Non-Scar Fixation Technique for Zygomaticomaxillary Complex Tripod Fracture Using Upper Lid Transconjunctival Incision.

BACKGROUND: Surgical approaches for zygomaticomaxillary complex (ZMC) fractures have evolved toward minimizing cutaneous incisions while maintaining adequate exposure. Conventional approaches for frontozygomatic fixation, such as lateral brow or transconjunctival incisions with lateral extension, may result in visible scarring or periocular complications. Building on previous transconjunctival techniques, we applied an independent upper eyelid transconjunctival incision as a non-cutaneous approach for frontozygomatic fixation in selected ZMC tripod fractures. METHODS: This retrospective compa

The multi-ligament ankle fracture: Epidemiology, key anatomical findings and fixation strategies in unstable open injuries.

BACKGROUND: Ankle fracture surgery has traditionally focussed on restoration of bony anatomy, with fixation of the malleoli and syndesmotic stabilisation where applicable. However, high energy open fracture-dislocations can also result in periosteal stripping of the stabilising capsuloligamentous attachments. As such, restoration of osseous anatomy alone may not result in sufficient articular stability. OBJECTIVES: In this series from a level 1 trauma centre, we report a subset of highly unstable open ankle fractures with combined capsuloligamentous injuries, in which restoration of osseous a

Limb reconstruction after high energy trauma.

Limb reconstruction techniques rely on stable external fixation to provide early limb function after major long bone injury. Bone may be generated by callus distraction techniques and internal techniques of moving bone segments used to fill bone defects. Soft tissue defects may be treated by acute shortening, although skin defects will also close spontaneously during bone transport as the leading edge of bone is covered with granulation tissue. External fixation is also used to cross joints permitting rest and repair of the joint. Hinges placed within the bars of the fixation frame may be used

Postinflammatory ossicular fixation: CT analysis with surgical correlation.

Postinflammatory ossicular fixation is a common problem encountered by the otologic surgeon upon exploration because of conductive hearing loss in patients with chronic otitis media. These nonotosclerotic noncongenital lesions take three pathologic forms: fibrous tissue fixation (chronic adhesive otitis media), hyalinization of collagen (tympanosclerosis), and new bone formation (fibro-osseous sclerosis). Fibrous tissue fixation appears on CT as nonbony, noncalcific soft-tissue debris encasing some or all of the ossicular chain. Tympanosclerosis appears as unifocal or multifocal punctate or we

Temporary external fixation for the management of complex intra- and periarticular fractures of the lower extremity.

Complex intra-articular and periarticular fractures of the lower extremity are challenging injuries to treat. Recently, the concept of utilizing a staged approach with temporary external fixation, a delay to allow soft-tissue healing followed by open reduction and internal fixation, has gained popularity. Historically high complication rates appear to be significantly improved with this strategy. Temporary external fixation is also useful in other settings, such as in the initial management of polytraumatized patients with multiple complex fractures, augmentation of internal fixation in osteop

Circular frame fixation for calcaneal fractures risks injury to the medial neurovascular structures: A cadaveric description.

AIM: There is a risk of iatrogenic injury to the soft tissues of the calcaneus and this study assesses the risk of injury to these structures in circular frame calcaneal fracture fixation. MATERIALS AND METHODS: After olive tip wires were inserted, an L-shaped incision on the lateral and medial aspects of 5 formalin fixed cadaveric feet was performed to expose the underlying soft tissues. The calcaneus was divided into zones corresponding to high, medium and low risk using a grading system. RESULTS: Structures at high risk included the posterior tibial artery, posterior tibial vein and poste

Recent advances in the treatment of gunshot fractures of the humeral shaft.

The increase in violent crime has lead to an increase in gunshot-induced fractures in the United States. Injuries to the upper extremity are common. The treatment of gunshot injuries depends on the extent of soft tissue disruption and the type and location of fracture. Most of these injuries are a result of low-energy gunshot wounds that may be treated conservatively under the appropriate conditions. Low-energy fractures that require operative stabilization may be done with predictable results for achieving union with intramedullary fixation or compression plating. Complex open fractures from

Pre-Analytical Variables in Digestive Cancer Pathology: A Systematic Assessment of Morphological Preservation in Tumoral and Normal Tissues.

Background/Objectives: This research covers both tumoral and non-tumoral (adjacent normal) tissues. Non-tumoral tissue samples were obtained from surgical resection margins located at least 5 cm from the tumor edge, with histological confirmation of the absence of tumor involvement. Methods: These samples, varying from 0 weeks to 1 week, were systematically evaluated. The assessment encompassed critical histological aspects such as tissue architecture, nuclear morphology, cytoplasmic features, and membrane characteristics. A scoring system comprising three categories (good, fair, and bad) was

Triple-Segmental Shaft Femur Fracture: A Case Report.

INTRODUCTION: Segmental femur fractures, an uncommon injury pattern, are usually caused by high-energy trauma such as road traffic accidents. Despite their complexity, early intervention is essential for maximising recovery and reducing complications. CASE REPORT: A 34-year-old male presented with multiple fractures, including a triple-segmental shaft femur fracture, following a high-energy road traffic accident. An intramedullary nail was used for closed reduction and internal fixation of the femur fracture, which provided stable fixation while preserving the blood supply and minimising soft

The renaissance of multipurpose AMeX method over conventional tissue processing method in the current diagnostic era: a comparative study.

Many attempts have been made to simplify tissue fixation and processing prior to hematoxylin and eosin (H&E) staining, immunohistochemistry (IHC) and genomic DNA extraction. The acetone-methyl benzoate-xylene (AMeX) method, is gaining popularity as an alternative to aqueous formaldehyde fixation followed by alcohol dehydration. We have assessed the efficacy of AMeX for the quality of extracted genomic DNA in addition to H&E and IHC in biopsy specimens of human oral mucosal lesions of size not more than 1 cm. H&E staining showed similar results when AMeX method was compared with conventional fi

Acute management of major trauma involving the foot and ankle with Hoffmann external fixation.

Massive trauma involving the foot was treated by external fixation in 33 cases and followed for 1 to 3 years. Amputations were performed in seven (21%) because of uncontrollable deep sepsis in two and massive injury in five. The other 26 (79%) had functionally acceptable feet at follow-up, but all had some stiffness and cosmetic deformity. Complications attributed to the Hoffman external fixation were limited to three pin tract infections. We recommend Hoffmann external fixation when it is necessary to: 1) stabilize major open fracture dislocations; 2) maintain length where bone is lost or ext

Effect of the subcutaneous route for iliac screw insertion in lumbopelvic fixation for vertical unstable sacral fractures on the infection rate: A retrospective case series.

OBJECTIVE: To report the perioperative results and surgical outcomes of patients with vertical unstable sacral fractures who underwent lumbopelvic fixation through a modified subcutaneous route for iliac screw fixation. SUMMARY OF BACKGROUND DATA: Treating vertical unstable sacral fractures is still challenging for orthopedic surgeons. Among various methods for treating these fractures, lumbopelvic fixation provides a high reduction quality and promising stability for early weight-bearing ambulation. However, wound healing disturbance and surgical site infection (SSI) are the drawbacks of thi

Arthroscopic management of volar lunate facet fractures of the distal radius.

The clinical outcome of an intraarticular distal radius fracture is generally thought to be associated with the following factors: amount of radial deformity, joint congruity, and associated soft-tissue injuries. The proposed technique to manage this fracture pattern that involves a displaced volar lunate facet fragment uses wrist arthroscopy and pinning. Distraction of the fracture before arthroscopy is accomplished either by external fixation or by the arthroscopy tower. A freer elevator is introduced dorsally to disimpact the fragments, and next, a nerve hook is used to reduce the volar lun

Arthroscopic-assisted fixation of Hoffa fracture: A case report and description of technique.

Medial condyle Hoffa fractures are intra articular injuries, which require prompt anatomical reduction and internal fixation. Arthroscopy-guided fixation of these fractures has also been advocated; the advantages include avoidance of soft tissue dissection, fast recovery and mobilization. We describe a case of a 28-year-old male patient presenting with a 10-day-old medial condylar Hoffa fracture. Skin condition prevented from conventional open reduction and internal fixation; hence, taking note of the urgency of fixation of an intraarticular fracture, arthroscopic-assisted minimally invasive a

Treatment of complex proximal humeral fractures with minimally invasive fixation of the humeral head combined with flexible intramedullary wire fixation - introduction of a new treatment concept.

The treatment of three- or 4-part proximal humerus fractures is still a matter of scientific discussion. The following study presents the results of a combined procedure using limited invasive fixation techniques for reconstruction of the humerus head and retrograde intramedullary wiring with elastic nails for the treatment of displaced three- or 4-part fractures. A prospective study of 24 patients with 3-part and 4-part fractures was performed from September 1995 to December 1998. Combined biologic fixation for reconstruction of the humerus head, including intramedullary wiring, was utilized.

External mandibular fixation for gunshot fractures: report of 2 cases.

High-energy ballistic injuries may cause comminuted facial fractures. Treatment of such fractures might be challenging because of in-fection and soft- and hard-tissue loss. These cases may not be amenable to open reduction and internal fixation. We present 2 cases of gunshot fractures, for which external fixation was used as a surgical step before definitive treatment. With the use of external fixation, existing infection had been controlled and soft tissues had been restored, which allowed oral rehabilitation with reconstruction plates and autogenous bone grafting, if needed.

Temporary external fixation across the ankle after tibial nailing.

We describe the application of a temporary spanning external fixation device across the ankle in conjunction with intramedullary tibial nailing. This technique can be useful in selected patients with open fractures associated with severe soft-tissue trauma, skin grafts, or muscle flaps. The external fixator allows for wound access and keeps the foot and ankle in a neutral position preventing equinus. A brief period of rigid ankle immobilization is beneficial in preventing muscle motion and sheer stresses on flaps and skin grafts. The external fixator is removed at 3 to 6 weeks once the soft ti

A cost-effective and efficient ex vivo, ex situ human whole brain perfusion protocol for immunohistochemistry.

BACKGROUND: Chemical fixation of the brain can be executed through either the immersion method or the perfusion method. Perfusion fixation allows for better preservation of the brain tissue's ultrastructure, as it provides rapid and uniform delivery of the fixative to the tissue. Still, not all facilities have the expertise to perform perfusion fixation, with initial high cost and complexity of perfusion systems as the main factors limiting its widespread usage. NEW METHOD: Here we present our low-cost approach of whole brain ex situ perfusion fixation to overcome the aforementioned limitatio
