Advances in the Treatment of Cholinergic Anti-Inflammatory Pathways in Gastrointestinal Diseases by Electrical Stimulation of Vagus Nerve.

BACKGROUND: The cholinergic anti-inflammatory pathway (CAIP) has been proposed as a key mechanism by which the brain, through the vagus nerve (VN), modulates the immune system in the body. Recent studies of VN stimulation (VNS) in vivo systems have shown that it plays an anti-inflammatory role through CAIP. Inflammatory diseases in the gastrointestinal tract are frequent and difficult to treat. SUMMARY: The mechanism of the anti-inflammatory effect of VNS through CAIP is not fully known. The current review covers anatomy, molecular mechanisms, and the application in gastrointestinal diseases

Beyond heart rate variability: vagal regulation of allostatic systems.

The autonomic nervous system (ANS) plays a role in a wide range of somatic and mental diseases. Whereas the role of the ANS in the regulation of the cardiovascular system seems evident, its role in the regulation of other systems associated with allostasis is less clear. Using a model of neurovisceral integration we describe how the ANS and parasympathetic tone in particular may be associated with the regulation of allostatic systems associated with glucose regulation, hypothalamic-pituitary-adrenal (HPA) axis function, and inflammatory processes. Decreased vagal function and heart rate variab

Fat redistribution in HIV-infected patients. A new hormonal-immune disorder?

Multidrug antiretroviral regimes in HIV-infected patients may have side effects. The most frequent side effects are changes in fat metabolism and distribution. We describe a particular pattern of fat redistribution (FR), characterized by a progressive enlargement of breast and abdominal girth and fat loss in the lower limbs, which occurs in approximately 10% of HIV-infected women treated with combined antiretroviral therapy. To elucidate the metabolic, endocrine, and immunologic consequences of the observed disturbance, we measured serum lipids, glucose, C-peptide, ACTH, plasma, urinary cortis

Incision-triggered neuroimmune remodeling of nociceptor ion channels: from postoperative inflammation to pain chronification.

Chronic postsurgical pain (CPSP) arises after a known tissue insult, yet no intervention reliably prevents its development. This Review examines whether incision-triggered inflammation remodels nociceptor voltage-gated ion channels in ways that sustain peripheral input and favor pain chronification. We integrate human genetic and clinical evidence, incision-specific perturbation studies, functional work in non-surgical models, and expression-level associations distinguishing mechanistic strength from directness to CPSP. We focus on NaV1.7, NaV1.8, NaV1.9, and CaV3.2 within a broader network th

Neuroimmune dysregulation in Alzheimer's disease: Mechanisms and therapeutic strategies.

Alzheimer's disease (AD) is a neurodegenerative disorder characterised by progressive memory loss and cognitive decline. With the global population ageing, the prevalence of AD continues to rise, posing a significant public health challenge. Historically, AD research has centred on two hallmark pathological features: β-amyloid (Aβ) deposition and tau protein hyperphosphorylation. However, repeated failures of therapeutic strategies targeting these pathways in clinical trials have prompted a paradigm shift toward a more integrated understanding of disease mechanisms. Accumulating evidence now d

Obesity-driven phosphatidylethanolamine dysregulation impairs neuroimmune crosstalk and accelerates Alzheimer's pathogenesis.

BACKGROUND: Midlife obesity is a major modifiable risk factor for Alzheimer’s disease (AD), yet the lipid-mediated mechanisms linking peripheral metabolic dysfunction to brain pathology remain poorly understood. In particular, how adipose-derived lipid perturbations influence immune and neuronal compartments in the brain has not been fully elucidated. METHODS: We employed an integrative multi-omics approach combining quantitative lipidomics, single-nucleus RNA sequencing, proteomics, and high-resolution imaging to characterize the metabolic alterations associated with obesity in both periphera

MASH-Associated T Cell Activation and Neuroinflammation: Evidence for a Hepatosystemic Immune Axis.

"The liver is the origin of the veins and the source of the blood."-De Usu Partium Galen.Galen's hepatocentric physiology foreshadowed the liver-brain axis, a network linking vagal signaling, cytokines, metabolites, and migratory immune cells. Central to the immune arm of this axis, the liver generates 25 to 50% of thoracic duct lymph, positioning hepatic lymphatic output as a significant, though not yet fully characterized, source of T cells and cytokines in systemic circulation, a contribution that increases with hepatic inflammation in chronic metabolic diseases. We propose that T lymphocyt

Harnessing neuromodulation to target microglial activity in neurodegenerative diseases.

Neurodegenerative diseases are increasingly understood as conditions of disrupted neuroimmune homeostasis in which microglia play an important, dynamic, and state-dependent role. Beyond their immune functions, microglia continuously monitor neuronal activity, participate in synaptic remodeling, and contribute to neural circuit regulation in ways that vary across brain regions, disease types, and stages of neurodegeneration. In parallel, neuromodulation can modify neural activity patterns, thereby influencing downstream cellular responses and immune-related signaling within the central nervous

Microglial State Reprogramming Across the Gut-Brain-Immune Axis: Limitations and Therapeutic Prospects of Traditional Chinese Medicine.

Bidirectional communication among the gut microbiota, immune system, and central nervous system shapes microglial maturation and responsiveness. This review evaluates microglia as context-dependent neuroimmune integrators rather than fixed M1/M2 populations. It organizes the literature into three linked layers: gut-derived signals and barrier physiology, microglial state transitions, and therapeutic modulation by Traditional Chinese Medicine (TCM). Experimental depletion, colonization, and metabolite studies provide causal evidence that the microbiota can modify microglial biology in animals;

Synergistic disruption of blood-brain barrier and neuroimmune homeostasis by sleep-related environmental pollutants drives sleep disorders: an integrated computational and experimental study.

Environmental pollutants are increasingly linked to sleep disorders (SD), affecting 27% of people globally, yet their synergistic effects remain understudied. Network toxicology identified 252 shared targets between sleep-related environmental pollutants (SREPs: NO2, formaldehyde, benzene) and SD. PPI network and diagnostic model identified four hub genes (HSP90AA1, RELA, PTGS2, MMP9) with moderate-to-high predictive value (AUC: 0.708-0.979). Immune infiltration analysis showing elevated T cells and reduced astrocytes and neurons in patients with SD. Molecular simulations confirmed stable SREP

Neurobiological and Neuroimmune Mechanisms Linking Chronic Pain, Sleep Disturbances and Mental Health Disorders.

Chronic pain, sleep disturbances, and mental health disorders such as anxiety and depression disorders frequently co-occur, forming a self-reinforcing cycle that impairs daily functioning and quality of life. Chronic pain is driven by peripheral and central sensitization, the latter sustained by reciprocal microglial-astrocytic crosstalk and maladaptive neuroplasticity. Poor sleep amplifies pain through inflammation and circadian disruption. Imbalances in serotonin, dopamine, and norepinephrine, together with limbic alterations and HPA axis dysregulation, contribute to comorbid anxiety and dep

Neuroimmune Clearance and EEG Biomarkers: A Unified Model of ASD and Dyslexia.

Recent advances in neuroimmunology and cerebrovascular biology have highlighted the important roles of microglial synaptic pruning and the brain's meningeal lymphatic system in shaping neural circuits during development. Disruptions in one or both of these systems have been reported in neurodevelopmental conditions such as autism spectrum disorder (ASD) and developmental dyslexia. This review synthesizes existing evidence suggesting that impaired meningeal lymphatic clearance may be associated with sustained neuroinflammatory states, which in turn could alter microglial homeostasis and contrib

Chronic spontaneous cholinergic urticaria: advances in immunological mechanisms focusing on neuro-immune crosstalk and novel targeted therapeutic strategies.

Chronic spontaneous cholinergic urticaria (CSU-Chol) is a chronic inflammatory skin disorder characterized by intense pruritus and small wheals triggered by a rise in core body temperature. This review aims to systematically elucidate its complex pathogenesis, with a particular focus on the pivotal role of neuro-immune crosstalk in disease initiation and progression. We will provide an in-depth analysis of key immunological components, including aberrant mast cell activation, autoimmune responses, and dysregulation of the cytokine network. Furthermore, the review will detail how neuropeptides

Sensory-secretory neuroimmune circuits of the ocular surface.

The ocular surface is a sensory-rich mucosal barrier in which trigeminal input, autonomic output, tear composition, epithelial stress, glandular secretion, goblet-cell function and local immunity are closely coupled. This review uses the sensory-secretory mucosal immune unit as a tissue-level map across four measurable output domains: secretory function, neural state, immune activity and barrier integrity. The term is not proposed as a new anatomical compartment or as a renamed lacrimal functional unit or eye-associated lymphoid tissue. Direct ocular evidence is distinguished from ocular-adjac

The innervated gut and critical illness.

PURPOSE OF REVIEW: This review highlights brain-gut neuroimmune interactions in the context of critical illness. Neural regulation of inflammation, gut innervation, and the brain-gut axis in critical illness are discussed. RECENT FINDINGS: Recent studies indicate that the brain-gut axis and the enteric nervous system are integral to the regulation of local and systemic inflammation. Experimental evidence suggests that neural reflexes control immune responses, and specific neural signals promote gastrointestinal homeostasis. The understanding of these interactions in the clinical context remai

Pathophysiology of Prurigo Nodularis: Neuroimmune Dysregulation and the Role of Type 2 Inflammation.

Prurigo nodularis (PN) is a chronic, inflammatory skin condition characterized by multiple, intensely pruritic, distinctive nodular lesions. Subsequent scratching can further intensify the pruritus, culminating in a self-reinforcing itch-scratch cycle, which drives lesion development. The latest data indicate dysregulation of the neuroimmune axis in PN pathogenesis, including the involvement of sensory neurons, key effector immune cells, proinflammatory cytokines, dermal fibroblasts, and pruritogens. In this review, we highlight evidence supporting the role of type 2 immune axis dysregulation

Neuroimmune recognition and regulation in the respiratory system.

Neuroimmune recognition and regulation in the respiratory system is a complex and highly coordinated process involving interactions between the nervous and immune systems to detect and respond to pathogens, pollutants and other potential hazards in the respiratory tract. This interaction helps maintain the health and integrity of the respiratory system. Therefore, understanding the complex interactions between the respiratory nervous system and immune system is critical to maintaining lung health and developing treatments for respiratory diseases. In this review, we summarise the projection di
