[Observation of the ventricular system and subarachnoid space in the skull base by flexible neuroendoscopy: normal structures].

Worldwide in the last two decades, in Latin America in the last decade, and Mexico in particular cerebral endoscopy as part of the minimally invasive neurological surgery, has constituted a significant advance for the treatment of neurological diseases such as congenital or acquired hydrocephalus, Chiari malformation, neurocysticercosis, stroke, ventricular and paraventricular tumors and cysts, arachnoidal cyst and hydrocephalus secondary to shunt malfunction. The lateral ventricles, the third ventricle, the cerebral aqueduct, the fourth ventricle, and the subarachnoid basal cisterns are among

[Changes in head circumference and ventricular system size after neuro-endoscopic third ventriculostomy in children].

UNLABELLED: The aim of the study was to compare changes in the head circumference ventricular system size after neuroendoscopic third ventriculostomy with those following shunt implantation in children suffering from chronic hydrocephalus. The data were analysed to establish criteria of success of neuroendoscopic procedures. In the years 1999-2001 neuroendoscopic third ventriculostomy was performed in 59 children at the Neurosurgery Department of the Research Institute of Polish Mothers' Memorial Hospital. However, the sample analysed in the paper consists of 29 children (16 boys, 13 girls age

Enlarged perivascular spaces mimicking multicystic brain tumors. Report of two cases and review of the literature.

The authors present two cases in which enlarged Virchow-Robin spaces were located in the basal ganglia and the thalamomesencephalic region. The incidence of such huge cystic lesions is extremely rare. The expanding nature of these lesions, demonstrated by the patients' progressive symptoms due to compression of the adjacent brain parenchyma and obstructive hydrocephalus, mimicked that of brain tumors. The two patients were successfully treated by neuroendoscopic cystocisternostomy or ventriculocystostomy. To the authors' knowledge there have been only two published reports on expanding Virchow

Multiple epidermoid cysts located in the pineal and extracranial regions treated by neuroendoscopy.

A 22-year-old woman presented with a rare case of multiple epidermoid cysts located in the pineal and extracranial regions. Magnetic resonance (MR) imaging showed a lesion in the pineal region as hypointense on the T(1)-weighted image and hyperintense on the T(2)-weighted image, without enhancement. Neuroendoscopic treatment was performed under a diagnosis of pineal cyst. However, the cyst wall was too thick to perforate, although third ventriculostomy was performed. Diffusion-weighted MR imaging demonstrated the lesions in the pineal and extracranial regions as marked hyperintensity. The diag

PsT1: A Low-Cost Optical Simulator for Psychomotor Skills Training in Neuroendoscopy.

BACKGROUND: Well-developed psychomotor skills are important for competence in minimally invasive surgery. Neuroendoscopy is no exception, and adaptation to different visual perspectives and careful handling of the surgical instruments are mandatory. Few training systems, however, focus on developing psychomotor skills for neuroendoscopy. Here, we introduce a new training system called PsT1 that provides visual feedback via the use of simple optics that emulate the endoscope at 0° and 30°. Time and error metrics are generated automatically with integrated software to ensure objective assessment

Novel multipurpose bipolar instrument for endoscopic neurosurgery.

OBJECTIVE: Hemorrhage control in endoscopic neurosurgery is critical because of the lack of suitable instruments for coagulation. One reason for this problem is that miniaturization of the instruments is still a technical problem. In this article, we present a solution: the use of bipolar microforceps with a small diameter of 1.5 mm. METHODS: With the use of modern synthetic and metallic materials, the construction of the bipolar microforceps was designed without the use of mechanical joints. All movable elements are integrated within the instrument shaft. This design provides optimal visibil

Third Ventricle Cavernous Malformation and Obstructive Hydrocephalus Thought to Be a Colloid Cyst.

BACKGROUND: Third ventricle cavernous malformations (CMs) associated with ventriculomegaly and obstructive hydrocephalus are quite rare in patients. Preoperative surgical planning can be challenging due to the lesion's non-specific appearance on CT and magnetic resonance imaging that can mimic other intraventricular pathologies, such as a colloid cyst. Management of these lesions can be varied in the setting of obstructive hydrocephalus. CASE DESCRIPTION: The patient is a 78-year-old woman who first presented to her primary care provider with balance difficulties and inability to ambulate on

Huge arachnoid cyst incorporating choroid plexus.

BACKGROUND: We report a rare case of arachnoid cyst incorporating choroid plexus. This 7-month-old girl presented with macrocrania. Magnetic resonance (MR) imaging disclosed a cystic lesion arising from the left prepontine cistern extending to the left middle cranial fossa. METHODS: First, we performed resection of the membrane microscopically and obtained an adequate reduction of the cystic size. However, 4 months after the first operation, the cyst was enlarged again, and bulging of the portion of the craniotomy was noted. Thus, we performed neuroendoscopic transcortical ventriculo-cyst-cys

TROPHY registry - status report.

INTRODUCTION: The TROPHY registry has been established to conduct an international multicenter prospective data collection on the surgical management of neonatal intraventricular hemorrhage (IVH)-related hydrocephalus to possibly contribute to future guidelines. The registry allows comparing the techniques established to treat hydrocephalus, such as external ventricular drainage (EVD), ventricular access device (VAD), ventricular subgaleal shunt (VSGS), and neuroendoscopic lavage (NEL). This first status report of the registry presents the results of the standard of care survey of participatin

Sellar cysticercosis and septum pellucidum cyst: a case report and review of the literature.

Sellar cysticercosis is a rare form of neurocysticercosis. A septum pellucidum cysts is a rare, often asymptomatic cystic structure between the lateral ventricles. We report here a male patient with sellar cysticercosis and septum pellucidum cysts who was successfully treated by neuroendoscopic resection. The patient was a 28-year-old male who presented with intermittent headaches for 5 years. A magnetic resonance imaging (MRI) of the brain revealed a well-circumscribed 13 mm cystic space-occupying lesion in the sellar region and the cavum septum pellucidum. The cyst in the saddle area was com

Clinical Value of 3D-Printed Navigation Technology Combined with Neuroendoscopy for Intracerebral Hemorrhage.

Intracerebral hemorrhage (ICH) is the most common form of hemorrhagic stroke with high morbidity and mortality. Rapid and massive bleeding may compress the brain tissue, causing space-occupying and pathological effects, such as reduced local cerebral blood flow, acidosis, and inflammatory and immune responses. Although the development of minimally invasive technique provides a new option for the treatment of ICH, their application is limited due to the difficulty in achieving accurate puncture localization under the guidance of the marks on CT. We selected 30 patients treated with neuroendosco

Endoscopic removal of a cystic choroid plexus papilloma of the third ventricle: a case report and review of the literature.

Cystic choroid plexus papilloma (CPP) of the third ventricle is rare. The authors have reported a case of cystic CPP resected successfully through neuroendoscopic approach. An 18-year-old young man was admitted to our hospital, presented with blurred vision and paroxysmal, intermittent, severe headache for 1 month as well as nausea and vomiting. The findings of neurological examination were normal except for a bilateral papilledema. Magnetic resonance and computerized tomography revealed a cystic lesion in the third ventricle, which did not furnish a definite diagnosis. The tumor was totally r

Ultrasonic aspiration in neuroendoscopy: first results with a new tool.

Neuroendoscopic techniques are often considered inapplicable to lesion resection because most lesions are too large for effective endoscopic resection in an appropriate time frame. To evaluate the potential of ultrasonic aspiration in neuroendoscopic procedures, the authors developed a new handpiece for endoscopic application. The instrument was subsequently tested in 10 cadaveric pig brains and applied in 5 clinical cases. In the pig brain, a precise and accurate aspiration of ventricular ependyma and brain parenchyma was obtained. Clinically, the device was applied in 3 patients with obstruc

Provision of a neuroendoscopy service. The Southampton experience.

BACKGROUND: A series of 21 patients (aged 1 week to 80 years) underwent a total of 22 neuroendoscopic procedures in our Unit in the period July 1993 to January 1996. METHODS: The procedures were performed by one surgeon familiar with the technique using the Stortz rigid neuroendoscope system. The most common indication for neuroendoscopy was obstructive hydrocephalus. The most frequently performed procedure was third ventriculostomy and tumor biopsy. The intended surgical procedure was successfully performed in all but two of the cases (attempted septostomy and internal cyst drainage) were bo

Neuroanatomical details under endoscopical view--relevant for radiosurgery?

Both, neuroendoscopy and radiosurgery, are upcoming techniques in neurosurgery and become nowadays more and more important. In planning radiosurgical interventions it is very important to have both, the information about the morphology of the pathology itself, and also a clear understanding from the surrounding structures. Neuroendoscopic techniques gives the possibility to demonstrate well known structures without prior dissection. This paper focuses on these anatomical informations which might be relevant in planning further radiosurgical interventions especially in cases of the vascularizat

Endoscopic anatomy of the skull base explored through the nose.

OBJECTIVE: Different surgical approaches have been used over the years in order to access skull base. The endoscopic endonasal approach represents a direct and minimally invasive approach to the suprasellar, retrosellar, and retroclival space, with the advantage of avoid brain retraction and visualize safely and effectively the surgical target. The present contribution aims to provide anatomical details of the skull base as seen from below (i.e., via an endoscopic endonasal approach). METHODS: Five human cadaver heads were dissected. The anatomical neurovascular structures within the skull ba

Endoscopic intraventricular surgery for treatment of hydrocephalus and loculated CSF space in children less than one year of age.

Controversy exists regarding whether children under 1 year of age have a higher risk of treatment failure after neuroendoscopic procedures for the treatment of hydrocephalus than older children. We retrospectively reviewed the surgical management and clinical outcome of 15 infants who underwent neuroendoscopic surgery for the treatment of CSF space loculation and hydrocephalus. We performed 8 third ventriculostomies, 3 endoscopic arachnoid cyst fenestrations, 2 aqueductoplasties, 1 septostomy and in 1 patient, three endoscopic fenestrations for isolated ventricular compartments.
