The BAHA HC200/300 in comparison with conventional bone conduction hearing aids.

A retrospective study was performed on 89 patients from a consecutive series who received a BAHA HC200/300 after having previously used conventional bone conduction hearing aids. The patients' performance with the BAHA HC 200/300 was compared to their performance with conventional bone conduction hearing aids. The patients were divided into two groups, depending on the time of implantation (before or after May 1992). The patients in group 1 (long-term users) were asked to fill in a questionnaire, the same one as they had filled in at the initial BAHA fitting more than 5 years previously. The a

Comparison of three procedures for initial fitting of compression hearing aids. I. Experienced users, fitted bilaterally.

We compared the effectiveness of three procedures for the initial fitting of hearing aids with multi-band compression: (1) CAMEQ, which aims to amplify speech so as to give equal loudness per critical band over the frequency range important for speech intelligibility, and to give similar overall loudness to 'normal': (2) CAMREST, which aims to amplify speech so as to restore 'normal' specific loudness patterns, over a wide range of speech levels; (3) DSL I/O, which aims to map the dynamic range of normally hearing people into the reduced dynamic range of hearing-impaired people, with 'full' re

Clinical and economic evaluation of minimally invasive cartilaginous palisade myringoplasty.

OBJECTIVES: To test the clinical and audiometric efficacy of a minimally invasive myringoplasty technique, combining cartilaginous palisades while avoiding flap elevation, for small and wide perforations. METHODS: Over 4 years, this retrospective study included all patients over 6 years of age presenting an indication for myringoplasty. Several clinical and economic criteria were noted at 7 d, 2 months, 6 months and 2 years postoperative. The main outcome was the absence of perforation 2 years postoperative. The secondary outcomes were an audiometric gain at 6 months and the evaluation of the

Exploring age-dependent speech recognition in noise for children with hearing loss: insights from hearing Aid or cochlear Implant rehabilitation.

OBJECTIVE: In this study we investigated whether speech recognition in noise improves with age in children with hearing loss using hearing aids (HA) or cochlear implants (CI). Secondly, we identified the audiological factors that predict these abilities. DESIGN: We performed a retrospective analysis of the Dutch Digit-In-Noise (DIN) test results from our clinical population of children rehabilitated with HAs (N = 163) or CIs (N = 83). Participants were aged between 6 and 18 years, had stable hearing loss and an open-set phoneme recognition in quiet > 50% at 65 dB SPL. STUDY SAMPLE: We includ

Speech Segregation in Active Middle Ear Stimulation: Masking Release With Changing Fundamental Frequency.

OBJECTIVES: Temporal fine structure information such as low-frequency sounds including the fundamental frequency (F0) is important to separate different talkers in noisy environments. Speech perception in noise is negatively affected by reduced temporal fine structure resolution in cochlear hearing loss. It has been shown that normal-hearing (NH) people as well as cochlear implant patients with preserved acoustic low-frequency hearing benefit from different F0 between concurrent talkers. Though patients with an active middle ear implant (AMEI) report better sound quality compared with hearing

Hearing function in patients living with HIV/AIDS.

OBJECTIVES: During the earlier years of the HIV/AIDS epidemic, initial reports described sensorineural hearing loss in up to 49% of individuals with HIV/AIDS. During those years, patients commonly progressed to advanced stages of HIV disease and frequently had neurological complications. However, the abnormalities on pure-tone audiometry and brainstem-evoked responses outlined in small studies were not always consistently correlated with advanced stages of HIV/AIDS. Moreover, these studies could not exclude the confounding effect of concurrent opportunistic infections and syphilis. Additional

Fitting range of the BAHA Cordelle.

The performance of the most powerful Bone-Anchored Hearing Aid (BAHA) currently available, the BAHA Cordelle, was evaluated in 25 patients with severe to profound mixed hearing loss. Patients showed bone conduction thresholds at 500, 1000 and 2000 Hz, ranged between 30 and 70 dB HL, and an additional air-bone gap of about at least 30 dB. With the BAHA Cordelle, free-field thresholds improve relative to bone-conduction thresholds with 1.5, 5.0, 17.8, and 4.3 dB at 500, 1000, 2000, and 4000 Hz, respectively, with substantial inter-individual variability. The differences in unaided air conduction

Remote self-report and speech-in-noise measures predict clinical audiometric thresholds.

Developments in smartphone technology and the COVID-19 pandemic have highlighted the feasibility and need for remote, but reliable hearing tests. Previous studies used remote testing but did not directly compare results in the same listeners with standard lab or clinic testing. This study investigated reliability of remote, self-administered digits-in-noise (remote-DIN) compared with lab-based, supervised (lab-DIN) testing. Predictive validity was further examined in relation to a commonly used self-report, Speech, Spatial, and Qualities of Hearing (SSQ-12), and lab-based, pure tone audiometry

Functional Outcomes and Quality of Life after Cochlear Implantation in Patients with Long-Term Deafness.

Background: Hearing-related quality of life (QoL) after cochlear implantation (CI) is as important as audiological performance. We evaluated the functional results and QoL after CI in a heterogeneous patient cohort with emphasis on patients with long-term deafness (>10 years). Methods: Twenty-eight patients (n = 32 implanted ears, within n = 12 long-term deaf ears) implanted with a mid-scala electrode array were included in this retrospective mono-centric cohort study. Speech intelligibility for monosyllables (SIM), speech reception thresholds (SRT50) and QoL with Nijmegen Cochlear Implant Que

The relation of hearing-specific patient-reported outcome measures with speech perception measures and acceptable noise levels in cochlear implant users.

Objective: To investigate the relation of a hearing-specific patient-reported outcome measure (PROM) with speech perception and noise tolerance measurements. It was hypothesised that speech intelligibility in noise and noise tolerance may explain a larger part of the variance in PROM scores than speech intelligibility in quiet.Design: This cross-sectional study used the Speech, Spatial, Qualities (SSQ) questionnaire as a PROM. Speech recognition in quiet, the Speech Reception Threshold in noise and noise tolerance as measured with the acceptable noise level (ANL) were measured with sentences.S

Binaural squelch effect in unilateral otosclerosis surgery: comparison of speech intelligibility in noise before-after surgery.

PURPOSE: We aimed to evaluate the speech intelligibility benefit in noise provided by stapedotomy in the treatment of unilateral otosclerosis. METHODS: We enrolled adults suffering from unilateral conductive hearing loss and followed them up until 9 months after surgery. The patients underwent a free field speech hearing evaluation using the French Matrix test before and after stapedotomy. Speech material was sent to the front of the patients (S0) and noise was presented either at the front (N0), or at the operated ear (N-90) or at the non-operated ear (N + 90). The speech intelligibility ben

Stapes surgery in Japanese patients with osteogenesis imperfecta.

Osteogenesis imperfecta (OI) is a heterogenous connective tissue disorder. The classical triad of symptoms involves a conductive and/or sensorineural hearing impairment together with a tendency to spontaneous bone fractures and blue sclerae. Between 1993-2004, primary stapes surgery was performed on 14 ears of 11 OI patients who presented with conductive and/or mixed hearing loss. Pathological findings included atrophy and/or fractures of the stapedial crura in combination with thickening and fixation of the stapes footplate and hypervascularity of the promontory mucosa. All the patients with

Chronic labyrinthine ischemia.

The purpose of the present study was to describe the audiological and vestibular deficit in patients presumed to suffer from chronic impairment of the blood supply to the labyrinth. Thirty-nine subjects affected by various pathologies often impairing systemic blood circulation, which also presented labyrinthine damage of unclear origin, were evaluated for a possible relationship between these two conditions. At the time of this study 80 per cent of the subjects showed also a certain degree of increased blood viscosity. The most common finding in the group was slow and progressive bilateral sen

Carotid-cochlear dehiscence: a dangerous mimicker of inner ear pathologies.

A 67-year-old woman was referred to the otolaryngology service after presenting to the emergency department for dizziness and loss of balance. She reported several similar episodes over the past years. Physical examination was unremarkable. A temporal bone CT scan revealed dehiscence between the bony carotid canal and the cochlea resulting in the diagnosis of carotid-cochlear dehiscence (CCD). CCD is an extremely rare condition involving the thinning of the bony canal separating the internal carotid artery from the cochlea. CCD is best diagnosed with temporal bone CT scan. Treatment options in

The Nobelpharma auditory system bone-anchored hearing aid: the Edmonton experience.

The unique ability of titanium to osseointegrate has been utilized in clinical practice for bone-anchored hearing aids in Sweden since 1977. The percutaneous design by Nobelpharma has been shown to be useful not only for pure conductive hearing loss but also mixed losses with a bone pure tone average of 45 dB (60 dB with the HC220). We examine our experience, since May 1991, with 15 patients aged 8 to 73 years, including indications, complications, and both objective and subjective results. In addition, we have found the second stage can be successfully completed during the initial anesthetic

Is Cholesteatoma a Risk Factor for Graft Success Rate in Chronic Otitis Media Surgery?

INTRODUCTION: In developing countries, chronic otitis media (COM) and cholesteatoma are relatively prevalent. Within the field of otology, COM surgery remains one of the most common surgical treatments. Most recent studies evaluating the potential prognostic factors in COM surgery have addressed graft success rate and types of middle ear and mastoid pathology. There has been much controversy about this issue until the present time. This study evaluated the effect of cholesteatoma on the GSR in COM surgery. MATERIALS AND METHODS: The present retrospective, case-controlled study investigated 42
