The authors report a patient with nevus of Ota (oculodermal melanocytosis) who presented with pigmented epithelioid melanocytoma in the right orbit. A 36-year-old female patient attended with proptosis OD and diplopia for 3 months. MRI disclosed an expansive lesion adhered to the right inferior rectus muscle, enlargement of the superior orbital fissure, and filling of the right cavernous sinus. Excision of the orbital mass through a transconjunctival inferior fornix access disclosed a heavily pigmented lesion. Clinical and experimental evidence explaining and supporting the role of UV radiation as a causal factor for the induction and promotion of nonmelanoma and malignant melanoma skin cancer are presented. The association of two different neoplasms in the same biopsy specimen is not uncommon and has been reported by numerous authors. Some associations may result from involvement of related cell types, however, most are serendipitous. Our study attempted to investigate the nature and types of contiguous or "collision" tumors seen in our dermatopathology service. Meyerson phenomenon (MP) is characterized by a symmetrical area of erythema and scales encircling a central lesion, which is most commonly a banal melanocytic nevus. In several studies from northern Europe, north America and Australia, melanocytic nevi are correlated with pigmentary traits and with intense sun exposure in a way similar to malignant melanoma. During the last four decades the incidence of malignant melanoma has increased seven fold. Clinically, it is often difficult to distinguish malignant melanoma from a benign naevus. The macroscopic clinical ABCD rule and the Glasgow 7-point checklist are didactically helpful, but inaccurate with many false positive and negative diagnoses. Dermatoscopy performed by a trained physician has increased the diagnostic accuracy to a sensitivity of 89% and specificity of 80% (meta-analysis of 11 studies). The dermatoscopic ABCD rule and the Risk Stratification method (RS) are reviewed.
