Pain predicts longer time to remission during treatment of recurrent depression.

INTRODUCTION: Pain and depression are mutually exacerbating. We know that both of these syndromes predict the future occurrence of the other. It has not been shown, however, whether the presence of pain slows the effect of treatment for depression. We hypothesized that greater pain and somatic scores prior to treatment with imipramine and interpersonal psychotherapy would predict a slowed time to remission from depression. METHOD: We performed secondary data analyses of an archived study. Subjects (N = 230) were between 21 and 65 years of age and were enrolled in a study of maintenance treatm

A randomized pilot study of psychotherapy and quetiapine for the acute treatment of bipolar II depression.

OBJECTIVES: The differential roles of psychotherapy and pharmacotherapy in the management of bipolar (BP) II depression are unknown. As a first step toward exploring this issue, we conducted a pilot study to evaluate the feasibility and acceptability of comparing a BP-specific psychotherapy [Interpersonal and Social Rhythm Therapy (IPSRT)] to quetiapine as treatments for BP-II depression. METHODS: Unmedicated individuals (n = 25) meeting DSM-IV criteria for BP-II disorder, currently depressed, were randomly assigned to weekly sessions of IPSRT (n = 14) or quetiapine (n = 11), flexibly dosed f

Long-term weight loss maintenance after inpatient psychotherapy of severely obese patients based on a randomized study: predictors and maintaining factors of health behavior.

OBJECTIVE: The objective of this study was to identify predictors of long-term weight loss after inpatient psychodynamic or behavioral psychotherapy of severely obese patients. METHODS: In a longitudinal study, obese patients [body mass index (BMI)>or=35 kg/m(2)] were randomly assigned to behavioral or psychodynamic inpatient treatment. The average treatment duration was 7 weeks. Two hundred sixty-seven obese patients, mostly female (85%), with psychiatric and somatic comorbidity (age, 20-64 years; BMI=35-74 kg/m(2)) were examined with standardized self-report scales at intake, discharge, 1-y

Analysis of the patient-therapist relationship in dynamic psychotherapy: an experimental study of transference interpretations.

OBJECTIVE: The purpose of this study was to measure the effects of transference interpretations (the assumed core active ingredient) in dynamic psychotherapy, using an experimental design. METHOD: One hundred patients were randomly assigned to two groups. One group received dynamic psychotherapy over 1 year, with a moderate level of transference interpretations, while the other group received dynamic psychotherapy with no transference interpretations. The most common axis I disorders were depression and anxiety disorders. Forty-six patients fulfilled the general criteria for personality disor

Losing One's Partner, Losing One's Self: Helping a Patient Process the End of an Early Romantic Relationship Through Psychodynamic Psychotherapy.

The literature on the psychotherapeutic treatment of individuals suffering from the dissolution of a significant romantic relationship is sparse, especially with regard to those who continue to reflect upon and struggle with a romantic breakup long after the event itself. The present case study explores the psychodynamically-oriented treatment of a young adult patient, with a focus on helping the patient process the ending of a significant romantic relationship from her late adolescence. A twice-weekly, long-term treatment provided a collaborative space to mourn the lost relationship, while fa

Interpersonal behavior in couple therapy: Concurrent and prospective associations with depressive symptoms and relationship distress.

Objective: This study investigated associations between couples' interpersonal behavior, depressive symptoms, and relationship distress over the course of couple psychotherapy. Method: After every other session of Integrative Systemic Therapy (M = 13 sessions), N = 100 individuals within 50 couples rated their in-session affiliation and autonomy behavior using the circumplex-based Structural Analysis of Social Behavior Intrex. Concurrent and prospective associations of interpersonal behavior with depressive symptoms and relationship distress were evaluated via multivariate multilevel modeling

Effects of clinician-assisted emotional disclosure for sexual assault survivors: a pilot study.

This study assessed the effects of clinician-assisted emotional disclosure (CAED), an integration of emotion focused therapy (Greenberg, Rice, & Elliott, 1993) and emotional disclosure, in ameliorating distress experienced by survivors of sexual assault. A total of 670 female university students were screened for both histories of sexual victimization and clinically significant levels of global psychological distress. Twenty-eight females entered the treatment phase of the study and were randomly assigned to participate in either treatment or no-treatment control conditions. Participants compl

A naturalistic study of psychotherapy for bulimia nervosa, part 2: therapeutic interventions in the community.

Data from naturalistic samples provide an important complement to findings from randomized trials of psychotherapy. A random national sample of US clinicians provided data on 145 completed treatments of patients with bulimic symptoms. We attempted to characterize the nature of treatments in the community and to examine the relation between treatment variables and outcome. Clinicians of all theoretical orientations report using interventions with polysymptomatic cases designed to address clinically significant personality characteristics and interpersonal patterns. Whereas cognitive-behavioral

Short-Term Effects of the Empathic Talk: The Role of Perceived Empathy, Interpersonal Dispositions, and Vagal Tone on Experiencing Depth.

The present study examines the immediate impact of empathic attitudes on the receiver, by comparing an empathic conversation (EC) with a neutral one (NC) on experiencing depth (EXP), perceived empathy and vagal tone. We also evaluate if empathy effects depend on personality dispositions relevant to interpersonal functioning. An experimental, counterbalanced, within-subject design was implemented wherein participants (n = 27, age M = 22.6, SD = 4.0, 52% females) talked about a personal, meaningful topic for 20 minutes under both the EC and NC conditions. At the group level, main results indicat

Psychotherapies for eating disorders: findings from a rapid review.

BACKGROUND: Psychotherapy is considered central to the effective treatment of eating disorders-focusing on behavioural, psychological, and social factors that contribute to the illness. Research indicates psychotherapeutic interventions out-perform placebo, waitlist, and/or other treatments; but, outcomes vary with room for major improvement. Thus, this review aims to (1) establish and consolidate knowledge on efficacious eating disorder psychotherapies; (2) highlight select emerging psychotherapeutic interventions; and (3) identify knowledge gaps to better inform future treatment research and

Borderline personality disorder and bipolar disorder: what is the difference and why does it matter?

Borderline personality disorder (BPD) and bipolar disorder (types I and II) are frequently confused because of their symptomatic overlap. Although affective instability is a prominent feature of each, the pattern is entirely different. BPD is characterized by transient mood shifts that occur in response to interpersonal stressors, whereas bipolar disorder is associated with sustained mood changes. These disorders can be further distinguished by comparing their phenomenology, etiology, family history, biological studies, outcome, and response to medication. Their distinction is of great clinica

Loneliness and Its Associations With Personality Functioning: Evidence From Longitudinal Inpatient Psychotherapy Programs in Germany.

Loneliness is an aversive state that occurs at elevated rates among individuals with mental health disorders and may reciprocally exacerbate psychopathological processes. Individuals with personality disorders (PDs) appear particularly susceptible; however, knowledge regarding the relationship between loneliness and facets of personality functioning (PF) remains limited. Greater conceptual and empirical clarity is needed to elucidate the mechanisms linking loneliness with maladaptive self- and interpersonal functioning and to inform clinical practice. We examined the association between loneli

The crossroads of gastroenterology and psychiatry - what benefits can psychiatry provide for the treatment of patients suffering from gastrointestinal symptoms.

INTRODUCTION: Gastrointestinal symptoms are very common in the general population. Many of them coincide with mental disorders (especially with neuroses, stress-related disorders, somatisation disorders, autonomic dysfunction, and anxiety) that are associated with psychological trauma, conflicts, and difficulties with interpersonal relationships. AIM: Assessment of the association between gastrointestinal complaints and stressful situations in relationships, among patients admitted to day hospital for neurotic and behavioural disorders. MATERIAL AND METHODS: Analysis of the likelihood of co-

Meaning-centered psychotherapy for existential distress in advanced cancer: A culturally informed case report from Vietnam.

OBJECTIVES: Existential distress is common among patients with advanced cancer receiving palliative care and is associated with hopelessness, loss of meaning, spiritual distress, and desire for death. Meaning-centered psychotherapy (MCP) has shown potential in reducing existential suffering and improving psychological well-being in patients with advanced cancer. This article presents a case report describing the application of MCP in a young woman with advanced cancer experiencing depressive symptoms, trauma-related distress, hopelessness, and loss of meaning near the end of life. METHODS: We

Interpersonal perceptions in treatment sessions: Pathological narcissism predicts self-rater discrepancies.

OBJECTIVE: Evidence suggests that pathological narcissism impacts psychotherapy process and outcome. This study investigates whether traits of pathological narcissism account for distinctiveness (construal) of patients' interpersonal perceptions in treatment settings. METHODS: Patients enrolled in psychological/psychotherapeutic treatment (N = 150) described a segment of a session in a written format and subsequently assessed both self- and clinician's behavior on the dimensions of dominance and hostility (patient-reported ratings), along with their pathological narcissistic traits. The efficacy of modified psychodynamic psychotherapy for patients with schizophrenia-spectrum disorders in Germany: a prospective, single-centre, assessor-blinded, parallel-group, randomised controlled trial.

BACKGROUND: People with schizophrenia-spectrum disorders have difficulties in interpersonal functioning that remain insufficiently addressed by standard care. Despite long-standing clinical use, psychodynamic psychotherapy has little empirical support compared with other psychosocial treatments for people with schizophrenia-spectrum disorders. We evaluated the efficacy of Modified Psychodynamic Psychotherapy for Schizophrenia (MPP-S), a manualised treatment tailored to the interpersonal vulnerability characteristic of this population, plus treatment as usual (TAU), compared with TAU alone. MDMA for the treatment of misophonia, a proposal.

Misophonia is a disorder characterized by negative physical and emotional reactions to certain trigger sounds, such as chewing food. Up to 50% of population samples endorse some symptoms of misophonia, with about 20% having symptoms that impair normal life functioning. Most misophonia patients exhibit intense negative emotions and autonomic arousal (the fight-flight-freeze response) in response to a trigger, similarly to how someone with post-traumatic stress disorder (PTSD) might respond to a trauma trigger. Curiously, misophonia trigger sounds are often most distressing when coming from a sp

Dyadic Interactions of Treatment-Resistant Schizophrenia Patients Having Followed Virtual Reality Therapy: A Content Analysis.

(1) Background: Very little is known about the inner therapeutic processes of psychotherapy interventions for patients suffering from treatment-resistant schizophrenia. Avatar therapy (AT) is one such modalities in which the patient is undergoing immersive sessions in which they interact with an Avatar representing their main persistent auditory verbal hallucination. The aim of this study is to identify the most prevalent dyadic interactions between the patient and the Avatar in AT for patient's suffering from TRS. The stepped-care approach in anorexia nervosa and bulimia nervosa: progress and problems.

The stepped-care approach is based on the assumption that treatment should be less intensive and hence less expensive and intrusive at the start. Current data suggest that cognitive behaviour therapy (CBT) should be the preferred first treatment for bulimia nervosa (BN) patients. By comparison with the numerous trials supporting its efficacy, in fact, the evidence sustaining similar therapies, e.g. interpersonal psychotherapy (IPT) or pharmacological therapy, is weaker. There are now sufficient data to justify the use of less intensive and shorter treatment (i.e. psychoeducational groups and s
