Urine diagnostics in paediatric intestinal failure: A prospective cohort study of alternative sampling methods.

OBJECTIVES: Regular monitoring of urinary electrolyte excretion and kidney function is essential in children with intestinal failure (IF) receiving long-term parenteral nutrition (PN). This study evaluated alternative urine sampling methods against 24-h urine collection (UC) and assessed the prevalence of chronic kidney disease (CKD) and renal abnormalities. METHODS: In this prospective single-centre cohort study, 31 paediatric IF patients underwent 24-h UC, spot urine (SU) sampling, and shorter timed collections at predefined intervals. Urinary electrolytes and derived ratios were analysed.

Physical exercise training improves muscle strength and self-esteem in children with intestinal failure.

OBJECTIVES: Pediatric intestinal failure (IF) patients are less physically active compared to healthy peers, exhibit an altered body composition and low bone mineral density, which can be improved through physical activity. We assessed the effects of a tailored exercise intervention on physical fitness, nutritional status, and psychological functioning in children with IF. METHODS: Pediatric IF patients, still on and weaned off parenteral nutrition aged 6-18 years were eligible. Physical fitness ([sub]maximal exercise capacity, amount of physical activity, motor performance, strength, and cor

Do functional gut parameters predict enteral autonomy and chronic cholestasis in pediatric intestinal failure?

BACKGROUND & AIMS: Parenteral nutrition (PN) dependency in patients with intestinal failure (IF) can lead to complications including liver disease. Therefore, IF management strives to wean patients off PN. In adult IF, chronic cholestasis is predicted by the functional gut parameters citrulline (CIT) and enteroendocrine fibroblast growth factor 19 (FGF19), which inhibits hepatic bile salt synthesis. We investigated 1) whether CIT, FGF19 and a marker for enterocyte damage (urinary intestinal fatty acid-binding protein (I-FABP)) are associated with enteral autonomy within 60 days after intestina

Lipid strategies to prevent intestinal failure-associated liver disease in neonates: A pilot trial.

BACKGROUND: Intestinal failure-associated liver disease (IFALD) occurs in up to 50% of neonates treated with prolonged parenteral nutrition. Preventative strategies for IFALD include soybean oil lipid emulsion (SOLE) minimization and use of mixed-oil intravenous lipid emulsions (ILE). We conducted a pilot study prospectively comparing these two ILE strategies in the prevention of IFALD in neonates who required abdominal surgery. METHODS: We randomized eligible neonates to SOLE at 1 g/kg/day (SOLE Min) or mixed-oil ILE containing fish oil (MOLE) at 3 g/kg/day. These treatment groups were also

Superior vena cava syndrome in chronic intestinal failure patients: When the going gets tough.

BACKGROUND & AIMS: Catheter-related venous thrombosis is a severe complication of home parenteral nutrition (HPN) with potentially devastating consequences such as superior vena cava syndrome (SVCS). Early recognition and awareness of factors leading to its development are of paramount importance. However, studies are lacking in HPN patients focusing on this topic. In this study, we aimed to determine the incidence of SVCS in HPN patients and describe SVCS-related outcomes. METHODS: This retrospective cohort study comprised all adult HPN patients who developed SVCS between 2000 and 2022 at ou

Chyme Reinfusion Restores the Regulatory Bile Salt-FGF19 Axis in Patients With Intestinal Failure.

BACKGROUND AND AIMS: Automated chyme reinfusion (CR) in patients with intestinal failure (IF) and a temporary double enterostomy (TDE) restores intestinal function and protects against liver injury, but the mechanisms are incompletely understood. The aim was to investigate whether the beneficial effects of CR relate to functional recovery of enterohepatic signaling through the bile salt-FGF19 axis. APPROACH AND RESULTS: Blood samples were collected from 12 patients, 3 days before, at start, and 1, 3, 5, and 7 weeks after CR initiation. Plasma FGF19, total bile salts (TBS), 7-α-hydroxy-4-chole

Liver Inflammation Relates to Decreased Canalicular Bile Transporter Expression in Pediatric Onset Intestinal Failure.

OBJECTIVE: Although liver disease is a major complication of parenteral nutrition (PN) for intestinal failure (IF), its pathogenesis remains unclear. We investigated potential molecular mechanisms of liver injury in pediatric onset IF. METHODS: Liver expression of canalicular phospholipid (ABCB4), bile acid (ABCB11), and sterol (ABCG5/8) transporters, their upstream regulators LXR and FXR as well as pro-inflammatory cytokines interleukin-6 (IL6) and tumor necrosis factor (TNF) were investigated among patients with IF [age median 3.8 (IQR 1.2 to 11)] in relation to biochemical and histologic l

Peripherally inserted central catheters for long-term parenteral nutrition in infants with intestinal failure.

BACKGROUND AND AIM: Infants with intestinal failure often require long-term central access for delivery of parenteral nutrition (PN). Traditionally, surgically placed central venous catheters (CVCs) have been used; however, the complications associated with these catheters can lead to significant morbidity. Peripherally inserted central catheters (PICCs) are potentially superior to CVCs because they tend to be smaller, and can be placed without general anesthesia. The purpose of the study is to report the use of PICCs for long-term administration of PN in infants with intestinal failure and co

Isolated liver transplantation for decompensated end-stage liver disease in children with intestinal failure.

Cholestasis progressing to end-stage liver disease (ESLD) is common in intestinal failure (IF) in infants. Isolated liver transplantation (OLT) is performed when eventual enteral sufficiency is expected. We reviewed our experience with OLT for ESLD in patients with residual IF. From 1998 to 2004, four IF patients underwent OLT for ESLD at our institution. Three were performed as UNOS status I for acute decompensation of chronic liver failure; one other with severe cholestasis with a living donor. The living donor recipient died within months after OLT of chronic respiratory failure despite nor

Congestive heart failure due to aortic incompetence with intestinal infarction due to endarteritis obliterans.

A 27-year-old man with congestive heart failure due to aortic incompetence and subsequent intestinal infarction was found at laparotomy to have extensive necrosis of the bowel due to proliferative endarteritis. Symptoms resolved following treatment with prednisolone and cyclophosphamide, and replacement of the aortic valve. The sub-total occlusion produced by endarteritis obliterans may lead to acute end-organ infarction if cardiac output is reduced.

Skipped Aganglionic Lengthening Transposition (SALT) is highly effective to achieve enteral autonomy in selected patients with intestinal failure secondary to total intestinal aganglionosis.

BACKGROUND: Intestinal Failure, parenteral nutrition (PN) dependence, and subsequent liver disease are the most challenging and life-threatening complications of short bowel syndrome experienced by patients with total intestinal aganglionosis. Skipped Aganglionic Lengthening Transposition (SALT) showed to be a promising procedure to overcome such problems. We herein report the results of two patients who underwent SALT at the Umberto Bosio Center for Digestive Diseases. PATIENTS AND METHODS: Between November 2019 and July 2022, 2 patients with total intestinal aganglionosis underwent SALT as

Teduglutide use in haemodialysis-dependent renal failure from secondary hyperoxaluria in short bowel syndrome and type 3 intestinal failure - A case report.

BACKGROUND: Renal impairment is common in patients receiving long-term home parenteral nutrition (HPN) for short bowel syndrome-associated intestinal failure (SBS-IF). While glucagon-like peptide-2 (GLP-2) analogues like teduglutide are effective in reducing parenteral support (PS) requirements, data on their use in advanced stage chronic kidney disease (CKD) or haemodialysis are scarce. CASE REPORT: We describe a 66-year-old female with SBS-IF on HPN, complicated by hyperoxaluria and nephrocalcinosis contributing to stage 4 CKD. Initial renally-adjusted teduglutide dosing (0.025 mg/kg/day) e

Reversal of intestinal failure-associated liver disease in infants and children on parenteral nutrition: experience with 93 patients at a referral center for intestinal rehabilitation.

PURPOSE: Intestinal failure (IF)-associated liver disease (IFALD) complicates the treatment of children with IF receiving parenteral nutrition (PN). We hypothesized that prevention or resolution of IFALD was possible in most children and that this would result in improved outcomes. METHODS: We reviewed prospectively gathered data on all children referred to the intestinal rehabilitation and transplantation center at our institution. Total bilirubin level (TB) was used as the marker for IFALD. Patients were grouped based on TB at referral and at subsequent inpatient stays and outpatient visits

Renal function impairment in children with intestinal failure receiving parenteral nutrition: A descriptive cohort study.

BACKGROUND: Children with intestinal failure are at risk for kidney dysfunction; however, the contributing factors are not well established. We aimed to describe risk factors associated with glomerular and tubular renal dysfunction in children with intestinal failure. METHODS: This retrospective cohort study assessed glomerular and tubular dysfunction in children receiving parenteral nutrition for intestinal failure at two subsequent time points. Glomerular function was assessed using the estimated glomerular filtration rate and urinary microalbumin/creatinine ratio. Tubular function was asse

Real-world experience of teduglutide for the treatment of short bowel syndrome-associated intestinal failure.

BACKGROUND & AIMS: Patients with short bowel syndrome-associated intestinal failure (SBS-IF) require parenteral nutrition and/or intravenous fluids (PN/IV); however, long-term dependence can lead to complications. Teduglutide enhances mucosal growth and the absorptive function of the residual intestine and reduces dependence on PN/IV. We aimed to investigate the use of teduglutide in real-world settings. METHODS: This retrospective study assessed outcomes in adult patients with SBS-IF who had received teduglutide (ever-treated cohort) or were eligible for teduglutide but had never received it

Microbiota-oriented strategies to mitigate parenteral nutrition-related complications in intestinal failure: A narrative review.

BACKGROUND: Parenteral nutrition (PN) is essential for patients with intestinal failure (IF) but is associated with complications such as dysbiosis, small intestinal bacterial overgrowth, catheter-related infections, and intestinal failure-associated liver disease (IFALD). Growing evidence indicates that gut microbiota alterations contribute to the pathogenesis of these complications, supporting microbiota-oriented interventions as potential adjunctive therapies. METHODS: Data sources: A structured literature search was conducted in PubMed, Web of Science, and Scopus from inception to Decembe
