Anemia as a predictor of cardiovascular events in patients with elevated serum creatinine.

Patients with anemia and patients with chronic kidney disease have elevated risks for cardiovascular disease. Available studies have been too small to provide details about the relationship or to provide for extensive covariate control. In a large insurance database with linked laboratory values, records of women with serum creatinine >1.2 mg/dl and men with serum creatinine >1.4 mg/dl, identified from July 2000 through June 2003, were sought, and the insurance claims searches for hospitalizations that were associated with myocardial infarction, coronary revascularization, unstable angina, str

Hospital use by adolescents and young adults.

This article provides a descriptive profile of hospital use by adolescents (10-14 and 15-18 years of age) and young adults (19-24 years) based on the 1987 National Hospital Discharge Survey. During 1987, nearly 5 million adolescents and young adult patients were discharged from short-stay hospitals. The discharge rate for adolescents was 56.5 per 1000 compared to 127.0 per 1000 for young adults. Two diagnostic categories--complications of pregnancy, childbirth, and the puerperium; and injury and poisoning--together accounted for 24% of discharges for patients aged 10-14 years, 51% for patients

Disparities in Cancer Patients' Portal Usage During Hospitalizations.

BACKGROUND AND OBJECTIVE: Patient portals are crucial for successful transition after hospitalization, especially for cancer patients with complex treatment plans. This study aimed to identify inequities in hospitalized cancer patients' portal use. METHODS: This retrospective analysis assessed portal usage at a high-volume cancer hospital in 2022 and 2023. We assessed whether patients ever logged in, and their login rates, during hospitalizations and Intensive Care Unit (ICU) admissions. Multivariate logistic and Poisson regression models evaluated relationships between demographic and care c

Intentional injury among the indigenous and total populations in British Columbia, Canada: trends over time and ecological analyses of risk.

BACKGROUND: Our objective was to explore intentional injury disparity between Indigenous populations and the total population in the province of British Columbia (BC), Canada. We focus on hospitalizations, including both self-inflicted injuries and injuries inflicted by others. METHODS: We used data from BC's universal health care insurance plan, 1991 to 2010, linked to Vital Statistics databases. Indigenous people were identified through the insurance premium group, and birth and death records. Place of residence was identified through postal code. We calculated crude hospitalization inciden

Payor type is associated with increased rates of reoperation and health care utilization after rotator cuff repair: a national database study.

BACKGROUND: Despite strong evidence supporting the efficacy of rotator cuff repair (RCR), previous literature has demonstrated that socioeconomic disparities exist among patients who undergo surgery. There is a paucity of literature examining whether payor type, including Medicare, Medicaid, and commercial insurance types, impacts early medical complications and rates of reoperation after RCR. METHODS: Patients with Medicare, Medicaid, or commercial payor-type insurance who underwent primary open or arthroscopic RCR between 2010 and 2019 were identified using a large national database. Incidence and related risk factors for postoperative delirium following revision total knee arthroplasty: a retrospective nationwide inpatient sample database study.

BACKGROUND: Postoperative delirium (POD) is a common surgical complication. However, the incidence and risk factors associated with postoperative delirium after revision total knee arthroplasty (rTKA) have not been comprehensively explored through extensive national databases. METHODS: Utilizing the National Inpatient Sample (NIS), the largest comprehensive U.S. hospital healthcare database, we undertook a retrospective investigation involving 127,400 patients who underwent rTKA between 2010 and 2019. We assessed various aspects, including patient demographics, hospital characteristics, pre-e

High Cost and Resource Utilization of Frostbite Readmissions in the United States.

Frostbite is a high morbidity, high-cost injury that can lead to digit or limb necrosis requiring amputation. Our primary aim is to describe the rate of readmission following frostbite injury. Our secondary aims are to describe the overall burden of care, cost, and characteristics of repeat hospitalizations of frostbite-injured people. Hospitalizations following frostbite injury (index and readmissions) were identified in the 2016 and 2017 Nationwide Readmission Database. Multivariable logistic regression was clustered by hospital and additionally adjusted for severe frostbite injury, gender,

Does medical insurance type (private vs public) influence the physician's decision to perform Caesarean delivery?

INTRODUCTION: US data reveal a Caesarean rate discrepancy between insured and uninsured patients, with the C-section rate highest among the privately insured. The data have prompted concern that financial incentives associated with insurance status might influence American physicians' decisions to perform Caesarean deliveries. OBJECTIVE: To determine whether differences in medical risk factors account for the apparent Caesarean rate discrepancy between Medicaid and privately insured patients in Michigan, USA. METHOD: A retrospective review was performed of 617 269 live birth deliveries in Mi

Statin Use and Influenza Vaccine Effectiveness in Persons >65 Years of Age, Taiwan.

Debates on whether statin use reduces the effectiveness of influenza vaccines against critical illness and death among persons >65 years of age continue. We conducted a study of 9,427,392 persons >65 years of age who did and did not receive influenza vaccinations during 12 consecutive influenza seasons, 2000-01 through 2011-12. Using data from Taiwan's National Health Insurance Research Database, we performed propensity score-matching to compare vaccinated persons with unvaccinated controls. After propensity score-matching, the vaccinated group had lower risks for in-hospital death from influe

Hospitalization, Underweight, Depression, and Cancer Increase the Risk of Suicide in Individuals With Bronchiectasis.

Bronchiectasis is commonly comorbid with depression and anxiety; therefore, bronchiectasis may be associated with an increased risk of suicide. We aimed to investigate whether suicidal risk is higher in individuals with bronchiectasis. We enrolled 3,903,636 individuals, including 50,975 individuals with bronchiectasis, from the Korean National Health Insurance Service dataset. Suicidal risk was significantly higher in individuals with bronchiectasis than in those without (adjusted hazard ratio [aHR], 1.27; 95% confidence interval [CI], 1.11-1.44). The increased risk of suicide was more evident

Impact of elevated pooling levels in basic medical insurance on insured individuals' inpatient service utilization-An equity analysis.

BACKGROUND: China has continuously promoted the transition of basic medical insurance from municipal-level pooling to provincial-level pooling to strengthen fund risk resilience, mutual-aid capacity, and benefit equalization. Although existing studies suggest that higher pooling levels may influence healthcare utilization, limited micro-level evidence exists on how provincial-level pooling of the Basic Medical Insurance Scheme for Urban and Rural Residents (BMISURR) affects inpatient service utilization, hospitalization costs, and income-related equity. This study examines the impact of provin

Neuraxial blockade for external cephalic version: Cost analysis.

AIM: Neuraxial blockade (epidural or spinal anesthesia/analgesia) with external cephalic version increases the external cephalic version success rate. Hospitals and insurers may affect access to neuraxial blockade for external cephalic version, but the costs to these institutions remain largely unstudied. The objective of this study was to perform a cost analysis of neuraxial blockade use during external cephalic version from hospital and insurance payer perspectives. Secondarily, we estimated the effect of neuraxial blockade on cesarean delivery rates. METHODS: A decision-analysis model was

Does more generous health insurance reimbursement lead to moral hazard? Evidence from large-scale administrative data in China.

This paper exploits the discrete increase in reimbursement rates at the statutory retirement age as a quasi-natural experiment to examine ex post moral hazard in inpatient care in China. Using nationally representative administrative data covering more than 450,000 hospitalization records across 29 provinces, we implement a cohort difference-in-differences design to identify the causal effect of insurance generosity on healthcare utilization. We find that a 5-percentage-point increase in reimbursement rates increases length of stay by 1.73 days and total inpatient expenditures by RMB 709.98. T
