Chronic Visual Abnormality in an Elderly Patient With Temporal Lobe Epilepsy.

A 79-year-old woman visited our department for chronic visual field abnormalities with a floating sensation for two months. Neurological and ophthalmologic examinations yielded normal results, except for brain MRI indicating left hippocampal atrophy. Cognitive function tests were normal. EEG revealed frequent spikes and slow waves in the left frontotemporal region, corroborated by reduced accumulation in 123I-iomazenil single photon emission computed tomography. A diagnosis of temporal lobe epilepsy was established, and treatment with lacosamide resulted in a remarkable improvement in symptoms

Synthesis and evaluation of anticonvulsant properties of new N-Mannich bases derived from pyrrolidine-2,5-dione and its 3-methyl-, 3-isopropyl, and 3-benzhydryl analogs.

The aim of this paper was to describe the synthesis of a library of 28 new 1,3-substituted pyrrolidine-2,5-dione as potential anticonvulsant agents. The anticonvulsant activity was evaluated using three acute models of seizures in mice (MES-maximal electroshock, scPTZ-subcutaneous pentylenetetrazole, and 6Hz-psychomotor seizure tests). The neurotoxicity was determined by rotarod test. The most promising compound was found to be N-[{morpholin-1-yl}-methyl]-3-benzhydryl-pyrrolidine-2,5-dione (15), as it was active in the MES (ED50=41.0mg/kg), scPTZ (ED50=101.6kg/mg), and 6Hz (ED50=45.42mg/kg) te

The relationship between anti-seizures medications and metabolic acidosis in craniotomy operations: is topiramate or zonisamide the cause of metabolic acidosis?

BACKGROUND/AIM: The most commonly prescribed anti-seizures medications (ASMs) for the treatment of epilepsy are currently topiramate, zonisamide, lacosamide, carbamazepine and levetiracetam. The objective of this study was to examine the correlation between preoperative, intraoperative, and postoperative metabolic acidosis and the use of ASMs prior to craniotomy operations. MATERIALS AND METHODS: This retrospective cross-sectional study evaluated patients who underwent intracranial surgery with craniotomy under general anaesthesia between May 2020 and April 2023 and used ASMs. The patients we

Medical management of refractory trigeminal neuralgia.

PURPOSE OF REVIEW: First- and second-line medical and surgical therapies for trigeminal neuralgia (TN) may have limited efficacy or tolerability in some individuals. In addition, even when these therapies are initially successful, efficacy frequently wanes over time. This narrative review presents medication options for intractable trigeminal neuralgia with a focus on new and upcoming therapies. RECENT FINDINGS: Lacosamide, eslicarbazepine, and botulinum toxin are increasingly used for intractable TN, with recent studies demonstrating efficacy. Basimglurant and vixotrigine (BIIB074) are inves

Nonconvulsive seizure control in the intensive care unit.

Nonconvulsive seizures (NCS) occur in as many as 20 % of comatose critically ill patients. These seizures need to be treated; however, the urgency with which this must be done and the medications that should be used are unclear. Often, data from treatment of convulsive status epilepticus (SE) is used to determine the best therapy for NCS. This may lead to "overtreatment" with sedating medications that prolongs hospitalization and worsens outcome. Nonsedating antiepileptic drug (AED) use is favored by many neurologists as the side effect profile is superior to sedating medications. Though limit

Antiseizure Medications and Bone Health.

Epilepsy frequently requires treatment with antiseizure medications (ASM). With the progressive rise in life expectancy in this population, patients are more exposed to potential undesirable effects, some of them on bone tissue. Here, we review current knowledge concerning the impact of ASM on bone biology. Cytochrome P450 inductors decrease serum concentrations of active vitamin D, increasing parathyroid hormone (PTH) secretion and hence bone resorption. Valproic acid also reduces active vitamin D, but in addition activates osteoclasts and impairs osteoblastic function through different pathw

Influence of Umbelliferone on the Anticonvulsant and Neuroprotective Activity of Selected Antiepileptic Drugs: An In Vivo and In Vitro Study.

Umbelliferone (7-hydroxycoumarin; UMB) is a coumarin with many biological properties, including antiepileptic activity. This study evaluated the effect of UMB on the ability of classical and novel antiepileptic drugs (e.g., lacosamide (LCM), levetiracetam (LEV), phenobarbital (PB) and valproate (VPA)) to prevent seizures evoked by the 6-Hz corneal-stimulation-induced seizure model. The study also evaluated the influence of this coumarin on the neuroprotective properties of these drugs in two in vitro models of neurodegeneration, including trophic stress and excitotoxicity. The results indicate

Epstein-Barr virus infection in a woman with aquaporin-4 seropositive neuromyelitis optica.

Neuromyelitis optica spectrum disorders (NMOSD) are characterised by pathological antibodies to aquaporin-4 water channels of astrocytes, resulting in severe brain and spinal cord injury. Serological evidence suggests that Epstein-Barr virus (EBV) reactivation may contribute to their pathogenesis. We describe an unusual case of a woman with fever, rash and headache preceding an Aquaporin-4 antibody positive longitudinally extensive transverse myelitis. EBV was detected in her cerebrospinal fluid by polymerase chain reaction assay. This case highlights the potential role of EBV in the pathogene

WITHDRAWN: Anticonvulsants for fibromyalgia.

BACKGROUND: Fibromyalgia (FM) is a clinically well-defined chronic condition of unknown aetiology characterised by chronic widespread pain that often co-exists with sleep problems and fatigue. People often report high disability levels and poor health-related quality of life (HRQoL). Drug therapy focuses on reducing key symptoms and disability, and improving HRQoL. Anticonvulsants (antiepileptic drugs) are drugs frequently used for the treatment of chronic pain syndromes. OBJECTIVES: To assess the benefits and harms of anticonvulsants for treating FM symptoms. SEARCH METHODS: We searched the

Use of Sodium Channel Blockers in the Thr226Met Pathologic Variant of SCN1A: A Case Report.

The Thr226Met pathologic variant of the SCN1A gene has been associated with the clinical development of an early infantile developmental and epileptic encephalopathy (EIDEE) different from Dravet's syndrome. The electrophysiological mechanisms of the mutated channel lead to a paradoxical gain and loss of function. The use of sodium channel blockers (SCB) that counteract this gain of function has been described in previous studies and they can be safely administered to patients carrying mutations in other sodium channel subtypes without causing a worsening of seizures. We report the use of SCB

Treatment of benzodiazepine-refractory status epilepticus: A retrospective, cohort study.

INTRODUCTION: Status epilepticus (SE) is a frequent neurological emergency, derived from the failure of mechanisms responsible for seizure termination. The present study aims to compare the efficacy of the most common antiseizure medications (ASMs) employed for the treatment of benzodiazepine-refractory SE. METHODS: We performed a retrospective cohort study of all SE episodes treated in our hospital between January 2016 and December 2020. Inclusion criteria were: age ≥ 18 years; a diagnosis of status epilepticus. Exclusion criteria were: status epilepticus resolved by initial therapy with ben

Clinical practice guidelines for the diagnosis and treatment of diffuse glioma-related epilepsy: 2025 update.

Diffuse glioma-related epilepsy (dGRE) frequently presents with epilepsy as the initial symptom and is closely associated with tumor progression or recurrence, imposing significant social and psychological burdens on patients. The pathogenesis of dGRE is highly complex, involving both peritumoral microenvironmental mechanisms and tumor-intrinsic factors. Diagnosis requires a comprehensive approach integrating neuroimaging, EEG, molecular biomarkers, and spatial correlation between the tumor and the epileptogenic zone. Management aims to control seizures and improve prognosis. Non-enzyme-induci

Case Report: Reversible myelodysplastic syndrome secondary to sodium valproate in an epileptic child.

BACKGROUND: We report a rare case of sodium valproate (VPA)-induced reversible myelodysplastic syndrome (MDS) in an Asian child with epilepsy, thus expanding the recognition of VPA-associated hematological toxicity. Although VPA has been widely used, the reversibility of MDS as a complication remains underreported, particularly in pediatric populations. CASE SUMMARY: A 9-year-old boy with epilepsy developed pancytopenia (hemoglobin level: 77 g/L, platelet count: 83 × 109/L) and bone marrow-confirmed MDS after 6 months of VPA therapy. Following a reduction in VPA dose to 250 mg/day with adjunc

New onset refractory status epilepticus diagnosed in the second trimester: A case report.

New-onset refractory status epilepticus (NORSE) is a rare condition in which a previously healthy individual develops refractory seizures without an identifiable cause. In pregnancy, management is particularly challenging due to the need to control seizures while minimizing teratogenic risk for the fetus. We report a 22-year-old woman who developed NORSE at 19 weeks' gestation following recurrent tonic-clonic seizures. Treatment included multiple antiseizure medications: levetiracetam, oxcarbazepine, lacosamide, clobazam, and lamotrigine. Due to super-refractory status, she required intubation

Rapid genetic testing in the PICU: uncovering COL4A2-related GOULD syndrome in super-refractory status epilepticus.

Gould Syndrome is a rare genetic disorder associated with COL4A1/COL4A2 variants compromising the blood-brain barrier and leading to neurological and muscular complications, including epilepsy. We report the first known case of Gould syndrome presenting with neonatal-onset panhypopituitarism and later developing super-refractory status epilepticus (SRSE). The patient, a girl diagnosed at 22 months old, required pentobarbital burst suppression, multiple anti-seizure medications (ASMs) and ketogenic diet therapy. SRSE resolved after 1 week of pentobarbital coma. She was discharged on Clobazam, P

Anterograde amnesia recurrence in temporal lobe epilepsy with amygdala-enlargement.

Temporal lobe epilepsy (TLE) can cause different types of memory impairments. Here, we report a case of immediate improvement of memory impairment following antiepileptic drug (AED) treatment in a patient with TLE with amygdala enlargement (TLE-AE), who rapidly developed recurrence. The patient was a man in his 60s whose family members complained of his amnesia. Neuropsychological investigations detected obvious recent and remote memory loss and executive function impairments. Our examinations revealed evidence of TLE and bilateral amygdala enlargement without any results suggesting organic di

Epileptic convulsions probably induced by desloratadine: a case report.

Desloratadine, a second generation H1-antihistamine, is generally considered to be safe. We found only one article reporting four children with a family or disease history of epilepsy who developed the condition after desloratadine treatment, with all four patients recovering well. Here we describe a healthy boy who developed left-arm convulsions on day 68 after taking desloratadine, at which point the desloratadine treatment was immediately stopped. Investigations were completed on day 83 and the patient was diagnosed with epilepsy. He was prescribed sodium valproate combined with oxcarbazepi

Reconsidering anion inhibitors in the general context of drug design studies of modulators of activity of the classical enzyme carbonic anhydrase.

Inorganic anions inhibit the metalloenzyme carbonic anhydrase (CA, EC 4.2.1.1) generally by coordinating to the active site metal ion. Cyanate was reported as a non-coordinating CA inhibitor but those erroneous results were subsequently corrected by another group. We review the anion CA inhibitors (CAIs) in the more general context of drug design studies and the discovery of a large number of inhibitor classes and inhibition mechanisms, including zinc binders (sulphonamides and isosteres, dithiocabamates and isosteres, thiols, selenols, benzoxaboroles, ninhydrins, etc.); inhibitors anchoring t
