[Suicidal fatal beta-blocker intoxication].

We report on the case of a 45-year-old female with beta receptor antagonist intoxication after swallowing about 30 tablets of Cordanum (Talinolol) with suicidal intent. The out of hospital and clinical management is discussed. Prior to admission to hospital the patient only showed a few signs of intoxication such as hypotension, central symptoms and cyanosis. There was no bradycardia but during treatment she developed cardiac arrest. Out of hospital it was possible to stabilise the circulation with catecholamines (norepinephrin) and the transport to hospital was uneventful but only a few minut

Thermal study of simple amino-alcohol solutions.

Widely regarded as the most promising approach to long-term cryopreservation of organs for transplantation, vitrification is a process where liquid is transformed into a disordered solid state free from crystals, known as the amorphous state. The vitreous state is obtained by rapid cooling to cryogenic temperatures in the presence of antifreeze substances called cryoprotectants, such as polyalcohols, which are known to be very good vitrification agents. This work reports on the thermal properties of a new class of compounds, the amino-alcohols, studied for its similarity to the structure of th

Use of esmolol to control autonomic instability of tetanus.

Tetanus is often accompained by autonomic instability, rendering hemodynamic management difficult. Death is frequently secondary to an inability to control this instability. A variety of modalities have been used to stabilize the cardiovascular system, but all are not ideal. Esmolol offers theoretical advantages over other modalities. We report a case of severe tetanus in which a continuous infusion of esmolol was effective in controlling the autonomic instability.

The effects and physiological consequences of raised levels of cortisol on rainbow trout (Oncorhynchus mykiss) erythrocyte beta-adrenoreceptors.

We have investigated the influence of cortisol on the beta-adrenoreceptor population of rainbow trout [Oncorhynchus mykiss (Walbaum)] erythrocytes and determined what impact it has on the adrenergic responsiveness of erythrocytes in vitro to exogenous catecholamines. To do so, the erythrocyte beta-adrenoreceptors were characterized in fish with chronically elevated plasma cortisol levels (118 +/- 5.9 ng ml-1, greater than 10 days) and compared with shams, using radioreceptor assay techniques. The number of 'internalized', low-affinity receptors was increased when cortisol levels were raised, b

The effect of epanolol on intra-arterial ambulatory blood pressure and baroreceptor heart rate reflex in essential hypertension.

1. The effect of chronic treatment with epanolol, a new cardioselective beta-adrenoreceptor antagonist with moderate beta 1-selective intrinsic sympathomimetic activity (ISA), on 24 h ambulatory intra-arterial blood pressure (24 h IABP) and the sino-aortic baroreceptor heart rate (SAB/HR) reflex was investigated in six hypertensive subjects. 2. All subjects demonstrated a greater than 10% reduction in mean arterial pressure with atenolol therapy (100 mg once daily) before entering a randomized, double-blind, placebo-controlled, crossover protocol with epanolol (100 mg twice daily for 4 weeks).

Evaluation of the risk for drug-induced postural hypotension in an experimental model: investigations with carvedilol, prazosin, labetalol, and guanethidine.

Postural hypotension is a common side effect observed in the treatment of hypertension with various drugs. Carvedilol, a vasodilating beta-blocker, was compared with prazosin, labetalol, and guanethidine for its ability to induce orthostatic hypotension. The comparison was carried out using doses of the drugs required to lower baroreceptor reflex hypertension in anesthetized rabbits by 30 mm Hg (ED -30 mm Hg). The following doses were found in independent investigations to be equipotent in this rabbit model: ED -30 mm Hg (mg/kg i.v.): prazosin = 0.03; labetalol = 1.8; guanethidine = 0.12; carv

[Penbutolol and arterial hypertension].

Penbutolol has proved particularly effective and suitable for the treatment, even on a long-term basis, of recently developed hypertension, especially in its hyperkinetic forms. The drug produces minimal side effects, is well tolerated and gives an early therapeutic response. In addition penbutolol does not cause any significant alterations in the biohumoral parameters of the patients treated and is ideal for combination with dihydralazine, reserpine and dihydrochlorotiazide in the treatment of more stubborn cases, making it possible to reduce the doses of the other drugs without causing brady

Effect of orally administered celiprolol in patients with chronic atrial fibrillation.

The heart rate increase induced by dynamic exercise in patients with chronic atrial fibrillation is competitively attenuated by beta-blockade. The influence of oral celiprolol on exercise induced tachycardia was evaluated in 23 patients with chronic stable atrial fibrillation in a dose-titration study. This was succeeded by a placebo-controlled double-blind, crossover multi-center trial. During the dose-titration phase each patient underwent a single-blind three week dose escalation period-taking celiprolol 200 mg once daily for one week, celiprolol 400 mg once daily for the third week. Liquid chromatographic assay and disposition of carvedilol in healthy volunteers.

Quantitative determination of serum concentrations of carvedilol [(+/-)-1-(carbazol-4-yloxy)-3-[[2-(o-methoxyphenoxy)ethyl) amino]-2-propanol], a combined alpha- and beta-adrenergic receptor antagonist, was obtained using HPLC with spectrofluorometric detection. Carvedilol was extracted from alkalinized serum with ether and was subsequently back extracted with diluted phosphoric acid. This method proved to be sensitive and reproducible (mean coefficient of variation of 6.1% for 0.25 to 150 nanograms per milliliter of serum). A single dose of carvedilol (5, 10, or 15 mg) was given as an intrave

Amidation of, and (R)-1-amino-2-propanol attachment to, the corrin ring during vitamin B-12 biosynthesis by Clostridium tetanomorphum extracts.

Two intermediate stages in cobalamin biosynthesis, amidation of carboxylic acid groups in the corrin ring and (R)-1-amino-2-propanol attachment at propionic acid position f, have been studied using cell-free extracts from the obligate anaerobe Clostridium tetanomorphum. The preparation of an incomplete corrinoid, probably cobinic acid-a,c,d,e,g-pentaamide, as an in vitro amidation substrate was accomplished via mild acid hydrolysis of cobinamide. Weak, but reproducible activities for both amidation and (R)-1-amino-2-propanol attachment were found in crude, nucleic acid-free and DE-52 column-pu

Beta 1 versus nonselective blockade in therapy of essential tremor.

The beta 1-selective blocker metoprolol was compared to propranolol and a placebo in a double-blind crossover trial in 24 patients with essential tremor. Both beta blockers suppressed the essential tremor, but metoprolol, which caused a mean reduction of 32.0% in tremor intensity from the base-line value, was less effective than propranolol, which reduced mean tremor intensity by 41.3%. Subjective benefit for their tremor was found by 15 of the patients taking propranolol and by one taking metoprolol. The tremor frequency was not affected. No serious side effects were observed. Metoprolol may

Comparison of withdrawal phenomena after propranolol, metoprolol, and pindolol.

Three groups of hypertensive patients were studied after they had received one of the three pharmacologically different beta blockers for at least 1 month: pindolol, 10 mg twice a day (n = 7), propranolol median dose, 80 mg three times a day (n = 9), or metoprolol, 150 mg (twice a day (n = 8). After abrupt withdrawal of drug and replacement with placebo, we measured the following on day 0 and approximately every 2 days up to 3 weeks; beta-adrenergic sensitivity (BAS) by the chronotropic dose of isoproterenol to increase heart rate by 25 bpm (CD25), resting heart rate and blood pressure, and sy

[Timolol response in denervational syndromes (author's transl)].

In eyes with pupillotony (Argyll Robertson) the effect of locally applied timolol maleate (0.5%) on intraocular pressure is considerably less pronounced than in normal control eyes. In Fuchs' heterochromic cyclitis the timolol response is the same as in controls. Infrared thermography of the anterior segment of the eye showed that the maximum drop in IOP was preceded by a fall in temperature. These findings suggest that the IOP-lowering effect of timolol depends on parasympathetic innervation and that timolol decreases the blood flow in the ciliary body.

Pharmacological effects of topical timolol in the rabbit eye.

The ability of timolol to act as a beta-adrenergic antagonist in the cornea and in the iris--ciliary body of albino rabbits is reported. In vitro, timolol potently blocks the isoproterenol-stimulated synthesis of cAMP. In the iris-ciliary body, the apparent inhibition constant (K1) for timolol is 0.6 nM, indicating that timolol is approximately seven times more potent than propranolol. Topical timolol (0.5% and 4%) rapidly inhibits the beta-adrenergic--stimulated synthesis of cAMP in both corneal and iris--ciliary body tissues. Within 3 hr, however, the iris--ciliary body tissue regains its ab

Inhibition by derivatives of diguanidines of cell proliferation in Ehrlich ascites cells grown in cultures.

The anti-proliferative effects of 1,1'-[(methylethanediylidene)dinitrilo]diguanidine [methylglyoxal bis(guanylhydrazone)] and 1,1'-[(metHYLETHANEDIYLIDENE)dinitrilo]bis-(3-aminoguaNIDINE) HAVE BEEN STUDIED IN Ehrlich ascites carcinoma cells grown in suspension cultures. Both compounds are potent inhibitors of S-adenosyl-L-methionine decarboxylase from the tumour cells. In the presence of putrescine (but not in its absence), the inhibition produced by 1,1'-[methylethanediylidene)dinitrilo]bis-(3-aminoguanadine) was apparently irreversible, as judged by persistent depression of the enzyme activi

The role of clinical drug trial methodology with respect to studies of new drugs. Clinical trials of timolol.

The use of the Randomized Clinical Trial (RCT) to assess the safety and efficacy of a new drug or treatment can provide a clear evaluation of the relative benefits and risks of the new therapy as compared to the longer-established treatment methodology. In some cases, such as insulin, the benefits are immediately apparent; in other cases, such as photocoagulation for proliferative diabetic retinopathy, the consensus is not so easily obtained, and the clinical trial can supply the answer. Furthermore, the RCT provides the opportunity to identify adverse reactions, in particular those which pres

Kinetics of pamatolol, a cardioselective beta adrenoreceptor blocker.

The systemic bioavailability, elimination half-life (t1/2), and plasma concentration--response relationships of pamatolol, a relatively cardioselective beta adrenoceptor blocker, have been measured in healthy subjects. Pamatolol is rapidly and completely absorbed after oral dosing. Elimination t1/2 ranged from 2.9 to 4.6 hr after oral doses and from 2.2 to 5.6 hr after intravenous doses. There was a clear relationship between log plasma pamatolol concentration and sympathetic blockade assessed by reduction of exercise heart rate. Concentration-response curves were essentially identical after o

Atenolol: a review of its pharmacological properties and therapeutic efficacy in angina pectoris and hypertension.

Atenolol is a beta-selective (cardioselective) adrenoceptor blocking drug without partial agonist or membrane stabilising activity. Its profile of action most closely resembles that of metoprolol which differs only in that it has some membrane stabilising activity. Atenolol has been well studied and is effective in the treatment of hypertension and in the prophylactic management of angina. Its narrow dose response range obviates the need for highly individualised dose titration. In patients with angina its long duration of beta-blocking activity allows once daily dosage, whereas other beta-blo
