Reflections on 20 Years of Research on Tai Chi for Chronic Musculoskeletal Pain: An Updated Review.

PURPOSE OF REVIEW: Chronic musculoskeletal pain (CMP), including osteoarthritis (OA), rheumatoid arthritis (RA), fibromyalgia, and chronic low back pain (cLBP), contributes substantially to disability and reduced quality of life. Contemporary pain science supports biopsychosocial, multimodal management addressing nociceptive, neuropathic, and nociplastic mechanisms. Tai Chi is a multicomponent mind-body exercise integrating gentle movement, breathing, relaxation, balance, and attentional focus. This narrative review summarizes approximately 20 years of research on Tai Chi for CMP and considers

A novel repurposing opportunity for valsartan: angiotensin II type 1 receptor blockade dampens muscle-to-brain sensitization in fibromyalgia by targeting lncRNA H19/miR-29a-3p, inflammatory mediators, AKT/CREB signaling, neurotrophic factors, and amyloid-Tau pathology.

Fibromyalgia (FM) is a debilitating nociplastic pain disorder increasingly recognized to involve skeletal muscle as an active contributor to peripheral nociception and central sensitization rather than a passive target of disease. Although angiotensin II promotes pain signaling through angiotensin type-1 (AT1) receptor activation, the role of skeletal muscle AT1 receptor signaling in orchestrating the inflammatory, trophic, and proteotoxic disturbances underlying FM remains unexplored. This study investigated, for the first time, whether AT1 receptor blockade by valsartan interrupts maladaptiv

Electrical injury: Chronic pain, somatosensory dysfunction, post traumatic stress and movement disorders.

INTRODUCTION AND AIMS: We aimed in this case series to identify shortcomings in assessment of long-term painful and psychosocial consequences of EI and to demonstrate the value of biopsychosocial assessment and the commonalities in outcomes. METHODS: We retrospectively analyzed 15 cases from 2004 to 2019 of adult claimants assessed in a medico-legal practice for complex chronic pain disorders secondary to EI. Extensive biopsychosocial information, including 165 data items on pre- and post-injury observations, were collected on each. Cutaneous and deep pressure somatosensory examination was pe

In-hospital outcomes of fibromyalgia patients undergoing primary total hip arthroplasty: a seven-year United States National Inpatient Sample propensity-matched analysis.

Fibromyalgia (FM) is a chronic nociplastic pain syndrome characterized by widespread pain, fatigue, and sensory amplification. While FM is linked to increased perioperative resource use and poorer patient-reported outcomes, its influence on in-hospital complications after total hip arthroplasty (THA) remains unclear. We analyzed the National Inpatient Sample (2016-2022) for adults undergoing elective primary THA for osteoarthritis. FM was defined as ICD-10-CM code M79.7. Exclusions included fracture, revision arthroplasty, infection, malignancy, inflammatory arthritis, COVID-19, and non-electi

Mechanisms Underlying Pain Avoidance in Complex Regional Pain Syndrome.

BACKGROUND: Maladaptive pain avoidance plays a critical role in chronic pain development and maintenance. Pain avoidance reinforced instrumentally through action-outcome contingencies remains poorly understood, specifically in complex regional pain syndrome (CRPS), a nociplastic condition. We aim to identify mechanisms underlying maladaptive avoidance to inform interventions and improve quality of life. METHODS: Twenty-eight lower extremity CRPS and twenty-seven healthy (HC) participants performed an instrumental pain avoidance task in which they chose between two options linked to different

Comparing three fibromyalgia screening tools: diagnostic accuracy and clinical utility.

OBJECTIVES: Brief self-administered screening tools have been developed to facilitate rapid recognition of fibromyalgia (FM) features. This study evaluated the diagnostic performance of three commonly used screening tools. METHODS: In this cross-sectional study, consecutive female patients with chronic multisite pain completed three FM screening tools, respectively the Fibromyalgia Rapid Screening Tool (FiRST), the Nociplastic-Based Fibromyalgia Features (NFF) tool, and the Self-Administered Fibromyalgia Screening (SIFIS) questionnaire. Patients were dichotomised in FM (2016 ACR criteria) and

Understanding the Psychophysiological and Sensitization Mechanisms Behind Fibromyalgia Syndrome: A Network Analysis Approach.

OBJECTIVE: Current evidence suggests that fibromyalgia syndrome (FMS) involves complex underlying mechanisms. This study aimed to quantify the multivariate relationships between clinical, psychophysical, and psychological outcomes in women with FMS by using network analysis to understand the psychobiological mechanisms driving FMS and generating new research questions for improving treatment strategies. METHODS: Demographic (age, height, weight), clinical (pain history, pain intensity at rest and during daily living activities), psychophysical (widespread pressure pain thresholds [PPT]), sens

Low back pain.

Low back pain covers a spectrum of different types of pain (eg, nociceptive, neuropathic and nociplastic, or non-specific) that frequently overlap. The elements comprising the lumbar spine (eg, soft tissue, vertebrae, zygapophyseal and sacroiliac joints, intervertebral discs, and neurovascular structures) are prone to different stressors, and each of these, alone or in combination, can contribute to low back pain. Due to numerous factors related to low back pain, and the low specificity of imaging and diagnostic injections, diagnostic methods for this condition continue to be a subject of cont

Reconsolidation blockade with propranolol as a novel treatment for fibromyalgia: A double-blind, randomized placebo-controlled feasibility study.

Nociplastic pain conditions such as fibromyalgia are characterized by changes in the central nervous system similar to those observed in post-traumatic stress disorder (PTSD). Reconsolidation therapy is a treatment for PTSD consisting in reactivating synapses underlying the excessive threat response and blocking their reconsolidation using propranolol. This double-blind, randomized pilot feasibility study aimed to assess the feasibility and acceptability of reconsolidation therapy with propranolol or a placebo in individuals with fibromyalgia, and to gather preliminary data regarding treatment

Central sensitization symptoms in vulvodynia: exploring the role of temperament, personality traits, childhood adverse events, defense mechanisms, and mental pain on quality of life.

BACKGROUND: Vulvodynia is a chronic pain syndrome characterized by persistent vulvar pain, occurring without clinically identifiable disorders. Central sensitization (CS) is suggested to play a role in the pathophysiology of vulvodynia, as for other nociplastic pain conditions. AIM: This study delves into the complex interplay between psychosocial factors and CS burden in women with vulvodynia, aiming to identify potential predictors (temperament, personality traits, childhood adverse events, defense mechanisms, and mental pain) and understand their impact on quality of life (QoL). Evidence-Based Self-Management Strategies for Fibromyalgia: Foundations for Digital Therapeutic Applications.

Fibromyalgia is a prevalent musculoskeletal pain condition that causes major personal, social, and societal burden. Pharmacological therapies often provide only limited benefit, making multimodal approaches and self-management the cornerstones of care. Such strategies, spanning lifestyle modification, physical activity, psychoeducation, and cognitive-behavioral approaches, target the biopsychosocial complexity of fibromyalgia and promote sustainable coping. In parallel, digital health technologies are transforming how these interventions can be delivered and coordinated in the form of digital

A systematic review and narrative synthesis of functional neuroimaging studies for chronic ocular surface pain.

Chronic ocular surface pain (COSP) is a debilitating condition of the eye driven by nociceptive, nociplastic, and neuropathic mechanisms. While individual studies using functional neuroimaging techniques to investigate ocular pain exist, the full extent, methodologies, and cumulative findings of this body of literature have not been systematically synthesized. Using a systematic review approach, we identified 5 articles evaluating the use of functional neuroimaging in COSP patients from an initial pool of 643 unique studies. All studies used functional magnetic resonance imaging exclusively as

Exploring Chronic Pain in Hemodialysis Patients: An Observational Study Based on the New IASP Classification for ICD-11.

INTRODUCTION: Pain is one of the most frequently reported symptoms in hemodialyzed (HD) patients, with prevalence rates between 33% and 82%. Risk factors for chronic pain in HD patients are older age, long-lasting dialysis history, several concomitant diseases, malnutrition, and others. However, chronic pain assessment in HD patients is rarely performed by specialists in pain medicine, with relevant consequences in terms of diagnostic and treatment accuracy. Furthermore, no study has used the recently introduced International Association for the Study of Pain (IASP) pathophysiological classifi

Elevated highly sensitive C-reactive protein in fibromyalgia associates with symptom severity.

OBJECTIVES: Fibromyalgia (FM), a common pain syndrome, is thought to be a non-inflammatory, nociplastic condition, but evidence implicating neuroinflammation has been increasing. Systemic inflammation may be associated with more severe symptoms in some FM patients. We studied healthy controls and FM patients with and without systemic inflammation detectable using high-sensitivity CRP (hsCRP) measurement. METHODS: We measured hsCRP levels and gathered clinical and questionnaire data [including the Fibromyalgia Impact Questionnaire (FIQ)] from 40 female FM patients and 30 age-matched healthy wo

Temporal Summation of the Thermal Grill Illusion is Comparable to That Observed Following Noxious Heat.

The thermal grill illusion (TGI) describes a peculiar or even painful percept caused by non-noxious, interlaced warm and cold stimuli. It involves the glutamatergic system and is affected in putatively nociplastic syndromes such as fibromyalgia. The glutamatergic system is also involved in wind-up, that is, the increased activation of spinal neurons following repeated noxious stimulation leading to a temporal summation of perceived stimulus intensity. Here we combined both stimulation methods to further investigate whether non-noxious stimuli as employed in the TGI can lead to a similar summat

Fibromyalgia in cancer patients: a systematic review and clinical implications for integrated care.

INTRODUCTION: In oncology, fibromyalgia (FM) is increasingly recognized as a clinically relevant comorbidity, as cancer patients frequently present overlapping symptoms such as pain, fatigue, and sleep disturbances, complicating clinical assessment and management. Moreover, anticancer treatments may trigger or exacerbate FM-like symptoms. This systematic review aimed to synthesize the available evidence on FM in oncological patients, focusing on prevalence, clinical characteristics, and its impact on pain perception, Health-Related Quality of Life (HRQoL), and treatment adherence. METHODS: A

Dorsal root ganglion stimulation for the treatment of joint pain with predominantly nociceptive characteristics: A case series.

INTRODUCTION: Dorsal root ganglion stimulation (DRG-S) has recently emerged as a novel therapy in neuromodulation that demonstrated a higher rate of success than spinal cord stimulation (SCS) in a prospective, head-to-head randomized comparative trial to treat complex regional pain syndrome (CRPS) and causalgia. In contrast to SCS, DRG-S also shows promise in treating conditions that are not purely neuropathic such as axial low back pain, which has a prominent nociplastic pain component. It is not known to what extent the effectiveness of DRG-S for such indications is due to effective treatmen
