Immediate post-abortal insertion of intrauterine devices.

BACKGROUND: Insertion of an intrauterine device (IUD) immediately after an abortion has several potential advantages. The woman is known not to be pregnant, a major concern for clinicians. For example, many clinicians refuse to insert an IUD in a woman who is not menstruating [Stanback 1997]. After induced abortion, a woman's motivation to use contraception may be high. Among women who have limited access to a clinician, abortion care may provide a unique opportunity to address a woman's need for contraception [Mahomed 1997; McLaurin 1993; Wolf 1994]. A copper IUD confers nearly the same contr

Three-year efficacy, safety, and tolerability outcomes from a phase 3 study of a low-dose copper intrauterine device.

OBJECTIVES: This study aimed to assess 3-year efficacy, safety, and tolerability of the Cu 175 mm2 intrauterine device (IUD). STUDY DESIGN: This single-arm trial recruited participants at risk of pregnancy aged 17 to 45 years at 42 U.S. centers to receive a Cu 175 mm2 IUD with a flexible nitinol frame. We assessed efficacy in participants aged ≤35 years at enrollment and assessed all other outcomes in the entire population. We calculated the Pearl Index (pregnancies/100 person-years) through 3 years as the primary efficacy outcome. The secondary outcomes included pregnancy percentages by life

Medical therapy for fibroids: An overview.

Uterine fibroids are the most common benign tumours in women of reproductive age group and are a cause of significant healthcare burden. Although surgical treatments have been the traditional gold standard for symptomatic uterine fibroids, several medical therapeutic approaches have been used to achieve symptom suppression in women who wish to preserve their uterus or are at elevated risk of complications during surgery. Medical therapies used for uterine fibroids include tranexamic acid, NSAIDs, contraceptive steroids, progesterone coil, GnRH analogues, aromatase inhibitors, SERMs and progest

New developments in intrauterine device use: focus on the US.

Many more women in the US today rely upon intrauterine devices (IUDs) than in the past. This increased utilization may have substantially contributed to the decline in the percentage of unintended pregnancies in the US. Evidence-based practices have increased the number of women who are medically eligible for IUDs and have enabled more rapid access to the methods. Many women enjoy freedom to use IUDs without cost, but for many the impact of the Affordable Care Act has yet to be realized. Currently, there are three hormonal IUDs and one copper IUD available in the US. Each IUD is extremely effe

Understanding benefits and addressing misperceptions and barriers to intrauterine device access among populations in the United States.

Three intrauterine devices (IUDs), one copper and two containing the progestin levonorgestrel, are available for use in the United States. IUDs offer higher rates of contraceptive efficacy than nonlong-acting methods, and several studies have demonstrated higher satisfaction rates and continuation rates of any birth control method. This efficacy is not affected by age or parity. The safety of IUDs is well studied, and the risks of pelvic inflammatory disease, perforation, expulsion, and ectopic pregnancy are all of very low incidence. Noncontraceptive benefits include decreased menstrual blood

Clinical performance of the TCu 380A and Lippes Loop IUDs in three developing countries.

The clinical performance of the Copper T 380A (TCu 380A) and the Lippes Loop intrauterine devices (IUDs) were evaluated for 12 months in a group of 710 women who had one of the two IUDs inserted. Results are from a randomized clinical trial conducted at three collaborating research sites located in three developing countries. The gross cumulative life-table pregnancy rate of the TCu 380A IUD was found to be lower than that of the Lippes Loop IUD at 12 months (0.7 and 2.1 per women, respectively). Although this difference was not statistically significant (p = 0.25), it is similar to findings f

Levonorgestrel-releasing intrauterine device used for dysmenorrhea: five-year literature review.

Intrauterine devices (IUDs) provide highly effective, long-term, safe, reversible contraception, and are the most widely used reversible contraceptive method worldwide. The levonorgestrel-releasing IUD (LNG-IUD), originally designed for long-term contraceptives, is now recognized to provide non-contraceptive health benefits. These include severe dysmenorrhea and/or heavy menstrual bleeding associated with uterine myoma, endometriosis, and adenomyosis. This report aims to review the last five-year literature on the efficacy and safety of the LNG-IUD in women with dysmenorrhea. Dysmenorrhea has

Pain control options for intrauterine device placement and endometrial biopsy.

PURPOSE OF REVIEW: To review the effectiveness of pain control methods for outpatient intrauterine device (IUD) insertions and endometrial biopsy (EMB) procedures. RECENT FINDINGS: Several approaches to decreasing pain with ambulatory IUD placement and EMB have been evaluated in the literature. Paracervical blocks, cervical lidocaine spray, nonsteroidal anti-inflammatory drugs (NSAIDs), and anxiety reduction techniques have been shown to decrease pain reported by patients. For both IUD insertion and EMB, evidence regarding NSAIDs for intraprocedural pain is mixed, and they should be advised t
