Permanent catheters for recurrent ascites-a critical and systematic review of study methodology.

PURPOSE: Management of refractory ascites traditionally includes medical treatment with diuretics or intermittent paracentesis. Patients with recurrent ascites may benefit from the use of permanent intra-abdominal catheters with more frequent drainage without hospitalization. The objective was to systematically asses the methodology of factors and endpoints reported in studies investigating permanent catheters for recurrent ascites treatment. METHODS: Using a systematic search strategy, we critically assessed the methodology when treating refractory ascites using a permanent catheter. Studies

Standardization of the Cirrhosis Admission Process and Its Impact on Inpatient Management and Patient Outcomes.

INTRODUCTION: An increase in cirrhosis-related hospitalizations has highlighted opportunities for improved patient care and standardization of practice. We created a cirrhosis admission order set with the goal of promoting guideline-directed care and improving patient outcomes. METHODS: We developed and implemented a cirrhosis order set that included dietary recommendations, diagnostic paracentesis orders and studies, common complications of portal hypertension and educational materials at time of discharge. We used a retrospective cohort design to compare post-intervention process measures a

Ascites and Chronic Liver Disease in Children.

Development of ascites in children with chronic liver disease is the most common form of decompensation. It is associated with a poor prognosis and increased risk of mortality. A diagnostic paracentesis should be performed in liver disease patients with- new-onset ascites, at the beginning of each hospital admission and when ascitic fluid infection (AFI) is suspected. The routine analysis includes cell count with differential, bacterial culture, ascitic fluid total protein and albumin. A serum albumin-ascitic fluid albumin gradient of ≥1.1 g/dL confirms the diagnosis of portal hypertension. As

'Blue bubble' technique: an ab interno approach for Descemet separation in deep anterior lamellar keratoplasty using trypan blue stained viscoelastic device.

BACKGROUND: In this study, we examined a novel variant of 'big-bubble' deep anterior lamellar keratoplasty using trypan-blue-stained viscoelastic device for the creation of a pre-descemetic bubble. METHODS: Ten corneoscleral rims were mounted on an artificial anterior chamber (AC). The AC was filled with air through a limbal paracentesis. A Melles' triangulated spatula was inserted through the paracentesis, with its tip penetrating the AC, was then slightly retracted and pushed into the deep stroma above the roof of the paracentesis. A mixture of trypan blue and viscoelastic device (Healon, A

Contributions of immunoblotting, real-time PCR, and the Goldmann-Witmer coefficient to diagnosis of atypical toxoplasmic retinochoroiditis.

Ocular toxoplasmosis is a major cause of posterior uveitis worldwide. The diagnosis is based mainly on ophthalmological examination. Biological diagnosis is necessary in atypical cases, and this requires aqueous humor sampling by anterior chamber paracentesis. We evaluated real-time PCR targeting the Toxoplasma gondii 529-bp repeat element, the Goldmann-Witmer coefficient (GWC), and immunoblotting for the diagnosis of toxoplasmic retinochoroiditis in 54 patients with atypical uveitis. The results of these biological tests, applied to paired aqueous humor-serum samples, were compared to the cli

Ocular penetration of intravenously administered enrofloxacin in the horse.

REASON FOR PERFORMING STUDY: Information on antibiotic concentrations in the equine eye following systemic therapy is limited. Reports that Leptospira spp. are frequently present in the eyes of horses with recurrent uveitis, emphasises a need for studies on ocular concentrations of specific antibiotics. HYPOTHESES: 1) Enrofloxacin, administered i.v. at 7.5 mg/kg bwt q. 24 h, results in aqueous humour concentrations greater than the reported minimum inhibitory concentration (MIC) for Leptospira pomona. 2) Aqueous humour paracentesis sufficiently disrupts the blood-aqueous humour barrier (BAB)

Pathophysiology, diagnosis and treatment of ascites in cirrhosis.

The mechanism by which ascites develops in cirrhosis is multifactorial Severe sinusoidal portal hypertension and hepatic insufficiency are the initial factors. They lead to a circulatory dysfunction characterized by arterial vasodilation, arterial hypotension, high cardiac output and hypervolemia and to renal sodium and water retention. There are evidences that arterial vasodilation in cirrhosis occurs in the splanchnic circulation and is related to an increased synthesis of local vasodilators. Vascular resistance is normal or increased in the remaining major vascular territories (kidney, musc

Clinical pattern of extra-uterine gestation in peri-urban Kenya.

One hundred and sixty five cases of extra-uterine pregnancy (EP) were managed in Thika District in peri-urban Kenya between January 1985 and June 1988, giving an incidence of 66:10,000 deliveries. 55.8% of the patients were in the age group 23-27 years, and 88.5% were para 0-2. 78.8% of the extra-uterine pregnancies were ruptured at the time of diagnosis. Paracentesis was performed in 126 cases with 13.5% false negatives. There were seven false positive paracentesis. All the false positives had conditions that would probably have required surgical intervention in any case. There were two patie

Severe ovarian hyperstimulation syndrome: a reevaluated therapeutic approach.

During the 10 years 1978 to 1987, 33 patients were hospitalized because of moderate and severe ovarian hyperstimulation syndrome (OHSS) in 39 treatment cycles. Twenty-five treatment cycles ended in moderate OHSS (group A), 7 had severe OHSS without a significant amount of ascites (group B1), and 7 had severe OHSS with ascites (group B2). Groups A and B1 received intravascular volume expander, electrolytes replacement, and indomethacin up to 300 mg/day. The patients in group B2 had significant clinical and biochemical improvement after abdominal paracentesis. Urinary output and creatinine clear

Reversal of retinal and optic disc ischemia in a patient with sickle cell trait and glaucoma secondary to traumatic hyphema.

A 14-year-old black boy with sickle cell trait, who sustained a traumatic hyphema, developed moderately elevated intraocular pressure that failed to respond to carbonic anhydrase inhibitors and osmotic agents. On the tenth postinjury day, a sudden increased cupping of the optic disc and partial central retinal artery obstruction caused painless loss of vision. Reversal of the cupping, the retinal ischemia, and the intraocular pressure was documented following anterior chamber paracentesis, and visual acuity returned to 6/6. Pathophysiology of the posterior ischemia is discussed. This case docu

Cirrhotic ascites review: Pathophysiology, diagnosis and management.

Ascites is a pathologic accumulation of peritoneal fluidcommonly observed in decompensated cirrhotic states. Its causes are multi-factorial, but principally involve significant volume and hormonal dysregulation in the setting of portal hypertension. The diagnosis of ascites is considered in cirrhotic patients given a constellation of clinical and laboratory findings, and ultimately confirmed, with insight into etiology, by imaging and paracentesis procedures. Treatment for ascites is multi-modal including dietary sodium restriction, pharmacologic therapies, diagnostic and therapeutic paracentes

Refractory Ascites: Pathogenesis, Clinical Impact, and Management.

Refractory ascites is defined as ascites that does not recede or that recurs shortly after therapeutic paracentesis, despite sodium restriction and diuretic treatment. To date, there is no approved medical therapy specifically for refractory ascites. Management of these patients is based upon procedures such as large-volume paracentesis and transjugular intrahepatic portosystemic shunts (TIPS), which temporarily alleviate symptoms but are not curative. These patients have a poor prognosis and are at risk for a series of complications that are associated with the condition or are secondary to t

Corneal blood staining secondary to hemorrhagic descemet membrane detachment.

PURPOSE: To present a case of corneal blood staining due to a hemorrhagic Descemet membrane detachment and describe a method of surgical intervention. METHODS: Observation of clinical features and outcome of surgical intervention. RESULTS: A 72-year-old man had an anterior-chamber intraocular lens removed for presumed uveitis-glaucoma-hyphema syndrome, and he developed an intraoperative hemorrhagic detachment of the Descemet membrane attributed to peripheral corneal neovascularization. Corneal blood staining rapidly developed, and a partial-thickness paracentesis was required to evacuate the

Medieval Islamicate Surgery and the Evolution of Ascites Management: Early Principles of Patient Selection, Controlled Drainage, and Postoperative Care.

Ascites management is currently guided by well-established surgical principles, including careful patient selection, controlled fluid removal, and prevention of circulatory dysfunction during paracentesis. Although framed in modern physiological terms, these principles have conceptual roots in earlier medical traditions. This study re-examines the surgical management of ascites within the medieval Islamicate medical tradition, encompassing medical cultures and scholarly practices that developed in societies shaped by Islamic civilization, rather than referring solely to religious doctrines or

Simultaneous portosystemic shunt and proximal splenic artery embolization for patients with therapy-refractory hepatic encephalopathy at risk of portal hypertensive complications.

BACKGROUND: Hepatic encephalopathy (HE) is a well-known complication of cirrhosis. When HE recurs despite optimal treatment or without a clear precipitant, a portosystemic shunt may be the culprit. We present a case of therapy-refractory HE caused by a spontaneous portosystemic shunt (SPSS). CLINICAL CASE: A 69-year-old male with MASLD-related cirrhosis, with a medical history of biliopancreatic diversion and variceal bleeding, presented with his fifth episode of HE despite maximal pharmacological treatment. Initial lab tests and infectious screen were negative. Abdominal CT scan showed a par
