Epidemiology and Economic Burden of Diagnosed Congenital Cytomegalovirus Infection in the First 2 Years of Life among Commercially Insured and Medicaid-Insured Individuals in the United States.

PURPOSE: Congenital cytomegalovirus (cCMV) is the leading infectious cause of congenital birth defects. Although approximately 20% to 25% of infants born with cCMV develop long-term health complications such as sensorineural hearing loss, developmental issues, and microcephaly, studies on the disease burden of cCMV are limited. In this study, we assessed the epidemiology, economic burden, and disease burden of clinically diagnosed cCMV in the United States using insurance claims data. METHODS: This retrospective study utilized Merative MarketScan Commercial Claims and Encounters and Multi-Sta

Evaluation of preventive care for swallowing difficulty through policy changes in Japanese long-term care insurance: analysis of a nationwide claims dataset for long-term care insurance.

BACKGROUND: Swallowing/feeding difficulty is a serious hidden health problem in the older population. Although oral intake services based on the degree of this difficulty have been provided and revised in Japanese public long-term care (LTC) insurance since 2006, their implementation has not been examined. We evaluated the impact of policy changes on these services. METHODS: The nationwide database of LTC service uses in Japan was analysed for three oral intake services: Enhanced Oral Function in daycare facilities whose users were slightly disabled, Maintenance of Oral Ingestion and Transiti

[Inequalities in health risks, morbidity and health care of children by health insurance of their parents (statutory vs. private health insurance): results of the German KiGGS study].

AIM OF THE STUDY: In Germany, differences in health risks, morbidity and health care between children whose parents are insured either in the statutory or in the private health insurance (in German: GKV or PKV respectively) have rarely been studied. Recent studies have shown that adults who are insured in the GKV tend to be less healthy and tend to seek primary health care more frequently than those who are insured in the PKV. The study presented here focuses on the hypothesis that similar inequalities also exist between their children. METHODS: The analyses are based on data from the German

Clinical-pharmacological medication reviews for insurants of a German statutory health insurance fund (Kaufmännische Krankenkasse (KKH)): a 5-year retrospective analysis (2019-2023).

This study sought to evaluate the characteristics of clinical-pharmacological (CP) medication reviews conducted by the Institute for Clinical Pharmacology of Hannover Medical School for older, multimorbid, polymedicated insurants of the German statutory health insurance fund Kaufmännische Krankenkasse (KKH). Special emphasis was put on potential sex-specific differences between female and male insurants, prescriptions of potentially inappropriate medications for older individuals (PIMs), and potentially inappropriate duplicate prescriptions (PIDPs). Medication reviews conducted for KKH insuran

Increased care-need in older long-term care insurance users after the 2018 Japan Floods: a retrospective cohort study based on the Japanese long-term care insurance claims.

BACKGROUND: Level of care-need (LOC) is an indicator of elderly person's disability level and is officially used to determine the care services provided in Japan's long-term care insurance (LTCI) system. The 2018 Japan Floods, which struck western Japan in July 2018, were the country's second largest water disaster. This study determined the extent to which the disaster affected the LOC of victims and compared it with that of non-victims. METHODS: This is a retrospective cohort study, based on the Japanese long-term care insurance claims from two months before (May 2018) to five months after

Experiences of Lactation Support Among Medicaid-Insured Mothers in Louisiana.

INTRODUCTION: Louisiana's breastfeeding rates are lower than most states, and even lower among its Medicaid members. Louisiana's Medicaid program does not cover outpatient lactation services, although expanded access to lactation providers could increase breastfeeding exclusivity and duration for women of diverse backgrounds. The purpose of this study was to explore the views of stakeholders in lactation support for Louisiana's Medicaid-insured women and increase our understanding of current barriers and opportunities to improving outpatient lactation support and decrease racial disparities in

Independent and interactive effects of race and insurance on preterm birth risk.

OBJECTIVE: To assess racial and ethnic disparities in preterm delivery in the US and evaluate whether insurance type modifies these associations. METHODS: We analyzed National Center for Health Statistics natality data (2018-2024) comprising 24,918,700 singleton live births. Preterm delivery was defined as birth before 37 completed weeks of gestation. Multivariable logistic regression with robust variance was used to estimate adjusted odds ratios, predicted probabilities, and absolute risk differences across race/ethnicity and insurance categories, adjusting for maternal sociodemographic, cli

Heterogeneous disability shocks and the dynamics of income, employment, and partial insurance.

This study examines the long-run economic consequences of disability, distinguishing conditions by the activities they impair. Using linked Canadian survey and administrative tax data, I estimate the effects of disability onset on detailed income components over a ten-year horizon and assess gaps in partial income insurance across disability types. Mental-cognitive disabilities lead to larger and more persistent losses in market income than physical disabilities. Despite this, both groups experience similar levels of partial insurance and comparable declines in after-tax income. Importantly, s

Impacts of health insurance on tympanostomy tube outcomes in the pediatric population.

OBJECTIVES: Tympanostomy tube (TT) placement is a common surgical procedure for treating pediatric patients with chronic otitis media with effusion (COME) with or without recurrent acute otitis media (rAOM). Prior work suggests children from low-income families face significant disparities in access to care for rAOM or COME. The impact of these health disparities in the care of children with rAOM or COME has yet to be investigated in a state with an expanded public health insurance model. We seek to examine differences in care for patients with rAOM and COME based on insurance status and socio

The Impact of Race, Insurance, and Socioeconomic Factors on Pediatric Knee Injuries.

Disparities persist in pediatric sports medicine along the lines of race, ethnicity, insurance status, and other demographic factors. In the context of knee injuries such as anterior cruciate ligament (ACL) ruptures, meniscus tears, and tibial spine fractures, these inequalities affect evaluation, treatment, and outcomes. The long-term effects can be far-reaching, including sports and physical activity participation, comorbid chronic disease, and socio-emotional health. Further research is needed to more concretely identify the etiology of these disparities so that effective, equitable care is

Prevalence and Financial Burden of Digestive Diseases in a Commercially Insured Population.

BACKGROUND & AIMS: Digestive diseases represent a diverse group of clinical conditions that impact the population. Their heterogeneity in classification, presentation, acuity, chronicity, and need for drug therapy presents a challenge when comparing and contrasting the burden associated with these conditions. Prior studies use an outdated classification system and aggregate costs at the population level or focus on specific diseases, limiting the ability to characterize the overall landscape. Our aim was to provide the most up-to-date assessment of cost, utilization, and prevalence associated

Enhanced INSURE (ENSURE): an updated and standardised reference for surfactant administration.

There is no firm consensus about the optimal technique for the administration of exogenous surfactant in preterm neonates, and different techniques may be equally effective. The intubation-surfactant-extubation (INSURE) procedure has not been fully described, and important details, such as duration and mode of ventilation, remain unclear, leading to significant clinical practice variations and influencing its suitability and feasibility. Since the first INSURE description, our knowledge in respiratory care has largely progressed, but the technique has not been updated according to current evid
