Acoustic biast trauma.

In this study 26 cases of blast injury to ear were studied. Both ears were affected in 9 cases and therefore the number of ears studied being 35. The audiological findings along with the treatment and a followup of 2 years is presented. We found that mixed deafness was commonest and the sensorineural element of the hearing loss started recovering before conductive element, but recovery slowed down later on and was incomplete in most cases. Approximation of torn fragments of tympanic membrane improved its healing. Vertigo lasting for 1-7 days was present in 46.15% of cases. Only one patient had

Targeted inner ear delivery of gadolinium using microbubble-assisted ultrasound in an ovine model.

Current strategies for treating sensorineural hearing loss include auditory rehabilitation, cochlear implantation, and systemic or local drug administration. Transtympanic injection (TTI) allows local drug delivery to the middle ear, but drug diffusion through the round window membrane (RWM) into the inner ear (IE) remains inconsistent. Microbubble-assisted ultrasound (MB-assisted US) has emerged as promising modality to enhanced RWM permeability. While feasibility and safety have been demonstrated in small animal models, translational validation in large mammals is necessary. This study aimed

The Endolymph Signal in Non-contrast Enhanced 3D-real IR Image Differs between the Ears with and without Significant Endolymphatic Hydrops.

PURPOSE: To investigate whether significant differences exist in labyrinthine lymph fluid signal intensities on non-contrast-enhanced 3D real inversion recovery (3D-real IR) images between patients with and without significant endolymphatic hydrops (EH), potentially enabling the non-contrast detection of EH. METHODS: Thirty-nine patients suspected of having EH underwent 3D-real IR MRI before and 4 hours after intravenous administration of a single dose of gadobutrol. Signal intensities of the cerebrospinal fluid (CSF), perilymph, and endolymph were manually measured on pre-contrast images usi

MicroRNA Profiling as a Methodology to Diagnose Ménière's Disease: Potential Application of Machine Learning.

OBJECTIVE: Diagnosis and treatment of Ménière's disease remains a significant challenge because of our inability to understand what is occurring on a molecular level. MicroRNA (miRNA) perilymph profiling is a safe methodology and may serve as a "liquid biopsy" equivalent. We used machine learning (ML) to evaluate miRNA expression profiles of various inner ear pathologies to predict diagnosis of Ménière's disease. STUDY DESIGN: Prospective cohort study. SETTING: Tertiary academic hospital. SUBJECTS AND METHODS: Perilymph was collected during labyrinthectomy (Ménière's disease, n = 5), staped

Poly-Lactic Acid-Based Biopolymer Formulations Are Safe for Sustained Intratympanic Dexamethasone Delivery.

HYPOTHESIS AND BACKGROUND: The clinical treatment of sudden sensorineural hearing loss currently relies on the administration of steroids, either systemically or via intratympanic injections. Intratympanic injections bypass the hemato-cochlear barrier, reducing its systemic side effects. The efficacy of the injections is limited through rapid drug clearance via the Eustachian tube, and through nonoptimal properties of slow-release drug carriers. A new slow-release drug delivery vehicle based on hexyl-substituted-poly-lactic-acid (HexPLA), with the highest possible safety profile and complete b

CO2 laser stapedotomy.

This clinical study was preceded by two laboratory experiments. The first experiment compared temperature changes in the vestibule while vaporizing a 0.6-mm stapedotomy with Argon, KTP-532, and CO2 lasers. Data demonstrated that the CO2 laser possesses superior tissue characteristics for stapedotomy. In the second experiment safe energy parameters were established for various Sharplan CO2 laser models. Using these safe power settings, 153 consecutive CO2 laser stapedotomies were performed under local anesthesia. No patient experienced intraoperative dizziness during or immediately following th

Quantification of furosemide from serum and tissues using high-performance liquid chromatography.

Since pharmacokinetics may play a significant role in furosemide (FSM) developmental ototoxicity, we developed an assay for the extraction and quantification of FSM in tissue and fluid from neonatal and adult rats. Rats from post-natal day (PND) 10, 30 and 50, were given an intravenous dose of FSM (35 mg/kg). Blood and tissues were analyzed by HPLC. FSM in serum, perilymph and liver was elevated in PND ten rats as was the body burden of FSM. Renal concentrations were higher in older rats. Altered clearance of FSM in developing rats may result in higher concentrations in the cochlea and ototoxi

Combining optimized three-dimensional ZOOMit real inversion recovery with T2-preparation to shorten the delay interval and scan time for endolymphatic hydrops evaluation in patients with Ménière's disease.

BACKGROUND: The scan times and delay intervals of three-dimensional real inversion recovery imaging are excessively long, which limits its widespread use in clinical settings. This study aimed to determine whether an optimized three-dimensional zoomed imaging technique with parallel-transmission (ZOOMit) sampling perfection with application-optimized contrasts using different flip-angle evolutions (SPACE) real inversion recovery under T2 preparation protocols could evaluate endolymphatic hydrops with a short delay interval and scan time. METHODS: Consecutive participants were recruited for th

Traumatic Stapediovestibular Dislocation With Intact Stapes: Report and Review.

Stapediovestibular dislocation is a rare disorder as a result of traumatic injury to the structures in the middle ear. We described a case of a 60-year-old female with stapediovestibular dislocation with associated perilymph fistula. She presented with symptoms including hearing loss, vertigo, and tinnitus after a penetrating injury by an ear pick. After 4 months of conservative management, her symptoms failed to improve. Therefore, she underwent surgery which resolved completely her vestibular symptoms and her hearing loss had partially improved. The restoration of the stapes to its normal an

Perilymphatic gusher in stapedectomy: demonstration of a fistula of internal auditory canal.

Gusher is a very rare phenomenon, generally associated with congenital stapes fixation or otosclerosis in adult age, that may present during stapedectomy. A sudden perilymph flow occurs following platinotomy, due to congenital malformation (abnormally wide cochlear aqueduct or internal auditory canal fistula), that causes an abnormal connection between subarachnoid and perilymphatic spaces. This report deals with a case of bilateral gusher, occurring during stapedectomy, caused by an osseous fistula between bottom of internal auditory canal and the osseous labyrinth later observed at computed

Biocompatible membrane of PDMS for the new chamber prosthesis stapes.

Stapes protheses are designed for patients with otosclerosis resulting immobilization or significant reduction of the stapes mobility. All currently used prostheses are called - piston prosthesis. However, its use to stimulate the cochlea is still imperfect. New chamber stapes prosthesis allows the perilymph excitation more effective than the piston prothesis. Moreover, the chamber prosthesis eliminates the common causes of piston-stapedotomy failures. The most important element of the new prosthesis is a flexible membrane. The membrane stiffness should be close to the stiffness of normal annu

Effects of perilymphatic pressure, sodium nitroprusside, and bupivacaine on cochlear fluid pH of guinea pigs.

CONCLUSIONS: Hydrostatic positive pressure and vasoconstrictor acidified the cochlear fluids, whereas the vasodilator made the fluids alkaline. CBF might play a role in regulating cochlea fluid pH. OBJECTIVES: Cochlea fluid pH is highly dependent on the HCO3(-)/CO2 buffer system. Cochlear blood flow (CBF) supplies O2 and removes CO2. It is speculated that cochlear blood flow changes might affect the balance of the HCO3(-)/CO2 buffer system in the cochlea. It is known that the elevation of inner ear pressure decreases the CBF, and local application of vasodilating or vasoconstricting agents di

Effect of glycerol and mannitol on perilymphatic PO2 in guinea pig cochlea.

The effects of glycerol and mannitol on partial oxygen pressure (PO2) in perilymph were studied with a polarographic technique in the guinea pig cochlea. Following a transient reduction immediately after the intravenous injection of each drug, the oxygen tension increased to reach a peak 5 to 10 minutes later, and then decreased gradually. The magnitude and duration of the PO2 increases were larger after the administration of glycerol than after that of mannitol. These results corresponded well with the clinical experience that glycerol was more effective than mannitol in improving hearing in

Labyrinthine otosclerosis and sensorineural deafness. Pathologic findings of the spiral ligament.

In severe labyrinthine otosclerosis, spiral ligament changes occur predominantly adjacent to the stria vascularis, and consist of a decrease or loss of the capillary-perilymph system. It is suggested that this results in increased concentrations of metabolic products in the cochlear fluids that can no longer be absorbed effectively. If vascular shunts form they might represent collateral channels, with the flow of blood from the cochlea into the otosclerotic focus, bypassing the deficient capillary network in the spiral ligament. The zone of dense collagenization is described as a widening of

Intracochlear PLGA implants for simultaneous controlled release of multiple drugs.

Reliable drug delivery to the inner ear is still an unmet clinical need, due to the blood cochlear barrier, which effectively protects this highly sensitive organ. To overcome this hurdle, miniaturized, biodegradable implants were prepared (0.3 mm in diameter), which can be administered into the cochlea via a tiny hole drilled into the round window. They were based on poly(lactic-co-glycolic acid) (PLGA) and prepared by hot melt extrusion. Three drugs were incorporated: (i) 20.0% dexamethasone with anti-inflammatory, anti-fibrotic and protective activity for hair cells (being critical for hear

Rat Model of Ménière's Attack: Intratympanic Injection of Potassium Chloride Produces Direction-Changing Spontaneous Nystagmus and Hearing Fluctuations.

The major symptoms of Ménière's disease are episodic vertigo, fluctuating hearing loss, and tinnitus. Direction-changing spontaneous nystagmus is a characteristic vestibular finding in Ménière's disease. In the acute stage, spontaneous nystagmus beating to the affected side (irritative nystagmus) is often observed, while paralytic nystagmus beating to the healthy side is found in the chronic stage. This direction-changing nystagmus can be reproduced in guinea pigs by increasing the potassium ion concentration in the perilymph. The objectives of the present study were to examine the effects of
