Extended wear contact lenses for aphakia.

The current modality for correcting unilateral aphakia is by the use of daily wear hard and soft contact lenses and by the use of intraocular lens implants. The use of the Cooper Permalens soft contact lens as an extended wear lens for correction of aphakia was investigated in 177 eyes. The relative merits of the extended wear lens are discussed with emphasis on fit, visual function, and contact lens-related problems.

Reverse Galilean telescopic spectacles in unilateral aphakia.

Minimizing aniseikonia in unilateral aphakia is often a difficult task. A case is presented in which a patient preferred a reverse Galilean telescopic spectacle of his own design to a contact lens. The design of the spectacles and the patient's visual performance with them are described. A reverse Galilean telescopic spectacle should not be excluded from the therapeutic alternatives in unilateral aphakia.

Depth of field for the presbyope.

As a general rule the near addition (add) for a presbyope is selected so that the range of clear vision is dioptrically and not linearly balanced. Implicit in such thinking is the assumption that the dioptric and linear range of clear vision cannot be simultaneously balanced. Assuming that the range of clear vision for the absolute presbyope is due to depth of field and assuming also that the blur circle theory adequately describes the limits of depth of field, I will show that the dioptric and linear range of clear vision can indeed be simultaneously balanced. The results are independent of t

The range of zero-associated phoria in an asymptomatic clinical population.

The clinical concept of the range of zero-associated phoria is developed, and normative data for this range are presented based on a clinical study of 201 asymptomatic patients. The relation of patient age, sex, and amount of near lens addition to this range is examined. The range at far is not related to patient age or sex. The range at near appears to bae independent of patient age, sex, and the amount of near addition required by presbyopes.

Contact lens intolerance in association with epithelial dystrophy.

Refractile changes in corneal transparency that are associated with epithelial basement membrane dystrophy have been detected in contact lens wearers who show a wide range of contact lens tolerance. In some of these cases, contact lens wear precipitates an episode of recurrent epithelial erosion. In other cases excellent contact lens tolerance is observed. The cases described indicate that pure bleb forms of epithelial basement membrane dystrophy may not be of benign significance when contact lenses are worn.

Corneal edema with hydrogel lenses and eye closure: time course.

Central corneal thickness changes during the wearing of hydrogel lenses under closed-eye conditions were measured for two subjects. Each wore an investigational ultrathin Bausch & Lomb Soflens contact lens (series X-176) on one eye only under closed-eye conditions for periods ranging from 30 min to 6 hr. The other eye served as a control. Corneal thickness of the control eyes increased gradually and reached a maximum increase of about 4% at the end of the 4-hr wearing trial. Corneal thickness in the experimental eyes also increased with wearing time during the first 4 hr and then remained fair

Adaptation to prism-induced heterophoria in subjects with abnormal binocular vision or asthenopia.

In a previous study we investigated how normal subjects adapted to prism-induced heterophoria. In this study we have continued this work by investigating this phenomena in 15 patients with abnormal binocular vision and/or asthenopia. It was found that the majority of subjects lacked or had a deficient adaptation system to base-in and/or base-out prisms, but all except two subjects examined adapted normally to vertical prism. These results suggest that it may be possible to treat the horizontal adaptation system independently from the vertical one. Prism vergences, heterophoria measurements, an

Optic nerve drusen and pseudopapilledema.

A case is presented in which unilateral visible optic nerve head drusen and a contralateral visual acuity loss are associated with moderate hypermetropia and the use of oral contraceptives. Optic nerve drusen are known to be responsible for visual field losses, but a reduction in visual acuity is a rare finding. Extensive investigation of this patient revealed no other cause for the visual acuity loss. However, the possibility of misdiagnosis of papilledema and papillitis is suggested by recent onset headache with unilateral visual acuity loss. A tentative diagnosis of buried drusen of the opt

Clinical investigation of the foveal light reflex.

Over a 2-month period, 1,516 optometric patients were evaluated by a collaborative study group of 30 staff doctors in Veterans Administration Medical Centers and Outpatient Clinics. The purpose of the evaluation was to determine the status of foveal light reflexes in the population, and thereby to establish normative data for this clinical phenomenon. Frequency data, correlation analyses, and literature citations are included in the paper.

Introduction to the Symposium on Basic and Clinical Aspects of Vergence Eye Movements.

The basic aspects of vergence eye movements revolve about a central theme of a critique of the Maddox classification. Topics include interactions between accommodation and convergence in the absence of visual stimuli, dynamic properties of accommodative and fusional vergence, interactions between vergence eye movements and other visual motor systems, violations of Hering's law associated with specific components of vergence, and sources of fixation disparity. The clinical reports examine the effectiveness of disparate and accommodative stimuli for driving vergence eye movements in strabismus w

Adaptation to prism-induced heterophoria.

Measurements of the way the oculomotor system adapts to prism-induced heterophorias have been made in eight normal subjects. Adaptation to 2 delta vertically and, on separate occasions, to 6 delta horizontally was found to be substantially completed after only 2-3 minutes of binocular visual experience. The form of the adaptive process for distant targets was asymmetrical in the horizontal meridian, being faster after the insertion of 6 delta base-out than for 6 delta base-in. This asymmetry largely disappeared for near vision.

Fixation of disparity: a steady state error of disparity-induced vergence.

Fusional vergence eye movements are controlled by a fast-acting mechanism that aligns the eyes in response to retinal image disparity and a slow-acting mechanism that sustains binocular alignment. These two control mechanisms are distinguished by their decay time constants and stimuli. Fast fusional vergence responds to retinal image disparity, and slow fusional vergence responds to the effort or output of the fast control mechanism. Fixation disparity is modeled as a steady-state error of fast fusional vergence. Fixation disparity is shown to be inversely related to prism adaptation, a manife

The dynamics of fusional vergence eye movements in binocular dysfunction.

Two subject groups were designated as either normal or abnormal on the basis of their clinically determined vergence and heterophoria data. The question was asked, can these groups be differentiated on the basis of certain objectively measured dynamic properties of fusional vergence response? Vergence tracking rate, percentage completion of step responses, response velocity, and latency discriminated between the two experimenal groups; latency of convergence responses did not.

Disparity-induced vergence responses in normal and strabismic subjects.

Latencies of vergence responses were obtained by presenting vertical line stimuli for 0.5 sec to the fovea of one eye and to various retinal locations of the other eye of four subjects with normal binocular vision, one exotrope with normal retinal correspondence, and four esotropes with anomalous correspondence. Vergence latencies were larger for squinters than nonsquinters and were especially long for the squinters with anomalous correspondence. For three of the four anomalous corresponders, the direction of the vergence movement (convergence or divergence) was based on the location of the st

An unexpected role for normal accommodative vergence in strabismus and amblyopia.

Accommodative vergence was measured using the Müller experimental paradigm in which target movement along the line of sight of the viewing eye results in large movement of the fellow (covered) eye. Our paper demonstrates an unexpected predominance of accommodative vergence over disparity vergence in patients with constant strabismus amblyopia, intermittent strabismus, and amblyopia without strabismus. Comparison of accommodative vergence responses in these patients with responses from normal subjects using symmetrical and asymmetrical targets under binocular and monocular conditions establishe

Krukenberg's spindle and contact lens-induced edema.

Krukenberg's spindle, a pigment deposit on the corneal endothelium, is frequently an inconsequential finding, although it can be associated with pigmentary glaucoma. A case report is presented describing corneal changes occurring with a gas-permeable contact lens (Polycon) in the presence of a Krukenberg spindle.

A visual profile of the alcoholic driver.

Some visual characteristics of the chronic alcoholic were investigated in a sample of 100 male alcoholic patients and 100 matched controls. The purpose of the retrospective study was to determine whether these characteristics may contribute to an increased motor vehicle accident rate among alcoholics. Significant differences between the two groups, which might affect driving abilities, were found for color vision deficiencies and the breakage and loss rate of spectacles. No significant differences were found for stereopsis deficiencies, limitation of visual field, distance phorias, ductions, o
