Optimizing vaccine uptake in sub-Saharan Africa: a collaborative COVID-19 vaccination campaign in Madagascar using an adaptive approach.

BACKGROUND: The COVID-19 pandemic has highlighted the need for more effective immunization programs, including in limited resource settings. This paper presents outcomes and lessons learnt from a COVID-19 vaccination campaign (VC), which used a tailored adaptive strategy to optimise vaccine uptake in the Boeny region of Madagascar. METHODS: Guided by the Dynamic Sustainability Framework (DSF), the VC implementation was regularly reviewed through multi-sectoral stakeholder feedback, key informant interviews, problem-solving meetings, and weekly monitoring of outcome indicators to identify and

Pilot study assessing the possible benefits of a higher level of implementation of biosecurity measures on farm productivity and health status in Belgian cattle farms.

Over the last few years, the interest of decision-makers and control agencies in biosecurity (BS), aiming at preventing and controlling the introduction and spread of infectious diseases, has considerably increased. Nevertheless, previous studies highlighted a low implementation level of biosecurity measures (BSM), especially in cattle farms; different reasons were identified such as perceived costs, utility, importance, increased workload and lack of knowledge. In order to convince cattle farmers to adopt BSM, it is necessary to gather more information and evidence on their cost-effectiveness

Improving Guideline Adherence in Urology.

CONTEXT: Clinical practice guidelines (CPGs) distil an evidence base into recommendations. CPG adherence is associated with better patient outcomes. However, preparation and dissemination of CPGs are a costly task involving multiple skilled personnel. Furthermore, dissemination alone does not ensure CPG adherence. Reasons for nonadherence are often complex, but understanding practice variations and reasons for nonadherence is key to improving CPG adherence and harmonising clinically appropriate and cost-effective care. OBJECTIVE: To overview approaches to improving guideline adherence, to pro

Growing implementation scientists: NHLBI's K12 programs in heart, lung, blood, and sleep research.

BACKGROUND: Implementation science seeks to accelerate the adoption, integration, and scale-up of evidence-based interventions (EBIs) to improve health and reduce disparities. As the field expanded, demand for trained implementation scientists rapidly outpaced workforce capacity. To address this need, the National Heart, Lung, and Blood Institute (NHLBI) launched the institutional Research Career Development Programs in T4 Implementation Research (K12) Next Generation of Implementation Science (iNexIS) in 2016. The program aimed to expand the national pool of rigorous implementation researcher

Barriers and facilitators at pre-implementation of a type 2 diabetes screening program in dental care using the consolidated framework for implementation research and the theoretical domains framework.

BACKGROUND: Screening and referral for type 2 diabetes mellitus (T2DM) during dental care visits has the potential for expanding preventive care. Using the consolidated framework for implementation research (CFIR) and the theoretical domains framework (TDF), we examined the barriers and facilitators at pre-implementation of a community-driven T2DM screening program in an urban dental clinic serving Alaska Native and American Indian (AN/AI) adults. METHODS: This convergent mixed-methods parallel study was informed by the updated CFIR. Data were collected in 2023 through a 13-item survey of adu

Shedding light on conditions for the successful passive dissemination of recommendations in primary care: a mixed methods study.

BACKGROUND: Passive dissemination of information in healthcare refers to the publication or mailing of newly established guidelines or recommendations. It is one of the least costly knowledge translation activities. This approach is generally considered to be ineffective or to result in only small changes in practice. Recent research, however, suggests that passive dissemination could, under certain conditions, result in modifications of practice, similar to more active dissemination approaches. The objective of our study was to uncover the conditions associated with the change in primary care

Designing complex health system interventions: an integrated theory-informed approach.

Health systems interventions are increasingly designed to address complex challenges in quality, safety, access, and equity. Yet many fail to achieve sustained impact, often due to misalignment between intervention mechanisms, organisational context, and broader system dynamics. Although theory is widely promoted to strengthen intervention design, frameworks from behavioural science, implementation science, and systems thinking are frequently applied in isolation, limiting their collective explanatory and practical value. This focused narrative review adopts a design-first perspective-defined

Adoption of a novel biomarker-guided quality improvement treatment bundle for patients with subclinical acute kidney injury after cardiac surgery: An implementation study.

BACKGROUND: Evidence-based peri-operative practices can deliver improved patient outcomes, but their benefits may be limited by obstacles to implementation into routine practice. A multicentre randomised controlled trial demonstrated a reduction in the incidence of moderate-severe acute kidney injury after cardiac surgery using a biomarker-guided renal care bundle, however these measures are not a routine part of clinical practice. OBJECTIVE: We set out to assess the key implementation domains of acceptability, feasibility and appropriateness of this technology-intervention bundle in the card

Call to action for health systems integration of point-of-care testing to mitigate the transmission and burden of sexually transmitted infections.

OBJECTIVES: In 2016, WHO estimated 376 million new cases of the four main curable STIs: gonorrhoea, chlamydia, trichomoniasis and syphilis. Further, an estimated 290 million women are infected with human papillomavirus. STIs may lead to severe reproductive health sequelae. Low-income and middle-income countries carry the highest global burden of STIs. A large proportion of urogenital and the vast majority of extragenital non-viral STI cases are asymptomatic. Screening key populations and early and accurate diagnosis are important to provide correct treatment and to control the spread of STIs.

Promoting early expression of breast milk in mothers of preterm infants in a neonatal unit: a best practice implementation project.

OBJECTIVES: The aim of this implementation project was to optimize lactation success in mothers of preterm infants by ensuring early (as soon as possible and within six hours) and frequent (eight to 12 times/day) milk expression. INTRODUCTION: Evidence suggests that breast milk improves enteral feeding tolerance and neurodevelopmental outcomes and decreases the risk of necrotising enterocolitis and late onset sepsis in preterm infants. It is widely accepted that breast milk expression should commence as soon as possible and within six hours of delivery. RESULTS: Following implementation of t

Updating the critical steps of the quality implementation framework: an umbrella review of reviews.

BACKGROUND: The Quality Implementation Framework (QIF) is a widely used process model in implementation science (IS). Since its publication in 2012, the field of IS has expanded considerably, yet QIF has never undergone formal revision. Given recent advances and the complexity in implementation research and practice, this study examines whether QIF continues to capture the full scope of implementation quality and its challenges. METHODS: An umbrella review of reviews was conducted on literature published between 2012 and 2025. Eligible articles reported on the development, application, or upd

Translating a Nurse-Led Coping-Sleep-Adherence Bundle Into Routine Cardiovascular Care: A Theory-Informed Implementation Blueprint.

BACKGROUND: Adults with cardiovascular disease frequently experience anxiety, sleep disturbance, and challenges with long-term treatment adherence. These factors are associated with poorer quality of life, reduced engagement in guideline-directed therapy, and increased healthcare utilization. Although nurses are well-positioned to address these needs, nurse-led behavioral interventions often encounter barriers to sustained adoption in routine clinical practice. OBJECTIVE: In this study, we aimed to describe a theory-informed implementation blueprint for integrating a nurse-led coping-sleep-ad

Adapting on-the-go in NYC: Ongoing implementation strategy and cultural adaptations of Problem Management Plus (PM+), a non-clinician delivered mental health intervention.

INTRODUCTION: Mental health service utilization remains disproportionately low among racial and ethnic minority populations, due to structural barriers and a lack of culturally responsive mental health programming in community settings. Adapting both intervention content and implementation strategies is essential to improving the fit and effectiveness of task-shared mental health interventions, yet most adaptation research focuses on pre-implementation changes rather than ongoing adjustments throughout a program's lifecycle. This paper examines adaptations to Problem Management Plus (PM+), a t

Locked in and left out: the "prison penalty" for implementation of evidence-based interventions.

BACKGROUND: While the broader medical community grapples with the widely accepted notion that it takes an average of 17 years for research evidence to be incorporated into clinical practice, the implementation of evidence-based interventions in carceral settings (i.e., jails and prisons) faces longer delays, exacerbating health disparities. MAIN BODY: The "prison implementation penalty" describes the significant delay in and limited adoption of evidence-based healthcare practices in carceral settings. We explore the complex challenges of implementing evidence-based interventions in jails and

Provider Perceptions of Bubble Continuous Positive Airway Pressure and Barriers to Implementation in a Level III Neonatal Unit in South India.

BACKGROUND: Bubble continuous positive airway pressure (bCPAP) is a simple, safe, and cost-effective strategy to provide respiratory support to newborns with respiratory distress syndrome in resource-limited settings. PURPOSE: To understand whether implementation of bCPAP, relative to other modes of respiratory support in the care of newborns with respiratory distress syndrome, increases positive attitudes about its potential for consistent and widespread use among providers in neonatal intensive care units (NICUs) of lower middle-income countries. METHODS: Semistructured qualitative intervi

A Systematic Review of Implementation Research on Determinants and Strategies of Effective HIV Interventions for Men Who Have Sex with Men in the United States.

Men who have sex with men (MSM) are disproportionately affected by HIV, accounting for two-thirds of HIV cases in the United States despite representing ∼5% of the adult population. Delivery and use of existing and highly effective HIV prevention and treatment strategies remain suboptimal among MSM. To summarize the state of the science, we systematically review implementation determinants and strategies of HIV-related health interventions using implementation science frameworks. Research on implementation barriers has focused predominantly on characteristics of individual recipients (e.g., et
