The treatment of bucket handle meniscus tear using "the pulling suture technique": A new surgical technique.

INTRODUCTION AND IMPORTANCE: Arthroscopic partial meniscectomy represents a challenge due to view obstruction by the tight posterior joint line. We are describing a new technique to overcome this obstacle using "the pulling suture technique" which is a simple, reproducible, and safe way to perform partial meniscectomy. CASE PRESENTATION: After a twisting knee injury, a 30-year-old man was complaining of left knee pain and locking. An irreparable complex bucket handle medial meniscus tear was found during diagnostic knee arthroscopy and partial meniscectomy was performed using the pulling sutu

An Analysis of Symptomatic Meniscal Re-Tear Incidence in Two Age Populations: Differences in Older versus Younger Adults.

A symptomatic meniscal tear is one of the most common problems treated by an orthopaedic surgeon. Treatment ranges from observation to complete meniscectomy. Prior research has shown that contact stresses increase within the joint compartment after partial meniscectomy, thus leading to arthritis of the compartment. The purpose of the current study was to determine the incidence of meniscal re-tears in two populations of patients and identify potential factors predisposing to re-tear. A retrospective review of our relational database identified 1,930 arthroscopic partial meniscectomies from 199

In Pursuit of Quantifying Patient Knee Contact Mechanics: Finite Element Model Validation of Cadaveric Knees in Axially Loaded MRI Scans.

Our long-term objective is to quantify patient-specific changes in contact mechanics after partial meniscectomy (PM) using knee-specific finite element (FE) models created from clinical MR scans under axial load. Before creating patient-specific models, a validation of our workflow and processes is required. The objective of this study was to validate knee-specific FE models of tibiofemoral joint contact mechanics by comparison to direct measurements of contact by electronic pressure sensors. We hypothesized that knee-specific FE model data would fall within direct measurements of the contact

Biological and Mechanical Predictors of Meniscus Function: Basic Science to Clinical Translation.

Progressive knee joint degeneration occurs following removal of a torn meniscus. However, there is significant variability in the rate of development of post-meniscectomy osteoarthritis (OA). While there is no current consensus on the risk factors for development of knee OA in patients with meniscus tears, it is likely that both biological and biomechanical factors play critical roles. In this perspective paper, we review the mechanical and the biological predictors of the response of the knee to partial meniscectomy. We review the role of patient-based studies, in vivo animal models, cadaveri

Discoid lateral meniscus: case report of arthroscopic attachment of a symptomatic Wrisberg-ligament type.

The symptomatic discoid lateral meniscus is a well-known congenital anomaly that is of three different types: complete, incomplete, and Wrisberg-ligament type. The Wrisberg-ligament type has no meniscotibial attachment posteriorly, and in the past has been treated by total (open or arthroscopic) meniscectomy. In this article, we review the literature and report a previously unreported case of arthroscopic peripheral attachment after central partial meniscectomy of a Wrisberg-ligament type discoid lateral meniscus, with documentation of healing at arthroscopic second look 1 year following surge

Restoration of Anterior Tibial Translation Is Achievable With an Anterior Cruciate Ligament Reconstruction and Ramp Repair With Increasing Posterior Tibial Slope While Medial Meniscus Deficiency Requires Concomitant Slope-Correcting Osteotomy.

BACKGROUND: Previous studies have demonstrated that as posterior tibial slope (PTS) increases, increased force is transmitted through anterior cruciate ligament (ACL) grafts, but limited literature assesses PTS effects on anterior tibial translation (ATT) with concomitant medial meniscus (MM) pathology. PURPOSE: To investigate the effects of varying PTS angles on ATT in the presence of MM ramp tear and MM subtotal meniscectomy in the setting of ACL-sectioned and ACL-reconstructed knees. STUDY DESIGN: Controlled laboratory study. METHODS: Ten male matched-paired cadaveric knees underwent a p

Gait Mechanics After ACL Reconstruction Differ According to Medial Meniscal Treatment.

BACKGROUND: Knee osteoarthritis risk is high after anterior cruciate ligament reconstruction (ACLR) and arthroscopic meniscal surgery, and higher among individuals who undergo both. Although osteoarthritis development is multifactorial, altered walking mechanics may influence osteoarthritis progression. The purpose of this study was to compare gait mechanics after ACLR among participants who had undergone no medial meniscal surgery, partial medial meniscectomy, or medial meniscal repair. METHODS: This was a secondary analysis of data collected prospectively as part of a clinical trial. Sixty-

Histologic analysis of postmeniscectomy osteonecrosis.

Bone marrow signal changes on magnetic resonance imaging (MRI) after meniscectomy have been reported as evidence of postmeniscectomy osteonecrosis, but this pathology is unclear. We conducted a study to follow-up cases with bone marrow signal changes on MRI after meniscectomy and investigate the pathology of underlying lesions. Of 136 patients with no presurgical evidence of osteonecrosis, 29 had juxta-articular bone marrow signal changes on MRI after arthroscopic meniscectomy and subsequently underwent conservative therapy. In 6 of these 29 patients, clinical symptoms and radiographic changes

Meniscus transplantation: preoperative planning.

Precise preoperative planning is mandatory to obtain good results of meniscus transplantation. Any pathology in addition to the missing meniscus should be detected, including malalignment, and ligament instability. A full status of the changes in the cartilage should be made. It is unknown how precisely the donor meniscus needs to fit the size of the original meniscus to allow healing and regeneration. The effect of incongruency and wrong insertion of the meniscal horns has been investigated in an animal experiment. The incongruous grafts did not entirely avoid chondromalacia after meniscectom
