Pre-Implant Substrate, Peri-Implant Electrical Features, and Outcomes Across Four Echocardiographic Phenotypes After CRT-D.

Background: Reverse remodeling after cardiac resynchronization therapy with defibrillator backup (CRT-D) is heterogeneous and may reflect both pre-implant substrate and post-implant electrical response, with implications for subsequent outcomes. Objectives: This study evaluated a graded echocardiographic response framework after CRT-D as an integrative phenotype linking pre-implant substrate, peri-implant electrical features, and post-landmark clinical outcomes. Methods: The analytic cohort included 334 CRT-D recipients with complete four-group classification based on relative left ventricular

Predictive value of QT interval correction formulas for all-cause mortality in patients with left bundle branch block.

BACKGROUND: QT prolongation is recognized as a prognostic marker associated with poor clinical outcomes and is frequently observed in patients with left bundle branch block (LBBB). However, the identification of QT prolongation in LBBB patients remains challenging. Although multiple QT correction formulas have been specifically developed for LBBB, the prognostic significance and predictive value of these corrected QT intervals in LBBB patients have not been conclusively established. METHODS AND RESULTS: This retrospective cohort study consecutively enrolled LBBB patients from 2011 to 2021. The

Electromechanical Profiling in Genotyped Dilated Cardiomyopathy with Left Bundle Branch Block.

AIMS: Genetic testing is routinely recommended in dilated cardiomyopathy (DCM), yet the prevalence and implications of pathogenic/likely pathogenic (P/LP) variants in patients with DCM and left bundle branch block (LBBB) remain unclear. We therefore investigated the electromechanical profile of genotyped patients with DCM and LBBB, and its relationship with cardiac resynchronization therapy (CRT) response and clinical outcomes. METHODS: Patients with LBBB were selected from a multicenter cohort of 1206 consecutive DCM patients undergoing genetic testing. All underwent sequencing of 20 clinica

Safety and performance of a novel defibrillation lead for left bundle branch area placement: Primary results of the LEADR LBBAP clinical trial.

BACKGROUND: Right ventricular placement of the OmniaSecure defibrillation lead has demonstrated high defibrillation success and low occurrence of lead-related major complications. With the advent of left bundle branch area pacing (LBBAP) as a physiological pacing option, the use of the OmniaSecure lead may be a viable alternative for LBBAP in the implantable cardioverter-defibrillator population. OBJECTIVE: This study aimed to understand the defibrillation efficacy at implant and the safety of the OmniaSecure lead for LBBAP. METHODS: The global Lead Evaluation for Defibrillation and Reliabil

Symptom subtypes and risk of incident cardiovascular and cerebrovascular disease in a clinic-based obstructive sleep apnea cohort.

STUDY OBJECTIVES: Patients with obstructive sleep apnea (OSA) are at increased risk of cardiovascular and cerebrovascular disease, but predicting those at greatest risk is challenging. Using latent class analysis, patients with OSA can be placed into discrete symptom subtypes. The aim of this study was to determine whether symptom subtypes are associated with future cerebrovascular disease in patients with OSA in a clinic-based cohort. METHODS: Patients with suspected OSA referred for a polysomnogram at an academic sleep center completed a comprehensive symptom survey. Patients with OSA (apne

Atrioventricular Block During Catheter Ablation for Ventricular Arrhythmias.

OBJECTIVES: This study sought to evaluate the incidence and significance of atrioventricular (AV) block associated with ventricular arrhythmia (VA) ablation. BACKGROUND: Attempted ablation of VAs that arise from the septum carries a risk of AV block. METHODS: Data from 1,418 patients who had catheter ablation for drug-refractory VAs were evaluated. Two analyses were conducted. The first analysis assessed the patient and procedure characteristics associated with ablation-induced AV block. The second analysis investigated outcome differences between patients with and without AV block. For the

Electrocardiographic patterns in biventricular pacing delivered by second-generation cardiac resynchronization devices.

BACKGROUND: With increasing use of cardiac resynchronization therapy (CRT), treating physicians should be familiar with different electrocardiographic (ECG) patterns of left ventricular (LV) lead and biventricular (BiV) pacing. However, there are a few publications on ECG patterns during BiV pacing. PURPOSE: This study was sought to determine different ECG patterns in patients with BiV pacing. METHODS: Twelve-lead ECGs during BiV pacing (right ventricular leads at apex and LV leads in one of the lateral coronary veins) were analyzed in 181 consecutive patients (121 male; mean age, 62.0 ± 13.

Left ventricular reverse remodeling in dilated cardiomyopathy- maintained subclinical myocardial systolic and diastolic dysfunction.

In idiopathic dilated cardiomyopathy (DCM), myocardial deformational parameters and their relationships remain incompletely characterized. We measured those parameters in patients with DCM, during left ventricular reverse remodeling (LVRR). Prospective study of 50 DCM patients (in sinus rhythm), with left ventricular ejection fraction (EF) <40%. LVRR was defined as an increase of ten units of EF and decrease of diastolic left ventricular diameter (LVDD) in the absence of resynchronization therapy. Performed morphological analysis, myocardial performance quantification (LV and RV Tei indexes) a

Late deterioration of left ventricular function after right ventricular pacemaker implantation.

OBJECTIVES: Right ventricular (RV) pacing induces a left bundle branch block pattern on ECG and may promote heart failure. Patients with dual chamber pacemakers (DCPs) who present with progressive reduction in left ventricular ejection fraction (LVEF) secondary to RV pacing are candidates for cardiac resynchronization therapy (CRT). This study analyzes whether upgrading DCP to CRT with the additional implantation of a left ventricular (LV) lead improves LV function in patients with reduced LVEF following DCP implantation. METHODS: Twenty-two patients (13 males) implanted with DCPs and a high

Pathophysiology of cardiorenal syndrome type 2 in stable chronic heart failure: workgroup statements from the eleventh consensus conference of the Acute Dialysis Quality Initiative (ADQI).

In cardiorenal syndrome (CRS) type 2, chronic heart failure (HF) results in the onset or progression of chronic kidney disease (CKD). Examples of CRS type 2 (CRS2) include progressive CKD resulting from chronic HF in congenital or acquired heart disease or from repeated bouts of acute decompensated HF. Animal data and clinical studies indicate that extended periods of chronic HF result in altered renal hemodynamics followed by progressive renal pathology. Experimental and clinical data indicate that CRS2 is characterized by mild to moderate proteinuria, a progressive decline of glomerular filt

Assessment of left ventricular dyssynchrony in pacing-induced left bundle branch block compared with intrinsic left bundle branch block.

AIMS: Although electrocardiographic and echocardiographic findings with right ventricular (RV) pacing mimic intrinsic left bundle branch block (LBBB), left ventricular (LV) mechanical activation pattern may differ. We compared mechanical activation pattern of the LV in RV-pacing-induced LBBB with intrinsic LBBB in symptomatic chronic heart failure patients. METHODS AND RESULTS: We studied 37 patients with chronic RV-pacing and 37 patients with intrinsic LBBB who were referred for cardiac resynchronization therapy. Ventricular dyssynchrony patterns in left bundle branch block, with and without heart failure.

BACKGROUND: Assessment of ventricular dyssynchrony in patients with heart failure is used for selecting candidates for cardiac resynchronization therapy (CRT). The patterns of regional distribution of dyssynchrony in a population with LBBB with and without heart failure have not been well delineated. This aspect forms the object of the study. METHODS: Tissue Doppler Imaging (TDI) data of consecutive patients with heart failure and LBBB (Group A) was compared with those with LBBB and normal LV function (Group B). All patients had standard 2D-echocardigraphic examination and TDI. Tissue velocit
