Correction of adult scoliosis via a posterior-only approach.

OBJECT: Adult scoliosis is a pathologically different entity from adolescent idiopathic scoliosis. The curves are more rigid, and rotational deformity and multilevel sagittal vertebral slippages compound the coronal malalignment. To correct these deformities, a surgical anterior release procedure is usually required, as well as posterior instrumentation-assisted fusion. This exposes the patient to the risks of a second procedure and of a thoracotomy or laparotomy. To decrease these risks, the authors have performed an anterior release, posterior release, and reduction via a posterior-only appr

Laminoplasty following anterior cervical fusion for spondylotic myeloradiculopathy.

Eighteen patients with spondylotic myeloradiculopathy who had undergone expansive laminoplasty after anterior cervical fusion were reviewed. The average period from the initial to the second operation was 7.8 years. Excellent neurological improvement occurred in 4 patients, good in 6, fair in 4 and poor in 4. Radiographs at the time of the second operation showed spinal canal stenosis in 11, and 12 showed dynamic spinal canal stenosis immediately above the fused area. A narrow spinal canal adds to the risk of recurrence. When spinal stenosis is treated by anterior cervical fusion, additional l

Cartilaginous endplate avulsion is associated with modic changes and endplate defects, and residual back and leg pain following lumbar discectomy.

OBJECTIVE: While cartilaginous endplate (CEP) avulsion is a common finding in discectomy due to lumbar disc herniation, its roles in residual back and leg pain, associations with Modic changes (MCs) and endplate defects (EPD) remain unknown. DESIGN: Patients with a single-level lumbar disc herniation who underwent endoscopic discectomy were studied. On MR images, the adjacent endplates of the herniated disc were assessed for MCs and EPD. The presence of CEP avulsion was examined under endoscopic and visualized inspection. Back and leg pain were evaluated by a numeric rating scale (NRS) and th

Cost-effectiveness Applications of Patient-reported Outcome Measures (PROMs) in Spine Surgery.

Because of the rising health care costs in the United States, there has been a focus on value-based care and improving the cost-effectiveness of surgical procedures. Patient-reported outcome measures (PROMs) can not only give physicians and health care providers immediate feedback on the well-being of the patients but also be used to assess health and determine outcomes for surgical research purposes. Recently, PROMs have become a prominent tool to assess the cost-effectiveness of spine surgery by calculating the improvement in quality-adjusted life years (QALY). The cost of a procedure per QA

Endovascular solutions to arterial injury due to posterior spine surgery.

Iatrogenic arterial injury is an uncommon but recognized complication of posterior spinal surgery. The spectrum of injuries includes vessel perforation leading to hemorrhage, delayed pseudoaneurysm formation, and threatened perforation by screw impingement on arterial vessels. Repair of these injuries traditionally involved open direct vessel repair or graft placement, which can be associated with significant morbidity. We identified five patients with iatrogenic arterial injury during or after posterior spinal surgery between July 2004 and August 2009 and describe their endovascular treatment

How to reduce recurrent laryngeal nerve palsy in anterior cervical spine surgery: a prospective observational study.

BACKGROUND: Recurrent laryngeal nerve palsy (RLNP) occurs as a complication during anterior cervical spine surgery. In 2005 the authors demonstrated the high incidence of asymptomatic RLNP in a right-sided approach. OBJECTIVE: This follow-up prospective observational study was designed to test 2 options said to reduce the rate of RLNP: reduced endotracheal cuff pressure and sinistral approach. METHODS: Two hundred forty-two patients in whom anterior cervical spine surgery was performed were examined postoperatively with indirect laryngoscopy to evaluate the status of the vocal cords. All pat

A Comparison of fixed-hole and slotted-hole dynamic plates for anterior cervical discectomy and fusion.

STUDY DESIGN: A retrospective review of clinical data at 1 institution was performed. OBJECTIVES: To compare the clinical and radiologic outcomes between fixed-hole and slotted-hole dynamic cervical plates. SUMMARY OF BACKGROUND DATA: Anterior cervical plating is commonly used to increase stability and promote spinal fusion. Two techniques, fixed-hole dynamic plating that uses variable angled screws and slotted-hole dynamic plating that permits sliding, are viable options, but there have been no clinical studies comparing their effectiveness. METHODS: Fifty-six patients at 1 institution hav

A new endoscopic spine system: the first results with "Easy GO".

PURPOSE: Endoscopy meets increasing interest by spine surgeons. However, endoscopic results are diverging and many spinal endoscopic systems are difficult to apply and handle. METHODS: A system for endoscopic spinal surgery was developed where the main goals were: (1) easy intraoperative handling with standard microsurgical techniques, and (2) avoidance of a prolonged learning curve. The system consists of various dilators, two different work sheaths, two different 30 degrees endoscopes, and an endoscope holder. RESULTS: Between August 2006 and April 2008, 80 spinal surgeries were performed

Long term outcome and adjacent disc degeneration after anterior cervical discectomy and fusion with titanium cylindrical cages.

BACKGROUND: Anterior cervical discectomy and fusion (ACDF) is widely performed for the treatment of cervical spinal degenerative disease. Autogenic or allogenic bone grafts are used for interbody fusion with satisfactory long term outcomes. However, harvest of the autograft causes donor site complications and allograft is associated with low fusion rate. Threaded titanium cages (TC) have recently been introduced to cope with these disadvantages, but there is little evidence of long term results. METHODS: The long term outcome was studied after ACDF using TC. Clinical and imaging follow up was

Clinical results and development of heterotopic ossification in total cervical disc replacement during a 4-year follow-up.

Cervical total disc replacement (CTDR) aims to decrease the incidence of adjacent segment disease through motion preservation in the operated disc space. Ongoing data collection and increasing number of studies describing heterotopic ossification (HO) resulting in decreased mobility of implants, forced us to carefully evaluate our long-term clinical and morphological results of patients with CTDR. We present the first 54 consecutive patients treated with 65 ProdiscC prostheses during a 12-month period (2/2004-3/2005). All patients signed an informed consent and were included in prospective lon

Two-level anterior cervical discectomy versus one-level corpectomy in cervical spondylotic myelopathy.

STUDY DESIGN: A retrospective investigation of clinical and radiologic outcomes after surgical treatment for 2-level cervical spondylotic myelopathy (CSM). OBJECTIVE: The study was undertaken to compare the outcomes of 2 different anterior approach types for 2-level CSM. Specifically, 2-level anterior cervical discectomy and fusion (ACDF) was compared with 1-level anterior cervical corpectomy and fusion (ACCF). SUMMARY OF BACKGROUND DATA: The optimal surgical approach for 2-level CSM has not been defined, and thus, the relative merits of 2-level ACDF and 1-level ACCF remain controversial. Ho
