Maternal and neonatal C-reactive protein after interventions during delivery.

To evaluate the effects of interventions during delivery on the maternal and neonatal C-reactive protein (CRP) we prospectively measured CRP by immunoturbidometry in 238 mother-infant pairs (179 normal vaginal deliveries, 36 cesarean sections and 23 vacuum extractions) at 24 and 72 hours after delivery. We additionally measured CRP in peripheral maternal and neonatal blood immediately after birth in 29 uncomplicated vaginal deliveries. CRP values in the mothers and their offspring were low at birth, but rose significantly during the first day after vaginal delivery. Cesarean section induced a

Retinal hemorrhages in the newborn.

There were 100 babies examined within 24 hours of birth. Retinal hemorrhages were found in 35% of newborns. Birth trauma from forceps delivery and prolonged labor were thought to be significant. Babies born by cesarean section were less likely to have retinal hemorrhages. Asphyxia was also found to be significant. The role of low plasma prothrombin levels, the vacuum extractor and increased intracranial pressure were discussed as etiologic agents. According to Von Noorden in 1973 there appears to be no positive correlation between hemorrhage of the macula and the occurrence of strabismus.

[From the impact of French guidelines to reduce episiotomy's rate].

OBJECTIVES: Following the publication of the French Guidelines on episiotomy in 2005 by the French National College of Gynaecologists and Obstetricians (CNGOF), our unit decided to adopt a restrictive politics to replace the former liberal one. The goal of this study was to evaluate the impact of this modification of trend in the Unit of Gynaecology and Obstetrics of the teaching hospital from Caen. This was the start point of an internal audit with the objective of an enhancement of the care during delivery. PATIENTS AND METHODS: It was a retrospective study concerning the period going from

Real-world use of a vacuum-induced hemorrhage-control device in births <34 weeks gestational age.

INTRODUCTION: The Jada System® is an FDA-cleared vacuum-induced hemorrhage-control device for the control and treatment of abnormal postpartum uterine bleeding or hemorrhage when conservative management is warranted. The instructions for use for Jada contain a warning stating that the safety and effectiveness of the Jada System in delivery at a gestational age less than 34 weeks or, if multiples, uterus judged less than 34 weeks size, have not been established. While the primary analysis of the RUBY registry, an 800 subject post-approval RWE study of the usage of Jada, included 50 individuals

Traumatic Decapitation of the Fetus During Birth: Criminalistic and Forensic Aspects.

Serious intrapartum fetal injuries are unfortunate events that confer severe consequences on medical personnel. Most birth traumas are noncritical and resolve for a few days. Permanent effects or fatal outcomes occur infrequently. We report an unusual case of intrapartum complete fetal decapitation. The labor was complicated by shoulder dystocia, with resultant repeated mechanical trauma to the fetal neck and, finally, decapitation. The tragic results of biological processes in human organisms do not automatically confirm medical malpractice. However, there may be grave ethical and forensic ou

Neonatal Alopecia Due to Birth Trauma: Case Report and Trichoscopic Findings.

A 4-day-old boy was referred for evaluation of an oval patch of occipital alopecia associated with caput succedaneum and ipsilateral eyelid ecchymoses. Based on the history of a prolonged, difficult labor with vacuum-assisted delivery, the diagnosis of neonatal alopecia associated with birth trauma was made. Trichoscopy showed purple dots corresponding to blood extravasation and follicular ostia. We also review the trichoscopic differential diagnosis of focal neonatal alopecia.

Subgaleal hemorrhage with dural tear and parietal-lobe herniation in association with a vacuum extraction.

Use of the vacuum for operative vaginal deliveries has become more favorable with fewer obstetricians and family practitioners trained in the use of forceps. When compared with forcep-assisted deliveries, the vacuum has been associated with a higher incidence of subgaleal hemorrhage (SGH), cephalhematomas, skull and clavicular fractures, Erb's Palsy, intracranial hemorrhage and need for ICN admission. We report the case of an infant who developed a large SGH with midline dural tear and herniation of the medial aspect of the parietal lobes bilaterally in association with a vacuum extraction (VE

Head injuries after instrumental vaginal deliveries.

PURPOSE OF REVIEW: The types, mechanisms and clinical manifestations of head injuries (extracranial, cranial and intracranial) after instrumental delivery are described along with current evidence of their prevention and management. RECENT FINDINGS: Asymptomatic subdural hematomas can occur in up to 6.1% of uncomplicated vaginal deliveries. Maternal nulliparity, incorrect placement of vacuum extraction cup and failed vacuum extraction are predisposing factors to subgaleal hemorrhage. Injuries associated with the vacuum devices may be minimized if the recommended limits for a safe traction for

Neonatal and maternal risk in Hermansky-Pudlak syndrome: peripartum management-brief report and review of literature.

INTRODUCTION: Challenges of modern medicine are peripartum bleeding complications as one of the most frequent cases of emergency in obstetrics with a prevalence of 0.5-5.0 %, meaning the main cause of maternal morbidity and mortality. In this context, inherited diseases such as Hermansky-Pudlak syndrome (HPS) should be recognized. HPS is a rare disease and belongs to a heterogeneous group of autosomal recessive disorders characterized by the triad of partial oculocutaneous albinism, disorder of "ceroid" metabolism and platelet storage pool deficiency with bleeding disorder. Mediolateral episiotomy and obstetric anal sphincter injuries in nullipara: a propensity score matching study.

BACKGROUND: Obstetric anal sphincter injuries are a notable adverse outcome of vaginal deliveries, with incidence rates ranging from 0.25% to 6%. Key risk factors for these injuries include primiparity and operative vaginal deliveries. In recent decades, the use of episiotomy as a preventive measure for obstetric anal sphincter injuries has been subject to extensive scrutiny. The objective of this study was to investigate the role of mediolateral episiotomy in preventing obstetric anal sphincter injuries during vaginal deliveries or vacuum-assisted deliveries within the context of selective us

[Pregnancy and labor in juvenile primigravidae].

A report is given on 100 juvenile primigravidae delivered in the period from 1965 to 1973. Course of pregnancy, delivery, postpartum period, rate of premature infants, and perinatal mortality are evaluated. EPH gestosis was observed in juvenile primigravidae at a rate approximately twice that encountered in the total number of all pregnant women. This group of patients is characteristic in that intensive care for pregnant women is required in both out-patient and in-patient treatment. In addition, intensive care for pregnant women has the target of minimising the increased rate of premature in

A simple model to predict the complicated operative vaginal deliveries using vacuum or forceps.

BACKGROUND: Complicated operative vaginal deliveries are associated with high neonatal morbidity and maternal trauma, especially if the procedure is unsuccessful and a cesarean delivery is needed. The decision to perform an operative vaginal delivery has traditionally been based on a subjective assessment by digital vaginal examination combined with the clinical expertise of the obstetrician. Currently there is no method for objectively quantifying the likelihood of successful delivery. Intrapartum ultrasound has been introduced in clinical practice to help predict the progression and final me

Foetal and maternal outcome of vacuum extraction.

In a prospective study, 167 vacuum extractions (VE) performed at the Kenyatta National Hospital, Nairobi, formed 6.3% of the total deliveries. Most patients were young and of low parity. There were no maternal deaths. The overall maternal morbidity rate was 22% and were mostly minor. Post partum haemorrhage was the most common maternal complication. There were 5 failed VEs. Perinatal morbidity and mortality rates were 16.2% and 4.8% respectively. Intrauterine asphyxia was the commonest cause of death. Several complications could have been avoided by more stringent use of the instrument. The pl

Neonatal subgaleal hemorrhage and its relationship to delivery by vacuum extraction.

Subgaleal hemorrhage is a rare condition of the neonate often associated with instrumental delivery. It is a potentially fatal condition that is often underreported and underdiagnosed. The vacuum extractor is being advocated as the instrument of first choice for assisted vaginal delivery, but appears to be associated with an increased incidence of subgaleal hemorrhage. It is widely believed that the vacuum cup will dislodge before causing serious fetal trauma. Because of the ease of application, vacuum extractors could be used potentially in circumstances in which forceps would not be attempte
