Assessment of hearing sequelae in individuals tested post-trauma.

BACKGROUND: Over 5% of the global population (430 million people) require rehabilitation for hearing loss. Individuals with hearing impairments face significant challenges in business, daily life, and social participation. Hearing loss (HL) and other permanent physical and sensory disabilities escalate dramatically in cases with brain damage and temporal bone trauma associated with head injuries. This study aims to identify the significant risk factors for hearing loss following head trauma, utilizing current data, and discuss the findings in the context of the literature. This could contribut

Clinical practice guidance for imaging selection in pulsatile tinnitus: an evidence-based expert consensus.

Pulsatile tinnitus (PT) is a rhythmic sound synchronized with the patient's heartbeat that represents a complex diagnostic challenge with a broad spectrum of vascular and non-vascular etiologies. Persistent symptoms can severely impact mental health, yet a specific causative abnormality can be identified via targeted radiological imaging in more than 75% of cases. Improper imaging selection frequently leads to misdiagnosis, reduced therapeutic efficacy, and increased healthcare costs. The choice of imaging protocol must be guided by a thorough initial clinical evaluation, including detailed me

Preservation of Inner Ear Function in a Case of Vestibular Fistula Caused by Cholesteatoma Using the Underwater Technique.

A 70-year-old male presented with intermittent vertigo, dizziness, and left facial spasms. Otoscopic examination and temporal bone computed tomography revealed extensive cholesteatoma with lateral semicircular canal erosion and a vestibular fistula. Preoperative audiometry demonstrated severe mixed hearing loss with partial preservation of bone conduction thresholds. The patient underwent canal wall-down tympanomastoidectomy using the underwater technique, which involved continuous saline irrigation to stabilize the inner ear environment and minimize perilymph leakage. The fistula site was sea

Expanding the Capacity of General Practitioners in Sub-Saharan Africa With Artificial Intelligence.

Low- and middle-income countries face substantial healthcare delivery challenges due to specialist shortages. This study assessed POE (Q-Exp-Claude-3.5-2kk), a multimodal large language model, for diagnostic accuracy and clinical utility in otolaryngology across 63 consecutive patients in rural Kenya. Patients underwent evaluation by both an otolaryngologist and primary care practitioner (PCP), with clinical data and micro-otoscopic images submitted to POE's interface. POE's primary diagnoses demonstrated 79.4% concordance with otolaryngologist findings, while management recommendations aligne

Foreign Body in the Eustachian Tube: A Challenging Diagnosis and Management.

Foreign bodies in the external ear are very common. The same cannot be said about foreign bodies in the Eustachian tube (ET). We report the case of a 63-year-old woman with a history of painless left side otorrhea and hearing loss. She reported a left ear surgery when she was 30-year-old but she did not know the diagnosis that was made at that time neither the kind of surgery performed. Otoscopic examination revealed an inferior perforation of the eardrum. Audiologic evaluation demonstrated a unilateral, moderate-severe mixed hearing loss. Computed tomography scan showed, in left ear, a soft t

Posterior condylar canal dural arteriovenous fistula as a rare cause of glossopharyngeal neuralgia: A case report.

BACKGROUND: The symptoms of dural arteriovenous fistula (dAVF) vary according to the location of the fistula and the pattern of venous drainage. Here, we report the case of a patient with a dAVF-induced glossopharyngeal neuralgia. CASE DESCRIPTION: We report a case of a patient with right glossopharyngeal neuralgia caused by a posterior condylar canal (PCC) dAVF. The glossopharyngeal neuralgia was accompanied by persistent tinnitus and repetitive right side otalgia, as well as ipsilateral shoulder and throat pain, lasting for about 30 s. However, there were no specific findings on otoscopic e

Undifferentiated nasopharyngeal cancer extending to maxillary sinus: a case report.

Undifferentiated nasopharyngeal cancer of the cavum (UCNT) is the most frequent neoplasm of the nasopharynx, having a close relationship with exposure to Epstein-Barr virus. It has a high potential for locoregional or distant invasion which are the cause of some treatment failures. The extension to the maxillary sinus is rarely described. We report here the case of a 38-year-old patient with headaches associated with epistaxis, left otalgia and facial pain. Examination by anterior rhinoscopy objectively revealed a polylobed ulcerating mass. Otoscopic examination revealed a left seromucous otit

A review of Horner's syndrome in small animals.

Horner's syndrome arises from dysfunction of the oculosympathetic pathway and is characterized by miosis, enophthalmos, protrusion of the third eyelid, and ptosis. It has been recognized in a wide variety of breeds and ages in small animal patients. The oculosympathetic pathway is a 3-neuron pathway. The central/first order neuron arises from the hypothalamus and extends down the spinal cord. The preganglionic/second order neuron arises from the first 3 thoracic spinal cord segments and travels through the thorax and cervical region until it synapses at the cranial cervical ganglion. The postg

Promontory osteoma mimicking otosclerosis: case report.

OBJECTIVE: This case report presents a middle-ear osteoma mimicking otosclerosis that was located at the promontory. The osteoma was successfully excised using an endoscopic transcanal approach without any complication. CASE REPORT: A 21-year-old man presented with a 4-year history of progressive conductive hearing loss (47 dB with a 30-dB air-bone gap) with intermittent tinnitus of recent onset in his right ear. Endoscopic transcanal middle-ear exploration showed that an osteoma located on the promontory was restricting the mobility of the stapes by affecting the anterior crus of the stapes.

Surgical management of giant intrapetrous internal carotid aneurysm presenting with coil exposure after endovascular treatment.

Giant intrapetrous internal carotid aneurysms (petrous ICA aneurysm) are rare. A giant petrous ICA aneurysm presenting with otorrhagia and coil exposure to the external auditory meatus (EAM) after endovascular treatment has never been documented before. The authors report here a case of successful surgical trapping with bypass intervention of a giant petrous ICA aneurysm presenting with coil exposure after endovascular treatment. A 58-year-old man presented with persistent otorrhagia having been admitted to our hospital because of the recurrence of a giant petrous ICA aneurysm after repeated e

Middle ear and hearing disorders of schoolchildren aged 7-10 years in South Sinai, Egypt.

Hearing loss among schoolchildren in developing countries is reported to be a significant health problem. Data on child hearing loss in South Sinai, a remote governorate of Egypt, are lacking. Middle ear diseases and hearing impairment were assessed among 453 primary-school children aged 7-10 years in South Sinai (906 ears). Otoscopic examination, tympanometry and pure tone audiometry (PTA) were done. Ear disease was found in 27.5% of the ears examined. The commonest cause was secretory otitis media (10.8%), followed by occluded earwax (9.5%). Mild and moderate hearing loss affected 8.5% of th

Bilateral aberrant internal carotid arteries: a case report presenting with pulsatile middle ear discharge.

An aberrant internal carotid artery (AICA) is a rare vascular anomaly. Misdiagnosis may lead to massive hemorrhage or other life threatening complications during even a minor surgical procedure of the tympanic membrane or middle ear. Knowledge of this rare entity is essential to any surgeon who undertakes myringotomy and middle ear surgery. Only 14 cases of bilateral AICA were reported in literature. We present a very rare case with bilateral AICA presenting with pulsatile middle ear discharge from his left ear and left sided conductive hearing loss. Tympanic membrane perforation and middle ea

[Tuberculous Otitis media - a rare differential diagnosis in Germany].

A 28-year-old female patient with a migrant background presented for surgery with a suspected cholesteatoma in the left ear. The patient reported having had an aural discharge for several months; otoscopic examination revealed a runny ear, and discrete granulation tissue was seen. Pure-tone audiometry showed conduction hearing loss of 30-40 dB across all frequencies in the left ear; high-resolution computed tomography of the temporal bone revealed that the mastoid and tympanic cavity were completely obscured. The intraoperative finding showed a caseous space-occupying mass that completely fill

Genetic predisposition for tympanosclerotic degeneration.

Atherosclerosis and tympanosclerosis are disorders that affect different organs in the body. When tissue samples taken from atherosclerotic and tympanosclerotic lesions are compared under light and electron microscopes, common pathological characteristics become apparent in both tissues. These similarities indicate that genetic predisposition to atherosclerosis is also seen in the middle ear. For the purpose of evaluating this genetic predisposition, the incidence of tympanosclerosis in atherosclerotic patients was examined and compared with the normal population. Otoscopic examination of 1,02

Transient otoacoustic emissions in children with chronic renal failure.

This study was conducted on 34 children with chronic renal failure: 27 on regular haemodialysis and seven on conservative treatment. Twenty normal healthy children served as controls. They were subjected to clinical examination including otoscopic examination, basic audiological assessment and transient otoacoustic emission testing (TOAE). Four patients had a conductive hearing loss and five had a bilateral moderately severe high frequency sensory neural hearing loss. TOAE testing was carried out for the rest whose ears displayed normal hearing. No response (fail) was obtained in 8% of them bu
