A Cystic Variant of Acinar Cell Carcinoma of the Pancreas with Repeated Bleeding from a Pseudoaneurysm: A Case Report.

INTRODUCTION: Acinar cell carcinomas are rare pancreatic neoplasms, accounting for approximately 1% of all exocrine pancreatic tumors. We describe a case of a cystic variant with intracystic hemorrhaging that was difficult to differentiate from a pseudocyst due to its morphology. CASE PRESENTATION: A 54-year-old man was admitted with severe left upper quadrant abdominal pain. Imaging studies showed a 7.0-cm internal heterogeneous cystic lesion with a splenic artery pseudoaneurysm near the lesion. Transarterial embolization of the splenic artery was performed, but rebleeding occurred 1 month l

Retroperitoneal paraganglioma presenting with pancytopenia: A rare case with rare manifestation.

INTRODUCTION: Paragangliomas are tumors that arise from extraadrenal chromaffin cells and most of them are asymptomatic presenting with painless mass. Retroperitoneal paragangliomas are mostly benign with good prognosis; however, they can present with abdominal pain, palpable mass, or hypertensive episodes. Surgical resection is still the main treatment and necessary for histological assessment. CASE REPORT: A 41 year old female patient presented with 6 months of loss of appetite, weight loss, weakness and breathlessness on exertion.. The patient's initial blood examination showed marked anem

Phenotypic heterogeneity of spontaneous lymphomas of CWD mice.

Animals of the inbred mouse strain, CWD, express endogenous murine leukemia viruses early in life and have a high incidence of spontaneous neoplasms. We found that approximately one half of these animals died of malignant lymphoma by the age of 16 months. Splenic enlargement was seen in all mice, but thymic involvement was unusual. One half of the CWD tumors were diffuse lymphoblastic or immunoblastic lymphomas while the remainder were large cell, small cell, or mixed cell lymphomas. Analysis of DNAs from 12 tumors for immunoglobulin and T-cell receptor gene rearrangements revealed that all si

Nodal marginal zone lymphoma: current knowledge and future directions of an heterogeneous disease.

Nodal marginal zone lymphoma (NMZL) is a defined, separate clinicopathological entity. NMZL is a B-cell neoplasm originated in the lymph node, whose histology resembles the nodal infiltration by mucosa-associated lymphoid tissue- or splenic-type marginal zone lymphoma, in the absence of clinical evidence of extranodal or spleen disease. The lack of characteristic phenotypic or molecular diagnostic findings is still hampering the reproducibility of this diagnosis. Herein, we review the main morphological and immunophenotypical markers, discussing the differential with other overlapping entities

Case report: a common presentation of a rare disease-hepatosplenic T-cell lymphoma.

Hepatosplenic T-cell lymphoma is a rare neoplasm characterized by systemic B-symptoms, hepatosplenomegaly, no lymphadenopathy, and lymphomatous infiltrates in the splenic red pulp, hepatic sinusoids, and bone marrow sinuses. The team presents the case of a healthy 30 year old man, active duty Marine, who presented with classic symptoms, yet obtaining a diagnosis took over three months from the onset of symptoms. This clinical entity initially described in 1990, is elusive, with vague and misleading symptoms. Despite aggressive conventional therapy with anthracycline-based regimens and stem cel

Pathologic and prognostic characteristics of splenomegaly in dogs due to fibrohistiocytic nodules: 98 cases.

Ninety-eight canine splenectomy specimens consisting of combined nodular lymphoid and fibrohistiocytic cell proliferation were evaluated for seven light microscopic characteristics. Electron microscopic features in eight primary and two metastatic nodules (liver) were also evaluated. Nodular fibrohistiocytic proliferation in the canine spleen is characterized by a mixed population of histiocytoid and/or spindle cells in varying proportions intermixed with hematopoietic elements, plasma cells, and/or lymphocytes. These nodules seem to form a continuum between splenic lymphoid nodular hyperplasi

Masked polycythemia vera presenting as non-cirrhotic portal hypertension with isolated gastric varices: a case report.

BACKGROUND: Myeloproliferative neoplasms (MPN) are an uncommon but important cause of non-cirrhotic portal hypertension (NCPH) and may present with variceal bleeding rather than overt hematologic abnormalities. Diagnosis may be further delayed when polycythemia vera (PV) is masked by iron deficiency or prior venesection, resulting in non-elevated hemoglobin despite an underlying JAK2-positive neoplasm. We report a rare case of isolated gastric variceal bleeding as the presenting manifestation of presinusoidal NCPH in a patient with early myelofibrosis evolving from iron-deficient PV. CASE PRE

Distribution and pattern of BCL-6 mutations throughout the spectrum of B-cell neoplasia.

BCL-6 mutations are accumulated during B-cell transit through the germinal center (GC) and provide a histogenetic marker for B-cell tumors. On the basis of a comprehensive analysis of 308 B-cell neoplasms, we (1) expand the spectrum of tumors associated with BCL-6 mutations; (2) corroborate the notion that mutations cluster with GC and post-GC B-cell neoplasms; and (3) identify heterogeneous mutation frequency among B-lineage diffuse large cell lymphoma (B-DLCL) subsets. Mutations are virtually absent in acute lymphoblastic leukemia (P <.001) and mantle cell lymphoma (P <.05), whereas they occ

t(14;22)(q32;q11) translocation involving IGH and IGL acquired at progression of splenic marginal zone lymphoma treated with rituximab, ibrutinib, and obinutuzumab.

Translocations involving immunoglobin (IG) are common in B-cell neoplasms. These IG translocations lead to the disposition of the enhancer or promoter of an IG locus, typically IGH, to a proto-oncogene, resulting in the elevated expression of the cancer gene. IG fusions play an important role in the diagnosis, prognostication, and therapy selection of B-cell lymphomas. A t(14;22)(q32;q11) translocation involving IGH and IGL is rare in lymphomas. We report herein clinicopathological characteristics, response to treatment, and outcomes of a first splenic marginal zone lymphoma case with a t(14;2

Pancreatic Endometriosis Coexisting with a Splenic Mesothelial Cyst: A Rare Case Report and Review of the Literature.

Endometriosis is a clinical entity affecting up to 10% of women of reproductive age, characterized by ectopic endometrial tissue outside the uterine cavity. While extrapelvic endometriosis has been documented, pancreatic endometriosis remains extremely rare and poses significant diagnostic challenges due to its similarity to other pancreatic diseases. At the same time, splenic mesothelial cysts are also rare and typically benign. This report presents a unique case of pancreatic endometriosis coexisting with a splenic mesothelial cyst in a 31-year-old woman. The patient presented to the emergen

A case of multifocal sclerosis angiomatoid nodular transformations of the spleen occurring after partial splenic infarction with transcatheter arterial embolization for splenic artery aneurysm.

A 48-year-old woman underwent transcatheter arterial embolization (TAE) for a splenic artery aneurysm, which resulted in a partial splenic infarction in the middle lobe. Five years after TAE, a 20-mm diameter mass in the noninfarcted area of the spleen was detected on imaging, which grew to 25 mm in diameter after 6 months. MRI after gadolinium administration showed a 35 × 34 mm mass within the superior pole and 15 × 12 mm mass within the inferior pole. The patient underwent laparoscopic splenectomy and had an uneventful postoperative recovery. No evidence of recurrence was observed during the

Splenic Rupture Secondary to Metastatic Undifferentiated Pleomorphic Sarcoma of the Spleen: A Rare Presentation of an Exceptionally Rare Entity.

Splenic malignancies are uncommonly encountered in routine practice, and they are often only detected incidentally, posing a clinical challenge. Primary malignant neoplasms of the spleen are rare, and sarcomas are by far the rarest. The undifferentiated pleomorphic sarcoma type is an exceptionally rare and aggressive type with only a handful of cases reported in literature thus far. Herein, we present the case of a 49-year-old, with a previous finding of a heterogeneous splenomegaly on imaging for an unrelated medical complaint, presenting with an acute abdomen secondary to a ruptured spleen.

Focal nodular hyperplasia of the liver associated with portal vein agenesis: a morphological and immunohistochemical study of one case and review of the literature.

Congenital absence of portal vein (CAPV) is a rare malformation. To our knowledge, sixteen cases are reported in western literature. All the cases are associated with other diseases, cardiac malformations (12/17 patients; 16 plus the present case) and hepatic neoplasms being the most frequent observations. We present the case of a girl with a complex malformative syndrome consisting of multicystic kidney dysplasia, CAPV and nodular tumor-like mass of the liver. Angiography showed that the splenic vein and superior mesenteric vein joined to form a common trunk directly entering the inferior ven

Splenic Irradiation Preceding Allogeneic Hematopoietic Stem Cell Transplantation as a Possible Risk Factor of Sinusoidal Obstruction Syndrome: A Report of Three Cases.

INTRODUCTION: Splenomegaly is frequently encountered in patients with myeloproliferative neoplasms. Splenomegaly is associated with an increased incidence of engraftment failure during allogeneic hematopoietic stem cell transplantation (allo-HSCT), and some centers perform prior low-dose splenic irradiation (LDSI) which has been reported to be both safe and effective. However, we report conflicting results by presenting three allo-HSCT patients undergoing LDSI that subsequently developed sinusoidal obstruction syndrome (SOS). CASE PRESENTATION: The underlying diseases of the presented cases w

A rare case of angiosarcoma with skull masses and erythropenia and thrombocytopenia: A case report and review of literature.

RATIONALE: Primary splenic angiosarcoma (PSA) is a rare, fatal neoplasm originating from sinusoidal vascular endothelial cells, and usually metastasizes and almost always has a poor prognosis. Surgical excision is the main treatment of this highly malignant disease. PATIENT CONCERNS: We reported a special case of a 68-year-old female who had a 6-month history of scalp masses. DIAGNOSIS: The patient was found to have 2 skull masses on computed tomography (CT). Laboratory findings revealed erythropenia and thrombocytopenia. Enhanced abdomen magnetic resonance imaging (MRI) showed multiple mass

A non-resolving splenic 'abscess' in an elderly male - An unusual manifestation of splenic lymphoma.

Splenic abscess is a relatively uncommon condition, posing a diagnostic and therapeutic challenge for the treating physician. It occurs due to haematogenous spread from endocarditis or other septic foci, especially in immune-compromised individuals and diabetics. We describe an elderly male who presented with splenomegaly and low-grade fever with no predisposing factors. Examination revealed a tender splenomegaly. Ultrasonography (US) showed a hypoechoic area within the spleen from which guided aspiration of pus grew Staphylococcus aureus and Klebsiella pneumoniae. Percutaneous drainage and cu
