A combined therapy was given to 30 patients with the acute stage of ischemic stroke that had complicated the course of cerebral atherosclerosis. The therapy consisted in exfusion of 250 to 300 ml of blood with subsequent infusion of low-molecular dextrans and various medicines. The changes of the general hemodynamic parameters at various stages of the therapy were compared with respective data obtained from 30 patients suffering from cerebral atherosclerosis but without acute cerebral circulation disorders, as well as from 20 healthy subjects. BACKGROUND: No reflow in capillaries (no reflow) is the lack of tissue perfusion that occurs once central hemodynamics are restored. This prevents oxygen transfer and debt repayment to vital tissues after shock resuscitation. Since metabolic swelling of cells and tissues can cause no reflow, it is a target for study in shock. We describe a new method for investigating ion and water transport in ciliary epithelium. A single ciliary process from the rabbit is isolated, placed in a chamber, and rapidly perfused with physiological Ringer. We studied the haemostatic and volume effects of synthetic plasma substitutes and Ringer's solution in 48 surgical patients and found that the measured fibrinogen concentrations of patients receiving either dextran or hydroxyethyl starch (HES) were significantly higher than those predicted by the dilutional effects.
