BACKGROUND: Different lateral and postero-lateral cranial approaches to the petroclival region and to the mid-upper brain stem have been described so far, some of which require extensive osseous demolition and possible damage of neurovascular structures. Neuronavigational systems are now extensively available for preoperative planning and intraoperative navigation to assist the surgeons in choosing the optimally invasive approach for each pathology. Herein, we describe a detailed navigation-augmented morphometric analysis to bring insight into the usefulness of an anterior petrosectomy (AP) to

OBJECT Surgery of suspected low-grade gliomas (LGGs) poses a special challenge for neurosurgeons due to their diffusely infiltrative growth and histopathological heterogeneity. Consequently, neuronavigation with multimodality imaging data, such as structural and metabolic data, fiber tracking, and 3D brain visualization, has been proposed to optimize surgery. However, currently no standardized protocol has been established for multimodality imaging data in modern glioma surgery. The aim of this study was therefore to define a specific protocol for multimodality imaging and navigation for suspe

BACKGROUND Multiple hereditary exostoses (MHE) is a rare genetic disorder characterized by multiple osteochondromas, often leading to joint deformity and early-onset osteoarthritis (OA). The knee is one of the most commonly affected joints, and progressive deformity and malalignment may eventually require surgical intervention. Total knee arthroplasty (TKA) in patients with MHE remains technically demanding because of distorted joint anatomy and soft tissue imbalance. Because of the rarity of this condition, published reports describing surgical strategies and clinical and radiographic outcome

Endodontic microsurgery (EMS) represents a significant advancement in the treatment of persistent periapical disease, evolving from conventional apicoectomy techniques into a precise, minimally invasive procedure. Indications for EMS include nonsurgical retreatment has failed or is not feasible, the primary endodontic treatment cannot be improved upon, insufficient residual dentin thickness, extra-radicular infection, among others. EMS follows a precise sequence: diagnosis and planning, surgical access, osteotomy, root-end resection, retrograde preparation and filling, and final closure; perfo

BACKGROUND: Achieving adequate alignment has traditionally been an important goal in total knee arthroplasty to achieve long-term implant survival. While accelerometer-based hand-held navigation systems (ABN) has been introduced as a way to achieve alignment, there is a limited body of evidence on its accuracy, especially in patients under 65 years with differing etiologies for knee arthritis. This study aimed to assess the precision of a specific ABN system in restoring the mechanical axis and report surgical variables and complications, with particular attention to younger patients.

PURPOSE: Fragility fractures of the pelvis (FFPs) are increasingly common owing to aging populations of several countries and the increasing osteoporosis incidence. Surgical interventions, including percutaneous screw fixation, are performed to avoid complications associated with prolonged bed rest for FFP management. However, conventional fluoroscopy for surgical navigation is difficult owing to issues such as insufficient tissue imaging, difficulty achieving accurate screw placement, and high radiation exposure to surgeons. This study aimed to compare the effectiveness of the O-arm-based nav

Nasopalatine cysts are common nonodontogenic cysts that occur in the maxilla. During the nucleation of large cysts extending to the floor of the nasal cavity, care must be taken to avoid damage to the nasal mucosa. In the present report, an innovative custom-made surgical guide made by a Three-dimensional printer is introduced for accurate enucleation surgery. The patient's cone-beam computerized tomography and dental model scan data were obtained, and a tooth-supported type of surgical guide was designed containing a circular plate structure showing the size of the cystic region, an indicator

Digital technologies such as navigation systems and robotics have substantially improved treatment accuracy, but they have also made frequent shifts of the operator's gaze between the surgical field and chairside monitor a routine part of clinical practice, increasing clinicians' physical and cognitive workload. Head-mounted display (HMD) offers a potential means of reducing such visual distraction; however, some currently available systems remain limited by a mismatch between their functional design and clinical needs, as well as by overly generalized use of technical concepts. On the basis o

INTRODUCTION: The development of computer-aided systems for endoscopic sinus surgery has enabled surgical navigation through diseased or surgically altered sinus anatomy with increased confidence. However, conventional computer-aided systems do not provide intra-operative updated computed tomography imaging. We describe the technical aspects of the xCAT, a new intra-operative mobile volume computed tomography scanner.
TECHNICAL REPORT: A patient with a malignant melanoma unwittingly removed at another hospital underwent surgery for removal of the lateral nasal wall and directed biopsies, in a

Instrumentation of the cervical spine particularly at the higher cervical levels like C2 presents unique challenges mainly because of their complex anatomy and proximity to neurovascular structures. The goal of the article is to demonstrate that using navigation technologies in inserting anterior odontoid screws can enhance the precision and safety of surgery. We describe a novel approach for anterior C2 odontoid fixation using a three-pin radiolucent Mayfield clamp with intra-operative CT registration and cranial brainlab navigation. A 28-year-old male patient who had a type II odontoid fract

This paper illustrates the feasibility and utility of combining cranial anatomy and brain function on the same 3D-printed model, as evidenced by a neurosurgical planning case study of a 29-year-old female patient with a low-grade frontal-lobe glioma. We herein report the rapid prototyping methodology utilized in conjunction with surgical navigation to prepare and plan a complex neurosurgery. The method introduced here combines CT and MRI images with DTI tractography, while using various image segmentation protocols to 3D model the skull base, tumor, and five eloquent fiber tracts. This 3D mode

BACKGROUND: Advancements in surgical navigation technology coupled with 3-dimensional (3D) radiographic data have significantly enhanced the accuracy and efficiency of spinal fusion implant placement. Increased usage of such technology has led to rising concerns regarding maintenance of the sterile field, as makeshift drape systems are fraught with breaches thus presenting increased risk of surgical site infections (SSIs). A clinical need exists for a sterile draping solution with these techniques. Our objective was to quantify expected accuracy error associated with 2MM and 4MM thickness Ster

UNLABELLED: Objective. Percutaneous balloon compression (PBC) is an effective and safe management for medically refractory trigeminal neuralgia; however, technical failure to cannulate the foramen ovale (FO) using only fluoroscopy is a significant problem in some cases. In this paper, we suggest the use of intraoperative navigation, in cases of reoperation due to prior technical failure to cannulate the FO under fluoroscopy. Methods. A total of 174 patients underwent PBC for TN since 2003. In 9 cases the penetration of the FO was not accomplished. Five of those patients were reoperated on for

RATIONALE: Deep neck infections (DNIs) in the head and neck area are difficult to treat due to the anatomical complexity of the cervical region. Since inflammation causes changes in anatomy, it is often difficult to find the exact location of the abscess, which leads to failed surgical drainage.
PATIENT CONCERN: A 76-year-old female patient was referred to our clinic with trismus and right-side facial swelling. After extraction of her lower third molar 2 weeks ago, due to chronic periodontitis, her trismus had aggravated and her maximal mouth opening was 20 mm.
DIAGNOSES: Computed tomography
