excitation and inhibition are often studied using pure tones; however, these responses do not reveal inhibition within the excitatory pass band. Therefore, we used a Wiener-kernel analysis, complemented with singular value decomposition (SVD), to investigate the immediate effects of acoustic trauma on excitation and inhibition in the inferior colliculus (IC). Neural responses were recorded from the IC of three anesthetized albino guinea pigs

The Ethics of Translational Audiology.

Translational research moves promising primary research results from the laboratory to practical application. The transition from basic science to clinical research and from clinical research to routine healthcare applications presents many challenges, including ethical. This paper addresses issues in the ethics of translational audiology and discusses the ethical principles that should guide research involving people with hearing loss. Four major ethical principles are defined and explained, which are as follows: beneficence, nonmaleficence, autonomy, and justice. In addition, the authors dis

OHC response recruitment and its correlation with loudness recruitment.

After proper noise exposure, Hensen's cells, which have been shown to follow closely the response characteristics of the outer hair cells, suffered a loss of sensitivity at low and moderate SPLs. The lower the stimulus level, the greater was the loss. When the low-SPL loss did not exceed about 40 dB, input-output functions showed an increased rate of amplitude growth, so that the post-exposure response caught up with its pre-exposure counterpart between 60 and 90 dB SPL, depending on the severity of the loss. These results, together with preceding clinical observations, led us to the conclusio

[Occupational hearing loss as probability diagnosis (author's transl)].

The paper discusses the question of which factors that are to be taken into consideration are of relevance to the expert in making his differential diagnosis in cases of occupational hearing loss. On the basis of the medical literature and earlier investigations the discussion is concerned with the proof of recruitment as well as with the effects of the exposure level and the exposure duration. Special reference is made to the "typical" hearing-threshold curve. The result of the examination with factor-analysis of 14,684 pure tone audiograms suggests that a small involvement of low and medium

Occupational Noise: Auditory and Non-Auditory Consequences.

Occupational noise exposure accounts for approximately 16% of all disabling hearing losses, but the true value and societal costs may be grossly underestimated because current regulations only identify hearing impairments in the workplace if exposures result in audiometric threshold shifts within a limited frequency region. Research over the past several decades indicates that occupational noise exposures can cause other serious auditory deficits such as tinnitus, hyperacusis, extended high-frequency hearing loss, and poor speech perception in noise. Beyond the audiogram, there is growing awar
