Distal humerus nonunion in adults: Management strategies and contemporary outcomes.

Distal humerus nonunion (DHN) in adults is an uncommon but highly disabling condition that poses significant reconstructive challenges. The complexity arises from a combination of poor local biology, limited metaphyseal bone stock, and the biomechanical demands of the elbow joint, often resulting in pain, instability, stiffness, and functional loss. We aim to emphasize current evidence on the pathoanatomy, diagnostic approach, and contemporary management strategies for distal humerus nonunion, with a focus on decision-making between osteosynthesis (including revision fixation of previously ope

Macroscopic cartilage degeneration despite partially preserved imaging appearance in chronic neurological hip dislocation: findings from surgically resected femoral head specimens.

BACKGROUND: Chronic neurological hip dislocation is a severe musculoskeletal complication in non-ambulatory patients with cerebral palsy. Surgical decision-making is largely based on standard radiographs, although radiography provides only indirect information on articular cartilage status. Whether preoperative radiographic findings accurately reflect macroscopic cartilage degeneration remains unclear. This study evaluated the association between standard radiographic appearance and intraoperative anatomical evidence of cartilage destruction in chronic neurological hip dislocation. METHODS: T

The proportional magnitude of rickets and diagnostic utility of radiography in children with cerebral palsy attending neurology clinic at tertiary hospital in a low resource setting: a cross-sectional study.

BACKGROUND: Cerebral palsy (CP) remains a significant public health challenge in Africa, compounded by economic burdens and limited healthcare resources. The co-occurrence of rickets in children with CP is poorly understood, particularly in low-resource settings, where its etiology and radiographic features remain unclear. Children with cerebral palsy often stay indoors, have poor nutrition, and low vitamin D due to limited sun exposure. Vitamin D deficiency plays a key role in the development of rickets. It is therefore crucial to investigate the magnitude of rickets in CP, its radiographic f

Risk of Distal Femur Fracture in Antegrade Intramedullary Nail Fixation of Proximal Femur Fractures: A Finite Element Study.

Surgeons often use long antegrade intramedullary nails (aIMNs) when fixing fractures of the proximal femur in geriatric patients. However, risk of peri-implant fracture in relation to aIMN termination level has not yet been studied. We therefore studied the correlation between aIMN nail tip termination level and extent of distal femur fracture in three different loading scenarios using finite element models of seven different length aIMNs. These were loaded into 3D models of synthetic osteoporotic femurs, which were loaded in single-leg stance (SLS) with body weight load at the femoral head, t

Will the Articular Surface Collapse After the Fixation of a Tibial Plateau Fracture?

BACKGROUND: Tibial plateau fractures present significant challenges, especially when joint depression is accompanied by metaphyseal comminution. The primary objective in these fractures is to attain, if feasible, an anatomic reduction of the articular surface, thereby restoring joint congruity and mechanical alignment to enhance functional outcomes and decrease the incidence of post-traumatic arthritis. Fractures with significant depression of the articular surface necessitate the elevation of the depressed fragments and stable internal fixation. OBJECTIVE: This study aimed to assess prospect

Time to Radiographic Union Following Minimally Invasive META Procedure for Hallux Valgus.

BACKGROUND: The use of fourth-generation minimally invasive hallux valgus surgery with metaphyseal extra-articular transverse and Akin osteotomy, recently dubbed "META," is a new generation of minimally invasive surgical (MIS) technique and a recent focus of foot and ankle orthopaedic literature associated with good functional outcomes and low complication rates. Literature in orthopaedic trauma has indicated that 2 or 3 neocortices on postoperative radiographs are associated with high likelihood of union. In this study, we conducted a retrospective analysis to investigate the time to bony uni

Does stem length affect proximal humeral stress shielding? A systematic review.

INTRODUCTION: Total shoulder arthroplasty (TSA) has increased in use, with revision surgeries also increasing. One cause of revision is aseptic loosening driven by stress shielding-a condition where altered load distribution leads to proximal humeral osteolysis surrounding the humeral prosthesis. This systematic review compares stress shielding between traditional-length (TL) and short-stem (SS) humeral prostheses, hypothesizing that SS stems reduce stress shielding. METHODS: A systematic search of Cochrane, Embase, and Medline databases was conducted. Inclusion criteria targeted studies from

Supracondylar ostectomy and shortening (S.O.S.) for distal humerus nonunions.

BACKGROUND: Many distal humerus nonunions are associated with bone loss, and rigid internal fixation is difficult to obtain, especially for low transcondylar nonunions and those with severe intra-articular comminution. The purpose of this study was to analyze the results of a strategy to address this challenge utilizing internal fixation using the Supracondylar Ostectomy + Shortening (S.O.S.) procedure for distal humerus nonunions. The goals of this procedure are to (1) optimize bony contact and compression through re-shaping the nonunited fragments at the supracondylar level with selective hu

Pilot study of three novel implants-GSK triangular system, locking hip screw and controlled sliding dynamic hip screw, for fixation of fracture of the neck of femur.

BACKGROUND AND OBJECTIVE: Intra capsular fracture of the neck of femur (FNF) treated traditionally with a dynamic hip screw (DHS) or three cancellous screws (3CS) has a high incidence of complications with reoperation rates between 20 % and 45 %. We hypothesized that FNF unites by primary healing. Therefore, intra-operative compression and absolute stability post-operatively until healing are essential. We postulated that FNF requires 2 types of implants- those which provide absolute stability for young patients with good bone stock and another with sliding mechanism for elderly patients with

The effect of shoulder prosthesis stem length on failure due to torsional loading. A biomechanical study in composite humeri.

BACKGROUND: Shoulder arthroplasty is becoming increasingly common. With evolving implant designs, multiple humeral stem options exist for the surgeon to choose from. New stemless and short-stem systems are modular, remove less native bone stock, and better adapt to patient anatomy. It has been suggested that shorter stem implants may be protective against periprosthetic fracture; however, this has not been mechanistically evaluated. Therefore, this study aimed to biomechanically test synthetic humeri with long-stem, short-stem, and stemless arthroplasty components in a torsional manner to eval

Hydrogel delivery of DNase I and liposomal vancomycin to eradicate fracture-related methicillin-resistant staphylococcus aureus infection and support osteoporotic fracture healing.

Fracture-related infection (FRI) is a devastating complication in orthopedic surgery. A recent study showed that FRI causes more severe infection and further delays healing in osteoporotic bone. Moreover, bacterial biofilm formed on implants cannot be eradicated by systemic antibiotics, warranting novel treatments. Here, we developed a DNase I and Vancomycin hydrogel delivery vehicle to eradicate Methicillin-resistant Staphylococcus aureus (MRSA) infection in vivo. Vancomycin was encapsulated in liposomes, and DNase I and Vancomycin/liposomal-Vancomycin was loaded on thermosensitive hydrogel.

Observation Versus Cast Treatment of Toddler's Fractures.

BACKGROUND: Nondisplaced metaphyseal fractures of the distal tibia, or toddler's fractures, are one of the most common pediatric injuries. Healing typically occurs quickly without sequelae. Treatment ranges from long leg cast immobilization to observation. This study compares short-term clinical and radiographic outcomes of toddler's fractures treated with long leg casting versus observation. METHODS: Patients with toddler's fractures were offered enrollment and randomization at diagnosis. Because many families opposed randomization, a preference arm was added after one year. All subjects wer

Diffusion Tensor Imaging of the Knee to Predict Childhood Growth.

Background Accurate and precise methods to predict growth remain lacking. Diffusion tensor imaging (DTI) depicts the columnar structure of the physis and metaphyseal spongiosa and provides measures of tract volume and length that may help predict growth. Purpose To validate physeal DTI metrics as predictors of height velocity (1-year height gain from time of MRI examination) and total height gain (height gain from time of MRI examination until growth stops) and compare the prediction accuracy with bone age-based models. Materials and Methods Femoral DTI studies (b values = 0 and 600 sec/mm2; d

Optimal blocking screw placement for retrograde IM nail fixation of distal femur fracture: a standardized biomechanical study of "osteoporotic" synthetic bone.

PURPOSE: Intramedullary (IM) nail use for metaphyseal fracture management is problematic in long bones like the femur because the larger medullary cavity allows increased fracture motion and potentially increased implant failure and malunion/nonunion risk; Achieving effective distal femur fracture fixation is even more difficult in osteoporotic bone. Blocking screws to control IM nail movement are known to be effective for tibia fracture management. Few studies have evaluated IM nail and blocking screw use efficacy for distal femur fracture fixation in osteoporotic bone. METHODS: This study u

Orthosis and Foot Structure Affect the Fifth Metatarsal Principal Strains During Simulated Level Walking.

BACKGROUND: Fractures of the proximal fifth metatarsal bone are common injuries in elite athletes and are associated with high rates of delayed union and nonunion. Structural features of the foot may increase fracture risk in some individuals, emphasizing the need for intervention strategies to prevent fracture. Although orthotic devices have shown promise in reducing fractures of the fifth metatarsal bone, the effect of orthosis on fifth metatarsal strains is not well understood. PURPOSE: To quantify the effects of different foot orthotic constructs on principal tensile strains in the proxim
