Sociodemographic Bias in Large Language Model Clinical Trial Screening.

BACKGROUND: Large language models (LLMs) are increasingly used in randomized clinical trial (RCT) screening, but their potential for sociodemographic bias remains unclear. OBJECTIVE: To determine whether LLM-based trial screening judgments vary with patient sociodemographic characteristics when clinical details and eligibility criteria are held constant. DESIGN SETTING AND PARTICIPANTS: Cross-sectional evaluation of Phase II-III RCT protocols from ClinicalTrials.gov (U.S. adult populations; 2023-2024). For each protocol, we created 15 physician-validated clinical vignettes rendered in 34 ver

A large language model framework to uncover underreporting in traffic crashes.

INTRODUCTION: Crash reports support the development of traffic safety countermeasures, but these reports often suffer from underreporting of crucial crash factors due to miscoded entries during data collection. To rectify these issues, the current practice relies on manual information rectification, which is time consuming and error prone, especially with large data volumes. To address these hurdles, we develop a framework to analyze traffic crash narratives and uncover underreported crash factors by capitalizing on the capabilities of Large Language Models (LLM). METHOD: The framework integr

Increased Prevalence of Large Language Model-Associated Language in US Residency Application Personal Statements.

BACKGROUND: Residency leaders increasingly rely on personal statements to assess applicant communication skills and individuality. Personal statements written by large language models (LLMs) have been rated more readable but more generic than applicant-written ones, yet the prevalence of LLM use and its effect on applicant writing remain unknown. OBJECTIVE: To measure the prevalence of LLM-associated language in personal statements from the 2017 through 2024 application cycles and assess associated changes in applicant writing. METHODS: This retrospective descriptive study analyzed personal

The utility of large language models in oncological multidisciplinary team meetings: A systematic review.

Large language models (LLMs) have emerged in recent years as innovative artificial intelligence systems with early potential in clinical decision-making. This is the first systematic review to evaluate LLMs' oncological decision-making and compare their treatment recommendations to "gold standard" oncological multi-disciplinary team (MDT) decision-making. PubMed, EMBASE and Medline databases were last searched on 20th January in line with PRISMA guidelines. All relevant peer-reviewed publications comparing LLM and MDT treatment recommendations in patients with cancer were included. Studies usi

Enhancing pancreatic cancer staging with large language models: the role of retrieval-augmented generation.

Retrieval-augmented generation (RAG) is an emerging technique that enhances large language models (LLMs) by retrieving relevant information from reliable external knowledge (REK). Despite its success in various language processing tasks, the clinical application of RAG in radiology is still novel. To evaluate the utility of RAG in radiological staging tasks, we compared the performance of NotebookLM, a RAG-equipped LLM (RAG-LLM), with its internal model, Gemini 2.0 Flash. Using Japan’s current pancreatic cancer staging guideline as REK, we compared three LLM settings—(1) NotebookLM with REK (R

Simulating insufficient effort responding with large language models: A persona-based prompt engineering approach.

Using large language models (LLMs) with persona-based prompt engineering, this study simulates realistic insufficient effort responding (IER) data under controlled conditions, overcoming the limitations of traditional methods in ecological validity and controllability. The core objective is to generate controlled, distinct IER and non-IER datasets, thereby improving further research on detection methods. Our strategy involved systematically manipulating persona attributes, such as behavioral descriptions and psychological attributes, to produce synthetic IER data under controlled conditions. W

Interpreting Omics Data Analysis with Large Language Models for Disease Target and Drug Discovery.

In biomedical scientific discovery, synthesizing prior knowledge from the literature is an essential component of interpreting numerical omics data analyses for disease target identification and drug discovery. Large language models (LLMs) alone can rapidly retrieve disease mechanisms from biomedical text, but text-only outputs are general and unreliable for target and drug prioritization without cohort-specific quantitative evidence. Herein, we propose a provenance-aware Text-to-Target framework that couples schema-constrained multi-model LLM retrieval with numeric omics data analysis. The ke

Reasoning or reciting? A temporal contamination audit of large language models in clinical medicine.

OBJECTIVE: Evaluate whether large language models reason or simply regurgitate training data in clinical diagnosis. MATERIALS AND METHODS: We audited 2000 clinical case reports from PubMed Central: 1000 from 2021 to 2022 (within training data) and 1000 from 2025 (after training cutoffs). Five frontier LLMs generated diagnoses evaluated by an independent AI judge validated against physician consensus (n = 10 000 evaluations). RESULTS: Diagnostic accuracy was virtually identical across temporal cohorts (66.8% contaminated vs 66.9% clean), directly contradicting the memorization hypothesis. Lex

Interpreting antibody identification tests with large language models: Critical role of tabular data formatting.

BACKGROUND: Antibody identification testing is essential for preventing hemolytic transfusion reactions from unexpected red cell antibodies. Although automated interpretation is desirable to improve efficiency and consistency, conventional algorithmic approaches show limited accuracy, particularly in complex cases. Large language models (LLMs) offer a potential alternative; however, their performance is known to be sensitive to tabular data, which is a standard format for antigen profiles in antibody identification. STUDY DESIGN AND METHODS: We generated 100 simulated antibody identification

Transforming Molecular Science With Large Language Models: From Molecule Understanding to Autonomous Scientific Discovery.

Large language models (LLMs) are driving a paradigm shift in molecular science, transitioning from molecule understanding to autonomous scientific discovery. By learning from multimodal molecular representations through advanced training techniques, LLMs address traditional limitations in molecular research, including expert dependency and limited experimental scalability. We analyze strategies for cross-modal alignment and domain adaptation that enable LLMs to interpret complex molecular semantics. This deep semantic understanding translates into broad versatility across key downstream applic

Introduction to Concepts in Artificial Intelligence and Machine Learning for Pharmacoepidemiologists: Large Language Models.

Large language models (LLMs) represent a type of generative artificial intelligence (GenAI) that generate and interpret text, with some LLMs able to process multimodal content (e.g., images, audio, video), and can be deployed as part of agents to perform users' tasks. LLMs can perform natural language processing functions such as summarization, translation, and extraction giving them the potential to enhance and scale pharmacoepidemiological and real-world research by performing tasks such as literature review, data extraction, and medical writing. Despite the growing integration of GenAI tool

Physicians and artificial intelligence diverge in evaluating large language models on real clinical cases.

While multimodal large language models (LLMs) demonstrate significant potential in healthcare applications, their clinical utility is difficult to appraise. Current evaluations of medical-assisting LLMs are often limited by sparse human expertise, narrow specialty scope, and reliance on multiple-choice benchmarks or synthetic vignettes, which can inflate performance and obscure clinical utility. We conducted a multicenter, multidisciplinary study in which more than 400 physicians-spanning seven specialties, varied experience levels, and multiple geographic settings-evaluated LLM-generated free
