Robot-assisted MRI-guided laser interstitial thermal therapy for a giant sessile hypothalamic hamartoma in an 8-year-old boy: illustrative case.

BACKGROUND: Hypothalamic hamartoma (HH) is a nonneoplastic tumor associated with precocious puberty and gelastic seizures, characterized by uncontrolled, inappropriate laughter. Successful treatment can resolve symptoms, but medications are often ineffective, and surgical treatment is complicated by the deep brain location. Laser interstitial thermal therapy (LITT) has been shown to be a safe alternative, but it has been suggested that it may not be suitable for larger lesions due to the increased risk of postoperative perilesional edema. OBSERVATIONS: The authors report the case of a giant s

Genotype-phenotype correlation over time in Angelman syndrome: Researching 134 patients.

Angelman syndrome (AS) is a severe neurodevelopmental disorder caused by the loss of function of maternal UBE3A. The major cause of AS is a maternal deletion in 15q11.2-q13, and the minor causes are a UBE3A mutation, uniparental disomy (UPD), and imprinting defect (ID). Previous reports suggest that all patients with AS exhibit developmental delay, movement or balance disorders, behavioral characteristics, and speech impairment. In contrast, a substantial number of AS patients with a UBE3A mutation, UPD, or ID were reported not to show these consistent features and to show age-dependent change

An open-source toolbox for measuring vocal tract shape from real-time magnetic resonance images.

Real-time magnetic resonance imaging (rtMRI) is a technique that provides high-contrast videographic data of human anatomy in motion. Applied to the vocal tract, it is a powerful method for capturing the dynamics of speech and other vocal behaviours by imaging structures internal to the mouth and throat. These images provide a means of studying the physiological basis for speech, singing, expressions of emotion, and swallowing that are otherwise not accessible for external observation. However, taking quantitative measurements from these images is notoriously difficult. We introduce a signal p

Quality of Life in Angelman Syndrome: A Caregivers' Survey.

BACKGROUND: Angelman syndrome (AS) is a genetic disorder, characterized by a cheerful disposition with bouts of laughter, developmental delay, speech impairment, ataxia, and seizures. Previous AS surveys have focused on the natural history, describing seizure types and response to anti-seizure medications. METHODS: A web-based survey was distributed to caregivers of individuals with AS to characterize motor function, cannabidiol (CBD) use, and factors affecting quality of life (QOL). RESULTS: Of a total of 183 individuals with AS (mean age 19.4 ± 13.4 years; 48.1% female), 72% had sleep prob

Scurvy as an Alarm Bell of Autistic Spectrum Disorder in the First World: A Case Report of a 3-Year-Old Girl.

BACKGROUND Scurvy secondary to deficiency of vitamin C is a rare condition in children. The polymorphism of clinical signs and symptoms makes scurvy diagnosis a challenge for the pediatrician. CASE REPORT A 3-year-old girl came to our observation because she refused to walk and to stand, she showed petechiae, follicular hyperkeratosis on the limbs, and bleeding gums. After a physical exam, laboratory tests, and X-ray examinations, we suspected scurvy disease and started treatment with vitamin C. During the hospitalization, some relationship difficulties of the little girl emerged such as absen

Development of zoophilic interests and behaviors in the example of an adolescent male.

BACKGROUND: In the research literature the phenomenon of bestiality is considered as a form of animal abuse or as an expression of love and affection toward animals or even as another sexual orientation. PARTICIPANTS AND PROCEDURE: The article elucidates the mechanisms of development and maintenance of paraphilia in an 18-year-old single male. Psychological evaluation was conducted with both clinical and psychometric measures. The patient's course of psychosexual development is characterized by two perspectives - his own and that of his mother. RESULTS: At the time of the assessment, the sub

Characterizing spine issues: If offers novel therapeutics to Angelman syndrome.

Angelman syndrome (AS) is a rare neurodevelopmental disorder characterized by severe mental retardation, microcephaly, speech impairment, frequent epilepsy, EEG abnormalities, ataxic movements, tongue protrusion, bursts of laughter, sleep abruptions, and hyperactivity. AS results from loss of function of the imprinted UBE3A (ubiquitin-protein ligase E3A) gene on chromosome 15q11-q13, including a mutation on the maternal allele of Ube3a, a large deletion of the maternally inherited chromosomal region 15q11-13, paternal uniparental disomy of chromosome 15q11-13, or an imprinting defect. The Ube3

Epilepsy in hypothalamic hamartoma: clinical and EEG features.

Hypothalamic hamartoma (HH) is a congenital malformation of the hypothalamus that may be asymptomatic or manifest with precocious puberty or seizures. Gelastic seizures often begin early in life, even in the newborn period, being manifest by frequent attacks of inappropriate laughter resulting from seizure activity in the HH. The scalp electroencephalogram (EEG) is often normal in children with gelastic seizures, such that the diagnosis of epilepsy and the finding of a HH are often delayed. In a proportion of children with HH, there is an epileptic progression, in which complex partial seizure

A cognitive and affective pattern in posterior fossa strokes in children: a case series.

AIM: Posterior fossa strokes account for about 10% of ischaemic strokes in children. Although motor and dysautonomic symptoms are common, to our knowledge cognitive and affective deficits have not been described in the paediatric literature. Our aim, therefore, was to describe these symptoms and deficits. METHOD: In a retrospective study, we included all cases of posterior fossa strokes in children occurring at a single centre between 2005 and 2007, and investigated cognitive and affective deficits. RESULTS: Five males aged 3 to 14 years met the inclusion criteria. They all presented very ea

The robustness of speech-like vocalization in typically developing infants and infants with autism.

BACKGROUND: Human infants produce speech-like vocalizations ("volubility") at very high rates, 4-5 per minute during waking hours across the first year, far exceeding rates of our ape relatives. AIMS: We document volubility in 127 typically developing (TD) infants, 44 with autism (ASD), and 21 with non-autism developmental delay (DD) through longitudinal recordings and human coding. METHODS AND PROCEDURES: Families of 302 infants (110 not yet diagnosed or with other diagnoses) supplied 8.6 all-day recordings across the first year. Trained coders analyzed 21 randomly-selected 5-minute segment

Gyratory Seizures in Hypothalamic Hamartoma.

Gyratory seizures (GS) are a rare seizure type characterized by body rotation of ≥180° around its vertical axis. While GS have been documented in various epileptic syndromes, their occurrence in association with hypothalamic hamartomas (HH) has not been reported previously. This case report introduces the first documented instance of GS in a patient with a HH, a non-neoplastic tumor originating from the tuber cinereum. The patient, a 25-year-old female, with a history of recurrent seizures since childhood, initially presented with gelastic seizures, marked by inappropriate laughter, and subseq

Ten-year updated meta-analysis of the clinical characteristics of pediatric mania and hypomania.

OBJECTIVES: The phenomenology and diagnosis of pediatric bipolar disorder has been controversial. We aimed to update a 2005 meta analysis of the prevalence of manic symptoms in youth, in order to determine whether the picture of pediatric mania has changed as research on pediatric bipolar disorder has grown. METHODS: We conducted literature reviews in PsycINFO and PubMed; studies with the prevalence of manic symptoms in youth were included. Two raters coded each study; kappa was 0.86-1.0. RESULTS: Twenty studies were meta-analyzed (N = 2,226 youths). The most common symptoms across bipolar s

"They all laughed and asked me if I enjoyed having sex with those guys": Exploring men's lived experiences when reporting rape to police in South Africa.

Rape is the most demoralizing type of crime violating human rights worldwide. Research has primarily focused on children and women's experiences of rape, even though victims include men and little documentation available concerning their experiences of reporting these incidents. The study aimed to investigate men's experiences when reporting rape to the police. An Interpretative Phenomenological Analysis was used to collect and analyze qualitative data from a purposive sample of eleven men who were rape victims. The findings of the study revealed three superordinate and twenty-two subordinate

Symptoms of narcolepsy in children misinterpreted as epilepsy.

Differentiating an epileptic seizure from some other paroxysmal event is a common challenge in clinical practice. Many paroxysmal events mimic epileptic seizures and misdiagnosis can have disastrous consequences. Incorrectly identifying an event as an epileptic seizure can lead to unnecessary investigations and instigation of inappropriate treatment regimes. We report five patients referred to regional Paediatric Neuroscience Centres for investigation of events initially suspected of being epileptic seizures. All five patients were subsequently diagnosed as having narcolepsy. Suspected diagnos

Pubertas praecox and hypothalamic hamartoma.

Precocious puberty of cerebral origin is classified into pseudoprecocious puberty and true precocious puberty. Pseudoprecocious puberty is caused by HCG secreting tumours. True precocious puberty is caused by various hypothalamic diseases. Among them, hypothalamic hamartoma is the most common cause. Precocious puberty is caused by elevated blood pituitary gonadotropin concentration, secondary to the elevated hypothalamic LHRH secretion. The hypothalamic hamartoma is not infrequently associated with laughing (gelastic) seizures as well as convulsions. Diagnosis of a hypothalamic hamartoma is ea
