Urban homelessness and poverty during economic prosperity and welfare reform: changes in self-reported comorbidities, insurance, and sources for usual care, 1995-1997.

Little is known of how homeless and other urban poor populations have fared during the robust economy and within structural changes in health care delivery and entitlement programs of the 1990s. This is important in determining the need for population-specific services during a vigorous economy with low unemployment and increasing Medicaid managed-care penetration. This study compared health insurance status and availability of a source for usual medical care, psychiatric and substance abuse comorbidities, and perceived causes of homelessness in homeless adults surveyed in 1995 and 1997. HepCCATT: a multilevel intervention for hepatitis C among vulnerable populations in Chicago.

BACKGROUND: Hepatitis C infection could be eliminated. Underdiagnosis and lack of treatment are the barriers to cure, especially for vulnerable populations (i.e. unable to pay for health care). METHODS: A multilevel intervention from September 2014 to September 2019 focused on the providers and organizations in 'the safety net' (providing health care to populations unable to pay), including: (i) public education, (ii) training for primary care providers (PCPs) and case managers, (iii) case management for high-risk populations, (iv) policy advice and (v) a registry (Registry) for 13 health cen

Brucella canis: An update on research and clinical management.

In Canada, Brucella canis remains a potentially devastating infectious agent that is still considered uncommon, despite the increasing international movement of dogs. There may be a growing risk to the Canadian canine population due to a reliance on outdated seroprevalence studies and the lack of federal regulation. With the complex diagnostic and management challenges associated with Brucella canis, a One Health approach is necessary to address the need for ongoing research, including updating canine and human seroprevalence rates in Canada, elucidating the pathogenesis, and determining the m

Hidradenitis Suppurativa in the Pediatric and Adolescent Population.

Hidradenitis suppurativa (HS) is a complex disease that manifests with nodules, abscesses, and tunnels in the intertriginous regions of the body. Symptoms often begin after adrenarche and may progress to severe disease with draining tunnels and permanent scars, leading to a significant reduction in quality of life and mental health. Diagnostic delays are common, increasing the risk of disease progression. Timely diagnosis and effective management are essential to the prevention of complications in the pediatric and adolescent population. This review will cover the currently available informati

Beyond the Waste: Reducing the Environmental Burden of Urinary Incontinence.

Urinary incontinence is a significant public health issue that affects a large proportion of the population in Europe. Incontinence not only affects the quality of life of patients but also strains national health systems and economies. Moreover, continence health care has a considerable environmental footprint. This mini review outlines the environmental consequences of urinary incontinence and current continence care practices. We describe the ecological impact of urinary incontinence management and potential sustainable alternatives. PATIENT SUMMARY: Our mini review describes the environmen

Women in Transition to Health: A Theory-Based Intervention to Increase Engagement in Care for Women Recently Released From Jail or Prison.

The time after incarceration is widely regarded as tenuous and stressful, and for women living with chronic illness, self-management is yet another stressor. Intervening before the individual is overwhelmed is critical to ensuring success. In this article the Women in Transition to Health, a nurse-led intervention based on Lazarus and Folkman's Transactional Model of Stress and Coping, designed to improve health outcomes in women recently released from jail or prison is described. Motivational interviewing and case management are used to strengthen coping skills and encourage engagement in car

[Acute pain management in opioid-maintained patients].

Treatment of acute nociceptive pain in patients with opioid substitution therapy (OST) is an actual topic. The clinical features of this population, as pain sensibility, and the pharmacological features of OST, require an individualized care which must be adjusted to the patient's pain and the OST used. This article offers a summary of the main possible pharmacological strategies by highlighting the features of pain in these patients and removing the barriers to an effective management. Generally, the OST is kept and a multimodal analgesia is added according to the intensity of the pain. Multi

The prevalence of pain and receipt of pain management in inpatient psychiatric settings in Ontario.

Pain is often underreported and undertreated among individuals with mental illness receiving health care services. Few studies have reported on prevalence of pain and pain management within psychiatric settings; this study aims to fill that gap. Secondary analysis of cross-sectional anonymized population-level data from the Ontario Mental Health Reporting System (N = 315,934 unique individuals) was conducted to identify associations between psychiatric diagnoses and recognition of pain management needs while controlling for factors previously linked to pain. Approximately 22% reported pain, ye

Barriers to, and Facilitators of, Diabetes Self-management in the Dialysis Population: A Narrative Review and Implications for Research.

PURPOSE OF REVIEW: Patients with both diabetes and kidney failure requiring dialysis are a complex population that is at risk of diabetes-related complications, hospitalizations, and mortality. Due to the significant illness burden, self-management of diabetes becomes challenging. The purpose of this review was to identify and synthesize the literature on barriers to, and facilitators of, diabetes self-management among patients with both diabetes and kidney failure requiring dialysis. SOURCES OF INFORMATION: We conducted a search of health care databases (CINAHL, PubMed, OVID Medline) to find

Population Genomic Screening and Improved Lipid Management in Patients With Familial Hypercholesterolemia.

BACKGROUND: The Helix Research Network program is a large population genomics initiative that screens an all-comers population of patients for Centers for Disease Control and Prevention Tier 1 genetic conditions, including familial hypercholesterolemia (FH). We evaluated changes in clinical management and low-density lipoprotein cholesterol (LDL-C) levels among patients we identified to have FH. METHODS: Participants across 9 US health systems provided samples that underwent clinical-grade exome sequencing. Individuals with a positive screening result for a Tier 1 condition were offered no-co

A large-scale genome-wide study of gene-sleep duration interactions for blood pressure in 811,405 individuals from diverse populations.

Although both short and long sleep duration are associated with elevated hypertension risk, our understanding of their interplay with biological pathways governing blood pressure remains limited. To address this, we carried out genome-wide cross-population gene-by-short-sleep and long-sleep duration interaction analyses for three blood pressure traits (systolic, diastolic, and pulse pressure) in 811,405 individuals from diverse population groups. We discovered 22 novel gene-sleep duration interaction loci for blood pressure, mapped to 23 genes. Investigating these genes' functional implication

Incidence and survival of multiple myeloma: a population-based study of 10 524 patients diagnosed 1982-2017.

Population-based studies from high-quality nationwide cancer registries provide an important alternative to clinical trials in the assessment of the impact of modern myeloma treatment. Based on data from the Cancer Registry of Norway, we investigated trends in incidence and relative survival (RS) for 10 524 patients in three age groups diagnosed between 1982 and 2017. Nationwide myeloma drug consumption statistics were obtained from the Norwegian Institute of Public Health. Patients aged <65 years had a steady increase in both 5- and 10-year RS across all calendar periods from 1982. For patien

Acceptability of safe drug consumption spaces among people who inject drugs in rural West Virginia.

AIM: Safe consumption spaces (SCS) are indoor environments in which people can use drugs with trained personnel on site to provide overdose reversal and risk reduction services. SCS have been shown to reduce fatal overdoses, decrease public syringe disposal, and reduce public drug consumption. Existing SCS research in the USA has explored acceptability for the hypothetical use of SCS, but primarily among urban populations of people who inject drugs (PWID). Given the disproportionate impact of the opioid crisis in rural communities, this research examines hypothetical SCS acceptability among a

Epidemiology of gastroenteritis on cruise ships, 2001-2004.

BACKGROUND: The incidence of diarrheal disease among cruise ship passengers declined from 29.2 cases per 100,000 passenger days in 1990 to 16.3 per 100,000 passenger days in 2000. In 2002, the Vessel Sanitation Program of the Centers for Disease Control and Prevention reported 29 outbreaks (3% or more passengers ill) of acute gastroenteritis on cruise ships, an increase from 3 the previous year. This analysis of gastroenteritis on cruise ships, conducted in 2005, details the increase in outbreak incidence rates during 2001 through 2004. METHODS: Using Gastrointestinal Illness Surveillance Sys

India's digital-first approach to sickle cell disease elimination.

The Government of India has launched the National Sickle Cell Anaemia Elimination Mission, a significant public health initiative targeting a genetic disorder that disproportionately affects the nation’s tribal populations. Sickle cell disease causes severe lifelong complications, and this mission aims to eliminate it as a public health problem by 2047. The primary objectives are to provide accessible, high-quality care to all patients and to reduce the disease’s prevalence through targeted screening and awareness. The mission’s comprehensive strategy is structured around three pillars: Health
